My mother, Barbara, was admitted to this rehabilitation facility after a fall that resulted in a…read morehead injury. What followed was a steady and heartbreaking decline due to what I believe was gross negligence and a lack of compassion from staff. Barbara was a vibrant, spunky woman who spoke her mind. But during her stay here, she became quiet, confused, and unable to finish her thoughts--nothing like the woman we knew. Repeated calls for help were ignored or significantly delayed. One day I waited 40 minutes after pressing the call button while nurses sat at the desk. I found her soaked through her bedding in the morning with no staff in sight. Her oxygen concentrator failed, and when I reported it to a nurse, I was told it "wasn't possible." Meanwhile, the emergency backup tank was also empty. Basic medical needs, like her Humira dose, were missed. I had to personally supply her medication because the staff failed to provide it in time. Eventually, her breathing became so labored I had to increase her oxygen myself while waiting for someone to take action. It wasn't until I pushed repeatedly that they finally agreed to order a chest X-ray. Throughout this, Barbara tried to express that something was wrong but couldn't communicate clearly--a heartbreaking shift from her usual self. During this time, I emphasized to staff that this was not her normal behavior. She was not passive or confused before entering this facility. And yet, no urgency was shown. Tragically, Barbara contracted Influenza A during her stay and passed away. She came to this facility for rehabilitation--to get better--not to be neglected. This place failed her. Failed us. And now we live with the consequences. I do not recommend trusting your loved ones to this facility.

do not work here or send your loved ones here. i am a worker here and the workers are snobby and…read morerude, and they are outrageously rude to the residents here. the workers put on a show for people who come visit their loved ones, all the nice, and happy, and smiles you see is all fake! today October 23, 2025 a nurse here was telling other nurses that a resident here was not liking the idea of him have a catheter, so he got upset and was saying he didn't want it on himself, and he was moving around, and the nurse told the other nurses she snatched him by his feet and yanked him to the edge of the bed and yelled at him in his face "you're going to keep this f**king catheter on rather you like it or not" she was demonstrating how she was pulling him as if she was proud of herself. this is a elderly who has pain and is fragile!!!! anything that you accidentally missed of your loved ones belongings after they've passed away, the nurses either throw the items away or give them to other residents or take them home for themselves.... to me that is just very disrespectful. about 2 weeks or so ago, i over heard a resident telling a nurse that another nurse slapped her in the face :( this place is just awful, and it's too expensive.

We had a reason. Six years ago Mom needed rehab from another fall and simply had a great experience at Forest Hills. I am pleased to report the high standards of patient care still exists. Simply put. The staff cares about the residents and patients! That includes the aides, nurses, OT and PT. It also shows with an attitude that the patients are adults. They even have a happy hour each Thursday. And My dad gets a free guest meal for lunch each day. So he and mom get to have lunch together like they have for the past seventy years. Pretty cool! Going through an experience like this is no fun. That's why it is really important to have a place like Forest Hills.

You feel like you're on vacation with a pool, 5 star chef. After my…read moreparent left the hospital, Sem Haven Rehab center took great care getting them back up on their feet and walking again. It was determined that my parent was in the beginning stage of dementia, so we have decided to keep our parent there, so they are safe, properly medicated, and cared for. I believe the SEM community is suffering through COVID like most care centers across the US. I am happy they keep our family posted on the number of cases and are doing their best to stay fully staffed.

Unfortunately the staff is not fully vaccinated and Covid has run rampant among the staff. Which resulted in my sweet mother's death. Yes, the un vaccinated staff killed her. Mom had a will to live but Covid was too much. Also due to the Covid dancing among the staff they were severely short staffed. We pulled our mom out after realizing they had only 2 staff members on shift for 15? Memory care patients? Disgraceful. I hope on judgement day those unvaccinated staff members see who they killed, a sweet, kind, gentle, vaccinated woman who had more life left in her. You should all be ashamed.
