    Great staff at both the front office and the PT team in Irving…read more Unfortunately, my experience with billing is the reason I'm not leaving 5 stars. First, I was charged a no-show fee for an appointment with a PA that I never scheduled. Second, I was charged a no-show fee for a Monday PT appointment during a major ice storm, when schools were closed for multiple days and the City of Irving Police Department advised residents to stay off the roads. Most businesses, including grocery stores and banks, were also closed. If the clinic had used its texting system to clearly notify patients that appointments were still being held and that no-show fees would apply, that would be different. But expecting patients to travel in unsafe conditions without any communication is unreasonable. Because of this billing approach, I'll be continuing my care elsewhere.

I recently lost my ACA health insurance and was very nervous about the cost of this. I was a walk-in and they treated me right away. Because I was uninsured, I had to pay the $478 visit fee up front. I was evaluated by a good nurse and very good Physician's Assistant named Evan. I was quite impressed with him. For the fee, I received his evaluation, x-rays, an ultrasound, a Toradol shot, and a sling to take home. I was so relieved. The care and the cost were all very reasonable. I didn't mind not seeing a doctor at all. I was scheduled for a follow-up visit in one week and was happy to return. However, at the follow-up visit, I discovered that the fee would be $203. I was never given any itemize documentation for what any charges covered. But what choice did I have? You can't exactly go shopping around for follow-up care without having to pay for a new place's initial care fees. The follow-up was with a different PA, who was also very nice. She spent 10-15 minutes with me and did her evaluation. She told me to go home and wear the sling for another week and then come back for another follow-up. That was it. A routine PA visit for a person with insurance earns the company around $115. If I had Medicare, it would be around $80. Every follow-up at DOC for uninsured patients is $203. It's like they get you in the door and make you happy, then gouge you.

Dr Gottlieb (Jamie) ensured the rep was at my first follow up visit to adjust the settings on the stimulator and also set me up for another follow up.     August 2005, 10:15am…read more Sign in at the doc's office, fill out forms, sign release forms since the damn HIPPA laws and the dorks at the MRI place sent them the 1st report but not the 2nd, sit down and wait.. and wait... and ask what the hold up is... he's stuck in surgery, he will be back soon... and wait... About NOON, we finally got to see the neurosurgeon (Nickname "TheCuteOne"). He asks me about my migraines, my numbness, my symptoms, and really listens to what I had to say. For once in this whole crazy shabang, I had a doctor listening to ME! He pulls out the MRI (and the X-rays sent with me by the Chiropractic Neurologist I had seen the week before) and shows the Chiari and starts drawing little happy faces and lines on it. The fact that my Chiari is only 8mm was a good thing and he said that it just didn't look like they were crowding each other that bad. It was possible that the "hole" that the cerebellar tonsils fell in is bigger than "bigger than normal." He says that he wants me to go get a second opinion (maybe a third if 2nd doesn't agree), but he just didn't think my condition was bad enough for surgery at this time. He wants to talk to the other doctor if Mr.2ndOpinion thinks I need surgery, and he wants to send me to a new neurologist to start a brand new pain medication and treatment plan to see if we can treat my Chiari without a knife and a zipper-scar. After he talks to me "TheCuteOne" asks my parents if they have any questions. My mother brings up the 2nd MRI that I had done last week and the absence of a syrinx. He flat out tells us that had my 2nd MRI from Tuesday resulted positive, and that I had a syrinx, then instead of pain medication and new doctors... that our conversation today would have been more along the lines of hospital gowns, designs shaved into the back of my head, and what color stitches I wanted in my zipper. He said that the Chiari surgeries were "FUN" and that you get to go in there and see all kinds of neat stuff.. but he frankly didn't think I needed it yet. He's a brain surgeon, he says he does necks and backs for "FUN." Then he seals the deal by saying, in front of my parents, "if you were my daughter, I wouldn't do surgery." This was the first neurosurgeon I saw for Chiari and I was a little put off by his suggestion that my surgery was something he did for fun, like waterskiing. The reason I can't give Dr. Cwikla more than the 1 star, is because he is the one who referred me to Deborah Cantrell... the drug dealer who made the first six months of my life as a diagnosed Chiarian a living Hell.

The receptionist who…read moreanswered the phone was very rude and preceded to hang up on me all because I asked to speak to someone else because I wasn't understating her question. I never once yelled at her yet she told me I was and instead of getting someone else to help me she hung up on me. If this is a reflection of how the doctors are as well I would highly advise fo stay away. It's a shame I'm in a great deal of back pain and seeking some medical help and I couldn't even get an appointment.

Unfortunatly today I am running low on my meds and so I rang her office to get a new prescription but the office staff said because she(the dr) last saw me last year I needed to be seen before she is able to write up a new prescription even though I told her I am running low on meds.What they didn't tell me at the initial appointment was that I would need to schedule an appointment yearly to get a new prescription. I' have never had this issue with any of my former neurologists. The neurologist I saw in Hawaii yes they did reinforce the fact I had to see him once to twice. year they still told me if I am running low or need a new prescription called in before the appointment to just call their office and it will be taken care of.So Shadad didn't tell me that an appointment is needed yearly or tell me to call her office to schedule an appointment for this year.Appointments such as speciality care I prefer making in advance so I have the appointment scheduled for that year or so many months. Because of the above, I am leaving her office and switching to someone closer who is more understanding with more experience under his belt. In my experience drs who have less than 20 years under them aren't very good. I wouldn't recommend her office .
