At some point it was clear at the end to provide a place for end of life care. To help a person be clean, cared for free of pain in the transition from life to death. He was there about a week and was clean and made comfortable for what their ability was. The rooms large, solid and private with other areas & break rooms for family to gather. 24 hours a day. Just want to say that the negative reviews seem to be based on people not understanding what hospice is. It is for end of life after doctors are unable to do anything more for them and they are near dying. I am sorry they had confusion at that time, but they should not detract from the facility. Some also seemed to expect the hospice to make arrangements for funeral homes, burials or cremations for them. No. When a family member is sick or at end of life YOU need to make arrangements for that. So I think some reviews were unreasonable in medical type expectations and also cremation and funeral type arrangements which they do not do and people should not expect them to make recommendations as potential conflict of interests. But, thank you to all the staff there and their kindness & care which is a tough situation & time.

It was anything but that. On day one we were told a nurse would contact us the next day. 3 days passed before anyone contacted our family. There was no clear directions provided upon admission. Our mother was only in their care for 11 days, if you can call it care, as the only contact was admission and then 3 days later and the day of passing. Then we were told someone would come pick up the bed and other equipment the next day which never happened and required us to call the day after that to ask someone to come pick it up. I understand that doesn't seem like that big of a deal however when you are going through that emotional turmoil you don't want an additional reminder of the pain you already feel. But the final straw for me was the fact that our mother had been gone for 10 days and we had been waiting to bring her ashes home yet we couldn't because the Unity doctor had not signed her death certificate. As a family who is already mourning the passing of their loved one I find it quite unempathetic of someone who is supposed to be a professional with compassion for others, especially in these circumstances. I would never in my life recommend Unity to anyone who wants their loved ones to have compassionate care at the end of their days.

She was supportive and communicated clearly and always timely. Unity provided all needed equipment quickly. The CNA was good with Mom. I cannot recommend Unity strongly enough! 5 stars meaning not just great but outstanding!

    My mom fractured her hip and we chose to admit her to McAuley Manor for her rehabilitation. I hope…read moremy mom never has to go to a short-term rehabilitation facility again, but if she does it will NOT be McAuley Manor. The facility seems very understaffed. The call button for the nurse could take 10 minutes for someone to respond. There was a handful of nurses and aides that seemed to care about my mom's well-being, but some of them made us feel like we were a huge inconvenience if we called them to the room to ask questions or ask for help. I'm sure this was due in part to the fact that there isn't enough staff and they were busy, but some need to work on their bedside manners with the patients and their guests. They put a commode in her bathroom that was missing at stopper on one of the legs and was not sturdy. My mom was a fall risk to begin with but no one was concerned that the commode was shaky and could cause her to lose her balance and fall again. I had to put a stack of napkins under the leg without the stopper so that it would not shake, but someone removed the napkins the next day which made it unsteady again. The bathroom was not cleaned frequently. I actually grabbed some paper towels and cleaned the floor myself a couple times I was there. One day that I went to visit, her whole room smelled like urine. Several nurses and aides had been in her room before I arrived that day, but none of them called housekeeping to clean the bathroom even though the smell was quite obvious. I had to actually request to have her bathroom cleaned. Since my mom had limited mobility because of her hip, she needed help showering. She was scheduled to have a shower twice week, but she got about three showers in the whole month she was there. The cafeteria also has a limited menu, so if a patient has any dietary restrictions you'll probably need to bring in food for them. The one good thing I can say about the facility is that the physical and occupational therapists were great. They were very caring and compassionate during her therapy sessions, and they always had a smile on their face when they came into my mom's room to do her therapy or when they just came to visit her to see how she was doing.

We were assigned Emily as our case manager and she was great to work with. However, we interacted the most with Grace who was on-call at the time we engaged them and helped my Dad a good bit during his last couple of days. Grace was super patient with us when we asked lots of questions about his care and hospice in general. LifeChoice also provided a rabbi for us and I think my father really appreciated that the rabbi visited with him for over an hour. I think they provided my father a lot of peace and comfort during his final days. My only regret was not engaging them sooner. Thank you LifeChoice for all of the help you provided my father and our family.

Their whole demeanor and attitude is that of angels, compassion, empathy, love..When he was in the hospital, he was not receiving the proper pain management, they kept delaying the hospitals hospice program while my dad was dying in pain. I was desperate to get him home. Alena, the administrator, moved mountains to make that happen. They got my dad home in less than 18 hours, they were by my father's bedside within two hours of me calling desperate for help, which felt like a miracle during such a difficult time. Daniel, the nurse who came to the hospital, was so attentive and caring such a such a genuine person. And once we were home, nurse Julie and nurse Grace took over and truly went above and beyond. They made sure the bed oxygen tanks supplies and meds was delivered before my dad even got home.They made sure my dad was comfortable, out of pain, and surrounded by dignity and respect--something he was not receiving in the hospital. there was also a Sweet CNA that came to help as well, Because of them, my dad was able to pass peacefully at home, just as he deserved. That means everything to me my father and our family. All of my father's friends and loyal family were able to come visit with him before he passed the next morning.. I highly recommend this hospice agency to anyone going through end-of-life care for a loved one--or even for themselves. Don't wait until the last minute. Have this hospice team set up ahead of time--you will not regret it. They are truly angels doing God's work. Shannon Johnson-parson
