Highly incompetent physyicians/NPs do not know how to read chest Xrays. My family member had a large pleural effusion which they sat on for three days and never sent copies of the Xray report despite multiple requests, which read large effusion. Grossly negligent. Nursing staff also very slow to respond to requests. Therapy delivery suboptimal. read more

A 1 star is generous. I wish there was a negative rating that I could give this place. Medications were not witnessed being taken. Loved one is now on a ventilator due to missed doses of medications due to nurses neglecting to ensure medications were being taken. Nurses had no idea how to work a PICC line. Call light times were up to 2+ hours of not being answered, if they were answered at all. Aides also stated that they would be right back to help and never came back; call light was on the entire time, and again, they never came back. There was one morning that family member had to go to the bathroom in the early morning. Family member waited and waited; had call light on. Had to go to the bathroom so bad that family member got up on their own (in their weak state) and they got to the toilet. Family member pulled the call light to get off the toilet, as they couldn't do it on their own, help never came for 2 hours. Family member sat on the toilet for 2 hours (from 4am to 6am) until the next shift was coming in and found her on the toilet. Family brought to the DON's attention about the nurse's not watching the family member in the facility take their medications; family found loose pills numerous different occasions (i.e. on the bed, on the floor, or left in the medication cup) as the nurse did not watch medications being taken. Multiple falls occurring in the same night and only first fall was reported to family. The other fall that was not reported family was the fall that initially ended family member back in the hospital. COVID booster was also given BEFORE medications were checked. Booster was given BEFORE it was cleared with the pharmacist. NEGLEGENT! Copper Trace does not have an ED or a DON that cares. FAMILY FIRST MY ASS. read more

Our 94yr old Mother lived in assisted living for 7 years before being placed in Copper Trace to recuperate from a fall and surgery on her foot. In just 3 weeks, she went from being very self sufficient to bedridden in a diaper that they never changed. She got a severe UTI, bed sores, and fluid around her lungs from laying down too much, and worst of all, they took no precautions what so ever for a fall risk patient. Of course she fell (see picture) she lay on the floor and yelled for quite some time before the staff, who was in the hallway chatting and laughing, even heard her. The day before she fell, we called every number on their list trying to check on her and never got an answer. They got covid while she was there and they never called us to let us know. We only found out when we went to get her belongings. Do not be fooled by fast talking admissions people and the pretty building. Copper Trace - No Precautions for fall risk patient - Assisted Living Facilities Near Me - Westfield, Indiana

I would give them zero stars if possible but you have to choose at least 1 star to leave a rating. My mother was sent there by the social worker at a large Indy hospital after coming down with Shingles. My mom was there less than 48 hours and had to be transported back to the hospital because she had sustained 3 separate injuries, 2 of which involved brain bleeds. I was not allowed in the facility when my mom was admitted which was the first red flag. Then I couldn't ever reach a "nurse" to check on my mom's status. I called at all hours of the day and night but the phone rang off the hook with no option to leave a voicemail. Another red flag. But when they wanted paperwork signed they suddenly wanted to communicate with me. At the time of admission my mom managed all of her own affairs but after the injuries sustained at Copper Trace she was deemed incompetent and was unable to manage her affairs going forward. Mom never fully recovered from her less than 2 day stay at Copper Trace and died a few months later. This place needs to be sold to a new company that will care more about the patients being kept safe from injuries. I did notify the hospital social worker that had my mom admitted there but she claimed they weren't aware of issues. Well just scroll down to the poor woman with the blackened eye and read their review. If I have found their review sooner my mom would NEVER have been at Copper Trace and might still be alive today. read more

I spent about 2-3 weeks there as a rehab patient after surgery. I found their lack of coordination/internal communication about my care to be very frustrating. One person would tell you one thing, the next would tell you something completely different. Trying to speak to their management about issues was like trying to see The Wizard of Oz. Each nurse would come in to change my wound dressing and say the nurse who did it the last time didn't know what they were doing. There was a lot of finger pointing that went on. Rarely did they all seem on the same page. I found the food to be inedible. Most things seemed pre-packaged and certainly not good for a diabetic. Soups appeared to be out of a can and way too salty to be healthy. The only fresh fruit I could get was a banana. I'm a diabetic and did not find much I could eat. Despite speaking to the person who ran their dining program, things did not improve. My husband wound up bringing in food from home for me. I felt sorry for the people who lived there full time. There were some bright spots, too. Their PT program was organized, well-thought out and the staff was very friendly and helpful. There were a few nurses who really knew their stuff: Stacey Triller, Jennifer Welfin. These few bright spots would never make me return here, however. read more

They almost let my mother die through neglect. They were understaffed, however the worst part was their incompetence and lack of knowledge on medical procedures and lack of care of/for their patients. Don't judge a book by it's cover. I now understand why out of all the sates in this nation Indian's elder care comes in DEAD LAST and why the industry is feverishly seeking to get legislation passed to keep people from being able to sue. We sent my frail 89 year old mother to rehab a Copper Trace for a broken shoulder. While she was there and while an aide was giving her a shower, an unsecured soapy bottle of shampoo fell off the little shelf in the shower and landed on my mother's foot and since she is on blood thinners it produced a massive hematoma. They assured us everything was fine, and my wife being a recently retired ER nurse not being able to asses it herself because of COVID restrictions on visitors, had to take their word. Weeks later they sent her home and told us to just rinse it with saline solution and change the dressing and that home health care would be here the next day. We called home healthcare and they had no idea what we we talking about and that it didn't matter anyway because they didn't have any nurses available. Then we unwrapped her foot and found a hole in her foot as big as the first IPhone produced and a half inch deep in her foot - how much flesh do you think an 89 year old had on top of her foot - and the doctors are working hard to keep her from losing her foot (wish I could post the pictures her). We have left messages with the social worker there .... no reply. We have left messages for the administrator there .... no reply. We've even left messages at the corporate office .... no reply. But ......... now that I am turning to social media and leaving pictures, guess what? I got a text today saying that they are forwarding it to the same people that are currently refusing to return my call. If I wasn't actually aware of the fact that their lawyers would just drag things out until my frail and failing mother died, I would sue them. I am, though, reporting this to the Indiana department of health and to state elder abuse officials. I also wonder if Medicare might have something to say about having to pick up the tab at the American taxpayers expense for having to treat this wound caused by IMO, their glaring negligence and clear and undeniable medical malpractice. read more

This beautiful community has changed Dad's life and has given all of us a new peace of mind. Great staff and very caring. The physical therapy staff have been excellent - pushing me to the limit and making me achieve my goals quickly but effectively. I am hopeful to be able to go home within the next couple weeks and get back to a normal life. The only issue I have experienced is issues with laundry return. But other than that - I am highly satisfied! read more
