With that being said: my experience with VNS was absolutely discouraging for our health care system/provider. My father came home for (home hospice) on a Thursday afternoon. The assessment nurse was kind: however I say this respectfully..I couldn't understand what she was saying because she had a heavy accent. She went over his medication and assessed his body, took pictures of wounds etc. she did not do vitals or anything. (I don't know if that was a requirement) We were not provided with any medicine or supplies. Not even a little starter kit until something came in the mail. Friday My dad was having labored breathing. I called the 212-609-1920 and they advised me to give him some morphine. Ummmm with what you guys don't give us anything. NO INSTRUCTIONS. NOTHING. I had to call 911 - he was unresponsive. He was rushed to the hospital on Friday afternoon and passed away early Saturday morning. My recommendation: DON' T USE THIS SERVICE!!!!! I am going all the way to the top with this complaint. I won't rest until I get proper ethical change with the company. It won't be about money. This is about PRINCIPLE.

I was glad when my mother was released, as I felt more comfortable with her at home than at this facility, despite my being the primary caregiver.

Now that she is ill and requires actual care/help there is NO support. They are beyond understaffed. She has been in very scary situations and when she rings the call button no one comes OR someone comes hours later. She fell once and we called the desk for help and we were told the nurse was on break and it would have to wait until she came back! She has been forced to sit in her feces for extended periods of time bc no one helped her to go to the bathroom. She has been left w/ her oxygen machine unplugged as she couldn't get to it and no one came to help her. We had to call 911 for her because the facility refused to. The facility is very dirty and the oxygen machines are not cleaned. It is inhuman and devastating to see conditions this bad. More importantly my Aunt has to live in fear. Even something as simple as going to the bathroom could be a traumatic , dangerous situation. Very sad.

At first, the place seems amazing: clean, friendly staff, consierge, bakery. But two weeks in, you…read morerealize your loved one is sitting or lying down 23+ hours/day, with less than one hour of PT a day, and nothing on weekends, without enough staff, and horrible family communication ....unless the patient is capable and motivated, they are getting weaker. So at week 3 we tried to take our sister out, and they blocked it with every delay tactic they could. Our assessor needed an appointment. One was given, only to arrive and find out they were all busy in a case management meeting (apparently they do this tactic all the time), and the next available appointment was in five more days. Even the MD agreed she should be discharged. She had fallen twice, with no supervision. Several staff members pulled me aside to tell me to get her out of there, she was in danger of breaking another bone. Finally, after forty phone calls (nurses station never available) and threats and emails to the social worker, she was discharged and now has 24 hour care, which is a much better/safer option. I'll never use rehab for her again, and certainly not Excel. This appears to be a business plan to KEEP BEDS FULL. Even if there are better choices for the patient. KEEP YOUR LOVED ONES AWAY!

Saying goodbye to a beloved family member is never easy, but Peaceful Transitions made it as positive an experience as it could be. Our sweet senior dog had been dealing with some health problems, but when we woke up this morning he was unable to walk and was showing signs of neurological issues. When he didn't improve throughout the morning we were about to take him to the emergency vet as his usual vet is closed on Sunday, but then I saw a listing for Peaceful Transitions and we decided to give them a call. They were so incredibly kind on the phone and even though it was a Sunday and they had a full schedule today, they were able to fit us in only a few hours later. They explained the entire process to us over the phone and answered all of our questions, helped us with planning arrangements for after he passed and we were able to pay our invoice easily online before our appointment . Our little guy always got very nervous at the vet, so having someone come out to the house where he could be comfortable and peaceful in his own environment was a blessing. Dr. Matheson was so kind and extremely patient, he examined our little guy, went over our options with us and reviewed the entire process once we were sure we wanted to move forward. They encouraged us to have a special treat for him and a favorite toy, to help distract him while the doctor got him ready. He never could eat bacon normally as it upset his tummy, but he loved it and was so happy to munch on pieces of it while the doctor got prepped for the procedure. We were able to say goodbye to him as he rested peacefully in his own bed surrounded by his favorite toys and blankets and the people who loved him. After the procedure was completed Dr. Matheson stepped out to allow us some time alone to say goodbye, and then made us a nose and paw print and framed them for us. He transferred him into a basket lined with blankets and let us carry him out to the car, to be with him for his final journey. He was so kind and supportive every step of the way, and we are forever grateful to have had this experience. Our hearts are broken over losing our baby, but this is the only way I would ever want to do this again in the future. No rushing to the vet, no fear for our dog in a strange environment, just love and respectful care as we said goodbye. We were also concerned that the price for in-home euthanasia would be too expensive for us, but it actually was in line with what our vet would have charged, and cheaper than the emergency vet. Thank you again to Dr. Matheson and Peaceful Transitions for making what is a stressful and difficult experience as positive as it could be, and for taking such wonderful care of our little guy and us.

Saying goodbye to my girl was the hardest thing I've had to do, but she got to go out peacefully…read moreand surrounded by everyone that loves her. I couldn't ask for more. Thank you, Dr. Louden, Kristin, and team for helping her transition. For reference, our appointment was supposed to be a week out but she made it out to Brooklyn sooner for us. The fee was $950 ($600 plus $350 travel fee).
