The search was terrifying...every single place had at least one horrible review. A family member who works at a law firm with lawyers who deal with nursing homes highly recommend this place. I can see why! Within 12 hours of being there my dad had a meeting with an occupational therapist and a few hours after that he had a session with a physical therapist named Daniel. He had my dad up, walking 10 to 15 ft with a walker...something he hadn't done in almost 2 weeks. We were very impressed. They were focused on getting him better. As with any situation, I would recommend asking as many questions as possible, double checking with them, and keeping up with ur loved one's progress, but after the first night I was confident that my dad was in the right place for his recovery. Unfortunately, my dad was only there for 1.5 days before his kidneys got so bad that he had to return to the hospital....all due to problems that we knew could end his life soon. He passed away a few days later. All in all, ManorCare West was highly recommended to us, and we would recommend it to anyone else in our situation.

My care there was adequate but with confusion about diet and medication often from the nursing staff. My doctor verified multiple times that I was to have a diet increase as I had lost roughly 100lbs and was denied adequate food. I was denied physical therapy of any kind and told that my insurance did not approve it. I was left there to die.....but I did not. On March 6th I was notified that there would be an additional patient sharing my room. This patient was Derk Ball and was brought in with his sister. The staff installed a bed but I do not recall a phone nor call button and the room was not ready when Mr. Ball arrived. I overhead and was told that he was brought in from Palos Hospital having undergone a biopsy of his lower back and suffering from Pneumonia. There was a minimal amount of care done to ensure that he was made comfortable or had adequate oxygen. He was not receiving fluids or nutrition and after his sister left was basically ignored by all staff other than to check vitals. Several times I heard the nurses comment that he was removing his oxygen tube from his nose and had it in his mouth. Food was brought to him, it went untouched and his tray was removed. On Friday night I commented to the nurse that he had not had food all day and nothing to drink and offered her to give him my milk. To my knowledge he had not had any nourishment all day. On Saturday morning Mr. Ball was awake partially and had a male visitor and by Saturday afternoon and into the evening he seemed to be asleep. Food was brought but removed shortly thereafter and other than a bed check or to check for vitals, Mr. Ball was ignored throughout the rest of the day and evening. Late into that evening the attending nurse seemed rather unprofessional in making comments to the CNA about subject matter that I found to be offensive. This escalated to the point that when they were changing Mr. Ball she began to make comments about his genitals and very off-colored remarks directly at him. The CNA was laughing histerically which only encouraged her cruel behavior. She then commented that he was sucking on his oxygen tube and that it goes into his nose and not his mouth. Mr. Ball was not vocal and did not seem to be lucid. At this point I was able to get myself out of bed and into a wheelchair and after she left the room I went into the hallway and notified the nurse that he had not had anything to eat nor drink since he was there. Her response was vague and basically that if he doesn't eat they remove his tray. Sunday morning I was awake from most of the night and the nurse checking in that morning was told by me that again he had not had anything to eat. Our breakfast came and I told the nurse to give him my milk which she brought to his tray. Mr. Ball did not seem awake for the rest of the day. After lunch came and went I again told the nurse that he has not had anything to eat. Late into Sunday evening the attending CNA did a bed check and said hi to me and was very pleasant. She went over to check on Mr. Ball and left his side of the room after less than a minute. She said goodnight to me and left. Within about an hour the nurse coming onto her shift was doing her bed check and we exchanged a few words and she proceeded to Mr. Ball's side of the room. After only a few seconds she ran out of the room and I knew something was wrong. The scene of this became very frantic, very quickly with multiple alarms and multiple people entering the room. A cart with life saving equipment was wheeled into the room as staff attempted life-saving procedures. The CNA who had last checked on Mr. Ball prior to shift change was present but admitted several times that she did not know what to do. She said that she did not know how to perform chest compressions. I was in my bed and because of multiple people in the room I was unable to leave. I heard mention that paramedics were on the way and one of the nurses got me out of bed and ushered me down the all to the lunch room area. It was very cold so I stayed in the hallway. By this time some of the patients that were mobile came out of their rooms out of curiosity. The paramedics arrived and assisted in the life-saving measures of Mr. Ball. The head first responder kept asking who was in charge and what was happening. After life-saving measures for Mr. Ball failed this paramedic started to become loud and was yelling that nobody seemed to be in charge and that there were needles left on the window sill and he tried to put them in the sharps box and it was full. He demanded multiple times to know who was in charge and what was going on at this place. He blatantly stated that they were not removing the decedent and to call the medical examiner's office immediately. He left in utter disgust over what he had witnessed.

I want to thank the great staff at Grace…read morePoint Place in Oak Lawn, Illinois for all of the hard work and care they put into creating a very memorable 100th birthday celebration for my very dear, elderly friend Irene. This is a serious milestone. Irene was happier and so animated yesterday, more than I have seen her in years. There are not enough words to say how much I love and appreciate her, so the above and beyond kindness that went into making Irene's birthday special was enormous to me. The facility and the grounds are very attractive, clean, and a very nice atmosphere. I was really impressed with how lovely everything is at Grace Point Place. Very friendly and helpful staff in all areas. If and when the time comes that I would need a care facility, I would definitely like to live in a place as nice as Grace Point Place in Oak Lawn, Illinois. Everyone is treated very well, with class, kindness, and some good humor which keeps the spirits of the residents uplifted. They decorated an entire section of the facility for her, her bedroom, and the dining room. The staff gave her gifts, and some cute little gifts for her guests. All of the powers that be showed up for her party including the Marketing Director, Activities Director, Director of the Facility, Her Public Guardian, and the Guardian's Boss, Maintenance staff, and Engineers all joined in the celebration, sang, danced and ate delicious food, cake, and some very tasty cider. The setup was beautiful with two gorgeous backdrops and lots of balloons and flowers. They played records and even hired a wonderful band, which was a great surprise. They showed up and showed out!! God love them for what they all did in support of Irene. I've never seen anything done up like that for a resident of a Senior Rehab Facility. God Bless Everyone and Their Big Hearts For Their Generosity and Kindness. Btw, the soul food luncheon was Delicious. Fried chicken, ham, macaroni and cheese, sweet potatos, cornbread, and some of the best collard greens I've had in a long time. Denise, the Head Cook and her culinary staff did a phenomenal job. I made sure that everyone knew how grateful I was to them. Irene showed so much love and appreciation to everyone for being there for her. I believe there were some people there from the news media, but there was so much going on that I didn't get a chance to check it out. Again, many thanks on behalf of Irene, my friend. I truly would not have chosen any other hospice, and I would not recommend anyone other than Shiloh Hospice and Palliative Care LLC. I actually found Shiloh on Google and chose them based on their reviews, and I can honestly say they did not let our family down. The day my mom came home, the nurse practitioner, Tobi, came to meet with me after her visit. I was also introduced to her nurse, Addie, for the first time, and we were visited by the social worker, Christina. A few days later, the chaplain also came by to provide support. Addie was an amazing nurse. She came twice a week, and one week she even came three times. I was always able to reach her by phone or text, even on weekends. She truly went above and beyond. Gabby was a gift from heaven. She came three times a week to bathe and care for my mom, and because of my mom's needs, she came multiple times a week when needed. Christina was an incredible social worker who provided wonderful resources and even continued to check on me after my mom passed, just to make sure I was okay. I honestly do not know what I would have done without the support, love, and care I received from Shiloh. As a healthcare worker, you want your family to receive the very best patient-centered care, especially during such a difficult time, and that is exactly what Shiloh Hospice provided -- and more. Not once did I feel alone, scared, or confused about what to do. Everything was taken care of. There were no surprise bills, no unreachable staff members, and no added stress during an already painful time. They are truly a blessing, and I would tell anyone to choose Shiloh.

The long-term consequences of their failures continue to impact her health today. Shiloh made several promises that were never fulfilled. The Executive Director assured us a minister would pray with my mother weekly, a doctor would visit every other week, and a care plan would be developed. None of this happened. The doctor never once saw her. The minister never came. We saw the social worker just one time. The only person who followed through was Nurse Brittany, who kept us informed and seemed to genuinely care. The worst part? When my mother began showing signs of a stroke, we called Shiloh's on-call nurse. He showed up hours later, asked me what I thought he should do, agreed that she needed to go to the ER -- and then did nothing. When I returned nearly 11 hours later, my mother was still at the facility, sitting alone, unresponsive. I rushed her to the hospital, but the critical four-hour window to treat her stroke had passed. She missed her chance for the medication that could have changed the outcome. To make matters worse, we never heard from Shiloh again. Not a single call. Not one word from the Executive Director. No apology. Nothing. This was more than poor service -- it was dangerous neglect. Please think twice before trusting this organization with your loved one's care. We wish we had.

Nurses routinely dismissed this, saying she was simply "tired." In reality, she was being administered PRN medications without our consent, leaving her heavily sedated. They have refused to release her medical records with his a violation of the state of Illinois law. During a surprise visit at 10 p.m., we found her in such poor condition that she had to be rushed to the hospital, where she remained for seven consecutive days. Not once did we receive a call from Peace Igbinovia ,, the Executive Director/Director of Nursing, during this crisis. Family concerns were consistently ignored, and promised care services were never delivered. The facility is grossly understaffed, and the competence level of the nursing staff is dangerously insufficient. Integrity -- or rather, the lack of it -- is evident and was even openly acknowledged by the Executive Director aas she noted that several of her nurses were dishonest and unethical, yet she refused to hold them accountable. . Medical professionals urged us to remove her immediately and seek alternative care. We did, and she is now thriving and receiving excellent, compassionate care elsewhere. Families deserve honesty and safety -- Autumn Leaves, under its current leadership, provided neither. If you care...you will look elsewhere. Your loved one deserves better.

But I'm writing this review to express that my family will forever be grateful for the exceptional care that our father received at Autumn Leaves of Orland Park. They made my father feel like this community was his home and we were treated like family. The wonderful caregivers, the nurses, management, cooks and even the custodial members of this community were so kind, caring and loving to my father and our family. I interviewed so many places before I found Autumn Leaves, and at first, it seemed too good to be true, that we had found a place where the staff genuinely cares about its residences and treats them with kindness, patience and compassion. But we certainly had found that place! Autumn Leaves has such a dedicated team; all hands on deck from every single worker! I never saw a staff member on their cell phone while working; they all make sure that the residences eat their meals, get plenty to drink and give them the help that is needed throughout the day. My father's laundry was always done in a timely manner, his room was always kept clean, and his bed made every day. I had a stereo in my father's room, and the caregivers made sure his Frank Sinatra music was turned on every single morning for him without fail while they would get him ready for his day. His medications were always given on time as well. And during the night, the staff would check on him multiple times. If you're looking for a wonderful place for your loved one, look no further than Autumn Leaves. Our family was blessed to have our father here!
