He was incredibly thorough during our tour--no rushing, no skipping over details. He patiently answered every question and made sure we clearly understood how the community operates. His calm, knowledgeable presence immediately built trust. The CNAs and nurses were equally impressive. They were attentive, compassionate, and consistently greeted residents with genuine smiles. What really stood out was how client-centered they were. Every staff member we passed made eye contact, said "hello," and acknowledged each resident by name--as if each person truly mattered. It's rare to find a place where excellent care, kindness, and professionalism all come together so naturally. This community exceeded our expectations in every way, and we left feeling confident, grateful, and comforted knowing that residents here are treated with dignity and heartfelt care.

Our 88-year-old mother was referred to this "beautiful" and highly recommended rehab facility in November of 2024 and spent 20 days there. It has a very low staff to patient ratio and my mother didn't get the care that she needed. She used her call button often if we weren't there with her and often needed repositioning. One of her complaints was that the call button was often placed out of her reach at nighttime. My family and I split time at the facility and were there 8-10 hours a day. We saw the doctor one time during those three weeks. It often took a number of days to get medication changed and implemented. Nurses and CNA are overworked and understaffed, therefore, toileting and dispensing medication was often late. My mother developed a horrible bed sore that we are still dealing with to this day. I'm well aware of the issues plaguing these types of facilities, but this experience was beyond disturbing. On my mother's last day there she told us about an "issue" from the night before with one of the CNAs. I immediately went to the director of nursing to inform them. The directors of nursing and administration hopped on the complaint immediately and interviewed with my mom with me present. My mother was able to articulate exactly what she told me when interviewed by the directors. I left at 3:30 PM and told the staff I would return at 5:30 PM to help her with dinner. As I was on my way back to the facility, I received a phone call that my mother was being transported to the ER on a 5150. During the course of the facility's due diligence, the Los Altos police department was called to come interview my 88-year-old bed-bound mother with no family present with her. I would have had no issue with it, save for the fact that there was no communication to the family that would happen as the outcome of the complaint, or I otherwise would not have left. My mother has always been a very timid and non-confrontational person and having two imposing men in dark uniform with guns on their belts come into her room made her anxious and frightened therefore she made some concerning statements, which is understandable. I also understand the need to 5150 someone when appropriate. The hospital never issued a 5150 and my mother's medication was changed, as needed. This change could have been done at the facility if the doctor was present half the time they said they would be and/or would have communicated with us in a timely manner. Granted, it was a blessing to get her out of there and home with us. It's unfortunate that some facilities have the reputation they do, but understaffing and lack of communication with a "non-present" doctor is the common denominator. This place was a horrible experience.

From the compassionate and attentive staff to the warm and welcoming environment, this facility truly felt like home for him. The caregivers went above and beyond to ensure his comfort, well-being, and dignity. They treated him like family, always engaging with him and making sure his needs were met. The medical team was knowledgeable and proactive, providing excellent communication and peace of mind. I want to especially mention Yazmin, Rajinder, Ed, Brisa and Karen. But all staff were great. I had several interactions with the cleaning and kitchen teams. They were highly professional and supportive. They were great at letting me know what was going on with my dad and making sure he was taken by ambulance to the hospital when necessary. They also provided a lot of valuable advice to me on his care/doctors including recommending a hospice provider when that time came. The driver, Ed, even visited my dad at the hospital several times while he was there. The facility itself is beautifully maintained, clean, and inviting. Each room has a private bathroom and patio. The meals were nutritious and delicious, and there were always engaging activities to keep residents active and social. The staff does their best to get residents to participate. It was comforting to know that my dad was in such good hands, surrounded by people who genuinely cared for him. I am forever grateful for the kindness and dedication of everyone at Sunnyside Gardens. If you are looking for a place where your loved one will receive the best possible care, this is it. I wholeheartedly recommend them without hesitation!

But he quickly made connections with other residents and eventually started thinking of the Avant as home. He was treated so well by the staff. They were unfailingly attentive and thoughtful. They looked after him and cared for him better than we could've possible hoped. When his health took a bad turn or he fell, they were on top of it immediately. They contacted us right away and gave us thoughtful guidance at every step based on their considerable experience. Li Li, Trish, and the entire staff are very professional, knowledgeable and kind. Mom was worried they would have to go to "The Other Side," also know as the memory care unit. Other residents say things to scare them about this arrangement. Dad loved the swimming pool in the apartment living, but once he 'crashed' into another swimmer who yelled at him, he no longer swam. Thus, my mom was ready to move out and live in their own home. Dad passed away a year ago and Mom, who is 93, is happy to live in her home. For me, I live 3 hrs away and always worry. I wish she continued to live at the Avant. She is isolated in her home, doesn't have transportation, but is happy to walk to her downtown. She is of sound mind, so I don't have any say, but that doesn't mean I don't worry. The Avant is a place I would live in the future and now that Mom is on her own, I think she would be more receptive to making friends with the other residents.

While moving into a senior community was a big adjustment, my Dad has come to really appreciate it and generally, every resident I talk to seems very happy at the Forum (you should ask residents you see how they like it when you visit). Some of the other benefits I've seen about the Forum are: * Management and management structure - The Executive Director (Nancy is great) and her management team and the staff are friendly and genuinely care about the residents, the cleanliness of the facilities, and the service provided. Additionally, the Forum is a non-profit benefit corp and is run somewhat like an HOA where the owners/residents have the ability to substantially influence overall policy and items important to them (e.g. food, entertainment, services, etc.). One key difference from a normal HOA is that the owners have the time and desire to make this model work as it requires participation to do so. * The naturally beautiful setting of Rancho San Antonio that surrounds the Forum - While not everyone takes advantage of all the hiking trails, just being in nature provides a relaxing, healthy setting. This is quite different from some of the senior centers that are in more urban settings. * Health facilities, library, ample activity spaces, planned activities (movies, presentations, etc,) and excursions (off site), many grassroots activity groups...uhh a wood shop? keep things more interesting while the services free residents up from the daily challenges (cooking, cleaning, etc.) and give them more time to enjoy. * Full continuum of care - You start of in independent living in a condo unit or stand-alone villa. Should your health needs and ability to live independently change, they have skilled nursing and assisted living facilities right there as well as a new memory care facility (on a fee for service basis after you sell your independent living unit). * Very pet friendly with many folk owning dogs, cats, other critters and visiting pets welcome. * Resident happiness - Perhaps the most important gauge is that I'm still surprised at the general level of satisfaction I sense from the residents ... and I visit pretty regularly. If your family is looking into senior living communities, check the Forum out!

Morgan Hill…read moreNov 14, 2025 About 15 years ago, while living in Sunnyvale, Ca, my aging mother was suffering from dementia and other age-related problems that had gone beyond my ability to provide the level of care she needed and deserved. I was fortunate to discover Madera Villa, a small, welcoming facility with around 17 rooms, each furnished and complete with its own bathroom. The staff there provided her with constant, loving care and attention until she peacefully passed away from her illness. I have carried gratitude for that experience ever since. Fast forward 15 years to March of 2025. My wife of 34 years was now struggling with dementia, mobility issues, and recovering from a broken arm after a fall. Once again, I reached a point where I could no longer meet her growing care needs on my own. Living about 30 miles south of Madera Villa, I searched for a nearby facility but found none that gave me a good feeling. Then I called Madera Villa--and to my delight, I was greeted by Allan Tuan, the facility administrator, and his wonderful assistant, Sarah. Both of them had been there when my mother was a resident fifteen years earlier. When I walked through those doors again after all those years, it felt less like entering a care facility and more like returning to a place of comfort and trust. Seeing Allan and Sarah still there filled me with a deep sense of reassurance. It reminded me that some places truly hold on to what matters--people who care for others not because it's their job but because it's who they are. Even in her condition, my wife recognized Allan and Sarah from visits long ago and connected with them instantly. Their compassion, attentiveness, and genuine kindness made her feel safe and loved. During her stay, she became friends not only with the entire staff but also with several residents. I started calling Sarah the "sweetheart lady of Madera Villa," and the title fits her perfectly. Madera Villa stands out as a place where warmth comes before profit and where every resident is treated like family. I cannot think of a better place for a loved one who needs care and community.
