He thoroughly arranged the necessary tests, including MRI, biopsy, and PSMA PET/CT, which led to a clear and timely diagnosis. Because I was concerned about the risk of infection, I specifically requested a transperineal (TP) biopsy, and Dr. Shinghal was very receptive and performed it without issue. I truly appreciated how he listened to my concerns and accommodated my request. After the cancer was confirmed, he took the time to clearly explain the available treatment options, discussing both the benefits and potential concerns of each. This helped me make an informed and confident decision about my treatment. His recommendation of Dr. Yao for surgery was excellent and made a meaningful difference in my overall care and outcome. Dr. Shinghal is professional, thoughtful, and genuinely patient-centered. I highly recommend him to anyone looking for a knowledgeable and trustworthy urologist.

He is kind, caring, empathetic and has a celebrity-like quality. I see angry/negative reviews and the problem I have with them is, I doubt Dr. Turek looked at anyone and said, "I can fix this without a doubt". If you're looking for certainty, I think it is highly unreasonable in this area. My wife and I understood this but would have stopped at nothing to build our family. I understand the cost involved is staggering but if Dr. Turek has any guidance it is worth following based on your desire and resolve. Diagnoses: Azoospermia Treatment: 1. Sperm mapping. 2. Varicocelectomy 3. As a result of the varicocelectomy not correcting, I underwent microTESE. One of the most important points I would like to make is, at the time of treatment I was in my mid to late 40's. I had never been under general anesthesia, been operated on nor had stitches. I was TERRIFIED knowing what I was about to go through. Literally my worst nightmare come true. I still have spontaneous thought inducing panic about the days leading up to my surgeries. I would do it all again without hesitation if it was Dr. Turek who was my physician/surgeon. I felt he was truly invested in our success and my care. He knew and understood my pain and anxiety. He knew how important this was to my wife and me.

He treated me like a moron, didn't listen, and didn't solve my problem. So he only gets 2 stars. I was born with my right ureter through a muscle, which caused many kidney infections as a child. The local doctor misdiagnosed this as backwash. Luckily, my mom persevered and got a correct diagnosis and I was successfully operated on when I was 6. Last year (at 28), I had 3 kidney infections within 6 months. I thought this was a problem, so I starting the slow path through the insurance and doctoral hoops until I finally got to a urologist, who had a test done, which revealed that (surprise surprise) my right kidney had reduced functioning. He said this was likely due to scar tissue from the childhood operation, and could be fixed easily enough. Unfortunately, I moved to SF before we could get on with the fixing. The old urologist recommended Dr. Rosenberg here in SF, and I immediately made an appointment in hopes that we could start out where I had left off with other urologist. I brought with me my chart from the other urologist, including the results of the kidney function test. I wrote down my whole history in their "new patient" forms, and then told it all to Dr. Rosenberg. He pretended to listen, and then proceeded with the generally uncomfortable process of manually exploring my urinary tract. Below is a small excerpt of the frustrating conversation that ensued: Dr: It's probably backwash. Me: Actually, that is what I was misdiagnosed with as a child. Dr: Hmm. So, where's your scar? Me: On my lower abdomen. (Dr. examines scar on lower abdomen) Dr: Well, that is not where the scar should be, if the operation was what you said. It should be up here on your side. (Dr. proceeds to physically turn me and search for a scar on my side. Like there really might be a scar there that I had not noticed in the past quarter century. Like maybe I got a long thin scar on my lower abdomen through some other childhood scrape, like, I dunno, falling off a bike, and that distracted me from the REAL surgery scar on my side, unnoticed by me until the astute Dr. pointed it out.) Me: Um, no, I am sure I don't have a scar on my side. I don't know exactly how they did the operation, but I know it was my ureter through a muscle. Dr. (giving up on searching for the elusive side scar, but with no explanation as to the actual scar on my lower abdomen): Couldn't have been. It's probably backwash. If you have backwash, go to Dr. Rosenberg. For any other problem and/or for an actual doagnosis, go elsewhere. I am.

However, sex is a very important part of my social life and how I deal with stress. I was having erectile disfunction issues that were changing how I lived my life and making me frustrated and depressed. I called his office and from that point on, everything was spotless. His receptionists were fast, efficient and helped me to figure out my insurance was accepted. I got an appointment and Doctor Karpman was helpful, frank, talked in a language I could understand, was very clear about the risks and outcome of the surgery and more importantly, kept an open mind about some peculiarities of my case. I didn't want to get a circumcision, so he offered an alternative treatment and I got another one, so my foreskin was expanded enough that Doctor Karpman felt he could do the surgery without risking infection. As a gay man dealing with mostly straight health care, even in San Francisco, sometimes it feels like people are not aware of a lot of differences and you have to be cautious about explaining things. I never felt that way with Doctor Karpman and even morr, when I neexed him to he listened, he kept an open mind, respected my autonomy, instead of telling me he knew better. And he incorporated the knowledge he gained from this as a new option for future patients. The surgery was not easy for me, but it was well done, the recovery process was uphill, but it had no unexpected issues, no infection and now I am very happy with my sex life. I am happier, calmer and more satisfied with life. Thanks, doctor Karpman!
