Horrible level of care provided. If you want to be treated disrespectfully and misdiagnosed then this is the clinic for you.

***REVIEW IS LONG BUT PLEASE READ*** I am really trying to…read morestart this review with something positive that I experienced through out my family members stay and only person I can think of is social worker Shannon and nurse Magdline (probably spelling it wrong). Honestly this heck of a place DO NOT deserve them. First week my family member was left in a recliner alone with no protection belt or recliner alarm (don't think they have it but if they do I've never seen it). When the fall incident happened I got a call from the male charge nurse who said she has "probably been on the floor for over 20-30 mins". Also said they take patients for a CT scan after a fall. That never happened. My family member complained to me that she is constipated and it's hurting. I expressed this to the staff BUT I was ignored and ofcourse they had the worse hemmoroid situation anyone could imagine. I personally had to go to Walgreens and got things to get her comfortable. Ofcourse afterwards they started her on stool softeners. Well let's not forget bed sores. She had so many bed sores that this establishment should be embarrassed of their existence. During all this going on my family member looked worse then when she came there from hospital. Had rash all over, back burning hot, confused, and many other things. They said it must be the soap they are using during bath!!!!!! I had to raise hel! and finally on 11th day of her having all these symptoms very ignorant short curly haired charge nurse called the doctor and got lab work done. Guess what? She had a UTI. WHENEVER I discuss her symptoms to anyone outside of this facility mostly medical professional's before I even finish my sentence they know it was a UTI but how in the heck this facility didn't???! After that she got 3 more UTI's at this facility. A day before her discharge from this heckhole I asked them I need it in black and white (in email) that I need to see if she is negative of UTI Ofcourse I got a call from a nasty nasty person going over her negative results but nothing in "black and white". Ofcourse when she arrived at the physical therapy inpatient rehab she tested positive for UTI. Made her stay from 12 days to 29 days. I know this review is long but please please please take your time and do some research and DONOT bring your worst enemy here unless you want them to suffer inhumanely manner. Feel free to reach out to me on any social media platform I could go in details even more.

The leadership and management fosters an environment of abuse/work place violence. Once the patterned abuses/violence was reported, they blamed the wrong person and terminated two people who were the victims of the violence. They are always under staffed and the nursing staff likes to sit and not actually do patient care. That is an unsafe environment for patients due to nurse patient ratios. Leadership is not much to be desired. They are lazy and do not help their so called team. A new nurse can learn good skills but the they boast of paying competitively but the reality was, they under pay even the national median hourly pay. Working at western peaks, the LTAC behind south Davis community hospital was my absolute worst employer. I do not recommend them.

They performed a nasal suction by putting a tube up our 8 month old's nose down her throat and completely traumatized her (though we thought this was the right procedure at the time.) We were sent home being informed she simply had a head cold virus. The following day and night her symptoms only got worse, we took her to a different instacare where she received 3 epinephrine treatments and we were taken in an ambulance to Primary Children's where our baby was hospitalized overnight for croup. This all could have been prevented had she had been properly diagnosed the night prior. The reason for this review: we attempted to dispute the bill due to misdiagnosis and were denied because this hospital provided "proper treatment." Had we had listened to the doctor's advice who provided care, our baby would have stopped breathing the night following. Do better, Lakeview Hospital.

Talked to the social worker there about her care and they said that they advised if she was still showing signs of mania to go to the nearest hospital. The thing is, we already did that and then she was admitted to THIS hospital where they absolutely did not provide her with any actual care. It felt like I had one impossible chance of her finally getting help and then no one did anything to help her or my family. I am heart broken and hope that no one suffers the same way as me or my family because of the lack of care put in by the providers here. Shame on them

We mostly saw APRN Lindsey Martinez who is kind and seemed to be a good listener, but after a nurse in training who was shadowing Miss Martinez said something about moms prescription being too large of a dose, APRN Martinez had to stop, go over moms medication notes and indeed admitted "oh yes, that is a mistake" then adjusted the prescription. It was not very reassuring and to be honest I am thankful for that shadowing nurse. Whoever she was she was a godsend because after that visit I began taking notes of our interactions. Researching mom's meds, so I could be informed. On one visit I questioned why mom was on Metformin and Metoprolol when she only has high blood pressure not high blood sugar and if we could eventually wean mom off of all 4 of them when she gets her blood pressure under control. I was told some BS about mom needing to be on all of them. I can call it BS because the new doctor I found closer to our new home questioned why mom was on those exact blood sugar medications and said he would like to take her off them on our first visit with him. He kept her on the two blood pressure meds and I am tracking her progress for our next visit. There was also a lot of confusion with scheduling. When mom had been in last and what she had been in for. I had to tell them what she had done on the last visit from my notes. They would give my mom cortisone shots for her bad knee whenever mom would ask for it. Mind you mom has early signs of short term memory loss, so she wouldn't remember when her last visit was. Only that her knee was in pain and that the shot helped. Once they gave her 2 injections in one month. That was the last straw because from what I researched you should not have another cortisone shot for at least 3 months from your last cortisone injection. It can cause more damage than good. Our new doctor confirmed that those injections should only be given 3 times a year. 4 at the most. Do your due diligence and find someone you connect with and listens to you. You have to advocate for yourself and your loved one's. I'm glad I finally found a doctor I believe genuinely cares about helping people and listens to their needs.

I went to pick up several prescriptions for several family members. When I looked through the…read moremedicines, I found one that shouldn't have been filled yet; and it was only for 30 days rather than 90 days. I called to resolve. I made the mistake of saying the meds keep getting f***ed up. The ire was not directed at anyone. He hung up on me. Ironically, I actually thought it may have been another computer error or glitch. That was what I kept being told every time it happened. I was thrown off checking on this one because I still had 1.5 month supply. The med shouldn't even have been on the agenda. I call again, and when I finally am passed on to the pharmacist, his immediate comment was if I was ready to be civil. There are a lot of nuances to being civil. Patronizing and talking over someone are very uncivil acts, which is exactly what the pharmacist gave me. He finally told me I could get a refund for the medication. When I asked him how that affected the prescription, I did not receive clarity and was still confused on that point. I go to the store to return the medication. After having the medication refunded, they gifted the 30 day supply due to legalities. Now, I don't really care about the "free" 30 day. I care about the 90 day prescription and the refills. This is important to me because our family insurance requires us to pay the deductible before they cover things, which is $20,000. We have so much cost in medical, it is actually a lot cheaper but does require budgeting. If the prescription was invalidated or shortened the number of refills or changed the amount from 90 days to 30 days (which they have already done previously), I have to schedule another appointment with the specialist which is a couple hundred dollars that is not budgeted. When, yet again, I ask about the prescription ramifications, I'm told that the refund was done, and here are your free pills because we can't take them back. My question again shut down and no answer. At this point I'm done with this pharmacy. I call another pharmacy and start the process of transferring all our family prescriptions. Four days later, I go to the new pharmacy to check that everything is in order. I find that of the 6 accounts with multiple prescriptions requested to transfer, only one prescription was transferred from one account. When the new pharmacist calls the previous pharmacist line for clarification and cooperative resolution, the technician states the pharmacist is unavailable, something about transferring everything and then hanging up without dialog. I would not recommend this pharmacy to anyone. The chronic mishandling of prescriptions was very painful. The dismissive and escalating nature of the pharmacist was beyond the measure of civility I could tolerate.

The total price before tax is 16.99. To get only two physical copy at USPS, it's only $15. So I decided to go to Walgreens instead of USPS because of the digital copy. However, based on my personal experience, walgreens do not provide a cut 2 x 2 passport size digital copy. Instead, they provide you the Original uncut photo and you have to cut it to 2 by 2 in an app by yourself. I talked to the staff at the store, they said the system automatically emails the digital version and they have no control over which picture (cut vs. uncut) is sent to Customer's email. They also said it happened to other customers as well. The issue was eventually resolved, but it wasted me 30 minutes to go to the store for the second time to get the cut digital version. The email for the digital version was not sent to me until a couple of minutes later after I pay. If you want to make sure you get the correct cut 2 x 2 passport digital version via email, it's better to wait for the email before you leave so that you don't have to make an extra trip. Below is what says on Walgreens website: Will I receive a digital copy of my passport photo? Yes, in addition to the two 2x2 photo passport prints, you will be given the option to receive a digital copy via email of your passport photo at no extra charge. A digital photo copy on a USB drive is also offered for an additional fee.
