My experience with this practice was fairly positive at first, but it ultimately became one of the…read moreworst dental experiences I've ever had. I initially came in because I had chipped one of my implant crowns and wanted both front implant crowns remade. Dr. Robert Swanson was very casual about the chip and even suggested simply leaving it as it was. I wanted it repaired properly, and he assured me it would be "no problem." He also told me he worked with a great lab and even stated that the lab was "the most expensive in town."... When it came time to remove my original cemented crowns, Dr. Swanson seemed unsure. I even asked how many times he had removed cemented implant crowns before, and he replied, "Not a lot." Despite that, the temporary crowns they placed were excellent. They fit comfortably, looked natural, and I almost forgot they were temporary. The problems began with the lab visit. Dr. Swanson told me I would work directly with the lab to customize the appearance of my new crowns. Instead, after waiting more than 30 minutes past my appointment despite being the only patient there, I was briefly color-matched and sent on my way with very little discussion. When I returned to preview the finished crowns, they were missing the gum porcelain entirely and were much larger than expected. I questioned the lab because they had color matched my gums during my first visit. After stating it wasn't in the work order, they acknowledged the issue and told me to give them another week to correct it, this was Friday. Surprisingly, by Monday morning, Premier Family Dental called to say the crowns were already finished and ready for placement. That timeline immediately seemed unusual. When I saw the revised crowns, they were extremely bulky. They felt unnatural, interfered with my tongue and bite, and the tooth/gum colors did not match at all. I expressed my concerns before they were permanently placed, but I was told my jaw just needed time to adjust and that I was "hyper fixated" on them. Unfortunately, things only became worse. Within days I developed severe jaw clicking, popping, and discomfort. Over the next two weeks, I returned FOUR separate times for adjustments. Each visit involved grinding the crowns down further until the undersides became uncomfortable yet the problems remained. When I continued asking for the crowns to be remade, Dr. Swanson refused and even tried to tell me I needed a mouth guard for sleeping. During our final conversation, he told me he would not remake them and stated that no one in the practice had the ability or skill set to correct the issue. The practice ultimately refunded the ENTIRE procedure, acknowledging that the crowns would need to be redone elsewhere even though Swanson told me the only way I would get a refund is if I could prove to him I was going to get them re done, which was EXTREMELY unprofessional and unethical to assume I was dining and dashing when they failed me and did terrible work. Since the new crowns were put in I have sought two independent second opinions from other dental providers. Both offices examined the crowns and independently concluded that they were poorly made and would need to be completely remade rather than simply adjusted. Hearing the same conclusion from two separate providers confirmed that the problems were not simply a matter of "getting used to them," but that the crowns themselves were not acceptable and required replacement. As a result of this experience, I am now starting over with another provider and am receiving treatment for significant TMJ symptoms that developed after these crowns were placed. I also received a patient relationship termination letter after requesting that the work be corrected. I would NEVER recommend this office or the work of Robert Swanson based on my experience. What should have been a straightforward replacement of two implant crowns resulted in repeated adjustments, a complete refund, ongoing jaw problems, a very trusting relationship tarnished and the need to have the entire treatment redone elsewhere.

First impressions matter, and the front desk completely ruined the experience for me. What stood out most was how she spoke to a minor about her healthcare card, insisting she should have it on her phone despite clearly being a child, and making inappropriate comments suggesting there were 'other things she probably has on her phone she shouldn't.' That kind of remark was unnecessary, disrespectful, and completely out of line. I understand offices can get busy and stressful, but that is no excuse for being dismissive or rude to patients, especially a minor. Overall, the lack of basic patience and professionalism at the front desk left a very negative impression.

Needless to say it was very painful, the topical numbing gel didn't do what I thought it would, and I vowed to NEVER do it again. So, four years later, I muster up the courage to give it a try again... This past Wednesday I visited In Shape MD, and I spoke with Dr. Shannon York about my concerns with lip injections and the last experience I had. Dr. York was understanding and very sensitive to my concerns and based on my history she suggested I get a dental block to numb the entire site. I agreed to give it a try again and as you know if you've been to a dentist and had a dental block, you CAN'T feel a thing. I spent a good 20 minutes in the chair with Dr. York as she was administering a dental block. Dr. York was very compassionate and extremely gentle in administering the shots. Given my history she wanted to make sure I was as comfortable as could be at all times and she took her time. After the dental block set in she started administering the lip injections, and I was so pleased, because I could not feel a thing!!! The procedure took about 45 minutes and by that time the dental block was starting to break -- thank goodness, because I did not want to walk around for hours with a numb face. Within two hours, I had complete feeling again and very minimal discomfort around the injection site. I would highly recommend visiting In Shape MD if you're considering any type of non-invasive cosmetic procedures, because Dr. York is a wonderful and very compassionate. It's been about a week since my lip injections, and I couldn't be happier with the look. The swelling is pretty much 100% gone and my lips look great. I am looking forward to visiting In Shape MD again for other cosmetic procedures. :P

I have filed a complaint against Dr. Smutko with the Arizona State Board of Dental Examiners. On February 11, Dr. Smutko performed a tooth extraction, bone graft and placed a membrane due to a sinus perforation evidenced on a scan my dentist provided. Dr. Smutko said I had an infection and that she had cleared it. On 2/13, I called to ask if the very considerable pain I felt was normal. On 2/14, I asked if an antibiotic might be helpful given that there was an infection. She ASSURED me I did not have an infection and that everything was fine, She said that an antibiotic would "only serve as a placebo."   On  2/18, I complained that I could not manage the severe pain with medication. She lectured me that it had been a "simple procedure," which completely contradicted her earlier statement that it had been a "complicated procedure" due to the tooth's proximity to the sinus cavity. " On 2/21, I called and asked for an appointment. On 2/25, again, I questioned the possible need for an antibiotic. I didn't know if I still had an infection but I did know that there was a serious problem. I had spoken to another oral surgeon who said I should have been put on an antibiotic immediately following the procedure. She coldly dismissed me. I asked if she didn't think it was possible that there could be a problem that is not visible to her eye.  She flatly said "no" but reluctantly prescribed an antibiotic. After taking only 4 pills over a 24 hour period, my pain had decreased substantially.   Unfortunately, after a few days, the pain slowly returned, now accompanied by the strong smell of sulfur (research will point to an infection in that instance.) I asked her if it was possible that the antibiotic which seemed very helpful initially, wasn't strong enough and I told her that now I was having to cancel my own clients for my aesthetics business I have owned for over 30 years and losing a considerable amount of income due to this situation. She completely dismissed me, actually rolled her eyes and left the room. I told one of her assistants that I was never returning due to Dr. Smutko's shockingly cold, dismissive and unprofessional behavior. She said that she understood, apologized and told me that "Dr. Smutko believes the body should be able to heal itself." I suppose that explains why the doctor coldly dismissed every question I had. She never took a single CT to see if there was actually an issue causing weeks of pain and suffering. She acts as though she is superior to her patients that their opinions or discomfort are of no merit or importance to her at all. I decided to get a second opinion from a new oral surgeon. On March 4, I went to see Dr. Nelson, for a second opinion. He immediately did a 3D CT scan and determined that there was a substantial issue, either mucus, infection or something in my left sinus cavity, which he called Opacified and Pneumatized left maxillary sinus, which likely explained why I was smelling sulfur and experiencing severe pain. He also checked that the bone graft wasn't contaminated, something Dr. Smutko didn't consider.    On March 6, Dr. Nelson performed a procedure to clear out whatever it was evidenced in the CT. Immediately following his procedure, I no longer could smell sulfur and miraculously felt no pain for the first time in weeks. Dr. Smutko had refused to consider, accept or scan for a problem. While under her care, I was left to suffer for weeks and had to cancel approximately $7,000 dollars of aesthetic procedures income I had on my books. Two different oral surgeons said it was negligent for Dr. Smutko to not to run a single scan for weeks following constant complaints after a dental procedure. Just one scan, less than 10 minutes and she would have had all the evidence she claimed she didn't have, to identify and treat my problem. Dr. Smutko is responsible for the worst medical experience I've ever had.

At last, a Dr. who really cares about a patient. It's incredibly difficult to plan around, especially when you think you have something confirmed and then it suddenly disappears. The lack of consistency and communication makes it hard to trust their process. One day an appointment is scheduled, the next day it's gone, with little to no notice or reasoning provided. For a service that people rely on for their health, this level of disorganization is disappointing. They texted me the new patient forms--which were super easy to read and complete on my phone--and stayed in contact, answering all my questions right away.
