This review of the facility is based on my experience as a staff nurse in 2019. My review will detail how I saw the resident of the facility being treated and the obstacles the staff had to deal with. Also, please note that since this review is based on my 2019 experience so there may be changes that have improve the facility since then or worsen due to the Covid-19 pandemic. This facility operate as a nursing home/rehab place. Upon entering the building there is a fireplace on the left, the cafeteria on the right, and in the middle is big bird cage filled with birds for the resident entertainment. There is four units with three unit in the top floor and one unit on the basement floor. I work on the basement unit and the name was Terrance. On the unit we have 17 bedroom that is shared so a max of 34 residents. For staff, this place is a 5 day a week for 8 hour work place. Work hours are 7am-3pm, 3pm-11pm, and 11pm-7am. To clock in you need a ID# type in and to scan your fingerprint which sometime doesn't accept the first time which can be an annoyance. The staffing ratio was horrible, on the ground floor unit it was 1 nurse to 17 residents and 1 CNA to assist you. The ratio was so bad how do find the time to do assessment, cleaning, feeding, passing meds, charting? Another thing that annoyed me as a staff worker was when trying to pull narcotic for the patient from the Pyxis you needed a second nurse but it was always hard to find someone because it was always busy and understaffed. I also witness a manager who would try to provoke a dementia patient by pretending to swing his fist at him. Lastly, another bad thing about working here as a nurse is that the facility doesn't use an insulin scale to determine how much insulin to give a resident who has a high blood sugar. You have to call the doctor every time and they will tell you how much insulin to give. I had a resident who blood sugar everyday was over 300 so I literally had to call the doctor every night to determine how much Insulin to give. Resident have a call light they can press to call the nurse and at the nursing station it ring very annoying like a very loud buzzing. For the resident they have events like music time and activities they can do. They get daily lab draw and if they need like an X-ray an outside company come to the facilities and does them. The building has a back yard type area where people can smoke. I have seen confuse resident be neglected and left outside in the hallway.They have an area in the basement where therapist come and do rehab with the patients. Resident are free to leave and come back. I worked at this place as a new graduate nurse and I would not recommend any RN come here to work at this place due to bad management & the nurse to patient ratio. This place was so bad that at some point during my time working here after finishing my med pass & charting, I would look for a new job and I was able to find a job at a hospital where it is 10x better for a new graduate nurse. Some of the coworker that I work with were so happy for me when I put in my 2 week resignation. All in all if you love your family member that I highly recommend you don't send them here.

My father resided at this nursing facility in Collingswood for over two years. Unfortunately, after…read morehe was hospitalized and required a feeding tube, I informed the facility that he would not be returning. I had a great rapport with many members of the nursing staff and encountered some genuinely caring and compassionate employees. However, I found that consistency in his care was a major concern, and I often felt that I had to constantly advocate for my father to ensure that his basic needs were being met. One of my biggest concerns was his hydration. My father became dehydrated multiple times and was repeatedly hospitalized because of dehydration. There were occasions when I had to travel to the facility myself to help feed and hydrate him. This was extremely concerning and frustrating as a family member. I also had ongoing concerns regarding his personal care and dignity. I purchased clothing for my father and clearly labeled it with his name, yet his clothes frequently went missing. Despite the fact that he was unable to walk, there were occasions when he was left in the common area wearing only a diaper, sometimes with parts of his body exposed. I repeatedly spoke with the nurse supervisor about making sure he was properly dressed. Unfortunately, I did not feel that my concerns were consistently addressed. At times, the clothing provided to him was either dirty, worn out, or not appropriate for him. I was also concerned about how frequently my father was repositioned while in bed. He developed multiple bed wounds, and I felt that more attention should have been given to regularly turning and repositioning him. Another major issue was communication with the social worker. When I was trying to have my father transferred to another facility, my emails were repeatedly ignored. I found this lack of communication extremely unprofessional, especially when I was trying to make decisions regarding my father's care. Additionally, my father's wheelchair was given to another resident, and staff were unable to tell me where it was. This was another frustrating experience, particularly because it was his personal equipment and an important part of his mobility and care. I understand that every nursing facility has challenges, and I recognize that there were staff members who genuinely cared about my father. However, having good individual staff members is not enough when consistency in care is lacking. As a family member, I felt that I constantly had to be present, ask questions, follow up, and advocate for basic needs such as hydration, proper clothing, personal dignity, repositioning, and communication. Ultimately, these experiences played a significant role in our decision that my father would not return to the facility. I hope the facility takes these concerns seriously and works toward improving communication, accountability, consistency of care, and the dignity afforded to its residents.

Terrible, after a leg amputation, my 80 year old friend was there for 9 hours before receiving pain…read moremedication. During this time the bandage fell of amputation, she had a painful bed sore. She was put in a very small hot room, with a broken, loud hvac unit. It took me 45 minutes to reach anyone there. The nursing unit would ring and then a message "there is no one to answer your call"....Total nightmare, she had been in Montcare, Potomac prior, like night and day.

A few specific issues: * He is supposed to receive "rehabilitation" (physical therapy) every weekday. As best we can tell, and from what he reports, it rarely happens. We're going to look into billings to see if they're charging for services not rendered. * We get the runaround whenever we ask about his care. For example, when I ask about him getting "rehabilitation" services I get different responses from different people at different times. Most recently I was told by the charge nurse that it's not her domain so she doesn't know anything about it, that I could talk to the people in that department if I see them, but she doesn't know how I can contact them. She doesn't know a phone number and she doesn't know if they have email. * His prescription glasses have been missing for the past two weeks. The empty case is still in his room. No one on staff knows anything about this. I was told that they don't touch his glasses, only he touches his glasses, they're not even allowed to touch his glasses at all. He's bedridden, so I'm not sure how they could've left the room if only he touched them. * He is never dressed. Every time we visit, no matter what time of day, no matter what day of the week, he is wearing just an undershirt and a diaper. We've bought all new clothes for him, so he has pants, shorts, sweatpants, and sweat-shorts all hanging in his closet. They've gone untouched. * Staff isn't taking him to the bathroom. He has two urinals hanging from his bed. These spill, so the room always smells of urine. Yesterday was the worst it has been. It appears he had to empty a urinal into the trash can next to his bed. *Whenever I suggest contacting staff (to help him to the bathroom, to empty a urinal, to help him make a phone call, to help him turn on or off the TV, etc.) he is very opposed to doing so. He implores me to NOT bother them. He genuinely looks scared. Why I gave 2 stars, not 1... on the positive side: * He gets a fresh cup of water every day. Staff writes the date on the cup each day. * Staff changes the lidocaine patches on his knees each day. They write the date on these too. In summary: We do NOT recommend the Hebrew Home of Greater Washington.

Where do I even begin…read more.. The level of care we've experienced has been deeply disappointing. Nurses frequently sit in the hallways on their phones or chart for long periods, yet little to nothing appears in the patient portal. Calls to the nursing station often go unanswered--even when messages are left. Patients are told to stay in bed due to fall risk, but then made to wait extended periods for assistance to the bathroom. When help doesn't arrive in time, they're then chastised for accidents. That is not care--that's neglect. Communication is vague and frustrating. "We can call the doctor" seems to be a standard response, but that doesn't mean a call is actually made. Doctors stop by infrequently and provide minimal updates. Meanwhile, families are left in the dark. Overall there is a troubling lack of urgency or accountability. Discharge has been unnecessarily difficult to coordinate. Physical therapy itself has been a bright spot, but it's limited to about an hour a day, which doesn't justify the broader experience. If you have a loved one here, be prepared to advocate for them daily. Ask questions, follow up constantly, and don't assume things are being handled behind the scenes. This has not been the level of care we expected or were led to believe we would receive.

Our loved one lived with fronto-temporal dementia requiring significant care and raising a host of challenging balance/falls, communication, and daily caretaking needs. The JK team worked tirelessly to come up with solutions that kept H safe and met her practical needs, while never losing sight of her as a real, present person and working just as hard to connect with her emotional and human needs. We know she felt as loved and cared for at JK as she did when we were providing care at home. From birthday celebrations to holiday traditions to everyday home-cooked family style meals, JK is committed to making a community that includes and supports all residents - and they succeed.
