Our loved one lived with fronto-temporal dementia requiring significant care and raising a host of challenging balance/falls, communication, and daily caretaking needs. The JK team worked tirelessly to come up with solutions that kept H safe and met her practical needs, while never losing sight of her as a real, present person and working just as hard to connect with her emotional and human needs. We know she felt as loved and cared for at JK as she did when we were providing care at home. From birthday celebrations to holiday traditions to everyday home-cooked family style meals, JK is committed to making a community that includes and supports all residents - and they succeed.     I've got a lot to say about this place after my mother spent almost four weeks here for subacute…read morerehab following a stroke. This facility receives $579 PER DAY to care for subacute rehab patients. This is what you can expect for that price.... They are pretty unorganized here. When my mother was brought in, they didn't have a bed ready for her and no one even seemed to know she was coming. The case manager at the facility that my mother came from told me she personally arranged the transfer with Mr. Williamson in the admissions department. I tried to call him to see what happened and he couldn't be bothered to return my voice mails. I toured this facility before deciding to place my mother here. During the tour I wasn't able to go into an actual room because Tiffany (admissions) claimed they were all occupied. This should have been a red flag for me and I wish I had seen the rooms first. There is mold on the sinks and they reek of urine since the bathrooms aren't properly cleaned and the smell wafts into the rest of the room. It's pretty disgusting. We had my mom move into a second room after about a week because her bed was broken. The bathroom in this new room was horrendous and the smell made us gag. There was dried urine all over the outside of the bowl and on the floor. I had to call an aide to have it cleaned. My family was there visiting my mother every single day so the restroom wasn't like that again but I wonder if that would have been the case if we weren't there as frequently as we were, especially since the woman residing alone in that room before my mother got there didn't have the ability to voice anything let alone displeasure over at the restroom. Most of the aides here are pretty terrible. Responses to calls for help can take as long as an hour. One aide would slam drawers and act huffy when she would have to bathe my mother. I don't know that she was ever bathed consistently and they never helped her brush her teeth. They don't watch the residents. My mother came in as a high fall risk and she fell a total of eight times (that we know of. I suspect that number is higher.)They would blame the family for some of these falls saying that we didn't tell them we left. The truth is we did tell them we left. One time I left my mother in the care of a physical therapist and an aide still tried to say it was my fault that she fell because I didn't tell them that I left. And when they blame the family they will actually yell at you over the phone. When you try to talk they talk over you even louder and don't stop. I had particularly heated phone calls with two aides on two separate occasions named Juliana and Mercy. I have never seen such unprofessionalism. My mother is covered in bruises and is lucky she didn't break anything. The staff's solution to the problem (after I called the director of nursing) was to park her at the nursing station and just let her sit there whenever a relative wasn't there to watch her for them. They still weren't watching her though even when she was at the nurses station and she wound up wheeling herself out into the parking lot. She's lucky she wasn't hit by a car. Dining staff seems to have no idea who should be eating what or even where they should be sitting. They are extremely unorganized. They are also pretty rude to the residents. If that's how they treat them when relatives are present I can only imagine how bad it is when there aren't any visitors. Family can dine with their loved ones for the reasonable fee of $5.00. If you're lucky Danine will be at the front desk and will gladly help you purchase your meal. If you're unlucky you'll have to deal with Barbara who is rude and acts bothered to help you. And if it's a minute past 10:30am forget it. Barbara won't even sell you a lunch ticket unless you get permission from the dining staff aides. Also I was concerned about the quality of food. My mother has diabetes and her sugar was running between 220-310 during her stay there. Her trays would frequently have ice cream, juices, and cakes. The equipment here is years old and barely functioning. My mom's bed was broken in the first room she was in. The brakes on the wheelchairs don't always work. We had an issue where the brakes on both side of a chair didn't work. The aides did nothing to help. The nursing unit manager finally got her chair replaced. Then the brakes on one side of the second chair stopped working. I told a total of SIX people (including the nursing unit manager) that the brake on one side of her wheelchair was broken. Three days later it remained unfixed. The therapist (the fourth person I told about the issue) had the solution of simply placing a weight behind the wheel on that side.

    I cannot speak for the rehabilitation part but my grandfather was here for hospice. Though it was…read morenot very long, the staff here had gone out of the way to make sure we were welcomed. This made the process of grief easier for us. I cannot express how grateful we are at seeing how they handled the care of my grandfather. The staff made us felt like family and some even cried with us. I really recommend this facility if you are going the same unfortunate situation. Thank you so much to all the staff members. We really appreciated everything you have done for our family.

The staff is caring, attentive and make sure all of the residents needs are met. Carol makes sure the house is clean, well decorated and has a real home feel. The food is home cooked (and cooked with love and care). The neighborhood is wonderful - safe and with ample space and a beautiful front and back yard. It is a wonderful, safe, loving home. The staff was very professional. It felt more like a family atmosphere than one of those cold impersonal retirement homes. There were just 7 residents when my mother was there. In this time of Covid-19, older persons are especially vulnerable. Carol made sure that all precautions were taken for a safe environment. I could only see my mother through a glass partition. Near the end, my mother needed hospice care. Once again, Carol came through and recommend an in visit hospice company. This way , my mother could stay at Rejuvenation House and not be transferred to a new location which am sure would have frighten my 98 year old mother. My mother passed away recently. I am sure that she was able to live longer due to the excellent care she received at Rejuvenation House. The rates are very reasonable. I would highly recommend this facility for your loved ones.
