    They are way under staffed to be able to properly care for the amount of patients they take in! My…read moremother called me while I was at work crying because she said no one was helping her. She rang the buzzer 20 minutes before she called me. She recently had a hip replacement surgery and could not get up without assistance. She had to use the restroom and ended up soiling herself because it was at least ONE HOUR before an aide came to assist her. I got there 45 minutes after her calling me and the aide was finally there helping her! I got a little loud and said how would you like it if you had to wait that long and don't you think it would make your job easier if you didn't make her wait!? Now you have to clean up sh*t! I also was told that 6 aides will be on the schedule to work a shift and they are lucky if 3 of them show up to work! I would never recommend this place because of this. I will say that the nurses are compassionate.

I have gone there at different time of the day, different shifts, different days of the week the experience has always been the same, fantastic . Mom needed skilled care and both the PT department as well as OT were extremely helpful. The professional team Stephen, Sophia and Brinden are second to none . Nothing can be done without the relentless efforts of the social worker Melissa Conrad . I can not thank her enough . JOHN the MDS was very helpful with insurance and the appeal process. The admission Director Rachel Holliday made the admissions process so easy . The place is always clean with no odor . Thanks to Dee and others for doing a great job. This says a lot about Housekeeping Supervisor Delia Diaz. The care from the LPN like Tania and other was greatly appreciated . This is due to the leadership of Director of Nursing Jennifer Mounts . The NP Rachel from Cleveland Clinic with Dr Vacca team we're open minded and willing to listen . The Nutrition Dept under the leadership of Traci with her team of cooks like Lynn and aids like Meagan offer great, tasty meals . Better than 5 stars restaurants . They even brought a live singer Chris for activity time .Thank you Angela for doing a great job in activity . This 75 bed skilled nursing and rehabilitation center in in the heart of Avon, Ohio. You can schedule a tour. If your looking for services like Chronic Disease Management, PT OR OT, Audiology Services, complex wound care, skilled nursing , short term rehabilitation , long term care, respite stay , hospice care, and much much more, I recommend The Woods hands down . It takes great leadership to run a team and the Administrator Mr. Alec Walker is Probably the best I have ever experienced because my visits every single time had the same outcome; GREAT

It is always…read morean easy process to pick up my meds and although there is usually a line, it goes pretty quickly. It is very clean inside and easy to find. The pharmacy is to the right side when you walk in and there is always someone at the entrance of the hospital to help direct or assist you. Also, the pharmacists are always friendly, helpful, and quick. I usually go in, but they also have a curbside pick-up option as well.

Jacob's Health Center's Emergency Room Department since…read more2:00p it's currently 10:45p and all they've done for my dad who came in struggling with heart pain and chest pressure also with extreme pain in his lower extremities is take blood, a ECG at triage and took him out of the waiting room to do x-rays and then brought him right back to the waiting room! We have been siting in this waiting room for over 8 hours with no test results, no one checking in on his chest pains and when my family or any of these other families waiting for help ask about how much longer themselves or their loved ones have to wait to be seen the nurses and receptionist just say (with a heightened volume and annoyance to their voice is... "you're on the list" and that "we don't know wait times" 8+ hours to wait to be seen and left in a waiting room when you came in with heart an chest pain is unexplainable for my father and the countless others that are in pain and just wanted some preview of when they'll be able to be seen and have some relief! Do better Cleveland Clinic and have your front line crew members show a little more empathy for people that have spent the equivalent to a full workday waiting in a lobby to get relief for their ailments. Also at the time of writing this we still have yet to be seen and are about to enter hour 9.

Having once been in the seminary and a pastoral minister, I've been in and out of a lot of nursing home senior living facilities. This place is tops! Really! All the staff are extraordinarily kind, warm, patient (no pun intended) and attentive to the needs of the residents. My mom couldn't have been better cared for. They always looked in on her to make sure she was alright. They treated mom (Marie) with dignity and respect, patience and kindness. Although once sponsored by the Lutheran Church, they have a very Catholic presence. The Diocese of Cleveland provides a priest to offer Mass there every Sunday, and they have a wonderful sister (nun) from Africa who is the coordinator. When mom died, I gave her whatever she wanted from her room. The food seemed adequate. But the most important thing was the atmosphere of real care and concern. It's nothing short of remarkable.

There are a lot of comfy common areas, complete with plenty of space to watch TV, listen to music, play piano, or read a book. They have birds in a large enclosure, a small aquarium, and greenhouse as well. They even provide a lot of enrichment for the patients, like live music, buffets, shopping, and exercise. The cleanliness of her bathroom (and overall of the home) was hotel-like in nature and didn't feel like (or even smell) like your typical old folks home. Overall, I would trust and recommend Lutheran home to anyone needing to stay here for a short term or long term stay.

What is presented during tours and brief visits does not consistently reflect how this building operates on a daily basis. When state surveyors or outside officials are present, everything suddenly functions the way it should. Call lights are answered quickly. Staff are attentive and engaged. Documentation is completed properly. Residents are closely monitored. It is obvious that management understands the standards required for safe and professional care. The concerning part is what happens when that oversight leaves. Staffing levels are frequently inadequate, especially during evenings and overnight shifts. Residents who require close supervision, including those at high risk for falls, are not always monitored appropriately. Response times can be delayed. Situations that should receive immediate attention sometimes do not. Families trust that their loved ones will be consistently supervised -- not only when inspections are occurring. There appears to be a significant lack of accountability within leadership. Concerns raised by staff often go unresolved. Employees are stretched thin, expected to manage unrealistic workloads, and provided minimal support. When workers are overwhelmed and morale is low, resident care inevitably suffers. Instead of addressing systemic issues, problems are often temporarily corrected only when outside oversight is expected. Professionalism inside the building is inconsistent. Staff members are frequently observed distracted by personal phones instead of actively supervising residents. There is not always a sense of urgency when residents require assistance. A long-term care facility should operate with attentiveness, compassion, and professionalism at all times -- not selectively. The activity department frequently operates with unsafe staffing ratios, at times leaving a single activity assistant responsible for supervising a large group of residents alone. On multiple Fridays in particular, only one staff member has been assigned to manage numerous residents without additional support. Residents who require close monitoring have been left without adequate supervision, creating an unsafe environment. Expecting one individual to oversee that many vulnerable residents is not only unrealistic, it places both the residents and the staff member in a difficult and potentially harmful situation. What is equally concerning is that members of management have been observed walking past these situations without offering assistance or intervention. This reflects a serious lack of leadership and concern for resident safety. The condition of parts of the building raises additional concerns. Some resident rooms show visible wear and maintenance issues that do not reflect the level of care families expect when paying for professional services. The physical environment should reflect dignity and safety. Perhaps most troubling is the inconsistency. This facility has demonstrated that it knows how to operate correctly -- because it does so when being observed. That means the standards are achievable. The issue is that those standards are not consistently maintained when oversight is absent. Placing a loved one in long-term care is one of the most difficult decisions a family can make. It requires trust in leadership, transparency in operations, and confidence that residents are being monitored and respected at all times. Based on repeated observations, families should ask very direct questions about staffing ratios, overnight supervision, response times, and accountability before making a decision. I worked in a jail for five years, and I say this with full sincerity: the conditions here are no better. Shortly after I moved back to the area, the administrator, Nikki, asked me to remove a negative review I had posted. I agreed in hopes that things would improve--but they didn't. In fact, I've seen more problems since. Some of the issues we've encountered: * My mother frequently misses scheduled showers--often multiple times a month. * She has been forgotten at mealtime on more than one occasion. * The air conditioning system is unreliable, creating uncomfortable and unsafe conditions. * My mother has had a broken tooth for months, and care has been delayed. * One night, she was coughing so badly her lips turned blue. The nurse on duty, Sonia, delayed calling 911 because she "had to finish paperwork." A staff member eventually urged us to call 911 ourselves. * Despite needing a Hoyer lift, a staff member improperly moved her, resulting in injury. * I've personally found dirty diapers discarded on the floor of her room. * She is often not assisted with tooth brushing or basic hygiene. * Many residents go days without being showered or changed, blamed on "short staffing." * Transportation to medical appointments is often missed due to poor coordination. * Staff often gaslight residents, dismissing their concerns or blaming them. There are smiles when you walk in, but walk a little farther--look at the peeling paint, broken equipment, and unattended patients. Families must be vigilant. No one should have to fight this hard for basic care.
