Our family had a very disappointing experience with this rehabilitation center while our father was…read morea patient. The lack of communication, poor coordination, and unprofessional behavior from the supervisor made an already stressful situation unnecessarily difficult for our family. My mother had spoken directly with the doctor the day before and was told that my father could go home the following day. However, when it came time to discharge him, the supervisor repeatedly insisted that they could not release him. We were left dealing with conflicting information and unnecessary frustration, with little clear communication about what was actually happening. To make matters worse, we were told that if my father remained at the facility, he would no longer be receiving physical therapy or occupational therapy. Essentially, he would simply be sitting there without receiving the services that justified his stay. Despite this, the supervisor continued to ask whether he was going home or staying, rather than professionally working with the team to resolve the situation. To her credit, the case manager was genuinely trying to find a solution. Unfortunately, the supervisor's behavior was particularly troubling. She came across as rude, confrontational, and completely unprofessional, even appearing to argue with the case manager in front of our family. This was unacceptable conduct, especially in a healthcare environment where patients and their families are already under considerable stress. The one positive aspect of our experience was Julio, the physical therapist. He demonstrated genuine care and compassion toward our father and our family. Although his ability to resolve the administrative problems was limited, his empathy and professionalism made a meaningful difference. He showed the kind of human decency and concern that we expected from the entire facility. A rehabilitation center should provide more than physical therapy. Families deserve clear communication, coordinated discharge planning, respectful treatment, and staff who work together in the patient's best interest. Unfortunately, our experience fell far short of these expectations. Management needs to take a serious look at how communication, discharge procedures, and staff professionalism are handled at this facility. No family should have to navigate confusion, conflicting messages, and confrontational behavior while trying to ensure their loved one receives appropriate care and gets home safely. I sincerely appreciate Julio for the compassion he showed, but one caring employee cannot make up for such a frustrating and disappointing overall experience.

The Springs Health & Rehabilitation Center - Negligence.. negligence... & more negligence! Mother was ambulanced that night thanks to me showing up! Not because she was being watched!

The first day there included a care conference with family, case manager, social worker, nurses, aides, physical therapist and Dr. Rolando Gutierrez. The plan of care was created after careful listening by the doctor and the staff to patient/family concerns. The plan was then implemented consistently over his 6 week stay. The nurses and case manager maintained necessary updates while also responding to my calls in real time. They were all professional and thoughtful. The facility itself is modern, clean and has large patient rooms. The physical therapy room is also large, modern and welcoming. There is a place for patients to safely sit outside. We feel fortunate in having the recovery phase in this skilled nursing facility. When we really needed help the staff who work in this modern facility came through beautifully. As a family, we were lucky to have found an excellent assisted living/care home for our dad. For…read morethe year and a half that dad was there, the workers and entire staff treated dad with the utmost love, care and dignity. It was a loving home and they enabled dad to live his best life well beyond 100 years of age. The staff always kept dad's loved one's informed and often asked if they could do more for him to keep him entertained. The in-home staff collaborated very well with the hospice staff. The Bristol hospice professionals were excellent in their own right. I would, with out a doubt, recommend this home to anyone. Unfortunately, I cannot leave this review without voicing my complete and total displeasure with Unikka Wil, the owner and head nurse at Always Heartfelt Homes. There are a few small items that I take issue with and I have let them go. Fine! One item, I cannot overlook... My dad died 3 months ago, still to this day I have not received any condolences from Unikka Wil. There is absolutely no reason what so ever for this massive lack of compassion to take place! Every invoice was paid on time with no issue. Every communication with our family was, respectful, cordial and professional. Still to this day I try to rectify this wrong in my head. My dad was a good man and a great resident, who was beloved by all of the staff. Many of the staff attended his funeral, not Unikka! I just don't get it. It's all very sad and horrible bedside manner on the part of Unikka Wil. Unikka, what ever you decide to do moving forward, let this be a lesson that you are able to live down. Make yourself better, rise above and give the dignity to the families who have lost their loved one's. Reach out and provide personal condolences and offer a shoulder to cry on. It's horrible to think that I have to tell an experienced care provider how to humanisticly conduct themselves in a compassionate manner. I hope you learn from this massive oversight and become a better person!

The owner and operator, Unikka, is not an invisible name. She is present overseeing, interacting, and supervising her hand selected staff. Situated on a sleepy country road just outside of old town, the facility is more like a large home. The kitchen is very visible and spotlessly kept. A large sitting room (family) greets you, bright and cheery. Having been in many assisted livings and nursing facilities, the demeanor and loving staff mark this as an exceptional rest for those in rehabilitation or needing consistent care and encouragement. They always receive visitors with grace and welcoming arms. Blessings and thanks for being above the requirements. Bravo!

We then had caregivers from this Brightstar office for 5 months and had an excellent experience. Upon discharge from a month long hospital stay, my mother was too weak to pull a blanket over her own head. She needed help with all basic activities of daily living: getting out of bed to the bedside commode, eating, dressing, bathing, assistance with oxygen, range of motion exercise and more. Brightstar Central Western Riverside's caregivers were 100% compassionate. All were well trained and transferred my family member safely. Mom even had a fainting incident and our caregiver Dania acted quickly to catch my mom and safely care for her until the family could get to the room to help. 100% score on safety and training. What was extra and unexpected to us as a Christian family: every single one of our Brightstar caregivers participated with us in family worship and were open in their expression of faith. This was very comforting in our time of need. When I wasn't around to have worship or pray with my mom, Wendy would. Our caregivers that stayed with us consistently through the end of our need were Wendy, Christina, Diana and Dania. These women cared for my mom like family. I had zero worries when these trusted caregivers were present. We also had great long term experiences with Brittany, Michelle, Asana and Margaret. From day one, special encouragement from Christina and Michelle helped get my mom moving and stronger with exercises each day. Brittany helped get my mom's brainwaves to function with writing exercises and puzzles. These women brought positivity into our home and inspired my mom to heal faster. (Hint: Christina also cooks excellent vegetarian dishes with an asian-inspired flare!). Wendy (a skilled CNA) was my mom's daily companion and was literally her extra hands. She would have thoughtful, God-filled conversations with my mom. She'd participate in her favorite hobby, card-making, and would help my mom meticulously complete each step. Wendy would go above and beyond the call of duty to be sure my mom was physically and emotionally supported. At night, Diana responsibly watched over my mom and our whole house. She cared for our home as if it were her own. I could sleep soundly knowing my mom was in her care. We had to request special assistance from two caregivers so my mom could attend my dad's funeral. Wendy and Diana worked as a team to be sure my mom was transported safely and cared for. This helped us as a family grieve and mourn. I can go on, but the bottom line is we loved our Brightstar caregivers. They will always be welcome by our family. I highly recommend hiring Brent's team at Brightstar Care Central Western Riverside.

He was pressing the call button because he was struggling to breathe, and no one came. By the time staff finally checked on him, he was no longer breathing. No family should ever have to live with that kind of nightmare. This facility was consistently inattentive and neglectful. Why are the employees constantly on their phones instead of monitoring patients? The atmosphere is dingy, depressing, and uncaring. Staff walk around with attitudes like the residents are a burden instead of human beings who need compassion and medical attention. If you love your family member, think very carefully before trusting them to this place. People in these facilities are vulnerable and depend on staff to respond quickly in emergencies. In our experience, that did not happen here, and the consequences were irreversible. *This review is a warning to other families: do your research, ask hard questions, and don't ignore the red flags. I wish I read the yelp reviews before considering this hell hole.

My brother and I went to five different assisted and Memory Care Facilities around the Temecula Murrieta area, and all of their memory cares look like zombieland. Very few caregivers, everybody sitting around doing nothing, no music, no one doing activities no laughing and very very depressing. And then we found Vineyard Ranch. The facility is beautiful and very well-kept. When we walked into the memory care, everybody was out of their rooms and doing something. People were laughing, talking activities were going on everywhere, and staff were visible throughout the rooms. Completely different environment and that made us very, very happy. She has been there for 3 weeks now, and she doesn't mention going home at all. She tells everyone this is her home and that she loves it there. She's always doing an activity and keeping busy. She's made a lot of friends. The food is good and she finishes all of her meals. Staff are always there to keep an eye on everyone. After any exercise, the staff brings everyone protein smoothies. Every Thursday they have an excursion to someplace fun and stimulating. At a time when dealing with a situation like this, it can be extremely stressful and create feelings of guilt.
