The plan of care was created after careful listening by the doctor and the staff to patient/family concerns. The plan was then implemented consistently over his 6 week stay. The nurses and case manager maintained necessary updates while also responding to my calls in real time. They were all professional and thoughtful. The facility itself is modern, clean and has large patient rooms. The physical therapy room is also large, modern and welcoming. There is a place for patients to safely sit outside. We feel fortunate in having the recovery phase in this skilled nursing facility. When we really needed help the staff who work in this modern facility came through beautifully. He was released after 2 wks, unable to walk & in tremendous pain. When he requested 911 to be called called staff manhandling error, they ignored him. They do not wash their hands from room to room, and due to this dismal like of basic hygiene, he now has MSRA. I just called Ryan @ the Springs the only Mgr that is proactive, and alerted him to MSRA is in the wild @ Springs. The food is processed & horrible. Most of the CNAs are untrained, and this is a cash cow for the greedy, and often sued owner. Google the 2.5 mil lawsuit he just lost, he owns hundreds of these Skilled Nursing Care Rehabs. Many, more than 10 per hall, sit in their wheelchairs, outside their room all day. Looking drugged & lost. Some of them repeatedly requesting a blanket. Most looking more than half dead. If you care about your loved one find another facility.

my husband had a stroke and his left side is paralyzed. this was supposed to be a great facility…read more it's impossible to get someone transferred out once your insurance company is paying, my husband's a diabetic. they don't have a dietitian there because they feed him just like a normal person and then have to give them insulin shots several times a day because you know insulin. the insurance company will pay for more for that drug than jardien or metformin. they give him ice cream puddings, sugary drinks and juice. he hasn't received any sugar-free drinks there they're charging us $2000 a day so i'm not sure what the issue is. do not send your loved ones here you will worry all night long you'll have sleepless nights. my husband is paralyzed from his left side. he cannot change his own depends so they leave them wet. he'll ring the nurses button in for one hour. he's still wet. i have to call the nurses station and they never answered the phone. i've got to keep calling and bugging just to get him changed. it's been absolutely horrible sleepless nights missing my husband. he's not being taken care of wow. now it's time for reporting this facility to agencies cause this is ridiculous my poor husband

They would never…read morebrush her hair so it became so tangled, when we asked that they please just brush it once a day a CNA cut off the tangled hair in the back and left the rest of the hair long. Her personal belongings were constantly "misplaced", her room and her were always unhygienic. We could NEVER get ahold of the nurses station, but when I would be there the RN would be sitting on her personal phone on Facebook. She developed multiple bedsores while there. And that's not even everything that was wrong with place. If you have the option to have yourself or a loved one in any other facility I would recommend never sending them to Menifee lakes. There were 2 CNAs we worked with occasionally that seemed to genuinely care, but other than that the experience was absolutely dreadful and made the already devastating process of losing a loved one even more stressful. Also I don't know when these pictures were taken but they are out of date and don't represent the facility well.

The Maxim office here could not take our case as they didn't have enough nursing for their current cases let alone new ones (or so we were told). Premiere took us in right away and within a month or so we had a new nurse who we absolutely love. We are sad but also happy that our beloved nurse is moving on to further her education/license and we are still staying with Premiere as we search for a new nurse. While many agencies don't have any nurses for us to meet or try, Premier has sent us several to try and we are continuing our search with them to find the right one for our home and schedule. The office staff is amazing, kind, helpful, responsive, etc. They know me when I call and make me feel welcomed. I am much happier with Premier than I ever was with Maxim. I also like that Premier has all of their charting on tablets instead of paper charts that took up half a drawer in my house and seemed less organized. Overall, we are very happy.

My brother and I went to five different assisted and Memory Care Facilities around the Temecula Murrieta area, and all of their memory cares look like zombieland. Very few caregivers, everybody sitting around doing nothing, no music, no one doing activities no laughing and very very depressing. And then we found Vineyard Ranch. The facility is beautiful and very well-kept. When we walked into the memory care, everybody was out of their rooms and doing something. People were laughing, talking activities were going on everywhere, and staff were visible throughout the rooms. Completely different environment and that made us very, very happy. She has been there for 3 weeks now, and she doesn't mention going home at all. She tells everyone this is her home and that she loves it there. She's always doing an activity and keeping busy. She's made a lot of friends. The food is good and she finishes all of her meals. Staff are always there to keep an eye on everyone. After any exercise, the staff brings everyone protein smoothies. Every Thursday they have an excursion to someplace fun and stimulating. At a time when dealing with a situation like this, it can be extremely stressful and create feelings of guilt. Vineyard Ranch has made the transition smooth, easy, and very pleasant. We are so happy to see Mom happy laughing and involved. Couldn't ask for more than that.

The woman that Gabby sent to care for my 90-year-old father was off her rocker and had to be removed from my parent's home. Also the woman apparently had good reviews online and had a lot of hospital experience. But bad people can slip through the cracks in any business, especially when they're being hired on a contract-type basis. My caution to anyone using any agency or hiring a private caregiver is to do your due diligence. Do not hire an agency and walk away feeling secure that your loved ones will be well taken care of. Had I not been watching on the camera to make sure that my parents were safe, they would have been traumatized by this woman being in their home all night. Even after getting her to leave, my mom who has Alzheimer's, was terrified that she would be coming back despite me ensuring her that she wasn't. Anyone who is doing long distance care of loved ones living at home, I recommend if you don't have them already, get cameras installed and diligently watch them, especially when a new person comes to the home whether they're from a private hire or an agency. This will ensure that your loved ones are safe and well taken care of.

I had no idea where to start. After being given a few options from various people I began making calls. As soon as I had Gabriela on the phone I felt immediately at ease. She went to visit my mom in skilled nursing so she would be able to find a caregiver that matched my mom's personality and boy did she knock it out of the park. The caregiver she assigned to my mom was kind, communicative, patient, reliable, and the list goes on. I was thrilled, but more importantly my mom was over the moon. When our caregiver was unable to make her shift due to illness, Gabriela texted me right away and was prepared to find a back up. Gabriela herself even offered to run an errand for my mom on the day that this happened. I cannot recommend Constant Companions Home Care enough! We had.a very positive experience and if I ever find myself in need of a caregiver again, I know who I am calling.
