The staff took excellent care of her--carefully monitoring her condition, administering medications, and supporting her recovery. Notably, she did not experience any falls during her stay, which was a significant relief given that she had fallen on her very first day at a previous skilled nursing facility. The reason I am giving four stars instead of five is due to a lack of communication after admission. Neither my husband nor I were contacted regarding her condition, diagnosis, care plan, goals, or discharge planning. It wasn't until I visited the facility in person four days later and requested to speak with someone that we received this information. My mother-in-law did mention that the care environment felt very constant, saying, "They never leave you alone--up at 5:30 for medications, bright lights, no privacy." However, I reassured her that the staff was being attentive and doing their job to support her recovery. By the fourth day, her infection had cleared, and she had regained independence in getting to and from the bathroom without incontinence issues. According to the discharge planner, she was also able to walk 175 feet with a walker under supervision. Special recognition goes to Sondra, the discharge planner, who was incredibly efficient. She coordinated the physician's discharge orders, medication reconciliation, and completed the necessary 602 forms for her return to assisted living--all by 2:55 PM on the same day I met her. We arranged private medical transportation, and my mother-in-law was comfortably settled back into her own bed by 6:15 PM that evening. Overall, we are grateful for the quality of care she received, with the main area for improvement being communication with family members.

Although I have mixed feelings about the place, I observed that many of the staff members are dedicated and are caring. I visited my son daily and what impressed me the most were how attentive and hard working most certified nursing assistance were to my son's needs. Although we did have a major problem with one certified nursing assistant, as far as I know there was an investigation into the incident we experienced and was happy to learn soon after that the individual in question was quickly terminated as she was found not suited for this type of profession. I was truly impressed with the rehabilitation center as well. My son needed both occupational and physical therapy. I have to thank the entire staff there for giving my son the push he needed to move forward. A special thanks to Jeanette who I felt went above and beyond as well as Kevin, and Sam who were dedicated in his success and wellbeing.

Our family's experience with my mother's care was extremely concerning. During my mother's stay, there were repeated issues involving her basic care and medical needs. We repeatedly had to advocate for things that should have been addressed as part of her care. This included problems with obtaining her commode and walker, monitoring her weight and fluid retention while she was taking Lasix for significant heart-related concerns, and getting assistance with her CPAP machine. There were also times when our concerns were minimized despite her documented medical history and needs. My mother was discharged from Kaiser on a normal diet, yet Asbury Park gave her pureed food for much of her stay. The facility later acknowledged that this was an error and apologized. She also has psoriasis and was supposed to have creams applied as part of her care, which was not consistently done. Even more upsetting, there were times she was left in bed soiled for extended periods. We raised concerns repeatedly, and far too many things seemed to be addressed only after our family escalated them. Families should not have to constantly fight to make sure basic hygiene needs, medical equipment, dietary orders, medications, and physician instructions are being followed. Because of the seriousness of what happened, I filed a formal complaint with the California Department of Public Health. CDPH opened an investigation, went to the facility, spoke with staff and administration, and reviewed my mother's records. That investigation has now been completed, and I was informed directly by the California Department of Public Health that Asbury Park was cited as a result of their investigation. For me, that is important to include because these concerns were not simply a family being unhappy with the care our mother received. We reported what happened to the state, the state investigated, and the investigation resulted in a citation against the facility. Our loved ones deserve dignity, proper hygiene, appropriate medical care, and for their documented care plans and physician orders to be followed without their families having to repeatedly intervene. If you have a loved one staying here, please advocate for them. Visit often, ask questions, document everything, and speak up when something does not seem right. I am grateful that our concerns were taken seriously and investigated, but I wish my mother had received the level of care she deserved from the beginning.

Let them know day agead and you can eat with your parent. Seasonal fruit was delicious. Everyone who works here is kind and seems happy to work here. They clearly treat staff well. there is no smell! other places smell like poverty and pee. this place is delightful. each room is private with nurse station close. There are mini fridges for drinks. Each room has a large cozy recliner. The hallways are well lit and the lobby is very welcoming. Don't expect your sick parent to eat or enjoy it, but they will be miserable anywhere less. my mom had surgery after a fall and she really loved it. The doc came every week and assessed her. The PT staff got her stronger, she was discharged on time. She asked me how to let them know. She is crying because the staff came to say goodbye and gave her hugs. She really felt cared for. Manzanita Healthcare Center bills itself as a rehabilitation facility for seniors; my spouse spent…read morefive days and nights there before checking out early. Staffing appears to be understaffed, with some dedicated caregivers and some who seem to be there to act as prison wardens, or sanitation workers who regret their status in life. Case managers are clipped, and not easy to communicate with, and my spouse was seldom ever offered insights by real nursing staff. She was quickly fitted with a catheter, apparently to save staff from having to help her to the bathroom and back. Physical therapy was hit or miss; in the first two days she was eligible, and was skipped by the staff. One of the physical therapists was probably 100 pounds overweight; and the physical therapy she did receive was minimal at best. The physical therapy department is not open on the weekends, so Saturday or Sunday PT is not a possibility. Food was passable to worse. My spouse was in a shared room with another client, a woman who loved to have her TV on late into the night with the sound at 100 DB - never did Staff come by to suggest the sound be turned down - giving my spouse earplugs which were little help. And result of the substandard nursing environment was that we checked out early, to the chagrin of several on the staff, who said "we were leaving against medical advice". We were pledged a referral for home-based nursing, but it took several phone calls and several days for Staff to file that paperwork. Overall grade, D. We would not recommend this place.
