I have outlined the key situations below: Thursday, May 22, 2025 - Cellulitis Diagnosis and Treatment: On this day, my brother Andrew and I noticed that our mother was not feeling like herself. Andrew observed that her legs felt like they were on fire. I immediately informed Dr. King, who briefly examined her and diagnosed cellulitis, but did not provide any further details or explanation. Later that day, another provider confirmed the same diagnosis and began antibiotics, but there was no improvement by the following day. I then requested more aggressive treatment. The provider suggested starting ceftriaxone, but it took over two hours for this to be administered. After that, we saw significant improvement by the next day. (pictures attached) Friday, May 23, 2025 - Bathroom Assistance: While visiting Ruth, she needed assistance to use the bathroom. Winston, who came to help, initially assisted Ruth onto the toilet, but then left to tend to another patient. He was gone for so long that Ruth became distressed and started to cry. Despite pulling the emergency cord, no one came to assist her. Fortunately, another nurse eventually arrived to help, but this delay caused unnecessary distress to my mother. Request for Air Mattress: On Thursday, May 22, 2025, I requested an air mattress for my mother. I followed up on Friday and again on Saturday. Unfortunately, the mattress was not brought to her until late Sunday afternoon, which was an unacceptable delay given her condition. (picture attached) Bathroom Assistance and Nurse Interaction: On another occasion, a nurse assisted me in bringing my mother to the bathroom. However, once we arrived, Ruth was too fatigued to use the toilet. The nurse then left to attend to another patient. I was left to manage the situation, but when I tried to assist my mother in getting back onto the toilet, a different nurse entered and raised her voice at me. (the nurse was white and on the hefty side) She insisted that Ruth be returned to her wheelchair and put back to bed, even though I had already nearly helped Ruth onto the toilet. Sunday, May 25, 2025 - Fall Incident: On this date, Ruth fell and sustained injuries. The images of her injuries are attached for your reference. This incident is of particular concern, and I believe it warrants further investigation into the circumstances surrounding her fall. I am reaching out with the hope that these situations will be thoroughly reviewed and addressed. The care my mother received did not meet the standards I expect from a healthcare facility, and I would appreciate any steps that can be taken to ensure better care in the future.

Three roommates I had were degraded and talked down to and sometimes completely ignored when they soiled their diapers or when they needed their ostomy emptied and cleaned. It was difficult and saddening to see this happen. The common theme with the CNA care is " don't make a fuss or they'll be repercussions. Fortunately, I did not have to depend on others to take care of me. I didn't experience these acts of degradation but I certainly witnessed it. Another huge problem here are the drugs and alcohol that get brought in by visitors and possibly staff. I've witnessed overdoses and death related to this problem. The reaction by management here is minimal. The way this facility is run and the lack of funding is readily apparent by the staff shortage, the dirty and poor condition of the facility and the low quality staff that is hired. Many of the CNAs and Nurses are overwhelmed and unhappy. This directly affects the patients here. They hire ONE nurse to distribute medication to 32 residents on one wing. This process can take up to 3 to 4 hours and many miss their time window for much needed meds. The food here is pretty good but a lot of times you do not receive what you ordered and get a random tray. Staff in the physical therapy component are truly incredible people. It's unfortunate they are restricted by lack of funding and staff as the "gym" has little, very little, equipment. On a positive note, about 50% of the staff like their jobs and it shows in their attitude and cordial behavior. The other half, only do their job for a paycheck and is apparent in the way they treat patients and lack empathy or compassion. All in all this place is not somewhere I'd recommend if you have private insurance or are self pay.

Let me start off by saying my Dad was in here for mobility issues only. He also has a lot of…read moremedical issues. Here are my observations while he was there. I visited every day. Wheelchair was old and one brake that didn't work and had to be fixed. When I had to bring him to a Dr appt I had to tell them he needed legs on the wheelchair and the legs they gave me didn't lock on the old wheelchair and I had to take them off at the Dr's office because they would swing out. Only one aid that was assigned to him actually did her job (shout out to Marie). He has urinals which either I or his gf would empty since no one would show up. One day there were three aids sitting at the desk chatting and hadn't attended to him for at least 4 hours. There were days I would come in and he would still be in bed (2:30PM) and they would say they didn't have enough help. They were using a hoyer lift on him after 5 weeks of being there. He was fully capable of standing, but this would take time and patience. There's only one social worker for the entire building. Sorry, but that is unacceptable and cruel to the social worker who has a load of responsibilities that one person is incapable of handling. His cardiologist wanted blood work done every week which was not done. Lastly, the building needs to be scrubbed down and sanitized. The smell is terrible and his room was filled with knats/fruit flies the entire time he was there. I do want to give a shout out to Eric (PT), Dave (PT), Ron (nurse) Sandra (OT) and Marie (aid) for being absolutely wonderful and caring. Unfortunately their heavy loads don't allow for them to spend the amount of time with these patients they need. My recommendation is to hire more professionals and put them through proper training. I have no idea who is running this facility but I would not put my loved one here ever again.

Over the following days, she continued to observe closely and thoughtfully refine her approach, which gave us tremendous confidence in my sister's care. On day one, the physical therapy team completed a comprehensive evaluation and quickly honed in on exactly where my sister needed support. Their ability to identify and target the right areas has already helped move her toward improved mobility at a higher level. It was clear from the start that they are focused, skilled, and genuinely invested in her progress. This stood in contrast to one of our prior concerns at Woodlake at Tolland, where we felt there was overmedication and a lack of direct, personal interaction with medical staff. Dining has been another highlight. The kitchen staff and head chef are not only talented but also very attentive to individual dietary needs and concerns. It's clear they take pride in both the quality of the meals and the well-being of the residents they serve. One of the most impressive aspects is the number of long-term employees. The food was disgusting and the breakfast "eggs" gave me such diarrhea that I actually had an accident in the doctor's office during my first visit post-surgery. There was one very nice and dedicated nurse but the rest were dismissive and all but two of the aides acted as if I was an inconvenience. I will say that the therapists were very good but also very overbooked. Only 30 minutes of therapy Monday through Friday and none on the weekend. The medical "staff" if you could call it that was seen via a cell phone link except for the nurse practitioner who was pretty much useless. They were also out of network even though the facility was supposedly in network.

She mostly ate cold cereal and snacks that we brought for her. My friend rang for assistance, but no one stopped into her room to see what she wanted. Good thing it wasn't an emergency. My friend said the physical therapist was nice. The front desk receptionist was polite and helpful. I don't know why the state of Connecticut allows this place to stay open. Disgusting.

Faith Care should not be in the supplying of aides/caregivers business. Their billing practices…read moreare, shall we say, questionable to say the least. Their aides - for the most part - are disrespectful ... and if you dare to say that to Faith Care, they accuse you of being disrespectful to their aides. In fact, Faith Care has/had a habit of putting the responsibility of whatever bad happening solely on their clients. Also, it appears to me that Faith Care's aides have/had more rights than their clients. When you question if you had made a duplicate payment, Faith Care will argue with you and demand you prove that you have, instead of checking their records. Because of verbal abuse at the hands of one of Faith Care's "best" aides, I switched agencies from Faith Care to an agency named CT IN HOME ASSISTANCE. Everything bad that Faith Care and their aides are, My new agency is not. Example: Totally on their own, CT IN HOME ASSISTANCE contacted me last week, and informed me that they had found a duplicate payment and would be returning it (which they have), and they told me not to pay that week's invoice because my account is/was up to date. AND REMEMBER - THEY DID IT ON THEIR OWN! Opposite of what Faith Care does. And CT IN HOME ASSISTANCE's aide? He's more respectful than the last aide from Faith Care, he knows his job better than the last aide I had from Faith Care. Bottom line? In regard to Faith Care? BUYER BEWARE.

Their desire was to remain in their home instead of a nursing home. The manager came to my aunt's house and sat with us - explaining the company's mission and philosophy. We were pleased to hire Faith Care - The caregiver they provided was incredible - personable, not under foot but completely attentive to their needs - responsible - the house was the cleanest it's been in a long time, the meals were healthy and prepared with care, reminders for medications were timely, hygiene improved dramatically....I recommend them with the highest confidence.
