Looking back over the entire process from the beginning to the present time, certain individuals stand out from the start. The first contact was Alice at the concierge desk. That contact was friendly, professional, and efficient. Although it was a weekend day, we were connected to Ginny in marketing within a few minutes. Ginny remembered s from the time we toured the facility a few months earlier and was ready to start our application for residency. Although there seemed to be a ton of paperwork, she led us through the process professionally and efficiently, giving us guidance on timeline, options on the move, information on required documents, etc. we found her to be knowledgeable, professional, and accessible as the move went forward. During our time here we have not experienced any employee being rude or uncooperative to either of us. We are happy here. A good beginning sets the stage for a good experience. It is a pleasure to give the facility and employees 5 stars.

Brentwood at Fore Ranch is an Assisted Living and Memory Care Facility (ALF) in Ocala, Florida. My…read moremother lived there from July 2019 until October 2022. The building is reasonably nice but deteriorating and in need of maintenance. My mom's room had a nonfunctional bathroom fan with moldy ceiling tiles around it that apparently had been there for longer than several days (see the included photo below). While I was there in September the hot water failed throughout the building. There were complaints about this from unhappy residents for multiple days. The rooms are nice but the floor covering is frequently peeling, creating trip hazards. The food is reasonably good. The staffing, management, and care provided is a significant concern. If you are evaluating assisted living in Ocala, ask how long the Executive Director has been in place. It is a telling piece of information about staffing turnover. Without consistent leadership to provide management and staffing oversight, the care for your loved one may not be as dependable as expected in an ALF. During the three years my mom lived at Brentwood, there were three different Executive Directors and the position was vacant when we removed her from the facility in October. On August 30th, 2022 my mother had a stroke at Brentwood. In fact, she likely had at least 2 strokes that day. The first stroke symptoms were in the morning and quickly observed by a dining room service employee who pushed mom's medical alert button that is provided by Brentwood. She went to the hospital by ambulance but was released back to Brentwood the same day. Upon her return she fell. She was evaluated by the Brentwood staff and it was determined she was okay. Later that same day in her room, at 4:39 PM, the button was again pushed on her medical alert necklace, presumably by mom who had fallen in the bathroom. At approximately 5:05 PM that day my brother made an unexpected visit to mom and found her on the floor in the bathroom. After repeatedly pushing the alert button on mom's pendant, my brother finally had to go down the hall to find staff to help. This was more than 25 minutes after mom's pendant was pushed at 4:39 PM. A Brentwood Director says the 4:39 alarm was responded to in less than 2 minutes, though Brentwood is unable to identify who responded to the alarm within 2 minutes of 4:39. The records supporting that assertion have been requested verbally and in writing without result. If your loved one does not need much assistance and can get around well on their own then Brentwood is an adequate facility. However, if you are searching for an ALF that can be counted on to provide the timely assistance your loved one may need, there are likely better options in Ocala. As an independent care manager said to me as we were looking for alternative care for mom, Brentwood is good at enrolling clients into their facility but not very good at providing the necessary care.

Visiting Angels Ocala (VAO) has been providing continuing outstanding and compassionate service for…read moreour 94-year old dad since the first week of March 2020. The entire operation and everyone involved has been professional and focused on providing the best care possible. Dad experienced some medical problems (completely unrelated to the care provided by Visiting Angels) after a year into the VAO care program which resulted in changes to his living arrangements. VAO was understanding, cooperative, flexible, helpful and supportive in making adjustments to Dad's care program. It has now been nearly 18-months since VAO began providing services. We have never had a problem with billings, services, care-givers, or compliance with the scope of services provided by VAO and the outstanding staff. Dad has been cared for by one person for the overwhelming majority of time - but there has been a limited number of days when other caregivers were required to fill-in (due to illness, doctor visits, family emergencies, etc) for Dad's primary caregiver. The "substitute" caregivers were each well informed of Dad's condition/personality before arriving and each provided good, friendly, quality support that was welcomed by Dad. If you're ever in need of a reliable home health care service to provide care, companionship, light housekeeping services, assisting with meal preparation and clean-up, personal hygiene and grooming support, and a compassionate friend to help care for a loved one, I would certainly strongly recommend VAO. Even though they have awesome neighbors and friends, it was time to have more direct home care for them. I have the pleasure of speaking with and dealing with Veronica, she jumped into play and organized a game plan to assist with our parents. Veronica is on top of her game, she right away called our Mom's Doctor to find out her medications and Doctor appointments. Veronica set her up with a Caregiver right away and started on Wednesday. If you have elderly parents and don't' live by them---I would 100pct support and recommend Visiting Angels... Thanks, Veronica for your continued help and quick response's...

Our family had a very concerning experience at this facility…read more My grandmother was admitted following a hospital discharge after a fall in which she hit her head and injured her shoulder. Despite this, neither issue was evaluated or addressed for more than a week, and only after our family repeatedly raised concerns with staff. She required a two-person assist for toileting, yet staff frequently refused to help her, leaving her in soiled briefs. When assistance was provided, the treatment was often lacking in compassion and professionalism. The facility was fully aware that our family holds power of attorney and health care surrogate authority and that we planned to relocate her closer to home. Despite this, care-plan meetings were held without our involvement. Additionally, attempts were made to pressure her into signing documents and transitioning into their assisted living program, which we found deeply inappropriate. Also, we needed a copy of her ID to take to the post office to close her PO box, and the Social services director Joyce treated us like we were criminals. Finally, while she was placed in a skilled nursing unit and billed as such, the level of skilled care expected did not appear to be consistently provided. Based on our experience, I would strongly encourage families to be vigilant, ask detailed questions, and closely monitor the care of their loved ones if considering this facility, and please find somewhere else to take them.

My mother was there for just two months and then we had to transfer her to a different facility. In the tour/interview process, they are extremely responsive. However, once you have your loved one placed, you can forget about timely responses, if they respond at all. Mom has heart issues and we were assured a cardiologist was coming onsite to see her. Never happened in the two months she was there. We could have brought her to one ourselves but were told it was handled. We could go on and on about similar things but suffice to say, what you are told during your tour/interview, probably is not completely accurate and I'm not even sure how to instruct on getting accurate answers. The facility is nice, it's clean, good food and activities for residents; however, the communication and response issue is NOT good at all. They also seem to lose medical records as well as treating physician names. Definitely could use some admin oversight or assistance in responding to family members of residents.

My mother was there for 9 months we had to place her there in the middle of covid. When they opened up and I walked in through the doors, the things that I saw were horrific. The place is filthy the management takes no culpability for the lack of care for the patients. When my mother walked through those doors she was walking, taking showers by herself, and feeding herself. By the time I removed her she had been left in bed with very little activity for 9 months, she had no muscle tone in her legs and could not walk on her own. I would come into her room and find her lying in bed in a wet pull up or full of faeces.This memory wing is like a morgue. These dementia patients need stimulating activity and there is none what so ever there. I do not blame the staff I blame the management, they take no accountability. When we had to make the difficult decision to place my mother there I provided her with beautiful linens, clothing, and new furniture. When I was able to come into the facility several weeks ago after they opened up, half of her clothes were missing all of her linens were missing she didn't even have her original bed that I provided for her when she moved in. Her room was filthy there were dust balls under her bed the size of baseball's, her bathroom was filthy, she was dehydrated. She was constantly getting UTI's (urinary tract infections) that was because nobody bothered to make sure she was hydrated. If that wasn't already enough to make me move my mother then they made the decision to remove Makayla Saunderson who's in charge of activities in the memory wing. She was excused from her position. With the exception of a few nurses and aids she was the only staff member there that I saw that truly care for these patients. After our tour we thought we had found a place for my parents so I paid the $1500 required to hold an apartment and booked a time that my folks could see it. Well when we got there they denied my dad due to his urostomy, told me that while they would like to have a night nurse they don't currently. Then told me I would get my $1500 back within 45 days. I can see 45 days if I cancelled but if they decline due to their deception a check should be available on the spot. If you don't need "assistance" than perhaps this is for you, if you are looking for some help go elsewhere.

My grandmother recently passed away and I wanted to write about where she spent her last couple of…read moreyears. When my parents and my grandmother moved to the Ocala area about ten years ago my grandmother was already starting to show some signs of short term memory loss and dementia, but she insisted on living in her own home and not with my parents. The retirement community they moved into was great and it's the kind of community where everyone seems to look after each other. When my grandmother got to the point where it just wasn't possible for her to take care of herself any more they sold her house and my Mom found her a place called the Bridge at Ocala. It was a difficult decision, and a very expensive one, but the employees at The Bridge cared for my grandmother until her last breath. When I say they cared for her, I don't mean they just took care of her, I mean they cared, they really did. When she was still able to get around they brought her into the community rooms and sometimes even forced her to have some fun (she'd fight it, but then tell my Mom afterwards that she was really glad she went). When over the past year or so she had gotten so weak and ill they would get her up into her wheelchair and bring her outside to sit with her or at least so my parents could sit with her. The place wasn't perfect, and there were times when there were miscommunications and even issues with things like her meals and the cleanliness of the place, but they were few and far between. The last time I visited my grandmother it took her quite some time to figure out who I was, but it was a nice visit for me...even if she didn't remember it the next day. My grandmother passed away yesterday in the early morning, in her sleep, and I'm happy she was at The Bridge with people around her who cared for her and about her. I can't thank you all enough for everything you did with my grandmother over the past couple years and for my mother who just simply couldn't be there all the time...I don't know what she would have done without you.

Abysmal facility and terrible employees. Lotti Smith had doctor's orders to distribute seizure…read moremedication to my grandma. My grandma indicated to multiple facility team members that she never received the medicine; to which they told her she was wrong and they know she took it this morning. She got denied today from each person she informed. I call and ask for the report of medicine distribution, at which point I was informed by Amber Grace that the delivery did NOT take place at 9am as I was first told. She will die without this medicine and nobody will give it to her without consent from a manager. None of the managers will answer...so the charge nurse Tamara refuses to give her the medicine that my grandma took by herself every day for over 60 years. I admitted her to this facility yesterday. I figured she could stay at The Bridge for around a month... Two days ago I took her out of HealthSouth acute rehab for a hip fracture. I will be taking her from The Bridge tomorrow if she is still alive and filing a lawsuit for incompetence and misconduct where they are medically liable.

Thankfully the lady said "would you please". I walked the lady to her room which was locked, then took her to a nearby office where the employees rolled their eyes exasperated at the poor lady. I went back to the salesman who was waiting 2 corridors away to continue my tour. Really?!! This is supposed to be memory care & no one wanted to help her? Harmony isn't compassionate or caring. Not a place for my relative for sure. There are better places that are the same price.

Needless to say walking in I had some anxiety about getting a Cpap machine but as soon as I sat with Kevin and he thoroughly explained everything to me from how to use, clean and order supplies and find a mask that would be perfect for me I felt comfortable with the fact that I would be using a Cpap machine and I enjoyed my time learning valuable information as well. Kevin is a great asset to the healthcare community and I'm extremely grateful to have had this opportunity to meet and pick up my Cpap machine from Kevin at Apria, thank you so kindly for the time spent and all your support!

Many stating that they consider the staff to be family, as that's how they get treated. The facilities were also impressive, from the rooms to the equipment available. This place may look polished on the surface but what I witnessed was anything but caring. My mother…read morewas admitted and within one day she was hysterical, crying, confused, and left without basic care. She returned to the ER in the middle of the night. And for six hours the next day, I never saw her assigned nurse. When I finally identified her, she was sitting behind a computer, smug and dismissive. She never delivered the medication my mother needed to help. Staff acted like they were above accountability, cold, oblivious, and completely unconcerned with any cries for help. I repeatedly voiced concerns about my mother's physical and mental state and they were ignored. Her decline was visible and urgent, yet staff acted as if nothing was wrong. I even had my friend come down to help me advocate for her because the situation was so alarming and I wasn't being heard. While my mother was in visible distress, I was approached by a woman who asked a series of questions, including how my mother voted. When I said I didn't know she continued to press me about it. They even made an alarming attempt to politicize and profile her. This was deeply disturbing and completely irrelevant to her care. When I returned to retrieve her belongings, no one knew where they were. It was a total circus. The man helping me look said they already had someone in her bed and her belongings were gone. It took over an hour to find her belongings. I truly believe that if I hadn't intervened my mother's life would have been at risk. She likely would have never made it out of the Palm Garden of Hell. Zero Stars!!!!!
