My wife's condition continues to decline as her disease progresses, and the staff's attentiveness to these changes is impressive. The care and the love that she received was nothing like I had ever seen. My dad had been in three different nursing homes back in 2018. The care that my mother received here at this place was a 12 if you were going one through 10. The staff is loving and kind and protects their patience 100%. They became my second family and were just as kind and loving to me as they were my mom. When they say Personal Care they mean Personal Care perfect name for this Nursing Home. All I can say is my mom died peacefully there with myself and a staff member who came back after working a whole day to be with my mom and I as she passed. Rating: "F", for failure to the residents of this community…read more I had personally witnessed the indifference of care and services my mother received for the past 4-1/2 years at the facility. My mother suffered UTI's, ear infections, severe skin conditions, suspicious bruising, falls, and anxiety due to the chaotic and unstable environment. Due to poor infection control and cross contamination while covid, influenza, pneumonia, and norovirus existed in the facility in December 2024 & January 2025, my mother became ill. During these two months she kept saying she didn't feel well even when she tested negative on tests. She had called 911 in late January because she had difficulty breathing. It was discovered at the hospital she had double pneumonia. Two weeks later she passed away. I had a camera set up in her room for the last 7 months of her life. This was after I found a nightgown ripped in half when doing her laundry. I asked her about it and she said someone ripped it off of her. As usual I got a fictitious explanation. There are a multitude of on-going issues, from late and inconsistent meal times, cold unappetizing food, lost menus, inattentive dietary and nursing assistants, brief changes not being done for over 10 hours. The list is unending. Filing numerous grievances with the facility and filing numerous MAARC reports with the State and communications with the Ombudsman caused retaliation from all levels of management and nursing staff. In just a year and a half I had furnished close to 400 documents which included email communications with the facility to the State. Even after providing photos and video clips of evidence and being cited and fined. Nothing had improved. I would like to point out that it's unethical and can be a legal liability for a business and their employees to post reviews. Management should be reflecting on their own part in contributing to the continuous obstacles. It starts at the top not the bottom up. The only justifiable solution is to start putting more effort into figuring out what they're doing that isn't working and to put forth new plans of action in addressing and improving the care, services, safety and infection control processes and procedures and following through with them. Implement a new effective plan on how to monitor employees to ensure they're doing what is required. Installing cameras throughout the facility would help tremendously in identifying and revealing many issues, which had been suggested time and time again. Management is the Beholder of the Dilemma's. Another deeply concerning conflict is the doctor's and social workers are contracted through the facility and seem to have their loyalties towards the facility, not in the interest of the residents. It's not necessary to respond or discuss anything further to this review. Management had their opportunity and sufficient time to prove they were capable in resolving matters and were unable to.

They will tell you they will make your outside specialist appointments and set up transportation but they don't do it for weeks. I finally ended up doing it twice myself and the third time they did it but then didn't have my mom up and dressed and ready to go in time! Run the other way - do not go there for rehab!

The meals if you're lucky enough to be the first person there you'll get the meal if you pay to have it delivered you just get leftovers which usually meant no meat and no vegetables, not away that seniors should get their nutrition only from pasta. My sister was dying of cancer and the extra help that they were supposed to receive we paid a lot for we never got so we had to hire a 24 hour nursing staff out of our own pocket I believe that this hastened my sister's death. The management team would tell my mom and sister to leave Stoney River if they weren't happy instead of trying to make them happy. They put cameras in and around the apartment in their bedrooms invading the privacy. Mom called this place the nursing home from hell people would check in but couldn't move out fast enough two were leaving at the time we busted my mom out of there. There are very few kind and compassionate staff however most of the management were found to be uncompassionate inept and incompetent most of the staff is young including management with a high turnover the management is immature often intimidating and hiding to avoid confrontation my mom is now in a real nursing home eating real nutritionist meals competent and carrying staff that check on her throughout the day and into the night which is what she wants the residents are happy clean welcoming you can talk to anybody her meds are over 50% cheaper and the rent is a lot cheaper too. It is such a shame that people that are entrusted with our loved ones and elderly people treat them so shamelessly.

The Director, Mary Hawkins, is amazing and so caring! Her door is always open for questions or concerns and she addresses them quickly. She was a big help in getting my parents on the Elderly Waiver. The nursing staff and aids are phenomenal! They always treat my parents with respect and positivity. The Physical and Occupational therapists are on site. They have done an awesome job helping my mom rehab from a broken leg. They are also helping my dad build back strength he's lost from cancer treatments. Not only were they neglectful, but they inflicted physical and lasting harm to one of my loved ones. These experiences are from 2018-2019, and I hope in the year since this facility has overhauled their practices to meet the bare minimum of care for their residents. I still harbor enormous resentment and anger towards the "caregivers" and hope current residents are in better hands. This facility was a blight on the last years of my loved one's lives. Don't trust your family members to be safe, respected, or cared for at Epiphany.

Then they pulled a stunt saying it would be shut down and moved her to the more expensive house to steal her money. The other house never shut down and the other 2 residents my mom lived with still live there. They wanted my mom's money. Thank you to the Guardian and the Ombudsman. You saw their true colors. My mom is free and safe and I get to love on her non-stop. The staff at her new facility are amazing and so understanding and loving. Night and Day Difference! Stay away please!

My Mother was at Amira for 4 years in the memory care unit. It was bad, and then it got worse…read more Every single time I went to visit, she was wet. Every single time! We even bought flow-through pads to use in addition to the depends, in case she was a "gusher". There were times she was wearing the depends and two flow through pads and was still soaking. FYI, that means she was sitting in 8 cups (1/2 gallon) of urine. I am just a dumb landscaper, but perhaps that explains the multiple UTI's that she had every year. She also had multiple falls (a result of dizziness from the UTI's) and a broken hip and later a broken femur. The best thing Almira ever did for us was kick Mom out, because it forced us to find another place for Mom. She spent the last 4 months at Breck Homes in Bloomington, where they made her feel useful and had so many outings that it was hard to visit her. Pretty is as pretty does, and Amira doesn't! If you are looking at memory care, visit here at meal time. Do these people look like they are enjoying themselves? Does the food look appetizing? Can they even hear each other, if they wanted to talk? Is there room to comfortably move around the tables with a wheelchair or walker? Yes, the fireplace is pretty. Is the room functional? Comfortable? Does the staff chit-chat with the residents, or do they just boss them around? Ask the employees you run into how long they have worked there, and ask if anyone has been there since the opening which is less than 6 years ago. There is only 1 who has been there the whole time, and she is the only one who knows how to get things done. Hint: it is Linda at the front desk. Raona

We would come visit her and she would smell like urine, she would have a dirty depends on for hours. She was barely washed with either a cloth or actual shower. She had 2 falls that didn't make sense, so we bought granny camera's, which were frequently turned off by staff until we told them it was illegal. Some of the staff did the bare minimum to get her ready for the day. She had 2 more falls which the staff was no where to be seen. She layed for 2 hours on the floor until they found her. Just when you think things couldn't get worse, it did. I sent flowers and a teddy bear to her one day and the teddy bear is no where to be found. My grandma contracted the Covid-19 virus here. She has since passed away. Unfortunately her death was not peaceful in any way. This company is absolutely horrible. The management is cowardly and quite literally pathetic. This company should be shut down. Our parents, grandparents, sisters, brothers and all other family deserve so much better. Please think twice before sending your loved ones here. I guarantee you, you will regret it, I do EVERYDAY. I wish she was still alive but at least she is no longer in pain .
