After looking at over a dozen locations we found Discovery Village at Southlake Memory Care. They have been truly wonderful to her. Are they overworked and overburdened at times? Sure they are, but who isn't these days. They have an amazing capacity for love and compassion. They have been taking great care of Mom during this extremely difficult and trying time and journey in her life and ours. This is end of life living after all. Not just a day at nursery school. We are very appreciative of the care and dedication our Caretakers, Med-techs, Nurses and all the staff have exhibited. they also keep me in the loop anytime anything happens. Thank you for being there for us, and I would recommend you for those needing your services, any day. God Bless all of you.

Food quality is excellent Hygiene is tended to Activities all throughout the day My favorite part of the whole facility is, the massive outdoor area the patients can go to any time of the day. Sitting right next to a large lake that I know helps with the overall morale. (Don't worry, it is completely gated all around, your loved one won't be in a situation that will bring worry) It's a great feeling knowing my grandmother is getting the care she deserves. We were new to memory care because my dad was mom's caretaker until he passed away suddenly, so we had no idea was was good or bad or right or wrong with memory care. Let me just say...facility on Precinct Line Road in Colleyville, Bad! Silverado Southlake, Good! We moved mom to Southlake in January 2018. As with most Alzheimer's patients, mom has declined in the last year. I've had at least quarterly care plan meetings with the staff and they are always tweaking her care. The staff always calls me to remind me its time and schedule the meeting. I usually drop in unannounced and always find the staff engaged. The residents seem to have full days and the staff tries to engage the residents with activities. Mom doesn't know me any more, but just last week, on a Saturday, they had a fellow with a guitar singing old songs. Those residents, including my mom, where sitting in a circle singing along to those tunes. I sat down with her and watched the response. She knew those songs but not me :-) I have found the staff to be very good, when compared with my previous experience. Is it perfect? No. It wouldn't be perfect if you had your loved one in your home. Is the care exceptional? Yes! There has been some turnover of staff, but that seems to be the norm in the industry. It is a tough business. In my view, the residents receive excellent care. The communications are excellent. If something happens, like a fall, which will happen, they are on the phone as soon as it is discovered, even if its in the middle of the night. Frankly, after getting involved in a family council with the other facility, I have come to learn that some people complain no matter what. Most of the complainers aren't interested in problem solving. Most of the folks that complained the most were on their fifth or sixth facility. At some point, you gotta ask yourself...what is the constant denominator? In summary, I love Silverado Southlake and the staff. I could not bring my mother to my home and care for her better. She is always clean, and mostly happy, considering she has a 60 second memory loop. The staff seem to really care about taking care of these vulnerable people and in my view, provide the best care available.

My father was in and out of the hospital a lot. Eventually I had to end his living at atria because he was in the hospital for over 3 months. The facility could/ should have notified me the family member that they were about to destroy his leftover medication , so that I could have saved him hundreds of dollars. Now that he is being discharged finally from the hospital I have to repurchase all his medication again. They should really consider rectifying this policy. I don't believe any new caretaker would know in advance that the medication would be destroyed so quickly.

BP/HTN is KNOWN as the silent killer among all healthcare professionals, this is KNOWN NELGECT. As a nurse for 10years, I highly recommend against this facility for the time being. The price to live at The Landing was exorbitant for what my Mother actually received in daily care. I naively assumed that with the overall beautiful look of the facility and the high cost to live there that the level of care would match accordingly. I was paying for EXCELLENCE! My hopes were quickly diminished. The Aides that were there- and to the best of my knowledge had been there a long time- had terrible attitudes. It was as though they hated being there. I communicated with the Director of the floor, regularly, to make problems known, as I hoped for change in behavior towards my Mom. The Director was super-helpful & did everything in her power to correct the problems! She had meetings, hired new people, & sadly, very little changed. As people were fired & some good new people were hired, it seemed as though this was more of a "systemic problem". Actual change did not occur. The food was very poor quality- I heard one woman describe it as worse than food given in prison. (Imagine school lunch 1975) My Mother lost 20 lbs in the first 3 months, she was down to 97 lbs! The Aides nor Nurses on staff did anything to try to help her find something else to eat. Simply shrugged their shoulders. Her daily routine was Get her out of bed in the morning for breakfast. They may or may not put clean clothes on her. May or may not brush her teeth and hair. After breakfast they Put her back in her bed, in the dark, for 4 hours until lunch. They'd get her up & take her to lunch. Bring her back to her room and may or may not change her dirty briefs, & Leave her in her bed, flat, in the dark, staring at the ceiling, for 4 more hours until dinner. They'd get her up for dinner, bring her back and put her in bed by 6:00pm, May or may not change her dirty brief, & Leave her in the dark in her bed until 5:00am the next morning when they changed her brief and left again. ****This is not a LIFE!**** It was almost as though they were just waiting for her to die. And I believe she would have if I had not intervened. When I realized the Aides had no routine they followed daily, & no one ever seemed "to know anything" about her care routine & who was responsible for what. I made a specific list of daily care for her since they had NO care report to follow other than 1. take her to eat and 2. make the bed. These aren't complicated things: Change her clothes daily. Brush her hair & teeth. Change her brief every 4 hours. Turn the lights on in her room- open the blinds. Make sure she has fresh water & it's beside her. Don't use her food tray for diaper changes! After several months of fruitless negotiations with her Aides. Some even had the gall to argue with me about what I was requesting for her care. I finally took my concerns to meet with the Executive Director & he was talking to Corporate & as it turns out, Corporate was very upset with ME that the Aides were having to follow private instructions daily! They clearly didn't want to make changes & I never heard back from them at all! I have everything on photos & videos as well as endless emails listing the lack of care & proper attention on any given day. *Bruises, cuts that took weeks to heal. *Sores on her bottom where her brief was not changed often 8-16 hours between changes. It happened MANY times! It's truly disgusting. *Dirty gloves, used in changing her briefs, were not removed- handling her clothes, wheelchair, tables, doorknobs, etc. *Diaper changing items placed on her rolling food table during changings. *Dirty clothes hanging all over the room & then put back in her closet dirty. The list goes on. *The RN they are so proud to have on the 3rd floor to have "Hospital Level Care" is actually responsible for making rounds to the other 2 floors in their shift. This means they are very rarely even at their Nurses Station seat to monitor the 3rd Floor because they are spread so thin working on the other floors. *Aides are given a certain number of patients they are responsible for, but some people require a lot of time in a shift due to their ailments, so the Aides end up almost ignoring the ones that are "easy" because they don't "need" as much. Or they hand them off to a newly trained Aide that doesn't have experience & doesn't take the time to look around to see what is actually needed. In the end, As I was preparing to move my Mom out, I found out that even the people in the leadership positions on the Landing 3rd floor started leaving over the last 2 months we were there. I hope things have improved in the last 4 months- and it's possible it has-but potential new Guests or current Guest families need to be aware! Get a good camera for your loved one! The saddest part is I know my Mom isn't the only one these things happened to!

Weekday staff are generally good and consistent but overall the weekends are filled with temp staff that either doesn't know or care about doctors orders etc. Simple weekly housekeeping ignited, sheets not being changed, hair not washed. The list goes on and on. We have met and discussed these issues numerous times yet it continues. Here we are Sunday morning and no one could be bothered to make sure she got breakfast. I have cameras in her room and can see and document everything.

From my first visit with Director Daniel Collins, then viewing the very beautiful pristine public areas, then the well appointed and comfortable apartments, I knew this was the place for my wife who suffers with Alzheimer's. Kim Ballard, Memory Care Director and her staff, the nurses, physical therapists, everyone, has welcomed us with kind, professional, caring, experienced help and support. My wife began her residency this week. she is content and flourishing. I am now able to rest day and night knowing my wife is being cared for so well. Throughout the years we have enjoyed many holidays, celebrations, and meals at the Discovery Village. They truly do provide a home like environment, along with meeting the needs of your loved one. The onsite therapy, entertainment, and transportation amenities are also a huge plus! We are thankful we found such a wonderful place for her to call home. Highly Recommend!

The rooms and bathrooms are kept clean, the residents are taken well care of and fed at the same time every day. The menus are really good. (They eat better than I do). There is one person in particular that has had a lot to do with the improvements and that would be Shae. She is The supervisor in my wife's building. She is doing a fantastic job. She fell on the floor three times in one night the first two times I caught her on my camera and I called somebody to come help her the third time happened at 1 o'clock in the morning and she was on the floor by herself all night, covering herself with a jacket, unable to get up and unchecked for over seven hours. They wouldn't change her on a normal basis she would wear the same clothes over and over, Ieven though I would tell them to change her the last straw is when she was all the sudden non-mobile and they put her in a wheelchair we had no idea what to do. They're the professionals finally, we made the call to have them call 911 to have her sent out. Turns out that one of her Falls gave her a hip fracture which she had immediate hip replacement. She got it replaced that day and was in rehab. She came back home to Mustang Creek Estates and within two days she woke up or should I say didn't wake up and was lethargic I looked at her on my camera and noticed she was still sleeping and it was almost past 10. I called her son to ask what's going on as she was lethargic and not responsive and he said that they took a blood pressure and just told him it was low and that was it so I asked her son to ask how low it was in the 50s over 40s, which is critical and her organs were failing. I told her son that they need to call 911 that second cause every time makes a difference between immortality. It's like they didn't even know how to read a blood pressure and that blood pressure clearly this place does not have the appropriate medical training. They don't even have a nurse on site. Just know that they have somebody that makes her rounds once a week and not everybody can even read a blood pressure that should be mandatory to facility like this thank God I had my camera because if it wasn't for that, she would be dead by now. She is now thriving in Roanoke at another facility and they are taking good care of her. They changed her every day. They bathe her appropriately. They have an on-site nurse five days a week. This place is a complete joke and my mother-in-law deteriorated so fast just within a couple weeks of being there and she lasted 4 to 5 months to say the least if you're looking to save money, this place may be a little cheaper, but sometimes for a little bit more, you can find care with dignity, and the turnaround of staff was also a huge concern. They pay these people peanuts therefore they don't stay. You can even read glass door and look at reviews of former employees. I did a ton of research after all of this and should've done my way more research before I even had my sweet mother-in-law walk through the door.. these people are all about profit and it'll probably take some even bigger act of negligence for this place to get shut down at some point. I do have pictures and videos to support what I have seen. they also just covered up her sheets with her comforter when she was incontinent and messed on it and they just put a comforter over it and I caught it on the camera and they said that she didn't have an accident and I said yes, she did. I could see it clearly on the camera and you just put a comforter over and just left it instead of washing her bedding . The laziness was astounding that happened twice. . Always get a camera. If I did t " babysit" her myself, she would have been gone sooner.
