I had the good graces to find him because I was researching about my symptoms and found that prolotherapy might help. I chose Dr. Juhl over the other NYC prolotherapists based on the simple fact that I liked his face. Dr. Juhl gave me an exceedingly thorough intake that included even looking at my nails (he identified a "digestive disturbance" that happened about 2 months ago, based on how far the ridge was from my cuticles). This was to gather information about what was going on in my back and pelvis. I was extremely impressed to find someone thinking so holistically about my body and symptoms. He advised against prolotherapy and recommended I get an MRI because I was exhibiting some signs of having a serious condition called cauda equina syndrome. I pushed to have it done ASAP, he found me a place that would see me the next day (Saturday). He called the MRI place to make sure I got there OK. On a Saturday. He called to make sure they read the results ASAP. On a Saturday. He called me the instant he got the results. On a Saturday. He was patient, gentle, and kind, through and through. He is incredibly thorough and extremely knowledgeable. He gave me enough information to make informed choices on my own, without pressing me one way or another (surgery or no surgery). He took extra time with me and didn't charge me for it. He referred me to some neurosurgeons, and after learning that I did go through with the surgery at the neurosurgeons' recommendation (I did have cauda equina), he called me several days later to see how my recovery was going. I have never felt in more caring hands in the medical industry. My only regret is that I found him just before I moved from NYC--I would have loved to have him as a practitioner for my entire time there.

    I was a counter strain patient of Stan Kupchenko's for a little over 6 months. He was great at that…read moremodality, and was professional and appeared empathetic during sessions. The staff were always polite and friendly, and the facilities well-maintained. I also needed to have my insurance cover my medically necessary treatment, which would require prior authorization paperwork. Stan and administrative (and supervisory; Dina Badami) staff verbally agreed to file that paperwork. The first email I sent them about it was in February 2024, to which they did not write back. By June, no paperwork had been started. As I was due to begin another health treatment (at about the same price per session: $235) in July and it was no longer financially feasible to continue even borrowing money from family, it became more urgent to address; I spoke with them about this insurance matter again, specifying the timeline necessary in order to ensure uninterrupted treatment. They all agreed again. June came and went with my following up with Stan at every appointment, and receiving verbal assurances with no paperwork. Repeated follow ups via email produced a draft of the letter of medical necessity written by Stan by 07/10/2024. Unfortunately it omitted a lot of information required by insurance for prior authorization approval, such as the number and frequency of sessions being requested and the criteria for evaluating my recovery. As my treatment had already been interrupted due to these paperwork delays, I created a document revising the letter of medical necessity so it included all the mandatory information and evidence to support the need for this treatment, and shared it with Physiofitness. I explained the changes, and requested that they update it with the numbers (so all they'd need to do it read it, and if there were no problems, just change the X sessions over Y weeks) so we can get this submitted. They wrote back, confirmed successful receipt, and agreed to read and update it. I followed up again the next week to find out the status of this request. Stan replied on 07/18/2024 "No edits have been made. Will let you know as edits are made. Thanks." which was the last time I heard from anyone at Physiofitness. I followed up twice after; once on 07/26/2024 (in addition to this letter I asked about the impact of insurance changes), and then again on 08/02/2024, this time asking if I am to understand I am no longer a patient of theirs. Given that it is now halfway through September 2024, and Physiofitness has declined to engage in any communication for 8 consecutive weeks, following poor communication/follow through for about 6 months, I am extremely disappointed that I am unable to access appropriate treatment from this physical therapy facility due to utilizing health insurance. TL;DR: Stan was skilled at physical therapy modalities such as fascial counter strain, but ultimately his expertise was irrelevant; as a patient who needed to use health insurance, Physiofitness was not interested facilitating insurance approvals to allow me to access appropriate care, and instead chose to misrepresent what services (such as administrative insurance-related tasks) they provide before eventually ghosting me.

I didn't immediately make an appointment, but I finally went in late Jan. 2025. I went on a Thursday and upon entering I saw a waiting area with people on top of people. Having retired after 50 years in healthcare in various settings, I knew this is not a good sign. I was seen sooner than I expected, but I suspect, it was only to input me into the system. It was a pleasant meeting and the doctor actually DID something. She gave me some trigger point injections! Relief. An MRI was ordered. Totally necessary since I hadn't one in years. In response to my question regarding the crowded waiting room, I was told Thursday is "procedure day" with patients requiring anesthesia needing someone to accompany them. I scheduled my f/u appt on a Friday. And then it began. The Friday I returned the waiting room was not crowded. I was hoping to discuss the MRI results with the doctor. I was told she was not in due to a last minute emergency. Possible, but not probable. The only medical practitioner onsite was the PA and she was overstretched. She discussed options and an epidural injection was scheduled for the next Friday, since I didn't need anesthesia. She recommended a baseline EMG, since I also have diabetic neuropathy. A sound idea, so one was scheduled for today, Thursday. When I expressed my reservations about Thursday, I was told it "was a different line". Arrived today, on time, to a room packed with people. I signed in and took a seat. After waiting an hour past my appointment time, I asked at the front desk how much longer. I was told there were 4 people ahead of me! I said I was told this was a different line. She said "it is". When I asked why I wasn't told there would be such a wait, the receptionist said "you didn't ask"!!!!!!! There are many types of business models for medical offices. Some are based on respect for patients and their time. Others are based on greed. An appointment is an agreement between the patient and the provider. There will always be instances where a patient may need more time. A medical complication or a little hand holding. But this practice only cares about generating income by overbooking time slots, at the expense of their patient's time. I canceled my EMG and my epidural injection. There are too many physicians in the area to be subjected to such treatment. Unfortunately, I never got to see if their treatment would have been beneficial.

She gave me a patch test, and three days later I learned that I'm allergic to a chemical present in some body wash products. Why Dr. Kulick never administered a patch test, I do not know. Maybe he wanted the hives to continue so he could get more money from me. In any case, I paid out of pocket for both appointments, and in 2013, to my surprise, Dr. Kulick's name appeared on my credit report! Now, in 2015, my credit report still shows a "delinquent" account with a collections agency named "Capital Accounts." I haven't received any bills in the mail from Dr. Kulick or Capital Accounts, and I've had the same phone number for like 10 years. No one has attempted to collect this apparent debt, but now it's ruining my credit report and I can't sign a lease on a new apartment. Dr. Kulick himself hasn't even responded to my emails about this - just his office manager, who sent me a PDF of my "billing history," which doesn't even add up the numbers correctly! Always double check your math, guys! ;)

The day I go on vacation was the day I was supposed to pick up my monthly Percocet but my flight left earlier than the pharmacy was open so I tried to get Turok to send me my meds to where I was going. I had multiple problems getting that done so I came home in alot of pain and extreme stiffness. His office then sent a much smaller dose and smaller quantity of my medication and I kept asking why and all I got was Dr Turok leaving nasty messages. Then I receive a letter that starting Jan 1st 2023, his office was no longer prescribing opiods and left me with NOTHING to hold me over AND still no reason why he, for the 1st time ever reduced my meds without reason! I now find out he lied to me, that letter was a complete lie and he has been prescribing opiods without interruption! STILL NO EXPLANATION and now he has blackballed me as no other pain mgt doctor will give me what I need, give me any opiods at all and some won't even see me!!! I not only have advanced DISH but now need a 4 level fusion in my neck, have Shmorl nodes eating away my lumbar spine and this POS doctor could care less! On top of that, he talked me into getting Testosterone implanted in me last year without any care that my BP spiked so high that I almost stroked out and refused to take any responsibility. Moreover, I found out after the fact that when I thought I was getting epidurals for my neck and back, they were either trigger point injections or nerve blocks. As others have pointed out, Turok is nice when you're giving him the high priced injections and other treatments that he can make the most money but if you mainly need pain meds to function and you cause any issue on getting your meds, he'll shut you down and lie his ass off! Avoid this POS! It's too bad I couldn't give a 0 Stars.

And as per the office stuff the test can't be scheduled before 2 weeks from the office visit as insurance not going to cover it!!! What a Joke ! Called the insurance and they told me it is not true!! Stuff and doctor is very unprofessional! I was devastated-and most doctors had no idea what to do and didn't want to bother with a case of systemic pain of unknown origin-especially after all of the more typical diseases were ruled out. Not only did Dr Shepelyak take me seriously but he has been successfully treating me with a combination of therapies, including prolotherapy and PRP. I am walking, typing and texting with far less pain. I can now sleep without any pain medication. Dr. Shepelyak is caring and listens while providing extraordinary care. He takes his time with each patient and thinks outside of the box-two things that are so rare in medicine these days. I recommend him to everyone I know, especially people with back and neck pain, migraines and other muscle conditions where they have been unable to get relief.
