My mom has been here 3 times in last 4 months or so. They have a great facility and for the most part the PT and OT therapists were outstanding. My mom can be difficult to say the least. I agree with previous comment about CNA and Nurse manning. Many patients are elderly and required basic assistance with going to bathroom. The lack of needed help is highlighted at night and on wknds when there are no OTs or PTs assisting. I was not impressed with the doctor, my mom only actually talked to him 3 times in person in 3 different stays and I never talked to him or heard from him about mom in the multiple times I was in the facility. i got transferred to The Center about 1:00 today, Wednesday. i was basically ignored for 4 hours. The only way I had water was when my husband went to the nurse's station to beg for a cup of water. If he had not been here, I would not received any until 5:00 when I finally got a jug of it. Then, I never received dinner until at 7:15. The only reason I did, was the person finally answered my call button. Dinner tray was received 20 min later.I then asked about my meds that I take with dinner. Well, it is an hour later 8:25 pm, and no meds. I am going to give up and go to sleep. read more

My mom is there right now. She is 93 and came in with a shoulder replacement. She is a strong lady, 1st they thought she had a broken leg, uh no. When she came from TMC she was able to get up and be asssisted to bathroom. 1week later, they have her Peeing and pooping in diapers. Not my mother. She's British and very proud. I don't like the fact as I come in the 2nd floor staff not only doesn't greet me , but Won't give eye contact. Very cold reception. Unacceptable in a supposed healing environment. read more

Do not have your loved one come here. The RN ration is 16:1 patient. The staff do not introduce themselves at change of shift, or check on patient unless you request. Follow up on medications given is not done. Call light are unanswered, I witnessed 4 call lights not being answered for 30 minutes as 2 staff member talked in hallway. If your loved one requires dependent care, staff do not assist with turning unless you ask them, VS are not taken, foleys are not emptied, trash is not emptied. Care is abyssmal at best. read more

We have been here fir about a month. It's horrible here. If your visiting your loved one no one goes in to check on patient. As I see and speak to others that sit all day no one goes in at all. As if they are to do the changing toileting. The food is horrible dry over cooked chicken and pork broccoli they give the steams really. Floor is always sticky I see a lady mopping her love ones room everyday and other wiping it does. It really is the cleanest room. My dad's is not. Lights on no one cares CNA on the phone see the lights and keep talking to there family on phone. Yes, correct on personal phone calls. Shower girl is rude and mean my dad has a hard time she doesn't care. I don't understand why some of these folks choice this field , if they are burnt out from it then move on. This is not the job any longer for you. Chris and Alex and Tracy nurses great helpful. We always ask him) shefor help for he is not hiding. Now CNA's Eva never comes in to check on dad Pyria great she always comes to ask if we are okay. Then I go home I know when she is her dad will be great. TJ rude talks rude yelled at my dad. That's not cool. Nor appriate. Manny he is okay Chelsea Destiny and Maria not friendly or polite. Annie she asked like she doesn't know what to do Never anyone in charge in the building. read more

The rooms are spacious and private but the care is mediocre. Someone I love had a brain injury and was at The Center, the staff were hard to get to respond to bed alarms or toilet alarms. If someone came and the patient needed something there was often a long wait to get the person "who's job it was" to do certain things to help, someone would come and say, "that is "someone else's" job, I'll tell them," and then disappear with no one to be seen until a meal or meds were delivered. It didn't feel like everyone there was there to help, many people seemed to just be there to do their job at the minimal intervals, rather than helping the patients. The patient aids and the caregivers here are amazing. The PT staff seem to have the credentials to help but don't seem to apply them in a meaningful way. I say this because my loved one was later sent to another rehab where I saw true healing because the staff was caring, educated, and properly trained. The Center is kind of bleak. You can do better for the people you care about. read more

My dad was at The Center for an extended period over the holidays which made it difficult for the whole family. Not only were they so caring and attentive to him, we were able to reserve a banquet room for the family to have Christmas time together. I can't say enough the thanks I have for the staff and the clean facility. read more

Terrible. Self serving butt covering bad people. Check on your loved one -- regularly -- if not constantly. These people lie. A lot! IF you love yourself --or are looking to help someone you love-- avoid this place. It's got all the trappings. Nice building. Nice rooms. Horrible staff. read more

5 Starts for Mary Belz, former Activities Director. I had worked for the Center at Tucson with Mary Belz, Activity Director as her assistant. I worked in the activities department for almost 3 years. I learned a lot from Mary. She was a great boss, She held the activities department to its highest standards, following Arizona State & Medicare/Medicaid guidelines & purpose of activities dept. in a SNF, which are serving all of the Residence. Providing them with variety of activities and themes to keep them engaged as part of their healing and rehabilitation, including sensory skills both as group and 1:1 visits. Families, residence appreciated Mary and the Activities department. I wish Mary all the best. Also best of luck to whoever the new Activity Director is. My dad there from VA hospital referral. Very nice building, clean and new.He had a private room that was nice. My problem with the Center was the case managers wouldn't return my phone calls. And the food menu seemed to be the same food items every day. Facility appeared to have enough staff and they were friendly. He liked the PT people. But why is it after leaving several messages I can't get a return phone call. read more

So here is my second review on the Center. In June of this year I had my second knee replacement. After the first replacement I knew what to expect so of course I wanted to go back to the Center. My recovery was so much better because I had prepared my self and knew what would be expected of me. I had received good care and rehabilitation recovery for my knee. It seemed like this second time around there was some confusion with staff (2nd floor) on what their responsibilities were when a patient checks in after certain hours and refused any physical therapy needed especially because it was a late Friday check in?? I don't want to go into all the specifics but I didn't like how I was treated. After the dust settled I was transferred down to 1st floor where I was familiar with some of the staff who were great! Front desk to nurse aids and Nurses were all great! During my stay night staff were the best. I was treated with kindness and respect. I just want to say that at least I can tell if someone doesn't like what their doing and shouldn't be in this line of work. My cup runneth over with the care to those who did a good job read more

The Center at Tucson is an amazing place to put your senior family members. Everyone is so loving, caring and welcoming to each person. My mom Was there for 3 months after a broken hip. They taught her how to walk again. The staff was emotionally and lovingly invested in her recovery. She went home and unfortunately got sick. We took her back for complete hospice care. She was loved, and cared for as if she was their mom. We couldn't have asked for so much love and support from everyone At The center. Thank you all for taking so much care of my mom and loving her as much as you all did. We could not have done all of this without you all. She has gone to heaven to be with our lord and savior. Warning! Nightmare on Glenn st. Checked in at 6 30 pm on Sunday, no one paid me any attention. All supplies out of my reach. Didn't get any ice water till 9 30 pm. Feeling feverish, I asked 3 different workers to take my temp. 36 hours later I got my temperature taken. The nurse was surley and the aids were frantic. Lots of hilarity in the corridor however. So much more, and then I SAW THE BED BUG and it went down hill from there. They took everything away from me including my therapy machine. I approched the door to ask for water and 2 nurses started screaming at the top of their lungs for me not to come out of the door. I felt I had to leave but they wouldn't let me till I signed a waver not to hold them responsible. When I got home I found they had bundled up my things along with their sheets, 8 pillow cases, blankets etc etc. I'm 83yo, live alone, I got bedbugs, my son who visited and transported also got bedbugs. One employee told me he had reported the bedbugs to them with pictures quite sometime prior so they have had them for a long time. Worst experience ever. read more

WARNING DO NOT USE THIS PLACE!!!!!! Being one of the newer facilities in town you think that they would be great but seriously DO NOT bring your loved ones here. They will not pay attention to them at all. From the doctors not remembering that my lady couldn't have surgery or any of her ailments to the food and lack of supplies. Thank god we were allowed to be there to advocate for her or else I swear to you she would be with her maker. No one answers call lights for anyone and don't bother asking the nurses for anything they are useless as far as helping you goes. Asked a nurse for gloves and trash bags because we had been out all morning and she told me to find a cna...REALLY!? Next time I'll leave it all and pray that state comes in. I feel like the center owes me a paycheck at this point. Could count the good ones on one hand. It is disgusting to witness the lack of care the rest provide. They were more worried about state coming in and fining them then they are about your loved ones. read more

My mom is at the center right now she's been there for five days 4 nights ago I spoke to her at 7 o'clock her time and she hasn't had dinner so after we got off the phone I called the center and spoke with the nurse at the desk closest to my mothers room she said she would look into it, when she came back on the phone she said that the girl who was supposed to hand out the dinners, actually didn't knowknow if that was her job and my mom didn't get her dinner until 730 and it was mushy cold mystery meat. I spoke with mom tonight December 12 she has not had a warm meal yet, the place is horribly understaffed, she waited 20 minutes tonight for someone to come in and help her after she hit her help button. What's up with the lack of good food and someone to come and answer a "help" call ? You people have to do a better job , this is the only mother I have . PLEASE DO YOUR JOB read more

We had my mom sent to the center at Tucson after breaking her leg and being at St joes. The staff and everyone there has been very accommodating and cooperative in helping my mother through her rehabilitation. she is not able to do the rehabilitation immediately on her broken leg as it is non-weight-bearing. however they are working on her upper body core strength which will help her once she is able to walk on it. a big thank you to all the staff and rehabilitation people who are helping my mom through this difficult time! read more

I drove from California to visit my aunt on her death bed and they made we wait outside in 105 degree heat. I understand the Covid guidelines as I am a nurse, but At least create an area for large families visiting hospice maintaining 6 feet guidelines . I know it can be done since I work in Emergency Dept and have to make accommodations for our lobby . Think outside the box and be compassionate about the families grieving. Create an area where you have Atleast 4 chairs maintaining But overall the facility is nice and clean except for a little ant issue in my aunts room. The decor is elegant and the lobby furniture that was all in one area taped off unused . I hope this feedback can be used for next family grieving a dying family member during this Covid crisis. read more

Had a two week stay here after three surgeries oh my right foot and a month in TMC. I'm going to be in a wheelchair for a few needed time to get my house set up with room rearrangements, grab bars in the bathrooms, etc. The place makes a good first impression. It was custom built new for them about a year ago and it looks like it. Two story building with a reception area and family bistin area downstairs, wards are upstairs via the elevators. Rooms are single occupant with private bathroom, a large screen T.V. and individual climate controls. Both PT and OT provided on site six days a week in a nice gym, and mini apartment training center on the second floor. The staff is for the most part friendly. The food is not bad to good. All that being said, there are some issues. Staffing levels for the nurses and CNAs seems low, which results in them having to run around like crazy just to get the routine stuff done. This leaves very little time for them to try to respond to unusual or unexpected problem, and that's where the breakdowns in communication start to happen. This both between the day and night shifts and the staff and patients. read more

This is a skilled nursing facility and rehab. They are best for milking every day your insurance company had for you whether you need it or not and keep patients until their days run out and then do a rushed discharge. They keep a male nurse at night that sleeps and snores at the nurses station or disappears for hours while you need medication. The same male nurse, Dan, will give you medication you don't have ordered and just throw a cup of pills at you and tell you to take them and then never return. You come there for therapy to get stronger and they make you use a wheelchair. Lab results don't get to the doctor. Medication never seems to come from the pharmacy, which is in Phoenix so you wait til the next day to get your medication. They can medicate you for pain from an emergency kit but the nurses tend to not bother and tell you you have to wait, except for Amber. She did everything she could to get me the medication she had available and to treat my pain after the previous nurse never did anything. I will not recommend this place and will also report it to my insurance, Medicare, the state. Their daytime nurses are horrible-Connie, Terry, Runee, they're angry, will ignore you, talk down to you, swear, call you an it, ignore you.... read more
