While they were sometimes over worked, under paid, short staffed and/or experiencing maintenance issues, they were caring, concerned and genuinely wonderful people. They treated my brother like they would have treated a family member. In addition, I was treated with courtesy and the utmost kindness. While the care staff was mostly kind, the food decent, and the common areas clean and homey, the administration was cold, unresponsive, and disorganized from start to finish. You can expect rent and care fees to increase by 40% in 6 months. They seem to have no capacity for digital communication and were slow to respond--except when raising prices. I was even mistakenly given another resident's financial and personal information, brushed off as a "mix-up." by the staff Due to their negligence, my mother's move-in date was delayed and I was told that she was "not allowed in the building" a day before. At one point, while I was out of town, she fell and broke her ankle in three places. I was initially told she only had a UTI that needed hospital treatment, it was only later that I found out about the badly broken ankle. Later, she developed pressure sores due to a lack of proper wheelchair cushioning. The day she entered hospice, they informed me same day that her care fees would now be at the maximum rate-- yet meaningful communication on other issues was nonexistent. Even after her passing, the reimbursement process dragged on for six weeks, with no response after multiple follow-ups. A heartbreaking and ironically uncaring experience from start to finish. Would never recommend Lakeside Park to anyone.

In late October of 2025, our family was scrambling to find a hospice care provider for my mother in law. Finding a reliable hospice provider for a loved one is a daunting task often made under stressful circumstances. Our family, after extensive searching and disappointing recommendations from other sources, found Sonata Hospice through positive online reviews on Yelp. This review comes from a genuine family who utilized their services for a critical 10-day period. The level of care provided by the entire Sonata team to both our loved one and our family was amazing and far exceeded our expectations. We felt completely confident that our loved one was in excellent hands, and we as a family felt fully supported throughout this difficult process. Several team members deserve specific commendation for their outstanding professionalism and genuine compassion: Rachael (Nurse/Case Manager): Rachael was outstanding. Her work is a true blessing and a calling. She provided expert clinical care while simultaneously offering immense comfort to our family. The attentiveness she provided our loved one was consistent with the level of care one gives their own family. Her clear communication, explaining procedures, answering questions, and involving us in the care plan was a model of professional and empathetic care giving. Marsha (Home Health Aide): Marsha treated our loved one with dignity and affection, like she was caring for a member of her family. Her hands-on care was superb. Maria (Chaplain): Maria brought much-needed peace, love, and spiritual support during a challenging time, helping us navigate the emotional and spiritual aspects of the process and providing our loved one with that extra level of care. The Office and After-Hours staff were consistently responsive and efficient in meeting all our immediate needs. The support they extended through the difficult moment when our loved one passed, provided ease and compassion during our time of grief. And our hearts were warmed when at her funeral we saw Rachael and Maria present and still supporting us. Deciding on end-of-life care is perhaps one of the most significant decisions a family can make. Concerns about the quality of care, trustworthiness, and prioritization of patient well-being over financial considerations are paramount. Sonata Hospice put those fears to rest. They are a trustworthy, compassionate, and highly professional organization that we wholeheartedly recommend to any family navigating the hospice journey. I just wanted to share how grateful we are for the support we received. It was mostly my aunt having to take on all the complicated decisions but the hospice employees provided compassion and support. The chaplain, Maria was very calming-never pushy, just compassionate and loving during a really overwhelming time. The social worker, Melinda was also wonderful, helping us navigate everything with so much patience. My aunt commented on how after her visits, she felt happier because of her positive presence. Thank you Sonata hospice, this experience was hard and I especially worried for my aunt but the team made the experience much more bearable. Both of them made a difficult situation feel a little lighter, and their kindness made a real difference for our family. We truly appreciated the care and respect they showed every step of the way

He had been hospitalized 2 or 3 times at the end of last year and began the conversation w/ medical staff about a referral to palliative care as well as starting the process of physician assisted death, which thankfully is an option in California. We had a telephone conference w/ the palliative care folks in early February and it became clear to us all that hospice was the agency that would be most appropriate to his case. The referral was made and the multidisciplinary hospice team was amazing, warm supportive to us all, available as needed. They arranged for medical equipment to make Alan more comfortable, including a better quality hospital bed. They arranged for him to have a massage. We had not had previous experience w/ hospice and I can unequivocally recommend Kaiser's hospice care. Tammy, who had experience w/ another hospice organization w/ her late grandmother several years ago, said the experience this time was much better. Alan's life ended March 27 and we miss him very much but are glad he was able to prioritize his quality of life over its longevity.

I googled Hospice and came across MD Choice that had some good reviews. I had no idea who I was calling but there had to have been some divine intervention pushing me to this call and this company. After a few questions I received a call from Dr De Leon who was the most compassionate and caring doctor I have come across. He sensed my immediate need, came straight out to the house, assessed my mother and went himself to the pharmacy to pick up the needed medications. He knew the urgency and took it upon himself to do all the footwork. He obtained the needed comfort medication, stabilized my mother and allowed my mother to pass peacefully all within a matter of hours from making that initial call. When she did pass that evening, he again, arranged for everything from the pronouncing to the calls according to the pre-arrangements. Without MD Choice and the connection with Dr De Leon, I don't know how I would have been able to handle the situation. His immediate response on a Sunday, his Taking control of the situation and quick actions made such a difference in the final few hours my mother had. If you find yourself in a situation like this and needing someone to help in this time, I highly recommend MD Choice and Dr De Leon. I am forever grateful to MD Choice and Dr De Leon for acting so quickly and with such caring and love in this terrible time of need. Thank you from the bottom of my heart and the hearts of my family.

Not only were they able to provide significantly more daily visits from their team of nurses and aids than the other options we considered, but they were incredibly responsive, efficient, and helpful with every question or request we had--and that includes their 24/7 on-call team. Cheryl, the RN Case Manager, in particular, was a godsend. It was a huge relief to be able to rely on them so thoroughly from day one. They delivered everything we needed within hours of getting the paperwork done; and both Cheryl and an aide were there for the last few hours my brother was alive--providing support and care. We really can't recommend them highly enough!

My mom is currently undergoing rehabilitation here but there is one particular employee I would…read morelike to mention who has gone above and beyond the call of duty and above her job description. Her name is Teri (not sure if I spelled her name correctly) and she works at the front desk answering the phones. One day I couldnt get ahold of my mom because they dont have phones in the rooms and my moms cell phone was dead, so Teri left her post at work and went into my mom's room and found her phone and plugged it in and charged it then had my mom call me. Another time I had one of my friends go down there who lives in Alameda (i live out of state) and facetimed me from his phone when he got down there and handed his phone to my mom so I could video chat with her. She had been sleeping and woke her up when he got there so she looked a little disheveled. The next day I had mentioned this to Teri and she got my mom cleaned up and then charged my moms phone and called me from it herself and then handed it to my mom when i answered so i could talk to her. My mom is not always with it and will forget to plug her phone in when it dies, so it is nice to know that Teri is there when I need her to go into my moms room and charge her phone for her when it dies, that is not her responsibility but she doesn't mind doing it at all. I appreciate that. It is employees like Teri who make me feel at ease during a time when i am so far away and can't go in to the facility myself. I am so thankful for Teri!

Negative stars if I could…read more Where to begin... You have a choice to choose your nursing home, I didn't know this! And if you are not happy with the care, you also have a choice to pick another place to get you or your loved one transferred to. This place should not still be in existence. Review long overdue. I tried to report them. But didn't know how. My 88 yr old dad got Covid May 2024. He was transferred here after his ER visit and few days of hospital stay to get OT/ PT. He got put into an isolation room where they moved the patient before him without letting the family know, they came looking for their mom after my dad had been transferred into the room. Then we saw the sign outside the front door that there was a scabies outbreak (that lasted at least another month more). My dad needed assistance to walk to the bathroom. The bathroom was Out of Order! They gave him a commode with no access to soap or sink for clean water. Because he was supposed to be in isolation for 4 more days, no one came to help him! The morale was so low here. Not a single heart beat in this place. He got up by himself to use the commode and walk around the room. Then one night (day 3), he was cold, so he asked for an additional blanket, the staff took the blanket from the previous patient's bed (the bedding had still not been changed and washed a few days later), and gave it to him! That is not ok in general, all kinds of health violations. did I mention earlier there was a scabies outbreak??!? #1 way to spread scabies in nursing home was thru bedding!!! The outbreak has been going on for a few weeks and was out of control. After the supposed 4 days of isolation, they still did not take my dad off. I argued with the staff. Still stuck in the same room with old bedding, with no help or any PT/OT services. We had to seek out his case manager to ask for a care conference. They did not call us first. So finally the next day we met up together, they said he needed PT/OT for at least a month. I laughed out loud. At the end of the meeting, they asked if we had any questions and we told them, yes, "I would like to have my dad discharged immediately." Thankfully he was completely able to walk around and use the bathroom by himself, we signed an AMA discharge agreement, and he left the next morning.
