Center Point Healthcare and Rehabilitation is terrible and should not be allowed to care for…read moreANYONE. My father recently suffered a stroke and was there to receive rehabilitation services as well as wound care. At the moment, he is incapable of moving on his own, he can't speak and has a peg tube for feedings. The facility is extremely unsanitary, you can smell it upon entry. The staff responsible for my dad's care were either incompetent, careless and/or reckless. He was there a little over three weeks. I never saw a Nurse Practitioner and the nurses were rarely in with him. CNAs were the only staff that were easy to find. When we had questions about his care and/or services being rendered, no one could give us any information. The excuse was they weren't able to see the charts that contained the day's information provided by the staff from the previous shift. One day during a visit, I noticed my dad's breathing had become noticeably rapid. I asked my mom (who was also there) to call the nurse ( they always had to be searched for).When the CNA checked his vitals and realized it was really bad, she tried to contact the on-call nurse, and no one answered the phone! To make a long story short, my dad had to be rushed to the hospital because his blood pressure was extremely low, and he had a substantially high-grade fever. The hospital informed us that he was less than a day away from septic shock! He spent several days in the hospital, had to be rushed to surgery for an extensive wound cleanse and was given several bags of aggressive liquid antibiotics before he was even considered for discharge. If it were not for my mom and I being there with him to notice the change, he would not be here today. Thank God he was transferred to another rehabilitation facility for care after discharge from the hospital. He was put on antibiotics for six months, that should let you know how bad the infection was. Sidenote: The same day I noticed the change in his breathing, my mom had to run to the nurse's station to inform them about a patient (using one leg to move their wheelchair) who was outside alone and feet away from the busy street out front. Two staff members ran outside (one with NO shoes) to get the patient. Mind you, there is a "protocol" ( which they don't follow) to be buzzed in the doors before entry. This is designed to keep the patients safe and inside the building at all times. There was never a day I visited my dad that I had to be buzzed in, I was always able to walk right on in. THERE IS ONE STAR ONLY BECAUSE I COULD NOT SUBMIT THIS REVIEW WITHOUT IT!

All is great as long as you are independent and do not require the attention or care from staff…read more Once you need care in the " wealth and hellness facility" ( as termed by residents) you see quite a difference Management is rude, staffing is minimal and inept, food was horrible, facilities we're dirty. Patients are left for hours with no attention.Do not leave your loved one there with the expectation that because you are paying a fortune they are getting exceptional care. . I had to pull my loved one out ....have never been so disgusted . I am an RN and would not recommend this facility to anyone I care about. They are more concerned with aesthetics of the independent living areas than patient care. Don't be fooled by the chandeliers and new paint ....its all for looks . Never assume you can forget about your loved one thinking they are being cared for. You MUST check on them daily .
