You can grab a quick order at the bistro or sit down for a more traditional restaurant-style meal. My 80-year-old father had just come out of the hospital after a serious medical crisis and was on dialysis. His cognition was significantly impaired, he could barely walk, and he was in no condition to safely care for himself. Despite this, they were attempting to discharge him home. I was so concerned about his safety that I had to pursue and file an appeal with the state to prevent his discharge and allow him additional time at the facility. I should never have had to fight that hard to keep an elderly man who could barely walk, had cognitive difficulties, and required dialysis from being sent home. What was especially frustrating was the attitude we encountered when we questioned the discharge plan. At times, responses felt condescending, as though our questions were unreasonable, yet when I asked specific questions about how their plan would actually work, I did not receive adequate answers. One example says everything. I asked, "How is he going to know when he needs to go to dialysis?" I was told they could set reminders on his phone. My response was: What happens when, because of his cognitive difficulties, he can't find his phone? What happens when he forgets to charge it? A phone reminder is only useful if the person has the cognitive ability to consistently locate, charge, understand, and respond to the phone. These were not hypothetical little details. Dialysis was essential medical care. I was asking how an elderly man with significant cognitive and physical limitations was realistically going to manage this safely after being sent home. I also repeatedly asked for actual evidence demonstrating that my father could walk safely and independently care for himself. They could not provide evidence that satisfied those very basic concerns, yet they were still attempting to move forward with his discharge. Our experience was that discharge planning seemed driven by a predetermined timeline rather than my father's actual physical and cognitive condition. It felt like there was a seven-day clock regardless of whether he was truly safe or ready to leave. Trying to reach someone in authority was another enormous frustration. Getting clear answers or reaching someone who could actually address our concerns was extremely difficult. From our family's experience, the problems did not appear limited to one employee. They seemed to reflect broader problems with communication, coordination, and leadership. What worries me most is what happens to an elderly or cognitively impaired patient who does not have someone willing to challenge these decisions. My father could not adequately advocate for himself. Thankfully, I could, and being treated as though I was difficult for asking legitimate safety questions was not going to stop me. I would not have an elderly parent go to this facility. Period. If your loved one is already there, stay extremely involved, document everything, ask for the evidence behind discharge decisions, and do not be intimidated out of asking questions when something does not make sense.

My father was temporarily admitted to Spring Tree for rehabilitation and continued treatment of an existing foot infection. He is visually impaired but cognitively aware and communicates clearly. He was there to recover and eventually return home. We did everything possible to establish a positive relationship with the staff. We communicated respectfully and even brought coffee and donuts as a gesture of appreciation. We were not looking for conflict. We simply wanted our father cared for properly. Unfortunately, I felt I needed to constantly monitor his care. There were times I found him dirty, problems involving meals and medication timing, and poor communication between nursing shifts. We were told that many nurses were per diem rather than regular full time staff. In my opinion, staffing arrangements should never become an excuse for inadequate care or communication. One night, my husband and I arrived unexpectedly and found my visually impaired father on the floor. We later observed bruising on his face. We have photographs documenting his condition but will not post them publicly out of respect for his privacy. My father also told us he felt he was being mistreated and abused. We took his statements extremely seriously. Our family was not the only one raising concerns. While I was in his room, a social worker came in to address and apologize regarding grievances that had been filed concerning his roommate's care. Witnessing that interaction while experiencing our own problems was deeply concerning. Then we learned my father's foot was fractured. According to the medical information provided to our family, imaging performed before his admission to Spring Tree did not show this fracture. It was identified afterward. When I questioned Estelle, the head charge nurse, about what happened, she suggested one possibility was that his foot had been fractured during ambulance transport. Our family witnessed that transport and watched the paramedics handle my father carefully. I will not speculate publicly about how the fracture occurred. The medical records, imaging, photographs, and other documentation have been preserved. The timeline speaks for itself, and our family wants answers. After leaving Spring Tree, my father required hospitalization and surgery and was treated for sepsis. Thankfully, he is now recovering. My experience raising concerns with management was equally frustrating. I felt talked over and dismissed by Estelle. My experience with Marion at the front desk was also extremely negative. Sandra was one of the few people who genuinely listened and tried to help, and I appreciate her for that. Overall, our experience left me feeling that residents were too often treated like numbers rather than human beings whose families had entrusted this facility with their safety, dignity, and care. If someone you love is staying here, PLEASE stay involved. Visit unexpectedly. Ask questions. Pay attention to their medications, meals, hygiene, injuries, and changes in their condition. Listen when your loved one tells you something is wrong and document your concerns. There are additional details regarding our experience that we will not discuss publicly. During her stay, my sister and I basically had to remain at the facility throughout the day because the staff did not appear to be on the same page when it came to administering her medications. It seemed that medications were not consistently documented in the computer system. As a result, when the next shift came on duty, they often did not know which medications had already been given to my mother. Thankfully, my sister and I kept our own detailed notes of the medications that had been administered by each shift. While I am grateful that my mother has recovered and is now home, there are several areas that I believe need immediate attention and improvement: 1. Medication Documentation and Communication There needs to be better communication between shifts and more consistent documentation of medications in the computer system. Families should not have to keep their own medication records to ensure that a patient receives the proper medications at the proper times. 2. Patient Rest and Wake-Up Times Why are patients being awakened as early as 5:00 a.m. to get up and get dressed? Patients are in a rehabilitation facility because they are recovering from an illness or medical condition. They need adequate rest and sleep in order to recover. Unless there is a specific medical reason for waking a patient that early, a more reasonable schedule should be considered. 3. Food Quality and Menu Options The food menu definitely needs improvement. Many of the meals served during my mother's stay were not very appealing or, in my opinion, appropriate for patients who are recovering. More nutritious, appetizing, and varied meal options should be provided. 4. Air-Conditioning in-Patient Rooms I also have concerns about the wall-unit air conditioning systems in the rooms. The unit in my mother's room blew directly onto her, which caused her to become congested. I eventually had to request that she be moved to the other side of the room to avoid direct airflow. The comfort and respiratory health of recovering patients should be taken into consideration when designing or operating the air conditioning system. In closing, I want to acknowledge that there were a few employees who genuinely did a good job and provided my mother with kind and attentive care. I appreciate those individuals and their efforts. Basically there are about 8 rider type machines a small walking track and stair rack and a machine to aid standing up. All these machines and tracks are very close together A few of the bicycle machines need repairs to the timing and difficulty ranging. I have had a very good therapist. I have not seen all the rooms but I assume they are generally as roomy as my room. Service is spotty. There are some very good CNA'S BUT IN MY OPININON NOT ENOUGH OF THEM. I have waited as long as 2 hours for service. i can honestly say that the upper level medical staff in my case and in my opinion are not at all competent. I have a script that requires 3 specific antibiotics Monday, Wednesday and Friday. I had to contact the head nurse because they could not do that correctly. We had come to an agreement that they would dispense the parts of that prescription noon day on the Monday Wednesday and Friday so i could verify the drugs. That lasted about 2 weeks. Right now It is 9:00 and I do not have my proper script from my Wednesday order. They have bee telling me that a nurse is coming. The last time I called they hung up the phone. Both do a fantastic job aligning your vertebrae to release pressure that has built up on your nerves. For me, the issue is my neck and lower back. My treatment plan consists of a 40 minute problem area massage with a massage therapist working with the soft tissues in my back and neck to achieve pain relief. The massage is not as thorough as others I've had at other offices. Next, is the quick stretch and about 20 minutes or so of electrotherapy with a therapist. Lastly is the manual adjustment with the doctor. The staff is friendly. Nice seating area with TV's and magazines. They also have complimentary coffee. The office is big with many treatment rooms. At times, you are sharing the room with another patient. I understand that they are a busy practice but the bathroom needs paint and some cleaning. I have been very happy with the latest treatment I received from Dr. Cherry. He always has time to check in on how you feel, how the last treatment worked and to address and discuss any concerns you may have. He is a great listener and has impeccable bedside manner.

There's a very 21st century check in kiosk that has very cool technology ( an AI program recognizes that you are nodding yes to answer or spells out what you said...sort of like being checked in by Siri). The main reception is very businesslike. The care staff is also uniformly friendly. You can just tell from just about everything they do, that most enjoy what they do and genuinely care about getting good outcomes for patients. My friend says the food is quite good, especially by hospital / healthcare commissary standards. The design layout is also well done, with almost every room and hallway getting sunlight, including a large central courtyard. Signage is easy and clear. The most impressive and surprising thing was how fast everything fell into place, and the organization. Within 15 minutes of my friend's first day arrival a fellow named Sam (who's sort of the case manager / concierge) came by to introduce himself, followed by other staffers pretty quickly.

I do not understand why someone would bring their loved one here. It's even more mind blowing that someone would leave their loved one here.
