Timeline summary: - October 29, 2025 - I returned for my third visit to complete and address ongoing issues following prior treatments. - October 30, October 31, November 1, and November 3, 2025 - I communicated concerns via WhatsApp regarding sensitivity, discomfort, and issues with my mouth. - November 26, 2025 - I sought an independent evaluation at Mint Dentistry, where concerns were documented regarding veneer condition and restorative issues. I was referred to a higher level of care due to the complexity of findings. - January 15, 2026 - I sent written correspondence outlining my concerns. - February 2026 - I followed up with certified written notice, after which your office responded. - March 2026 - I provided photographs, external clinical documentation, and additional information as requested. Patient impact and treatment experience: - I traveled to Miami on three separate occasions for this treatment. - During the course of treatment, my teeth were shaved/prepared and left unprotected for approximately five days without adequate temporary coverage. - During this time, I experienced severe sensitivity and discomfort, was unable to eat normally for several days, and was unable to work for approximately three days due to the pain. - I had to travel with exposed teeth in this condition, which caused significant distress and functional impairment. Medical findings: - March 17, 2026 - I was evaluated by Dr. Alberto Lazaro, DDS, at North Texas Dental Surgery (prosthodontic-level care). - Clinical and radiographic findings documented multiple defective restorations, including: * Open margins, overhangs, and over-contoured veneers * Dislodged and chipped crowns * Secondary decay * Gingival inflammation and bleeding on probing * Radiographic evidence of pathology - The specialist concluded that the defective restorations were contributing to plaque accumulation, inflammation, and my ongoing symptoms. - A comprehensive full-mouth rehabilitation was recommended due to the extent of the defects. Refund and resolution attempts: - March 2026 - The provider stated in writing they were "100% sure" a refund would be processed. - April 18, 2026 - I was sent a refund agreement and agreed to a reduced amount ($6,050) in good faith to resolve the matter. I signed the agreement based on the representation that payment would be issued promptly. - I was told payment would be sent that same day; it was not. - Several days later, I was informed that $2,000 would be sent first and the remaining balance within 24 hours; this has not occurred, and no payment has been received. Additional documentation discrepancies: - The treatment plan I was given in person after payment in Miami differs from the treatment plan that was later sent to me upon request in November 2025. - This inconsistency raises concerns about the accuracy and integrity of the documentation provided to me. My Dad went in 2 weeks ago and he quoted him $15000.00 worth of dental work that he claimed was necessary. When my Dad told him directly that he didn't have that kind of money, he persuaded him to apply for Care Credit. My Dad is very conscious of his credit, but he signed the form to apply for the credit, feeling he had no other option, and was in a lot of pain! Subsequently, he went to another dentist for a second opinion and ended up getting the work done for much less - an affordable, reasonable amount. Within 48 hours, he went to Dr. Otero and informed him that he no longer required his service - he did this in writing as well. Fast forward 2 weeks, and my Dad receives a bill from Care Credit informing him that his payment is due in July - for 488.00! We immediately call Care Credit and they close the account and tell us to call the Dentist, as they have already received the funds (15K). We have been given the run around from Dr. Otero and his staff, as they claim that Care Credit has to send them paperwork, and 3 different representatives from Care Credit have told us that there's nothing to be done on their end. Dr. Otero's office has to issue the refund, in order to clear this account of its balance. Meanwhile, my Dad is a wreck worried about the credit that he guards with his life!
