I'berb here for 2 weeks now. All of the staff are so amazing, kind, caring, responsive and attentive always smiling and happy in giving me the best care I have ever had from other families except the hospital Another I like imy hospital is only 3 minutes away I love my private room It has a nice private bathroom, closet, counter with sink, cabinet, microwave, fridge, esk, chest of drawers l, beautiful lamps and a nice big porch with a nice view My bed is comfortable and I sleep good The food so far has been good except a few times That's not an issue since I food here I can eat I would highly recommend The Solera at West Houston because of the amazing and phenomenal staff that are giving me best care always happy and smiling, my nice room and all of the amenities

These type reviews are tough to write because if you're experiencing it, you likely have a friend or family member temporarily residing in a facility like this. I was surprised I was so impressed with the center. My uncle is currently residing here and though his condition is not good, I feel confident he is getting great care. The staff are all very friendly, the rooms are clean and it smelled nice through the entire interior. It is a one story facility with easy parking. There are several restaurant's conveniently located nearby. They have a couple of common area rooms with pretty decor (of of which has a piano), a small shop with boutique items, and a salon for hair cuts and a shave service for the men. Each room has a mini fridge, a single sink and a microwave. The bathrooms are spacious. All rooms, as far as I could tell, are private rooms. Some rooms even have a private exit to a terrace. The TV channels are limited. I think there's only a total of 10. The facility was quiet. I couldn't think of any reason to give less than 5 stars. Again, it's a tough situation and no one wants to be going through, but if this is where he has to be, I feel it's a great option. He is entering hospice, so it's all about keeping him calm and pain free right now. If anyone is going through this and looking for a place they can rely on to care for their loved one, I would recommend this place. Since he has been sick, we have put him in 2 places. The Concierge was the first one. We did not care for that facility at all. Prayers to anyone reading this, but I also hope it helps you make a decision. Side note: I visited on a weekend. Everyone who knows anything about facilities like this knows they don't have their "A Team" on weekends. Not sure why. The patient still need the same care no what day. If the staff was this great on a weekend, and a weekend is not the "A Team", then I suppose the weekday crew is even that much better. I hate to even make that comparison. Anyone who searches for a facility or has experienced loved ones in a facility, will know this.

Her prognosis was grim at first, but she is exceeding their expectations and is now on to skilled nursing here at the rehab. I was told it is called TIRR, but nowhere on the building does it state this so don't let that confuse you. The facility is easy to find, parking is a breeze and navigating through the building is simple with everything marked so well. During my visit, I was pleased with the cleanliness, attentiveness, responsiveness and overall room layout. She is in a private room with a private bathroom. I'm not sure if all rooms are private. A schedule is given to the patient each night with what is expected/planned for the next day. The goal is for her (and all patients) to get better and stronger, so I like the planned therapy and for her to know what is expected of her to achieve the results needed. I've heard excellent feedback from other people about this place. Everything I have read and now seen/experienced is A+++. My friend even said the food was good. The Houston Metroplex is home to fantastic healthcare and I think this is another representation to that statement. Anyone going through tough situations like this, my blessings to you. Stay strong.

I would give it 0 stars if I could. This place needs to be shut down!…read more My experience there was worse than anyone could ever imagine. The social worker and the director of hospitality both are there only for the paycheck. They do not care about the patients at all! My husband had a terrible experience. I just wish I would have known what I know now. My husband was there only a few months ago and I DO NOT believe they have changed for the better as they reply that to eveyone that gives them a negative review! They meds were continued to be given incorrectly, they broke his glasses and would not replace, they lost his wrist brace that he was required to wear as he came in with a broken wrist. I could go on and on but these are some of the more serious actions that happened there. I never got updates on his rehab. He was home 6 hours and fell again and ended back in ER. I do not believe he was ready to be released! They want to admit patients, collect their money and move them on. The patients are not cared for at all Please please consider other places over this awful place!!! You will regret it if you put a loved one there.

With any facility, the staff support goes up and down, and we were fortunate to have caring staff at the time of our visit. We were impressed by the hands-on Physical Therapy in the gym room I did stay overnight the first night with my family member and found that staff were there to keep an eye on the residents Appreciate the care for my family member from all the staff. Being in the medical profession, we had a positive experience with short-term rehab here

Do not come to this hospital as a patient, and do not bring a loved one here either! I'm here now,…read moreand the level of care that I'm receiving is horrendous! I am a complicated case with multiple health issues and lots of chronic pain from different cancers. I was told before I was transferred here that my medications that I was receiving at my previous hospital would not be touched or changed essentially. As soon as I got here that was the first thing they did! They changed the one medication that helps me the most! Then, when they supposedly changed my medication back to IV I felt like I was not getting the correct dose. So I asked SEVERAL nurses if I was getting the right dose, and they all lied to me and said that I was! Come to find out a week later after several bouts of nausea, vomiting, and rashes I was told that I was only getting HALF the dose! And when I talked to the doctors here about it they would not change it back! The hospitalist (Dr. Moosavi) refused to listen to me much less let me even speak! I have never been treated so poorly in my life over a medication that I clearly need! It got to the point where I had to talk to the patient advocate here, and he set up a meeting for my husband and I to speak to the manager. She basically pretended like she cared but did NOTHING to change the situation. It was all for them to save face! And it gets worse. It literally just happened again today after I had a procedure! This time with my pain meds! The horrible palliative doctor here changed my medication to not only half my dose, but also for it to be administered intramuscularly instead of just through my IV, and the pain of receiving the medication that way was horrendous!!! Absolutely beyond painful!! Why would I want to take half of my dose of my pain med and it cause me pain like that?! I cried both times I tried the medication that way because it hurt so bad!! And on top of it all it did not affect my pain whatsoever! It just doesn't make any sense! And I had no warning for the change! This particular doctor I haven't even seen in over a week! So I don't even understand what prompted her to even touch my medication to begin with! I feel like it was done purposely because she doesn't like me! I see no other reason why. The palliative doctor (Dr.Gail Donley) was so rude and condescending to me when I first met her. I could literally feel how much she disliked me when she would speak to me. She wouldn't listen to me when I would tell her what worked for me medicine wise and when I told her that I was allergic to NSAIDS she literally asked me how do I know. I told her that it caused me bleeding. And she was like yeah but when was the last time, and was trying to push me into taking an NSAID when I clearly told her that I could not take it. It was absurd! And now come to find out that she's the doctor that changed my pain medication without coming to see me, or letting me know anything whatsoever. I just don't understand why I am being treated this way here. It's beyond scary to be here as a vulnerable patient. Some of the nurses are wonderful here don't get me wrong. There's always some good people mixed in with the bad. But the bad ones are actually the doctors here! I have never felt so unsafe and uncared for in my life at any other hospital facility EVER!! I'm truly scared to be here, and I'm in SO much pain! The only reason I'm even here is because I was transferred from another Methodist location that said that they can't keep patients long term, and I had already been there for 50 days. So they sent me here to the Methodist Continuing Care location in Katy. Please please please do NOT come to this place for yourself or anyone else that you care about! The care here is below subpar, and that's coming from the actual doctors here! They are already trying to discharge me without even fully helping me with my issues. They will be sending me home with more problems than I came in with, and I am terrified! Please go somewhere else!! Anywhere but here!! I should've read the reviews to this place before I got here because that would've told me everything I needed to know! If you've gotten this far thank you for taking the time to read this. And if you ever feel like you are being mistreated don't be afraid to speak up for yourself! You are your biggest and best advocate! I plan on not only writing many reviews about this place online, but also contacting the higher ups here and filing a grievance! They will not get away with treating me this poorly! And I would hate for anyone else to go through what I've been through so far here!! Take care and God bless!

Patients do not receive basic routine care or medication as ordered. I had to report them to the government "DADS" Department of Aging and Disability Sevices Services department to report lack of basic care & nutrition. Unless you go there every day to check in on your loved one, they will be poorly cared for. The facility is not staffed with enough CARING employees to adiquitly a take care of your loved one. Please search and research to find a caring facility other than 'Grace Care'. A good resource to start with is a free website called "A Place for Mom". Also FYI, a written complaint form is available at every nursing home to register a formal complaint. It has to be addressed by the facility by holding a meeting with you within 2 weeks of the complaint being registered. A discussion of the corrections they will make will be signed by you and the facility. You are to be given a copy for your records. You should bring someone with you such as a hospice nurse. You are entitled to have hospice services if your loved one is in declining health. You do not have to wait "until the end is near". Hospice is a great service. Seasons Hospice which has been great offered 5 days for mom to go to center. I told them I needed a week so I would have to pay for 4 days. Which all good with that. $824. For 4 days no big deal. Fast forward to unlimited calls to my cell phone at roaming charges on a ship. Mom was hungry, nurse aid from hospice found her laying in wet bed once ask for lady to change bed, she laid sheets on bed. Then two days later my daughter witnessed same exact thing. Then mom ask for some coffee was told she had to go to dinning room. Was told she had not had her meds as our nurse from hospice had left for her. Her blood pressure was 186/100 when we got her home. Again meds not given for blood pressure. Again you ask how I know meds were not given I counted pills before turning them over to them and then when I got in car. I go to take mom home and they offer a wheel chair, it has a plastic cushion in seat I lift it up and OMG it had food from weeks and weeks in the seat. I have been told sheets were changed twice only when bed was wet. Her clothes were balled up in drawer with clean clothes. This is only to list a few items. I get a call on my cell telling me they forgot her meds then I go today to collect. They say it is liquid morphine. I was told our nurse from hospice ordered it but then another nurse head said no so not sure who ordered it but has moms name on. They kept it which is fine if it is destroyed. I bet not. Fast forward to stories from mom which I weighed as her demented state. But o no after hearing stories from my daughter, grown granddaughter (18), and the care taker. I was sickened at the care my mother got. Not one great moment and in my opinion Totally Fraud! I trusted them with MY mother and they allowed her to fall twice in 8 days! This is unexplainable and wrong on so many levels. Yes I am taking this to the top and have filed reports with State and will continue to tell others of my experiences so No other family has to go through this! Pattie Creel 281-455-8001

This is a Skilled Nursing Facility (or SNF as they're called) and from what I know, they focus more…read moreon rehab following a hospital stay now and not long-term care like what one might think of when they hear about a Nursing Home. I mention this because if a loved one was in the hospital for something other than a routine/elective procedure and after a lengthy hospital stay and you are told that the doctor is recommending transfer to a Nursing Facility instead of discharging them home, it creates a great bit of anxiety for both you and your loved one because "Nursing Home" is a scary, perhaps the scariest word for an older loved one but we were assured that this was indeed temporary and in the best interest of our loved one and in retrospect, it was the right decision. Their ultimate goal is to rehab your loved one and get them ready and strong enough to be discharged at home and have things ready for them at home (Oxygen, equipment, etc.) when they are. To be very honest, it won't be easy for you and your loved one especially with all the COVID-19 protocols that limit visitation - by limit I mean there are no visitations unless you go outside on the window or use Facetime but you can register to be an "Essential Caregiver" by taking a COVID-19 test, making an appointment to get trained for it, and then making appointments to see your loved one. You are however able to drop off things for your loved one. There will also be times that you will be very frustrated because communication could use some improvement. The problem is that the nursing staff take instructions from the hospital doctor (at least in our case) but don't really know what the doctor's longterm plans are. Your point of contact is the social worker who gets her information from the Director of Nursing who again, may or may not know what the doctor's goal and timeline is. If you call the doctor's office, they might refer you back to the Social Worker and so as time goes by, your level of anxiety keeps building up. At the end of the day though, I can say without hesitation that every single person that myself and my loved one came in contact with there was a dedicated and caring professional who had my loved one's best interest in mind. The rooms were nice, bright, and clean and resembled a nice hotel room. The food was pretty good from what I was told as well. When the time came to be discharged, they helped us set up the medical equipment and other things we needed.

Communication with about 55% of the CNA's Is mostly unfamiliar careful wording is needed for expression. This is called now Avir. The administration is cold, distance, and totally unavailable. For instance, I was requested to submit a sample of my urine. This was a 2 day attempt. First the bowl wasn't very secure, fell in the water. Second attempt was much better, caught what was needed, they failed to be picked up my the 6 pm - 6am, so my 3rd attempt I was successful. Now I have to wait to see if it will be picked up. Another insult I was waiting outside for my 98 year mother to arrive which made me late for lunch. A troop of culinary staff came to my door asking when I would be finished with my lunch, I explained I had received my warmup food and was eating it now. When I finished with my one plate, bowl, and dessert. Called for the pickup, no one showed. Most recently the fire drill was blasting, being from the public sector I am aware of the procedure if I was informed of the drill. Was not informed. A staff member hurriedly past me grabbing the extinguisher. Didn't say anything about a fire drill so I continue on the hallway, as I got into view of the gather CNA's/ maybe maintenance member. One pointed to me saying "why is out". It could have been a better way of communicating to me such as "we are having a fire drill please return to your room" no one told me it was a drill, if someone had said this you me I would have wholeheartedly participate correctly. My linen hasn't been changed in over a week. Asked for a change nothing happened. There're are other incidents I would like to mention I have written to long.
