I rented from the Sixt location at Barajas Airport in 2014, 2015, and 2016. I've also used Sixt in the USA. Overall, it's been a positive experience all around. I find that Sixt in general has rates on-par or even better than some of the larger chains based out of the states. For further context, I'm Emerald Elite with National and will choose Sixt over them because they are *that* good. For those curious about the models I received at a specific reservation class, see below.
2014:

All the cars were new, low mileage, and overall fun to drive. I'd rent from them again. Two observations from my experience:
1. They may pressure you to buy their insurance (what car rental company doesn't)? When I picked up my car in 2015, I told them I would be driving in Spain and Portugal and the woman told me I would make a mistake not buying insurance saying, "Have you seen Portugal's roads?" Trust me, the roads are fine - they're a part of the EU and were a joy to drive. Decline the insurance if you already confirmed you have coverage from your CC or insurance at home.
2. Be mindful if you leave Spain regarding the options for roadside assistance. In 2016 my rental needed a tow from a rest stop near Nimes, France. Calling the number on my agreement, I was told Sixt Spain couldn't help and that I needed to call Sixt France. I called the latter, and they instructed me to call Sixt Spain. After a bit of back and forth I finally got a tow and given a new car in Nimes (BMW). Not the end of the world but just be prepared if you have to get assistance from outside of Spain.
