My mom past almost 6 years to the day and I still think about all of them with warmth in my heart. The attention my mom got was top notch. They titrated meds so mom didn't have pain but still was able to function almost to the end. When one medication stop working tegu switched it to another by admitting her into hospice facility. I will never forget nurses there and how compassionate and understanding they were. The violinist came and played music in my moms room and that was just so wonderful. After switching meds she was able to function and enjoy pain free days and our company longer. The person who was coming to bathe her hugged and kissed her as if it was her own mother,per my mom's admission. Humanity of staff and compassionate care she had was unmatched . I heard this company was bought by another company, I surely hope that they would continue their wonderful care of terminally I'll patients as they did when my mom was transitioning. P.S. if you think that all hospices like this don't be mistaken: first hospice we got into, I would not name it, almost killed my mom by overdosing her on morphine in the first week , while I was at work. Thank God I came on time to find her almost unconscious. The nurse gave her huge dose of it and quickly left, leaving her to deal with it. That what prompted me to switched to Seasons and it was the best decision I ever made!

My mom fought pancreatic cancer for just days short of a year. She left the hospital to come home to hospice. We didn't know what to expect but they far exceeded any expectations we had. Every single person we interacted with was knowledgeable, professional and above all compassionate. I don't know how they do what they do. They became a part of our family. This all happened in the midst of the corona virus quarantine and they balanced protecting themselves and us yet still giving all the care needed and never making us feel like we were a burden. Our main nurse was Tami and the CNA was Iris. They both took amazing care of her and trained us to do what was needed when they were not with us. My mom instantly bonded with both. They were gentle and caring. I don't know what else I can say but we made the right choice with Seasons Hospice. I didn't see any reviews at the time I was looking and I hope this helps someone who might be on the fence.

Rainbow Hospice was nothing short of a Godsend for my mother and our family. After a stay in the…read moreICU at Resurrection Medical Center, we spoke with Ashley, the director of Rainbow Hospice. I did not know she was a nurse at first since she did not share her credentials up front. Instead, she came off as someone who was warm, and welcoming, patient, and wanting to be transparent when it came to explaining what they were about and what hospice looks like. We met her at the hospital while we were trying to plan for our mom's next steps. She gave us a tour of the Rainbow Ark Hospice inpatient unit, which looked more comfortable, cozy, and single rooms looked less like a standard hospital room, but more like bedrooms at home. The family waiting areas with large sofas and a fireplace were a special touch. It was our mother's wishes to have comfort care and refuse life-sustaining procedures if it meant her quality of life would not improve. After a long battle with dementia, it was time to make that step and honor her wishes. Rainbow Hospice made sure all of the equipment needed for my mom was at home before discharge. They actually started calling about the delivery of her hospital bed and other equipment while we were still at the hospital! They were very quick. They transported mom back home with dignity and grace using a private ambulance. The EMTs were so kind and gentle with our mom, explaining everything they were doing and taking their time. All of the staff that came to see our mother - nurses Maria, Shirley, Marylou, CNA Debbie, social worker Megan, and chaplain Concy were phenomenal. They were so compassionate, caring, answering every question our family and I had about mom's prognosis and care. Due to being in the end stages of dementia, our mother could no longer speak, eat, hold her head up (she was completely bed bound) , but every single one of them greeted mom warmly, looked her in the eyes, and talked her through what they would be doing for the day. They showed her such respect. Chaplain Concy was so thoughtful and touching in his prayers with our mom. He even played church hymns our mother loved on his flute the last time he visited her before she passed. This was a touching surprise that she loved and really responded to with her eyes, showing she enjoyed it. Concy knew we were interested in the getting blessing from the priest at a home visit, and within a couple of hours of his last visit, we received a call from my mom's parish. The priest came shortly to give my mom the sacrament of the Anointing of the Sick. He was so attentive and very thoughtful to have done this. Sadly, our mother had passed a few days later, only after being on hospice for 11 days. The on-call nurses at Rainbow were so helpful: Jillian, Kathy, Ann Marie. The last couple of days, we called around the clock, and they did not make it seem like we were a bother at all. Mom never got to spend time at the inpatient unit, but bringing her home and seeing her loved by her hospice team is more than we could ever ask for. I could not end this review without giving a special thanks to Marylou, the nurse who came to see our mom shortly after she passed, and pronounced her. Marylou had other appointments before being scheduled to see my mom that day (for a regular visit), but after hearing of her passing, she prioritized our mom. She helped us clean her and prepare her with such care and integrity. She took her time with us, and even called the funeral home on our behalf. She was really such a blessing, and we expressed our gratitude for her that day. Thank you so much to the wonderful team at Rainbow Hospice! You gave our family a wonderful send off for our mom by helping make her comfortable in her last few days at home. Words are not enough to express how much you have made such a lasting beautiful impact on mom and our family. It means the world.

Planning for the end of a life is painful and difficult, but Horizon made my father's last few…read moremonths a little bit easier and for that, my mom and I are forever grateful. When my father was given a terminal diagnosis and a dementia diagnosis on top of that, I became his advocate and caregiver. I received a lot of conflicting information from our doctors about when we should reach out to hospice, whether palliative care was worth our time, if we should sign a DNR, etc. Horizon wasn't the first place I reached out to or engaged; I spoke to another hospice and while very informative, they lacked the follow through we sought. More than once we fell into a black hole where calls and emails went unanswered. They also hadn't sold my mom on their services; maybe it was timing, but she felt pushed too far out of her comfort zone as it pertained to electing to stop curative treatment and other difficult decisions. When my dad's meds stopped working, there was no question it was time to engage hospice. I reached out to a few providers and was immediately impressed at Horizon's responsiveness. I filled out a web form early one Saturday and my phone rang less than 15 minutes later. Working with a non-profit hospice appealed to me as did their inpatient unit at Rush (midway between my house and my parents). Our social worker, Amy, was wonderful. My mom liked that she and our nurse, Kathy, lived in our community; they were very familiar with the local services available to us and never pushed us to make decisions we weren't ready to make. More than once Amy made herself available to us when she thought we might need support. When changes would agitate my dad, like the presence of oxygen tanks, Amy and Kathy helped us figure out how best to calm him. In my dad's last few hours, before we knew as much, Amy helped us think about what we wanted for my dad and she and Kathy helped guide us to keeping him safe and comfortable at home. My parents were skeptical about some of the services Horizon provides (masseuse, art and music therapy, etc.); I think some seemed too granola for them, but my dad quickly adapted, wishing his massage therapist could come every day. Horizon was responsive to our needs; emails and phone calls were returned quickly, medications and equipment arrived promptly and the folks staffing the 24-hour hotline were always friendly and patient, even when we were tired and panicked. The folks at AdvaCare, their equipment partner, were very kind and efficient as well. Horizon's engagement with our family didn't end when my dad passed away. Our nurse and social worker reached out to express their condolences. Our nurse also wrote a very kind note on the staff card we received. Additionally, we were notified of the support services available to our family in the wake of my father's passing and I've received at least one check-in call since his passing. As I said, end of life planning is painful and difficult, but having an organization like Horizon to lean on helped. * Horizon has merged with two other area non-profit hospices - Midwest and JourneyCare.

The hospital recommended a…read moredifferent company but after much research and several recommendations from close friends/nurses I decided to go with them and I'm really glad I did. I'm truly grateful for the wonderful CNAs, social worker, bearevement therapist, and their whole team. My husband chose home care so I wasn't sure they were the right fit since we're located in Chicago and some of reviews for the city area were not great but went with my gut on this and they've exceeded my expectations. I believe our team was out of the Deerfield office. My husband was in his 50s, had a strong will to live despite battling stage IV cancer for 9 yrs, and a dark sense of humor. It wasn't an easy transition but the whole team worked with us both to address all of our concerns and helped him find comfort and peace. Every time I called the on-call nurse/manager they were able to help out and come up with solutions tailored to his needs. Communication was key here since my husband was reluctant to taking pain meds and anything of the sort. We didn't get to use their facilities so I don't have that experience as others have mentioned but the home hospice care was just what we both needed. I just felt very well supported throughout the entire process and was consistently kept informed.

Positives: The facility is nice and peaceful, depending on what other families are there…read more Nursing/PCN staff was mostly compassionate and kind. My mother-in-law was brought in on Mother's Day. We were originally told she was only approved for 14 days of hospice, no mention on what would happen if it took longer than 14 days. It took a week to hear from social worker regarding her situation because social worker was out of town. no on-site chaplain, but one was available upon request. Social worker promised support with FMLA and a note for work, never received either. My husband was promised bereavement/mental health support, but only after she passed. it's been over a week and nothing. We have to track down the social worker to do her job. The last week of her stay, several loud families were visiting patients in the hospice house, with no enforcement of rules, rowdy guests, or inappropriate behaviors after we were told the teenagers in our party need to be on good behavior. The last few days of her stay, visitors, nurses and staff were talking loudly outside of her room about their personal lives. Completely inappropriate. Have had to ask staff several times for pain meds for MIL, she would be moaning and saying "ow" - nobody would check on her. Would tell staff at desk to get her more meds, they said I had to tell the nurse in a closed-door office. Why is it so hard to identify who is actually providing care? We were told by staff to plan funeral arrangements and they would contact our provider. After she passed, we were told she was a coroner's case and had to go to the Cook County Medical Examiner. We were NOT told this and have no record of this conversation or procedures (but a handout with funeral homes/crematoriums). Staff insisted they told us she was a coroner's case. We have no record. She passed on Memorial Day, the nurse on-site was unprepared for this and kept us at the hospice after she passed to talk to coroner. Did not release us, PCN staff told me it was okay to clean out her fridge and go home. We did not know we would have to talk to the cops, nobody told us that anyone who is seen for a fall and passes within 1 year has to go to the ME. No clue what to expect. More trauma after a traumatic day. We have now had to contact the funeral home on our own and hope they transport my MIL from Cook County to their facility without incident. This has been botched from the get-go. No continuity in training, communication, or actual support for families. The social worker here does not care about the families at all.

Without a doubt, all of the…read moreprofessionals who visit us on a weekly basis are phenomenal people-Alex, the nurses, the CNAs, and the "emergency" times we have needed people here. We get a call when there is a substitute coming, we get a call if someone is running late, we get a callback when we have questions. Medicine is delivered to our door within 24 hours of the request, but usually the same day. The seamless care from everyone is remarkable. When hospice is mentioned, most people have no understanding of what the wonderful benefits are for them. At a time of life when support is crucial, LifeChoice Hospice is there-for my husband and for my family and me. Again, thank you LifeChoice! Peggy T.

We were assigned Emily as our case manager and she was great to work with. However, we interacted the most with Grace who was on-call at the time we engaged them and helped my Dad a good bit during his last couple of days. Grace was super patient with us when we asked lots of questions about his care and hospice in general. LifeChoice also provided a rabbi for us and I think my father really appreciated that the rabbi visited with him for over an hour. I think they provided my father a lot of peace and comfort during his final days. My only regret was not engaging them sooner. Thank you LifeChoice for all of the help you provided my father and our family.
