I offered to fill out although not necessary as I only had a couple more sessions...left THEY INSISTED THAT IF I COULD NOT FILL OUT I WOULDN'T RECEIVE TREATMENT. Further as I became annoyed they offered a print out and quiet room to accomplish the request.Later in a text message, I was told it was a consultation appointment. I did not need consultation...I WANTED WHAT I PAID FIRST...AS INEFFECTIVE AS IT IS TURNING OUT TO BE. Since I have been denied service on a paid for service I am requesting my money back...on services not rendered $1277.31 They wasted my time and so I will not be returning nor wish to debate/ mediate the situation further. I sent a letter to their email site customer care....and received a response saying send to patient care which I did attention officer manager. Additionally they advise that services are...non refundable. However if they refuse or can not provide services,,,which I was told then monies have to be returned. In my opinion this was a HARD CORE PLOY to get you to sign up for something else while they have a captive audience.
