My son is receiving good care while at Sapphire. And every once in awhile, great care depending on the Nurse and CNA attending to him. But hope you never need anything from any other office to help with Dr's info, insurance help, or any other information you may be looking for. No one ever returns a call. Never. It doesn't matter what department or who you want to communicate with, we have never received a call back. Very disappointing. Thanks for responding. But apparently your response is the same to all your reviews which seem to be much worse than mine- call to discuss your issue. That's one of your biggest issues, nobody ever calls you back! We keep calling and no one cares. read more

This place is now called Sandstone Estates on Wilmot Rd. Had to change the name bc of poor reviews is my guess! A CNA is not to change dressings, yet a male CNA changed my loved ones heal dressing causing him to scream. He thought he could yank the dressing and the padded covering off in one fell swoop from the knee to the bottom of the foot. I wrote the chief of nursing and two others who are their top dogs. NO response from any of them! My loved one while still living at home had 10x4 inches skin off the back of his thighs pulled off several years ago and he did not scream at all. So you know this really hurt! The wound nurse said the next day she would change the dressing.... lying to my face. She did not change the dressing. This loved one might have to have a second amputation and this is how they treat him! They also refused him antibiotics prescribed by the VA wound department. He has lost 2 lbs per month since being admitted 36 months ago. Keep this address you will need it! Worst place i've ever worked at, the staff is unprofessional and will believe absolutely anything, If you're planning on working at the Sandstone on Milber street good luck! The most unprofessional, gossipy and laziest people work here. HR and management are so out of touch no wonder so many people have gotten wrongfully terminated and or quit. I was wondering during the time I worked here why so many people have left, now I know. Also not to mention they hire absolutely anybody! Those background checks obviously don't do much. Take whatever there is with this info and again decide if you want to be employed here! read more

So disappointed in the service. When my mother was transferred here for therapy, I thought this would be a good place for her to recuperate. The therapy staff was good, but no one could ever tell me which days were her rest days or when therapy was coming to work with her. As far as nursing and nursing assistants, this facility was so short-staffed that no one came to answer a call light for over an hour at night and often over 45 minutes during the day. I met two different nurses working with my mom who stated that they couldn't answer my questions as "this is my first day." In addition, one of the kitchen staff who came into the room daily to help my mom with the menu would take off her mask if my mom had trouble understanding her. And when I met her, she wore her mask UNDER her nose. I had to ask her to pull it up. She also told my mom she was against vaccines and was unvaccinated which upset my mom. The reason I'm bringing this up is the fact that my mom ended up with COViD after she was there 10 days. Then her meals came in plastic and styrofoam which is totally unnecessary as COVID is airborne/droplet. Trying to talk to anyone from social services was super frustrating as no one ever called me back to discuss discharge plans or medical equipment needed for her mobility. I had to call and demand to stay on the line until they found someone from social services to talk with. I realize that many people left their positions during the pandemic, but the staffing shortage seemed to be particularly bad at this time. Service and communication were almost non existent. read more

This facility is a death trap! My Father was admitted there after being released from UMC. Within 72 hours he was found unresponsive with low blood sugar. This again happened within in another 24 hours after that same incident, which resulted in an ambulance ride to TMC. The emergency room Physician said that when he was admitted, his blood sugar was "dangerously low" which could have resulted in a coma or DEATH. DO NOT CONSIDER this facility for your loved ones, or this could be their DEATH SENTENCE! WHY IS THIS FACILTY STILL OPERATING??? THIS CENTER SHOULD BE SHUT DOWN!!! HORRIBLE MANAGEMENT!!! read more

DO NOT USE THIS FACILITY! I hate that I can't give this zero stars. My grandmother is experiencing punishment for asking for the care she is there for. The staff has taken her phone, her call button, shut her door completely and left her with no way to contact anyone. She asks for food or drinks and doesnt get it for hours, even a simple glass of cold water or coffee in the morning. She is sitting with wounds unchanged and burning, calling repeatedly for change. Same with bathroom calls, she's sitting on dirty pads and in her own filth for hours. This place is completely unacceptable, they are mean, angry, lacking in empathy, and obviously don't care about their work because it's not like this place is bustling, and the patients are not cared for effectively. Any review source you look at, this place has majority bad reviews. Believe them. read more

DO NOT GO TO THIS PLACE. TL;DR: Mom had hip surgery. She didn't get her meds at this place. Had diarrhea for two weeks without care, had her walker sabotaged one night which made her fall. Nurses didn't answer call button for hours. And a nurse straight-up yelled at her. There's other stuff too. Read on if you want more. Long Version: My mother had hip surgery and came to this place because it had a decent rating from her insurance company. I want to be fair about this (it's hard because it's my mom), so I do want to say there were good nurses/attendants at this place. Maybe three to six that helped her. But all of their good deeds were blotted out by the people who did not do their jobs. Just to set the scene: She's got out of surgery two days before with a new rod in her hip. She has some mobility (can get out of bed, walk with a walker to the bathroom), but it's extremely painful to do this. When not recovering from surgery, she's very active and able-bodied. She also takes a lot of different prescribed medications because of previous medical conditions (including cancer). A couple of which are pain meds that she's been on for awhile. I also want to emphasize that this is during COVID restrictions and so no one can visit my mom. No one can pull up to her window to check on her. She's all alone and has to advocate for herself. This list is based on our conversations, some of things here were going on while I was on the phone with her. So, here's the list: The BIG STUFF: --So the biggest issue, and this alone should give anyone pause, was that the meds my mom normally takes, prescribed by her GP and oncologist, and has taken for years WERE NOT GIVEN TO HER. FOR DAYS. My mother would complain and tell them that she takes these drugs and they would tell her that they're not on her list and so she'll have to wait for the doctor fix it. They kept telling Mom that they told the doctor but never heard back. For at least three days, maybe four, my mother did not have her medications. She could not sleep. It's dangerous to stop some of these meds cold turkey. Yet, no one would follow up or seriously attend to this. --She was told that she couldn't see the doctor on call because he wasn't her doctor. He refused to see her for this reason. My mother was under the impression her doctor was on vacation. But when she spoke to him the day before she finally left this horrible place, he said he wasn't and that he hadn't heard about any of this at all. --They also would not call her doctor or give her meds when she had diarrhea for FIFTEEN DAYS. They wouldn't give her Immodium, an OTC drug, that they did eventually give to her when they finally addressed the problem. FIFTEEN DAYS. --One of the biggest issues was lack of attention. Nurses/attendants not answering the call button. I was on the phone with my mother for two hours and she told me that she'd pressed her call button an hour and half before I even called. This happened over and over and over again. She needed her pain meds. They would tell her they're coming to give them to her and then it would be from two to six hours later before they arrived to do so. --Staff leaving things across the room. For example, my mother asks for a cup of water. They bring it to her, maybe fix her bed or do some other task, and then leave, and her water is on the other side of the room where she would have to get out of bed, grab her walker and painfully roll over to get it and come back without falling or spilling the water everywhere. Which she did fall. --So, after a lot of this happened my stepdad got on with the facility and insurance company and complained (tore them a new a-hole) and they started to get a little better. But right after it happened, one of the nurses YELLED at my mother for telling her husband. Straight up yelled at her. --Might've been the same nurse, but one night my mother asked for water, nurse came in, gave it to her. Mom went to sleep later that night and had to get up to go to the bathroom and as she was walking with her walker she fell because THE NURSE WRAPPED THE CALL BUTTON AROUND THE HANDLE OF THE WALKER which yanked back from underneath my mom and sent her plummeting to the floor. Remember, she's in for hip surgery. SMALL THINGS, or, things you'd expect to be taken care of even in a bad hotel. ---No utensils given with lunch/dinner. And it's not like they answered the call button for her to get some. --Misinformation (this a bigger one). One nurse would tell her it was okay to move in a certain way (like put her leg underneath her) and then her PT would come in shocked that she was doing that. --Pillows given to her (which she was using) were then taken away. --TP was not refilled. NO TP! I am not in the habit of giving bad reviews, but this was an outrage and disgusting by professional standards. The board of directors need to do some serious soul searching as far as its patient care. DON'T GO HERE! read more

Not good place to leave your family members there!! specially people for the covid -19 they don't have enough staff, they where rude to my dad he couldn't move they will throw there medicine and water and leave it there knowing that he couldn't get it himself. They didn't give them the medicine on time, call him Mexican, this is horrible place to recover. no communication also if you call a lot of time and there very rude, they ignore your calls. because we had to call from another phone so they can answer. I DO NOT RECOMENT THIS PLACE AT ALL. read more

I worked for this company. Do not trust them, go somewhere else. They treat their staff and their patients absolutely terribly. They are constantly understaffed, several employees were threatened to have their licenses reported even though they have done absolutely nothing wrong. They continue to take on more patients when they do not have the staff to do so, and no one is willing to help those on the floor when things get rough. Please don't do this to your love ones. It was heartbreaking working here and I am so glad to be going somewhere else. read more

I just discharged from Sapphire Estates after an extended stay (40 days). I was there due to falling from a ladder and dislocating my elbow and fracturing my tibia. Both pre and post operatively I was on the first floor of the west wing. In spite of the restrictions in place due to COVID-19 I was pleasantly surprised by the care and compassion given to me by the day to day staff responsible for my well-being. The Physical and Occupational therapy staff made sure I never gave up and that I did my exercises. They cheered me on and celebrated each success! They definitely motivated me and got me out of my funk and into a positive headspace! The wound care team was gentle and kind as they daily changed my dressings. My surgeon was impressed by how good my incisions looked when I went to my follow up appointment. I attribute this to the excellent care they gave me. Both the day and night team of Nurses and CNA's made sure my medications were received in a timely manner and that my daily needs were anticipated and met. From fresh linens to cold water, help with tasks I could no longer perform by myself due to my injuries they took wonderful care of me. In addition to doing their job, they offered compassion and would talk with me and lift up my spirits when their busy schedules allowed. I also must mention others who were integral to the smooth operation of the facility. The housekeeping team, the kitchen staff, and the transportation crew. The housekeepers made sure my room was always well maintained. The kitchen staff, led by Chef Robert, took obvious pride in the food they made. The transportation team made sure I was never late to an appointment and they took care to avoid potholes and routes that would cause delays. Finally, I would like to acknowledge those behind the scenes who worked with my husband to make sure both my check in and check out went smoothly. I appreciated the compassion visits that they allowed my family, so I wouldn't be so lonely. Thank you from the lady in 112/120! read more

ABSOLUTELY DO NOT trust this place with your loved ones care!! I'd give them ZERO stars if I could. They are the worst! My Mom was a patient at Sapphire for 10 days. *Call lights were not answered for up to an hour....or completely ignored. *Understaffing all around( especially on the weekends) I'd walk around the building looking for a tech or nurse and couldn't find anyone. *Most staff is rushed or completely disconnected. *Gross negligence resulting in physical injuries that include diaper rash, open wounds and bruising. *Unwillingness to provide food for patients specific medical needs. Literally the grossest food ever! *They did not give her medications as prescribed. Many times we had to ask MULTIPLE times for them to bring meds or change pain patch. Often times it was never done. *On site Dr only saw her twice in 10 days resulting in a delay of her care and recovery. *She was there for REHAB but it took 4 days for her to even get an evaluation by PT. Our family had to take turns staying with her every day because we didn't feel comfortable leaving her there alone. We brought in her meals ourselves. We had to constantly ask for her meds. I also took it upon myself to change linen, clean feces off of the faucets, clean up and apply moisturizer balm and do other things that I feel the staff should have done. I had to move her to a new facility where she's now getting the most incredible care! read more

PLEASE DO NOT ALLOW YOUR LOVED ONE TO SPEND ANY TIME HERE. My 95 year old dad was sent here after two weeks in the hospital. The people who did physical and occupational therapy were pretty good and kind to my father. And the food was OK. Beyond that, trying to get someone to talk to you is almost impossible. And when I did find a nurse or a caseworker, they acted like I was imposing on them. The employees all seem angry. When I asked how to get help from the nurse's station, I couldn't even get one of the girls to look up from their computer until I got angry. Dad was given a laxative for an unknown reason for days that gave him terrible diarrhea. Even when I asked them not to give it to him, they continued. Only when dad absolutely refused to take it were they forced to not give the laxative to him. Without any warning, they moved my dad from a private room to a semi-private room that didn't even have a working TV. I was told only two days beforehand that my dad was to be discharged. I didn't feel he was ready to go home but, by the time I was told he was being discharged, it was too late for me to appeal his discharge to Medicare. No one cares. About anything. I was able to talk to a doctor who assured me that dad was fine to go and that everything would be in place when he left this place. I was given a discharge packet but with almost no guidance for his aftercare. PLEASE BELIEVE THE PREVIOUS BAD REVIEWS AND GO SOMEWHERE ELSE. read more

My mom was a patient Sapphire Estates Rehab Centre, if I could give this facility zero stars, I would! DO NOT EVER place your loved ones in this rehab facility!! Their level of neglect is criminal!! Call lights go unanswered or unchecked for 45 minutes or longer, it's no wonder they have the highest death rates in Pima County!! Basic personal hygiene care is ignored! It is unacceptable for someone to sit in their feces for an hour or longer! Unexplained bruising appeared on mom and when we questioned the bruising, finger pointing commenced and no one could offer an explanation! The attending physician did not come and evaluate mom until 5 days after she was admitted, delaying her treatment plan. My mom sat in a bed with zero therapy evaluation until day 4, again delaying her treatment. This place is not in the business of rehabbing patients, they are in the business of letting their patients die!!! I will be reporting this facility to the state, city & county! read more

If I could give this dump zero stars I would. I am an former employee that witnessed the pain, suffering and death of patients that arrive with minor issues leave dead. Call lights of patients take hours to answer. Trach patients that wanted to be weaned were denied services because long term insurance brings in more money. I have seen hospice patients get treated so cruelly it brought tears to my eyes. Workers have been escorted our because they were hysterical having been tested positive for Covid 19. No one had been fitted for an N95. They use non disposable PPE gowns. 1/2 the staff has or has had the virus. If your loved one are here, please get them out before they perish. read more

I went to visit a friend at Sapphire. This place does not pass the "smell" test. You can smell poop and pee when you enter the hallway. My friend had a bedside commode, it had not been emptied. She said they empty it once a day No wonder the place smells. She reported, and I got to see in action, that when she pressed the call button for toilet assistance, no one replied. We timed how long it took them to respond, I finally went in search of someone and was rudely told by 2 women chatting at the nurses stations that they were busy- so it took 47 minutes! My friend was weeping she had to go so bad. I left when assistance arrived after asking them to empty to commode when she had finished. This is shameful! read more

One person can make a difference. Sara Johnson showed my Dad kindness and personal attention. She made him feel like he mattered and like a human being again. Her can do attitude was encouraging and refreshing. When you can't visit with your family you have to rely on the employees of the facilities to provide some human interaction and kindness it is unfortunate many of the employees just don't see to care. Sara Johnson is an example for others to emulate. Overall was clean and the food wasn't bad and somebody answered the phone. read more

Our mother received absolutely the best care there while being a patient. She became extremely fond of the Respiratory Staff who checked on her throughout the day and night. Do not bring your loved one here. They literally torture patients. I wish I could leave negative stars. My dad came here after being in the VA hospital and he was put on hospice care. Some of the CNAs have been great and the ONE nurse that I saw ONCE in a week was nice but they haven't been actually caring for my dad. He sits in unchanged adult diapers for hours and when they did change it in front of me yesterday, they pulled off the poopy one and just pulled on a clean one without wiping or cleaning him in any way. I'm trying to get him moved to a different facility but it's complicated and takes time. In the meantime, they are not giving him his pain medications and he's laying there suffering as he's dying. Where are those pain meds going? I'm sure all those oxycontin that he doesn't want to take have gone somewhere. I know he has refused some pills but he's good about the liquid but after hours of sitting with him last night and asking about his medications even the CNA couldn't find the nurse to even tell me. There was no supervisor available. To top it all off, he has a capsasin (hot pepper) cream that is very very warming for his knee but they have been applying this burning cream to open sores from shingles on his neck instead of his arthritic knee. He was in tears from the burning. They have literally tortured him by inaccurate treatment and intense neglect. I chose this because the VA assured me that it had good ratings and they needed a quick decision about placement. How is this place even allowed to be open? They definitely don't have adequate nursing staff and it's extremely dangerous. They don't help the patients in the slightest. They have left my dad, who can't move on his own because he's in his last days sitting up for hours while he's uncomfortable and moaning in pain. When I asked about it they said "well he didn't ask to be moved or for medicine". He clearly is not fully present mentally and isn't able to ask. It's extremely obvious that he's very very uncomfortable. If I cannot get him transferred from this facility today then I am going to revoke his hospice status and put him on acute care so he can go back to the hospital just to get out of here. Please believe all the negative reviews on here and not the big sign in the lobby saying they are one of the best. read more

It pains me to write this review. One star is too much to give this place. It is poorly managed. You are unable to get a hold of any staff member to inquire about your loved one. When you call you are greeted by it's a great day at Sapphire Estates. When being transferred to to the nurses station where my mother was supposedly at I was transferred to a voicemail. I left a message ad the call was never returned. When I asked to speak to a nursing supervisor I was told there wasn't one at this building. I received a call from a social worker who didn't leave a return number. When I called the facility I was placed on hold and eventually hung up because I was on hold for 12 minutes. When my mother was released she told me she fell twice while there and I was never notified. My mother also stated that she would call for her nurse and would wait for sometimes longer than half an hour. Her trays of food were always cold and out of her reach. The noise level in the hallways was intolerable. This was a place of pure torture and confusion. It is not a place of healing like the website states. I am going to file a formal complaint with Caremore in regard to this. When I have called wanting to speak to someone in charge I have been told the Director is based in Las Vegas. read more
