We were nothing but pleased with her stay at this center in so many ways. My mother felt safe and well taken care of. Her day and night nurses were so sweet and caring towards her and the aides were fantastic in helping her in so many ways. Peggy was her day nurse and Krishna was her aide. The night nurse (sorry I can't remember her name) and all the other nurses & aides were very attentive and soft and gentle with her. They kept us (my family) well informed with her progress and that really eased our worries. Rehabilitation services were awesome! They worked with my mom pretty extensively and was very motivating. They did an initial assessment that helped encourage my mom on specific goals and motivated her towards achieving those goals plus more. They worked with her twice a day - PT and OT. She was in the center for two weeks, towards the end of her stay, she was wheeling and walking (w/walker) to the PT room on her own! They really gave her the confidence to improve and recover that she really needed. My mom is a total introvert. The center provided many group activities. My mom was completely uninterested. I wish she would of tried. She stays alone a lot (with my father & family) and not really comfortable around people should doesn't know. We (the family) spent as much time with her as possible. The staff was so awesome in welcoming us even when my niece would play the guitar for her granny. It meant a ton for my mom to have her safety net of family around her during this time. The food was good - coffee delivered throughout the day. My Mother was very picky on what she was eating - she really doesn't have taste-buds (for many years); the staff would often ask her what she wanted to eat and bring that. I would often come visit during my lunch break and we share lunch. The food was good. The center was super clean; absolutely not issues. Visiting Physician, Care Managers, - all great. More than anything that staff was really great.. I want to thank you for making my mom feel safe and secure during this scary time for her. You all did a fantastic job helping her to recover. She is at home now and stronger than she was before her fracture. We believe this fall was a "eye-opener" to getting her physical health back on track. She attends outpatient physical therapy twice a week and is 100% off the wheelchair and about 80% off the walker. She uses the walker when she's out in public just for a sense of security. Because she had a stroke (about 7 yrs ago) her balance will never be a 100% but with the rehabilitation she is definitely stronger; so much stronger. This was all possible because of Bear Canyon. So thank you for your diligence and hard work and caring staff. She is doing this good because of your work with her. I am so appreciative in so many ways and so glad we chose your center. Huge appreciation.

At the same time my mother was in hospice for metastatic breast cancer. I had to divide my time between both facilities. Half the time there was no one at the front desk to allow me entrance to the facility. We would have to wait for someone to notice us standing at the door. On one occasion I was there early enough to actually see the "physical therapist " in his room. She placed a weight in his right hand and told him to lift it. He was paralyzed on the right side so she just removed the weight and said ok for today. The next day I was told that he had fallen out of bed during the night and found the next morning. The head nurse told me that they placed a mat next to the bed in case he fell out again. There were no rails on his bed and they could not explain why rails were not supplied. After I filed a second complaint about the lack of care I received a call about 5:00 am to be told he was being transferred to a hospital because he was unresponsive. They could not tell me which hospital he was being taken to. After multiple phone calls I located him at Lovelace downtown. When I arrived he was receiving CPR. He passed away at 9:11 am after several attempts at resuscitation. I was told that he was heavily sedated that might have been a contributing factor. I was not given an option of nursing homes when he was released from his initial hospitalization, but Genesis' reputation is terrible. Please don't allow your loved one face such irresponsible treatment.

It seems like the people that work here are fed up with their jobs and just want the residents to rot in their beds. My mom had to stay in this hell hole for a month, which was gravely short staffed and over worked. Violeta a female tech, made fun of my mom in front of her face for not being able to walk. My mom suffers from a neurological disease that causes her to basically become paralyzed. Because she was unable to walk, Violeta made fun of her for that, calling her dramatic. My mom was often left soaked in diapers for hours which gave her a uti which they tested her for and two weeks later her results came back "negative" but then they realized it was "positive". She ended up getting a bed sore that my mom's friend had to point out and Manzanos response was that they won't do anything about it until it's a stage 3 bed sore. This place is a money hungry hell hole that lies to their residents. They quoted my mom $250 for a transport and once the transport was done they said it was $450. I would highly recommend all the people with their negative reviews to report this place to Ombudsman 18664512901. There have been a few CNAS at this place who were compassionate and amazing. Those nurses are Anastasia, Destiny, Shana & Mabel. Kitchen staff and food was great.

This review is specifically for the Las Palomas Center. All three previous reviews (April 2015, July 2018, and November 2019) rate this Rehab & Long Term Care center as a one (1) star facility. It was even mentioned that they would rate the Center as a zero (0), if they could. Needless to say, none of the reviewers were pleased with the care that they, or their family member, received here. I have a long-time friend who was first in the Rehab section and eventually the care she needed required her to be moved to the Long-Term Care section. Having visited there for a couple of months now, I would tend to agree with the previous three reviewers. The care is mediocre to severely lacking. I have met a couple of caregivers who genuinely seemed to be working in the best interest of the patients; but I have also seen a lack of empathy by some for their charges that bordered on mistreatment. These patients are primarily old and ill. They have lived full lives and now either do not have family who are in a position to physically care for them, or no longer have family nearby. Every time I visit, I hope and pray that the parent company - Genesis - looks at these people as more than $$, but as human beings who have contributed to the greater good of our world. I am giving them 2 stars, because I am hoping that Genesis steps up and corrects the areas that are lacking. P.S. It is a beautiful facility. Lets hope I will be able to update this review in the future to say the care is now "beautiful." In the meantime, if your loved ones need to be cared for, even temporarily, check out other facilities in your neighborhood.

He is beginning to walk again at 102 years old, his mental state is far better, as are his cognitive abilities. Woodmark administration is lousy, non-caring, non-responsive, non-accountable. I unfortunately left my father there for one year, believing he's self injured as I was told until he himself told me that he will not move or ask for anything because they hurt him, and he is scared for his life. I quickly removed him before they killed him, they demanded the next months rent anyway. I called APS and they failed to comply to their recommendations. Woodmark feels to distribute medication's timely and left my dad without morphine after being prescribed for injuries, they caused. Heartless! They do not chart properly, and his injuries do not match up with those that Hospice came in treated. I trusted them, and all they did was lie to me. I am so glad that I remove my father and put him somewhere where he now enjoys life. When I attempted to contact their corporate heads, which are national, they do not respond. It is their culture. This is their culture. It's a horrible money making machine off the backs of the helpless.
