I am blown away at how much he has changed in the three months he has been there. I did a lot of research on Rehabilitation Centers; including conducting many interviews. I only found a handful of facilities that I felt genuinely were in business to help people as opposed to making money off of desperate people. I let my adult son choose from these and he chose wisely CTR based on his personality. It turned out to be a lovely open place with really good energy. I felt great about his choice! He is very happy he made this choice and calls it a "great place". He has replaced the substance abuse with maintaining a healthy body and mindset. He still knows that he has to keep this momentum going and has taken the next step by entering into sober living. My son can be a hard one to crack and they didn't crack him; they got him to want to be sober and to love it. That doesn't mean that it was easy for him. It wasn't; especially at first with the withdrawal symptoms and mood swings that occur for awhile even after detox. He felt like quitting several times, but this is a determined group of compassionate people who were obviously very skilled at keeping him on track. I think one of the biggest problems with many rehabs is that they are not compassionate for the people they are serving. CTR treated my son with dignity. I can't thank everyone at Circle tree enough. Thanks to Karla. Roki, Eric, Mark, and everyone else there that touched my son. You are all angels to me.

Some of the therapists are amazing, others not so much. It is very expensive, and the financial side of the facility is unprofessional and rude. There was major miscommunication and they outright ignored my multiple calls and emails before sending my account to collections after insurance paid their part. I learned that in the second half of 2025, major corporate changes gutted the first responder program (called Red, White & Blue, or RWB), and they increased the patient to staff ratio to unsafe levels. So very sad. It's a gorgeous facility and the clinical staff is exceptional. So while my healthcare was fantastic at that time, I'd be wary and look at other options, especially veterans and first responders, as that program has been gutted.

I went there for help with severe chronic pain, depression, and trauma, and I left feeling much worse, not better. From the very beginning, I experienced a lack of professionalism and care. I was picked up from the airport by a staff member who immediately asked me if I smoked so he could smoke on the drive. The van smelled heavily of cigarette smoke the entire hour-long ride. That was my introduction to a place that claims to prioritize health and healing. My first night, after finally falling asleep around midnight, I was woken at 2 am when a staff member turned on all the lights and loudly announced I had a roommate. I woke up in a full trauma response. I was there to heal trauma, not be retraumatized. Within the first couple of days, I spoke with staff to try to arrange to leave. There were so many red flags within the first 24 hours, and it was pretty clear that this was not going to be a healing experience for me. I wanted to leave. They quickly arranged a meeting for me with one of the higher ups and he manipulated me into staying, taking advantage of my vulnerable state. If I had been in a better state of mind, I would have booked a flight home immediately. The environment was chaotic and disorganized. My toilet ran nonstop for over a week despite multiple reports. Staffing was inadequate, and there were many times I needed support and had to wait hours to speak to someone. The scheduling was a constant issue and incredibly stressful and exhausting. My schedule was almost always wrong, so every morning I had to go to the scheduling desk to get it corrected. I often had to wait because the staff were almost always late, and when they did arrive, they were unfriendly and unhelpful. I waited for appointments that didn't exist and missed ones that were never properly scheduled. I was told before coming that I would receive EMDR, which was one of the main reasons I chose this facility. After weeks of asking, I was eventually told I would not be getting it at all. I submitted many written complaints during my stay, and nothing was ever addressed. There was no accountability or follow-through. The financial aspect was incredibly misleading and exploitative. My family paid $45,000 for my stay. Only after arriving did I learn that many of the "integrative services" presented on their website were not included and required an additional $2,500 to access. By that point, I had already rearranged my life to be there for a month and had already committed financially, so I reluctantly paid the extra money. The additional services were not worth it. It was a bait-and-switch. There were also serious issues with professionalism and basic respect. My belongings were damaged, including my ukulele. A $45,000 check from my father was lost for over a week. I also overheard staff gossiping about me which was shocking, emotionally painful, and deeply inappropriate in a treatment setting. My experience with my individual therapist Minnie was very unprofessional and harmful. She was unkind and unsympathetic and shared personal information with my father without my consent, including information that was completely untrue, which caused serious harm in my family. There were many situations that were actively retraumatizing rather than therapeutic. During my stay, a patient died by suicide in a room just a couple doors down from mine. One minute I was sitting across from her at dinner, and the very next morning I found out that she had hung herself in her room the night before. That alone speaks to the level of safety and support in the environment, and it had a profound emotional impact on me while I was already in a vulnerable state. Even the programming did not match what was advertised. Even the programming did not match what was presented. For example, "equine therapy" consisted of a single brief session where we all stood around and briefly touched a horse. That was the extent of it. When I got home, I immediately wrote to Sierra Tucson and provided detailed documentation of everything that had happened and asked for my money back. I did not receive any resolution. Overall, I spent a significant amount of time trying to advocate for basic aspects of my care while dealing with chronic pain and emotional distress. I spent an enormous amount of time trying to fix issues that should never have been my responsibility. It was incredibly frustrating and exhausting. Instead of being able to focus on healing, I was dealing with constant stress, instability, and emotional harm, which made my condition worse. People come to places like this when they are at their most vulnerable. What I experienced was not just disappointing, it was deeply harmful.

The rooms are not stocked with basic linens and towels. There are no slide boards to assist moving pts from a chair to the bed. The night staff are no where to be found, there was a 3 hour delay in response from nurses. Healthcare staff do not follow safe medication practices-not following the 5 rights principles. They did not use two identifiers when meeting the Pt for the first time or any time. This same practice was repeated by nurses, CNAs, MD. There are no boards in rooms to identify nursing staff, treatment goals, etc..The beds are too large to fit through the door-this is a fire hazard. They were disorganized at intake-pt had been in bed for 5 hours before they started the admit process and several staff did not know he was there. The staff has been dismissive-making false promises and lying to Pts. There has been no Medicare annual inspection since 2023. The evening meal was 2 slices of bread, one slice of processed turkey and a small amount of frozen vegetables-not sufficient for the Pt to maintain their nourishment without family members supplementing the food. This facility is run by a for profit organization but it is clear the money brought in doesn't go into the facility. Someone from the state needs to intervene quickly. The staff need remedial medical training, the facility needs serious upgrades

I had heard…read morethe horror stories about this place, both in reviews and direct from previous patients, but it was the only facility my employer's insurance would pay for. The doctors here disregarded my prescriptions from my surgeon, deciding they could alter them without examining or speaking to me. As a result, my pain, which had previously been managed effectively, exploded. Rather than keeping me on a set schedule and receiving my three different pain medications simultaneously, they arbitrarily decided that I have to explicitly ask for each of them separately, at least an hour apart. They also lowered the dosages while increasing the frequency. This is especially problematic overnight, as I don't want to set alarms that will wake my roommate 5-6 times throughout the night, so I have to rely on my pain to wake me, which means I end up waiting too long between doses, which makes the medication less effective at controlling it. The nursing staff is rude and argumentative. They gave me a wheelchair that was cobbled together out of parts that don't fit or lock in place properly, and the thermostat is set to 78 degrees. The splints wrapping the lower halves of my legs were already causing me to be warm in the typically cold hospital. Here I'm sweating my balls off, but I'm somehow still supposed to keep my splints clean and dry? Don't go here if you have any other choice. EDIT: Every nurse here has a different set of rules as to what medications I'm allowed to take, and when. Every time there's a shift change, the new nurse tells me they can't do what the last shift insisted was how I had to do it. One nurse says none can be taken together, and there must be at least an hour between different meds. One says two can be taken together, and there must be at least two hours between doses in the rotation. One says I'm only allowed one of them every four hours. Nobody knows wtf is going on.

    this skilled nursing facility /rehab & long-term care / nursing home is horrific in every way! the…read morecare from CNAs, nurses is a travesty! personal care, if one is confined to bed and cannot go to the bathroom themselves, they are changed by an aid a CNA actually. that stands for certified nurse assistant! where they got trained I do not know!?, as each and every time, and each and every cna does not know how to do this properly. urine on your butt can cause bed sores which can be extremely serious! if they do use the wipes, they never and I mean ever wipe the cheeks on your butts! also, there were a few times where the CNA decided not to use any wipes at all! when I pointed it out to her, she was defensive. I watched how they rolled the chuck/pads under you and the brief how they taped it and how you rolled from side to side. so, from there on, I changed myself! they may not do personal care for me any longer! it boggled my mind! I was a patient there, I'm in a wheelchair for 10 years now but I broke my leg in a car accident and had surgery. so this is what I experienced above. it's difficult to write this in a review. but people need to know the personal care, and how they do not care! I am compassionate and realize they have many other patients, too many in fact, and some CNAs and nurses work double shifts. it has become the last couple of months, like a revolving door. people are quitting and I'll see a new face each and every day! this is difficult on the patient even though I do feel sorry for the employees. they should be paid more. also, they should be able to hire more medical professionals so they do not have to have 25 to 30 patients for one employee! therefore, the patient suffers as your call button will be going for one, two even 3 hours without help. suppose you're choking? suppose you're having a heart attack? after all, we are in these rehab facilities after coming from a hospital from many illnesses, falls and fractures, some are on oxygen etc. secondly, they seem to only care about money, about filling a bed! the meals are awful! they come cold, & every day three times a day the trays and the silverware are incredibly filthy! if you are one of the 10% of elderly that 'speak up', advocate for yourself, your own body and mind. watch out! they do not like that. so be very careful. it's like walking on eggshells. I lived it for 7 and 1/2 months. I didn't even have a way or the time to grieve for my husband that passed away only months before I entered on June 4th 2025. I left January 16th 2026. if you have a loved one, please do NOT bring them to Park Avenue health and rehabilitation Center. do not be fooled by fancy decor, as chandeliers and such as you walk in the front door..( at any place). looks are deceiving. and you will be greeted kindly by the staff and office people. you have to visit your loved one or be a patient moreover to know what you got yourself into! if you really love your loved one you will be aware of where you are taking them to live or to heal. and never sign anything before reading every word!

It should be shut down, and I have filed a formal complaint regarding the care and treatment my mother received. While at this facility, my mother--who was unable to walk--fell out of bed, hit her head, and had to be transported to the hospital. Their approach to patient hygiene is appalling, as they simply spray patients down instead of providing proper bathing. There is no respect for modesty, compassion, or basic human decency. I have attached a photo of my mother's feet as evidence of the neglect she suffered. When I saw the condition of her feet, I immediately went to the administration office and brought a staff member to witness it. Only then did they take action, calling someone to medicate and wrap her feet. Tragically, my mother passed away shortly after leaving this facility. In my opinion, the poor care and negligence here contributed to her death. No one should have to endure what she did.

I was somewhat worried after reading some negative reviews in Yelp, but my experience was a good one. The staff were very cheerful, friendly and helpful, especially Nicole and Mila. Anytime I needed help, medication, or had questions, the staff quickly responded at the push of a button, day or night. The food was varied and generally good and nutritious. The kitchen gave us two options for our entree, and allowed us choices of beverages and dessert. We were given 2 showers weekly, along with plenty of towels and toiletries.The shower was clean and I never saw any cockroaches or other insects. My room was cleaned daily and my bed linens were changed twice a week in contrast. I had therapy in the facility gym twice daily Monday - Saturday. My two main therapists, Andy and Dave, were kind, patient, and helpful. My leg was non-wait-bearing, so they showed me how to use a walker with one leg, and taught me how to use a wheelchair to get around. They also demonstrated some "hacks" for getting around and give me a list of excises I could do on my own. Foothills Rehabilitation Center thoughtfully provided tv headsets on request so my roommate could sleep and not be disturbed by noise from my television. Finally, there is an activities center where residents can meet people, play games, have sing-alongs near the gym. The only down point was that it was quite cold in my room the first few days of my stay. However, I requested extra blanketsand I was fine from then on. All in all, I was extremely pleased with my stay there, and highly recommend the Foothills Rehabilitation Center.
