They were caring and compassionate and spent the necessary time needed to answer any questions or to just put my uncles mind at ease that she was being looked after. Colby was exceptional at gently talking with him and explaining her decline. Fast forwarding to this past week, my uncle who started off 2026, in the hospital with some unexpected issues, declined quickly. After 12 days he came home on hospice and our logical choice was to use With Grace once again. We were blessed by the staff, but Colby in particular. She checked in on him daily and would gently talk with him and play calm music for him during her visits. When he became non-responsive she would continue to check in on him to monitor each stage and to make sure that he was comfortable. I appreciated her communication with me, our caregiver, and her amazing and calm approach. Each step of the process and engagement with their staff with both my aunt and my uncle, were exceptional.

My daughter and I researched numerous facilities in and around Morgan Hill, California, for many months. We were looking for the perfect Assisted Living residence for my 90 year old Mom. A large part of our decision to select Westmont of Morgan Hill was due to the confidence we felt in its Nurse Director, Ria Hernandez, to deliver the level of care and service we were looking for. Ria does an amazing job of keeping her Care Team functioning at peak performance in providing excellent care and genuine concern for my Mom and all the residents at Westmont. We are at the facility several days a week and are so delighted that Ria and her team are constantly checking in on Mom to make sure that she is comfortable and is receiving all the services she needs to feel safe and "at home." The facility is very clean and well maintained and we especially appreciate all of the creative, decorative touches the team provides at each and every holiday event. The residents there all seem happy and well cared for. It's like one big, happy family! We gladly refer friends and relatives to reach out to Ria if they are contemplating moving a loved one into Assisted Living or Memory Care.     This review is for in home PT services with Saher. Saher visited our home to work with my two…read moreelderly friends. I oversee their home and care. Saher is very caring, but her shortcomings far outweighed her attributes. She admitted to being disorganized; claiming that remembering appointments and important rules of our home were too difficult for her. With indoor pets and two elderly residents; safety is of the utmost importance. The rules of our home are set up to maintain safety and are very simple. She was often unfocused and spent the scheduled appointment time socializing rather than providing the services for which she was hired. I was greatly disturbed by her care of my elderly friend who is physically the more compromised of the two. She asked him to help her set up a heavy and cumbersome massage table, compromising his safety! This was very dangerous for him and she was aware of his limitations prior to requesting his assistance. With her expertise, she should not have enlisted his aide for such a task. If she cannot manage such tools independently, she should not be using them. The most upsetting incident that happened with Saher is when she let my elderly friend fall WHILE doing PT with him. This was irresponsible and negligent. Her scope of duties is to ensure that the exercises are done as safely as possible and within the reasonable ability of the client/patient. Upon subsequent visits she started soliciting me to help with the exercises. I am not a trained Physical Therapist and I was busy with my own tasks. She should have brought help, planned and executed more appropriate activities and/or avoided exercises that required two people to manage him. Lastly, we received numerous invoices 5-6 months after we had ceased working with her. She was astonishingly disorganized with her paperwork; which caused me an unreasonable amount of extra work to research and cross reference her invoices. They were out of order, some were already billed, some were unclear, some were from her forgetting to bill. It was very confusing. It saddens me to have to leave this review. Unfortunately, the care we received from Saher was not the quality that I expect from an out of pocket PT and I ultimately found her to be unsafe for my friends. That in conjunction with her unprofessionalism has prompted me to leave this review.

In late October of 2025, our family was scrambling to find a hospice care provider for my mother in law. Finding a reliable hospice provider for a loved one is a daunting task often made under stressful circumstances. Our family, after extensive searching and disappointing recommendations from other sources, found Sonata Hospice through positive online reviews on Yelp. This review comes from a genuine family who utilized their services for a critical 10-day period. The level of care provided by the entire Sonata team to both our loved one and our family was amazing and far exceeded our expectations. We felt completely confident that our loved one was in excellent hands, and we as a family felt fully supported throughout this difficult process. Several team members deserve specific commendation for their outstanding professionalism and genuine compassion: Rachael (Nurse/Case Manager): Rachael was outstanding. Her work is a true blessing and a calling. She provided expert clinical care while simultaneously offering immense comfort to our family. The attentiveness she provided our loved one was consistent with the level of care one gives their own family. Her clear communication, explaining procedures, answering questions, and involving us in the care plan was a model of professional and empathetic care giving. Marsha (Home Health Aide): Marsha treated our loved one with dignity and affection, like she was caring for a member of her family. Her hands-on care was superb. Maria (Chaplain): Maria brought much-needed peace, love, and spiritual support during a challenging time, helping us navigate the emotional and spiritual aspects of the process and providing our loved one with that extra level of care. The Office and After-Hours staff were consistently responsive and efficient in meeting all our immediate needs. The support they extended through the difficult moment when our loved one passed, provided ease and compassion during our time of grief. And our hearts were warmed when at her funeral we saw Rachael and Maria present and still supporting us. Deciding on end-of-life care is perhaps one of the most significant decisions a family can make. Concerns about the quality of care, trustworthiness, and prioritization of patient well-being over financial considerations are paramount. Sonata Hospice put those fears to rest. They are a trustworthy, compassionate, and highly professional organization that we wholeheartedly recommend to any family navigating the hospice journey. I just wanted to share how grateful we are for the support we received. It was mostly my aunt having to take on all the complicated decisions but the hospice employees provided compassion and support. The chaplain, Maria was very calming-never pushy, just compassionate and loving during a really overwhelming time. The social worker, Melinda was also wonderful, helping us navigate everything with so much patience. My aunt commented on how after her visits, she felt happier because of her positive presence. Thank you Sonata hospice, this experience was hard and I especially worried for my aunt but the team made the experience much more bearable. Both of them made a difficult situation feel a little lighter, and their kindness made a real difference for our family. We truly appreciated the care and respect they showed every step of the way

    My elderly relative got sent here after a physical injury. I had reasonable expectations going in,…read morebut this place was bad. Patient was depressed, crying daily and upset by lack of care/professionalism/responsiveness/etc. Everything from simple questions to getting help being rolled down the hall were quite difficult. Front desk and some of the workers were helpful, but not consistent and real lack of communication among parties. Real issue is the Patient Coordinator we had assigned - Juana. I found myself multiple times asking myself what here motivations were (from money, to laziness, etc) for why she would be doing certain things or communicating the way she did to me. She tried to discharge the patient early multiple times without important test results which would then later show complications (like pnemonia that developed) and ignored concerning medical results multiple times and didn't even follow up to understand if results had been received or communicated. She didn't really "coordinate" much as we were told to figure out any transportation needed, followed up with doctors directly for information she had, followed up directly to get any medical records needed, medications, etc.) She also lacked professionalism/ethics by being told we were going to protest a discharge and then proceeded to try to get the patient to sign forms that she was explicitly was told the patient would not be signing. As the rooms are shared (3 patients per room) I also saw similar observations with others. One patient developed sepsis and I witnessed the family member crying and pleading for help as the center took over 6 hours to try to have an ambulance coordinated to move the patient properly. It was hard to watch. All I can say is if you end up here visit often and be prepared for a fight for basic care. I was there multiple times a day to assist my family member and still found it difficult to get basic answers/care. Glad we got out and have moved on.

She is an expert in what one might need - something that we really appreciated as this was new space we were navigating. Grace has a well trained team who are compassionate, lovely and gracious. She is impeccably organized, creating a month-long calendar and schedules so that your loved one will have as few disruptions as possible - providing a consistency in which only a few caregivers are chosen for your week or month. This was a stressful time with lots of unknowns (including financially) and all we wanted was to ensure the best care for our loved one. Grace will make it as straight forward as possible and provide care givers that you and your loved one will love. Although our family is moving out-of-state, I hope to find another care agency with as wonderful a team as the one here with Advanced In-Home Care.
