They were caring and compassionate and spent the necessary time needed to answer any questions or to just put my uncles mind at ease that she was being looked after. Colby was exceptional at gently talking with him and explaining her decline. Fast forwarding to this past week, my uncle who started off 2026, in the hospital with some unexpected issues, declined quickly. After 12 days he came home on hospice and our logical choice was to use With Grace once again. We were blessed by the staff, but Colby in particular. She checked in on him daily and would gently talk with him and play calm music for him during her visits. When he became non-responsive she would continue to check in on him to monitor each stage and to make sure that he was comfortable. I appreciated her communication with me, our caregiver, and her amazing and calm approach. Each step of the process and engagement with their staff with both my aunt and my uncle, were exceptional.

And, his nurturing presence made such a huge difference as the days wore on and became more difficult. I don't know how Dindo is able to retain and exude joy and caring in such a difficult profession -- the fact that he can hold so much and help during the most harrowing times in such an impactful way is astounding. My daughter and I researched numerous facilities in and around Morgan Hill, California, for many months. We were looking for the perfect Assisted Living residence for my 90 year old Mom. A large part of our decision to select Westmont of Morgan Hill was due to the confidence we felt in its Nurse Director, Ria Hernandez, to deliver the level of care and service we were looking for. Ria does an amazing job of keeping her Care Team functioning at peak performance in providing excellent care and genuine concern for my Mom and all the residents at Westmont. We are at the facility several days a week and are so delighted that Ria and her team are constantly checking in on Mom to make sure that she is comfortable and is receiving all the services she needs to feel safe and "at home." The facility is very clean and well maintained and we especially appreciate all of the creative, decorative touches the team provides at each and every holiday event. The residents there all seem happy and well cared for. It's like one big, happy family! We gladly refer friends and relatives to reach out to Ria if they are contemplating moving a loved one into Assisted Living or Memory Care. This review is for in home PT services with Saher. Saher visited our home to work with my two…read moreelderly friends. I oversee their home and care. Saher is very caring, but her shortcomings far outweighed her attributes. She admitted to being disorganized; claiming that remembering appointments and important rules of our home were too difficult for her. With indoor pets and two elderly residents; safety is of the utmost importance. The rules of our home are set up to maintain safety and are very simple. She was often unfocused and spent the scheduled appointment time socializing rather than providing the services for which she was hired. I was greatly disturbed by her care of my elderly friend who is physically the more compromised of the two. She asked him to help her set up a heavy and cumbersome massage table, compromising his safety! This was very dangerous for him and she was aware of his limitations prior to requesting his assistance. With her expertise, she should not have enlisted his aide for such a task. If she cannot manage such tools independently, she should not be using them. The most upsetting incident that happened with Saher is when she let my elderly friend fall WHILE doing PT with him. This was irresponsible and negligent. Her scope of duties is to ensure that the exercises are done as safely as possible and within the reasonable ability of the client/patient. Upon subsequent visits she started soliciting me to help with the exercises. I am not a trained Physical Therapist and I was busy with my own tasks. She should have brought help, planned and executed more appropriate activities and/or avoided exercises that required two people to manage him. Lastly, we received numerous invoices 5-6 months after we had ceased working with her. She was astonishingly disorganized with her paperwork; which caused me an unreasonable amount of extra work to research and cross reference her invoices. They were out of order, some were already billed, some were unclear, some were from her forgetting to bill. It was very confusing. It saddens me to have to leave this review. Unfortunately, the care we received from Saher was not the quality that I expect from an out of pocket PT and I ultimately found her to be unsafe for my friends. That in conjunction with her unprofessionalism has prompted me to leave this review.

In late October of 2025, our family was scrambling to find a hospice care provider for my mother in law. Finding a reliable hospice provider for a loved one is a daunting task often made under stressful circumstances. Our family, after extensive searching and disappointing recommendations from other sources, found Sonata Hospice through positive online reviews on Yelp. This review comes from a genuine family who utilized their services for a critical 10-day period. The level of care provided by the entire Sonata team to both our loved one and our family was amazing and far exceeded our expectations. We felt completely confident that our loved one was in excellent hands, and we as a family felt fully supported throughout this difficult process. Several team members deserve specific commendation for their outstanding professionalism and genuine compassion: Rachael (Nurse/Case Manager): Rachael was outstanding. Her work is a true blessing and a calling. She provided expert clinical care while simultaneously offering immense comfort to our family. The attentiveness she provided our loved one was consistent with the level of care one gives their own family. Her clear communication, explaining procedures, answering questions, and involving us in the care plan was a model of professional and empathetic care giving. Marsha (Home Health Aide): Marsha treated our loved one with dignity and affection, like she was caring for a member of her family. Her hands-on care was superb. Maria (Chaplain): Maria brought much-needed peace, love, and spiritual support during a challenging time, helping us navigate the emotional and spiritual aspects of the process and providing our loved one with that extra level of care. The Office and After-Hours staff were consistently responsive and efficient in meeting all our immediate needs. The support they extended through the difficult moment when our loved one passed, provided ease and compassion during our time of grief. And our hearts were warmed when at her funeral we saw Rachael and Maria present and still supporting us. Deciding on end-of-life care is perhaps one of the most significant decisions a family can make. Concerns about the quality of care, trustworthiness, and prioritization of patient well-being over financial considerations are paramount. Sonata Hospice put those fears to rest. They are a trustworthy, compassionate, and highly professional organization that we wholeheartedly recommend to any family navigating the hospice journey. Sooo, I don't know how your relationship with hospice started, but here is mine…read more Our loved one was admitted to the hospital with some very serious medical issues. The hospitalist (treating doctor), sent around a palliative care doctor (doctor who specializes in improving the lives for people with serious illness), who explained that our loved one would benefit from hospice services. The doctor went on to explain that it doesn't mean our loved one was going to immediately pass away, but just that hospice, amongst many things, gives more care to people who need it and it was the hospital's opinion that our loved one would benefit from this service. She went on to recommend a hospice service her mom had and left me with "Great!", we'll check in tomorrow and let us know who you selected. "Wait, whut?!" So I only get *24 hours* to select the service providing critical care to my loved one? So with that, I was off to find a hospice service, but not confident it would be *the best* hospice service, if only given 24 hours to find them. I was lucky enough to think of someone I trusted (the Executive Director of our loved one's care home), who immediately provided me with five hospice services. We ended up with the first service we contacted, Sonata Hospice, on the basis that 1) they were very local to our loved one's care home (2 miles) 2) most of the nursing staff was local to the office 3) most of the nurses were educated with the issues of our loved one (they had the specific speciality within their RN designation) 4) the very arbitrary fact that the owner was born and raised in Berkeley and lives in San Diego and our loved one was born and raised in San Diego and lived in Berkeley for most of their life. (That is to say, by that time, I had a good feeling about them). I will just briefly state what happened to the other four services - two were very aggressive and just turned me off, the other was outside of their service area, and one didn't answer the phone. Sonata turned out to be the right choice! Our loved one got such stellar care, that a significant number of other resident's families saw what good care our loved one got (full bathing in the shower every other day, not a sponge bath, consistent specialized care and respectful and compassionate communication) that they actually switched from their hospice service to Sonata! Sonata provided exceptional care to our loved one. From the beginning, when meeting with the very knowledgeable and very thorough Director of Patient Care who comes to you and takes a medical history and gives her patient the "once over". The Certified Nursing Assistants come to provide patient hygiene and are extra eyes for the nurses (looking for skin issues, etc) to the volunteers who sit with patients (because, let's face it, family is not the bomb all the time) to the social workers and religious personnel; they all provided seamless medical expertise, compassionate care and super prompt service to our loved one. Another thing that makes Sonata outstanding is the way they treat their patient's families. Having your loved one in hospice is so stressful! Worries abound - are all the patient's issues being addressed, HOW are all the issues being addressed? What if something comes up? Sonata treats all the people involved with the patient (family, caregivers) with comprehensible and simple medical expertise (they never speak above you), compassionate understanding and most of all, they are available and ready to go if something comes up or are there if other issues come up and more discussion or action is needed. If their patient passes away, if needed, they provide grief support for everyone, even after the patient passes. Hospice has one of the hardest jobs in the world. They help patients through what may be their patient's last time on Earth. Patients are not super bueno with their situation as is also their family. They are tasked with providing medical expertise, end of life care and compassionate attention to everyone concerned (patient and family) in very emotional and could be tumultuous times. We entrusted Sonata Hospice with escorting our loved one through the last phase of their life. What we got was exceptional medical expertise, the most compassionate and caring staff ever, comprehensive care, not only for our loved one, but for our family too and 24-hour open communication. Hospice care is an essential need in palliative care. For my family and for all the reasons stated above, we are forever grateful to Sonata for the service provided to our family in a difficult time. They are our only choice for hospice care.

I'm still very mostly happy with this place in the care she gets like any place. There's some people that take a lot more time and caring attitudes towards the patients., and there's some that just try to get their job done.. But mostly I'm satisfied with a caring staff that tries to do the best for my mother. the facility has changed hands. I cannot remember the name of the company it has now but it's no longer covenant care. Something things have changed, but mostly the same staff is there. There's hardly ever any bad smells and if so someone has had an accident and it just got cleaned up. For the most part the place is very clean. Like I said before check it out yourselves. And remember that kindness works both ways.

Patient was depressed, crying daily and upset by lack of care/professionalism/responsiveness/etc. Everything from simple questions to getting help being rolled down the hall were quite difficult. Front desk and some of the workers were helpful, but not consistent and real lack of communication among parties. Real issue is the Patient Coordinator we had assigned - Juana. I found myself multiple times asking myself what here motivations were (from money, to laziness, etc) for why she would be doing certain things or communicating the way she did to me. She tried to discharge the patient early multiple times without important test results which would then later show complications (like pnemonia that developed) and ignored concerning medical results multiple times and didn't even follow up to understand if results had been received or communicated. She didn't really "coordinate" much as we were told to figure out any transportation needed, followed up with doctors directly for information she had, followed up directly to get any medical records needed, medications, etc.) She also lacked professionalism/ethics by being told we were going to protest a discharge and then proceeded to try to get the patient to sign forms that she was explicitly was told the patient would not be signing. As the rooms are shared (3 patients per room) I also saw similar observations with others. One patient developed sepsis and I witnessed the family member crying and pleading for help as the center took over 6 hours to try to have an ambulance coordinated to move the patient properly. It was hard to watch. All I can say is if you end up here visit often and be prepared for a fight for basic care. I was there multiple times a day to assist my family member and still found it difficult to get basic answers/care. Glad we got out and have moved on.

She is an expert in what one might need - something that we really appreciated as this was new space we were navigating. Grace has a well trained team who are compassionate, lovely and gracious. She is impeccably organized, creating a month-long calendar and schedules so that your loved one will have as few disruptions as possible - providing a consistency in which only a few caregivers are chosen for your week or month. This was a stressful time with lots of unknowns (including financially) and all we wanted was to ensure the best care for our loved one. Grace will make it as straight forward as possible and provide care givers that you and your loved one will love. We are beyond grateful we found Grace and team and highly recommend to all of our family and friends. You will be thrilled you called her!

Although our family is moving out-of-state, I hope to find another care agency with as wonderful a team as the one here with Advanced In-Home Care.
