Meds on the floor

My mom was transferred over to Powerback Rehabilitation from virtual voorhees hospital after she suffered a stroke. First couple of days she was taken care of very well by a male nurse. After that it was just horrible. Went to visit my mom and few minutes later the nurse came to check her vitals and give her meds. Few minutes after she left my mom asked me to massage her back and feet, as I went over to her I saw that there was a tablet on her chest that must have fallen out of the plastic dose cups the nurse was using to give her meds. At 75 my mom takes meds for her health issues. And it was upsetting to see that nurse overlooked the tablet on her chest, specially since she was giving her water through a straw after she gave her the meds and it was right there. I took a pic of it before giving her the med and was moving the tray table to get her water. As I was moving it I noticed meds on the ground and as I bend down to look at it I saw there was another half tablet near the couch. As soon as I try to call my brother someone walked in to check on her. She introduce herself as nurse practitioner. I told her about the tablets and showed her the ones on the floor. She said she'll check on it. While I was leaving she told me it must have been someone over night because the "nurse on duty is awesome and very good". I told her that's the same nurse who was giving her the meds earlier when one of them fall out and was on her chest. My brother called the supervisor about it and she said that our mother had an episode where her sugar and oxygen fell and during that time the meds must have been dropped. The tablet on her chest was from that morning and there were 3 tablets on the floor, 2 of them were same one. So she didn't get 2 dose of that medicine. On top of all this our mother got pneumonia while there and they transferred back to the hospital. We are looking to get her into another rehab facility. I'm not the one who complains about things but the negligence here is beyond anything I have seen. If you truly love the person make sure they do not stay here. I have attached pictures and a video.

Bruises on my mom from PowerBack

My mom went to PowerBack from Virtua. She was completely fine but needed physical therapy. Six days later she was sent to the ER, unresponsive, two HUGE bruises on her side and in very bad shape. Six days and we never spoke to a doctor or nurse. Called over and over and over. Not one update on my mom until one hour before she went to ER. Please fo not use or recommended this rehab.

Blood stains and trash on the bathroom floor

Think long and hard before you send your loved ones here. I spent ten days at the facility and could not wait to leave. Day 1 - check in to a room that 3pm which doesn't have a working bed, tv, clock and partial air conditioning, 74 degrees in the room. Asked nurse Robin for pain medication she said not your time honey closed the door, never to be seen again. I rang the bell numerous times only to have the aid come in and say, we will be with you soon. I finally got my medication at 9pm. Well after there four hour between. Day 2- AC still not cooling the room. The room is a mess and my sheets need changing. Night nurses are like day workers, they can just barely complete their jobs. It got to the point where my wife had to call the location multiple times to get anything done for me. The administration at the Voorhees location is ignorant and rude to the point of telling at my wife for calling about my well being. The only bright spot of the entire adventure was nurse Chastity and Jackie the Respatory therapist were angles and could not do enough for me. To top it all off, I was discharged on a Friday and on Monday I got a bill on the mail for $30 for the rental of the TV in my Room. Laughable, cause the tv was old and the remote never worked properly. If I had to raise or lower the volume, I had to get out of bed and do it at the TV. Stay away from here if you can

I just had my father at this facility. We had such a horrible experience. I would never recommend or EVER place a person in this facility.

If you know anybody that's even remotely looking into this place do not do it. My dad went in for rehab from a UTI to get his strength back. He is 68 years old he was there a month. They said they needed to keep him longer because of a sore on his heel. We knew the store was there. It wasn't that bad, but I knew it needed to be taken care of so I agreed to it while he was there. They did two other procedures on him did not go through his primary care. doctor did not go through his specialist for this problem. They did it with their own doctors. I was not even notified. I am a primary care caregiver at home. His next kid he has no other family members besides me against their wishes. I took him to another foot doctor his heel on his right foot was so blistered and oozing, and it looked horrible. It was 100 times worse than when he went in which they were supposed to be carried for it. His left foot. The heel was fine no problems with it when he went in today for his foot appointment for the specialist. It was worse than the first the other foot when he went in. He was neglected. They did not care for them properly. They're overnight. Crew doesn't do a damn thing. My dad set on a wet mattress stoke with urine and feces for six hours before they even got anybody in there to care for him and clean him up. This is the worst place for anybody I would not wish my worst enemy on this place do whatever you can to put your family member anywhere else but here.!

Unfortunately since we needed a rehab and there was no where to place her, she was placed here. Huge mistake on our part. She was there approximately two weeks ago for only two days total and it was two days too many. The building looks like converted Section 8 and that is an insult to Section 8. It's clear that it is run by the slum lords of nursing care. The minute you step in the vestibule there are random old boxes and packages thrown about. The supposed refreshed lobby looks rundown, dim, and not secure. I'm not sure I saw one security person in the multiple trips I made nor an administrative figurehead. The elevators look like something out of a horror movie and do not appear to have ever been wiped in the entirety of their operation. My mother, who is immune compromised, was on the 5th floor, which contained their COVID unit, which consists of a plastic sheeting separating one side of the floor from another and that's it. But more on that later. Upon stepping off the elevator you are smacked with a smell of rancid urine and feces, which only gets stronger when you enter a patient's room. The walls are an off white as they haven't been painted and minimally cleaned. The floors look dirty. The day room looks sad and depressing. There is NOTHING for the residents to do; the furniture is falling apart; and the scant books clearly have never been cycled out. I never saw anyone come in to clean the rooms. Only someone to collect the tiny wastebasket that is in each patients' room, which are not private. Btw, there are no biohazard large closed lid baskets to be found in any of the patients' room or out on the floors so it was concerning to understand what they were doing with the medical waste. The rooms looked disgusting. My moms sidetable still had the food contents inside from the prior patient. The baseboards had all kinds of disgusting buildup. Regarding care, it was subpar to nonexistent. My mom was wheeled in from the hospital and the sheeting and padding from the hospital, including diaper was still there the next day (nearly over 24 hours). That means they left her sitting in her own mess for that length of time. Mind you my mom was hospitalized with a urinary tract infection and now they were about to create a new one. We had to complain that they hadn't check her since intake and they still didn't come in for over another hour and a half from our complaint. In that timeframe my sister and I change our mother ourselves. Multiple times we asked to speak to the doctor but couldn't because he wasn't there. It seems the facility only has one doctor for the entire facility and he may not even be there sometimes. Similarly they only have one RN so good luck getting any seasoned care. Each floor has appears to be two LPNs or someone with less training and they are handling a significant number of patients each. We we complain that our mother had be changed the one LPN told us they were short staffed as if that's an excuse for neglecting patients. That's a administrative issue and it needs to be addressed as patients cannot be expected to get sick, injured or die because the administration has determined to cut corners. My moms condition continued to deteriorate in the two days there as we could not get the facility to get the doctor in on the weekend to look at her meds and care so she wound up with another infection and we unilaterally called the ambulance and thankfully got her out of that hell, back in the hospital and working on a better placement. If you love your family, do not put them in this facility and, if they are there, do everything in your power to get them out. No good can come of them being warehoused in this place.

I didn't think I needed rehabilitation. I was ambulatory, going to the bathroom on my own, feeding myself and gave myself a shower. I believe, they saw my very good insurance and didn't want to loose the opportunity. The food there, I wouldn't give to a stray dog, they deserved better food. I thought maybe it's because I had tested positive on Covid the week before and they had to serve a paper bag lunch with crap food. I was literally starving, I had to order out Saturday night cuz I was so hungry. The next day, I saw my roommate was feed the same stuff. I had asked if I could get a couple yogurts etc. I would be told sure and they never back back. I would turn on my light, a hour later they would come in ask me what's wrong I'd tell them, they would say I'll be back with your meds, never to see them again. They would just turn off my call light all night and not show back up. I was prescribed, pain medicine for pain management. Been on pain management for 20+years for my failed back and neck surgery. I was supposed to get 30mg of pain meds twice a day(12hours) I was left in pain for 24hrs and then was told, I only had to ask for the pain meds, that it was an as needed order. I was furious, I said, I did, 3 times you all just kept turning off my call light and not come back. I'm also on psych meds for Bi-polar, severe depression and helps me sleep at night. The 1st night,I received them. The next day, at bedtime I asked for my sleep/psych meds, the nurse says, your not ordered those . meds. I wanted to run my head into the wall cuz I was so angree. I was like, you can't make this stuff up. On Monday morning, I at 9am I called the front office and told them I was leaving at 10am. They tried the scare tactic about not getting scripts for my meds etc. I said, don't need them, my meds are all at home. I don't need to be here, even the doctor said I didn't need to be here, he had already told me he would write for the one med that the hospital had lost on me. So, I was properly discharged anyway and left by 10 30am. So, there is so much more I can write, and would like to but I don't have time. I'm pretty sure 'you get it' I Just wanted to warn any other potential patients or patient family members to think twice about coming here.
