I stayed at CareOne Lexington from Friday, 2/27/26 to Wednesday, 3/4/26. Let me start with what's…read moregood about the place: Helga, V., Marvin, Sandy, Lydia, Irene, and Jae. These nurses and aides are genuinely kind people working above and beyond in a hellishly penny-pinching for-profit nursing home that has clearly experienced years of top-down CEO neglect. They do this as the physical plant literally falls apart around them (see photos). If it were not for these caring people, I doubt I would have survived my stay. Nurse Helga is wonderful. On Friday, staff from the cafeteria brought me the meal plan for the week. It consisted of meat for breakfast, lunch, and dinner. I explained I was vegetarian and asked for the relevant meal plan. Staff had never heard of such a thing. They sent me a salad (see photo). It consisted of half a tomato, some lettuce leaves, and onion. I was supposed to be eating plenty of calories and healing after surgery, but this "meal" was under 100 calories - a starvation diet. I showed Helga about catastrophically non-nutritive meal. Out of the goodness of her heart, she bought me a vegetarian bowl at Qdoba. It was delicious. I was deeply touched by this act of extreme kindness. I asked to pay her back, and she wouldn't take my money. Helga is an angel on earth. Incredibly, once Helga was off for the weekend, I had to continue to beg staff for a non-starvation diet. They continued to send me food that amounted to under 600 calories per day. I called Robby, the dietician, but he was off for the weekend. I was hungry and dizzy. A kind nurse got me a bargain-basement Ensure knock-off, but it was lukewarm and tasted bad, so I threw it up. On Saturday, I called friends to let them know I was starving. On Sunday, my friend Julianne came with hummus and pita. On Monday, my friend Dennis came with food that my friend Amy bought. On Monday, when staff started bringing actual vegetarian food (bargain-basement Cheerios, the egg glop that comes frozen in large containers), it was so disgusting and full of chemicals that I threw it up, every time. This meant that I kept losing my medication. (I'm on over a dozen meds a day due to severe neurological and autoimmune disease.) I brought the electolytes my neurologist told me to take three times a day for orthostatic intolerance, but I was never able to get staff to bring me a container to mix them up in, so I became progressively dizzier and more dehydrated throughout my stay. Also: 1) I was told to call Jen, the discharge planner, Wednesday, 3/4 at 10:00 am. I did so. I let staff know that my ride was coming at 2 pm. Four hours later, my ride came, and Jen had still not returned my call. Staff was surprised that she never got back to me. Today, Friday, 3/6, I called her again, because I don't know if I'm still supposed to be taking antibiotics that they gave me at CareOne. Jen has never returned my calls, and I have received no discharge planning. Does she still work there? 2) Social work asked if I needed services at home. I told them that I applied for a home health aide through MassAbility in 2024, and that I have not yet received those services, despite being over 90 percent bedbound and 95% housebound due to neurological disability. (MassAbility gave my case to Elara Caring. Elara Caring has not provided the home health aide that they are required to send me.) Social work said they would try to get me services at home. I never heard back from social work. I called them today, 3/6, to ask what services they found. I still haven't heard back, and I have received no services, including the home help and the post-surgery nursing care I requested while at Care One. 3) The call alarm at the nurse's station typically rings for 35 minutes or longer. It turns off for a couple minutes... then starts back up. My neurological disease comes with severe sensory limitations. I was in the room next to the nurse's desk, with its constant beeping, for my entire stay. I had a severe migraine every day. This is not the nurses' fault. It's the fault of CareOne for understaffing to the point where the call alarm barely ever stops. 4) There was no phone reception in Rm 125P where I stayed. When I needed to beg friends for food or ask my surgeon what to do about the fact that I couldn't stop throwing up, I had to walk far to use the phone. Given that I wasn't allowed to treat my orthostatic intolerance in keeping with my neurologist's directions, this was dangerous. 5) My room was extremely bright even in the middle of the night. This is because there was a huge chandelier outside my window that was kept on all night. The meager shade did almost nothing to block the light. This added to my constant exhaustion and migraine. 6) The issues documented in the photos (exposed wires, insufficient calories, patients being forced to hop in and out of bed to manage their own light soon after abdominal surgery) are dangerous. DO NOT SEND A LOVED ONE HERE.

Not only did She detect issues that the doctors were not pointing out, Brittany was able to come up with effective solutions to soothe or make the problems go away completely. Unfairly she wasn't able to begin working on my broken foot until months after my crucial injury. I'm sure if she was my first P.T we would have definitely had better results and understanding about the real problems sooner. Overall Brittany's professionalism even rubs off on her interns who I thought already graduated and were working as full time P.Ts. In addition I want to thank the staff and Interns who I had the pleasure of meeting during my time there. Heartbreaking Experience…read more I honestly wouldn't recommend this Rehab to anyone. The staff here move incredibly slow and seem overwhelmed all the time. Most of the nurses are retired and just don't have the energy or attentiveness that patients need. There are no staff doctors, only nurse practitioners, and it feels like there's no real leadership or plan in place for anything. There's no protocol for emergencies -- during a fire alarm, everyone looked confused about what to do. We also had two power outages in one week, and instead of fixing the issue or using proper backup power, they handed out battery-operated LED lights to patients. It was honestly shocking.Language barrier is an issue especially when reciting the menu options The worst part is how long it takes to get help. I've seen patients left sitting in their own feces for up to 40 minutes before anyone came. No one deserves that. It breaks my heart to see people treated this way when they're supposed to be cared for and respected in their later years. This place seriously needs to be inspected and held accountable. Families deserve better, and so do the people living here.

The nurses and CNA in the Brooknoll unit were excellent. I literally felt that they loved him. He was always happy, clean and very well cared for. He just loved living there. I also watched how other residents were cared for and they were also well cared for and happy and to me that speaks volume. If i could give 20 stars i would.

It's kept much cleaner than other facilities I've visited. That's it's best attribute. Care is…read morewildly inconsistent with basic needs such as access to water varying day by day. Weekends are particularly bad. Staffing is inconsistent and many are portly trained. An example of this is that some CNAs pick up patients by hand for bed to wheelchair transfers instead of using a Hoyer lift. If they DO use the Hoyer, many times it's only one person when it is supposed to be two. The administration will tell you this never happens, but they are flat out lying as I have witnessed it. I have seen patients who are on oxygen not being reattached after a transfer. Once again, you can raise these issues with nursing staff or even in team care planning meetings, but they will tell you it's untrue. I actually stood with a nurse in front of a patient who did not have their oxygen cannula on tell me that the patient was on oxygen! You just can't make this stuff up. Many times, you cannot reach a loved one OR their unit on the telephone, the first because the aids don't set up residents who are not ambulatory or able to get to things, the second because they are short staffed and/or inattentive. Patients are not always provided access to call bells. Personal items left in drawers, etc. are treated with disregard. For example, one might find their items smeared with toothpaste because an aid shoved an open tube into a drawer after use. I have seen open moisturizer packets tossed into bedside drawers where they then get smeared all over personal belongings. One might find their limited storage space (drawers and such) inundated with towels, diapers, pads, wipes, etc. which get left by one aid and not used by the next. The more care a loved one requires, the sloppier it gets. The administration will take a position that patient reports are undependable and that what family reports isn't actually true. I've rated this place a 2 ONLY because it is sadly better than many other places. That said, the fact that what should be considered baseline is often not met is disgusting. It is apparent that supervision is minimal because the things I've mentioned, along with countless others, can be and have been observed repeatedly.
