I stayed at CareOne Lexington from Friday, 2/27/26 to Wednesday, 3/4/26. Let me start with what's good about the place: Helga, V., Marvin, Sandy, Lydia, Irene, and Jae. These nurses and aides are genuinely kind people working above and beyond in a hellishly penny-pinching for-profit nursing home that has clearly experienced years of top-down CEO neglect. They do this as the physical plant literally falls apart around them (see photos). If it were not for these caring people, I doubt I would have survived my stay. Nurse Helga is wonderful. On Friday, staff from the cafeteria brought me the meal plan for the week. It consisted of meat for breakfast, lunch, and dinner. I explained I was vegetarian and asked for the relevant meal plan. Staff had never heard of such a thing. They sent me a salad (see photo). It consisted of half a tomato, some lettuce leaves, and onion. I was supposed to be eating plenty of calories and healing after surgery, but this "meal" was under 100 calories - a starvation diet. I showed Helga about catastrophically non-nutritive meal. Out of the goodness of her heart, she bought me a vegetarian bowl at Qdoba. It was delicious. I was deeply touched by this act of extreme kindness. I asked to pay her back, and she wouldn't take my money. Helga is an angel on earth. Incredibly, once Helga was off for the weekend, I had to continue to beg staff for a non-starvation diet. They continued to send me food that amounted to under 600 calories per day. I called Robby, the dietician, but he was off for the weekend. I was hungry and dizzy. A kind nurse got me a bargain-basement Ensure knock-off, but it was lukewarm and tasted bad, so I threw it up. On Saturday, I called friends to let them know I was starving. On Sunday, my friend Julianne came with hummus and pita. On Monday, my friend Dennis came with food that my friend Amy bought. On Monday, when staff started bringing actual vegetarian food (bargain-basement Cheerios, the egg glop that comes frozen in large containers), it was so disgusting and full of chemicals that I threw it up, every time. This meant that I kept losing my medication. (I'm on over a dozen meds a day due to severe neurological and autoimmune disease.) I brought the electolytes my neurologist told me to take three times a day for orthostatic intolerance, but I was never able to get staff to bring me a container to mix them up in, so I became progressively dizzier and more dehydrated throughout my stay. Also: 1) I was told to call Jen, the discharge planner, Wednesday, 3/4 at 10:00 am. I did so. I let staff know that my ride was coming at 2 pm. Four hours later, my ride came, and Jen had still not returned my call. Staff was surprised that she never got back to me. Today, Friday, 3/6, I called her again, because I don't know if I'm still supposed to be taking antibiotics that they gave me at CareOne. Jen has never returned my calls, and I have received no discharge planning. Does she still work there? 2) Social work asked if I needed services at home. I told them that I applied for a home health aide through MassAbility in 2024, and that I have not yet received those services, despite being over 90 percent bedbound and 95% housebound due to neurological disability. (MassAbility gave my case to Elara Caring. Elara Caring has not provided the home health aide that they are required to send me.) Social work said they would try to get me services at home. I never heard back from social work. I called them today, 3/6, to ask what services they found. I still haven't heard back, and I have received no services, including the home help and the post-surgery nursing care I requested while at Care One. 3) The call alarm at the nurse's station typically rings for 35 minutes or longer. It turns off for a couple minutes... then starts back up. My neurological disease comes with severe sensory limitations. I was in the room next to the nurse's desk, with its constant beeping, for my entire stay. I had a severe migraine every day. This is not the nurses' fault. It's the fault of CareOne for understaffing to the point where the call alarm barely ever stops. 4) There was no phone reception in Rm 125P where I stayed. When I needed to beg friends for food or ask my surgeon what to do about the fact that I couldn't stop throwing up, I had to walk far to use the phone. Given that I wasn't allowed to treat my orthostatic intolerance in keeping with my neurologist's directions, this was dangerous. 5) My room was extremely bright even in the middle of the night. This is because there was a huge chandelier outside my window that was kept on all night. The meager shade did almost nothing to block the light. This added to my constant exhaustion and migraine. 6) The issues documented in the photos (exposed wires, insufficient calories, patients being forced to hop in and out of bed to manage their own light soon after abdominal surgery) are dangerous. my sister was there for several months. after her part A had run out she was marginalized. they refused to start her part B benefit, they refused to let me bring in a private PT. they refused to let me pay for PT. When we rang for help it would be 30 min before anyone checked, then if she had to go to the bathroom, or get cleaned up it would be two hours. and nothing happened between 2:30 and 4. I was told that it was shift change. The grounds were dirty, and repairs were not being done. The food was monotonously meh! I'm so glad I got my sister out of there!!! if you have a minor problem it might be fine but if you need serious help , go somewhere ekse!! read more

I was a patient at the CareOne facility in Lexington for a little more than three months while recuperating from a broken ankle in one leg and a serious infection in the other leg. My overall opinion of the treatment I received was excellent. The nursing staff provided outstanding care seven days a week. Experts from other local hospitals would visit when a second opinion or another level of expertise was required. The therapy department at CareOne in Lexington has a well-deserved reputation for effective treatment and results. Both the physical and occupational therapists excelled at setting goals for me and working diligently to help me achieve them. The food was surprisingly good. The kitchen staff records your preferences and needs when you first arrive and three times a day, they provide nutritious and enjoyable meals. There are always several choices to choose from. Special note has to be made here for Mark, the recreational director, who is amazing. He knows everyone's name in this large facility and always has a smile while he engages patients with endless games, movies, special holiday events, and even live music once or twice a month. The last performer I heard before I left was a well-known Elvis Impersonator and he put on a fantastic show! Mark's loving and upbeat personality is a powerful medicine you can't dismiss. I also want to single out the executive director of this complex facility, Ryan, who has skillfully hired some of the best nurses and aids in the area. While I am certainly glad to be home and walking on my own two feet again, I miss some of the wonderful friendships I enjoyed at CareOne in Lexington. Also, getting a warm breakfast served in bed every morning at 8 a.m. was nice, too! read more

Transferred to Care One, Lexington Ma on 1/11/24 from hospital for Rehab. Ambulance attendants wheeled me into a bed at 4:00pm. Discharged 1/15/24. No member of staff was in attendance for the entire night with the exception of someone who gave me meds, and approx. 1 oz. of water. I was never greeted. I was never given an orientation, nor a schedule. I never received water. I never was asked if I needed toiletries. Quality of care was awful. I rang the call bell to go to the bathroom. Took 25 mins. for anyone to respond. There was no rehab provided. Never met with occupational therapist. One visit from physical therapist on 1/14/24 who assessed my skills, and deemed me discharge ready. She said she would notify case manager. Case manager told me she was never notified. Physical therapist was summoned to my room. She said, " ready for discharge", did not mean "discharge ready" ???. I had to firmly take her on as she made no sense. No menu, no food choices, meals arrive within a 2 hour time frame, always cold, not edible. This for profit company is a disgrace. It is not rehab. It is, in my opinion, a money maker for the owner. Do not send your relatives there without thoroughly checking them out prior. read more

I would never recommend sending a loved one to this facility. My 88 year old mother stayed at Care One in Lexington for almost six weeks.The rooms are very small with two patients to a room, there is very little space to have visitors and only a thin sheet that can be drawn to separate patients. There was a serious mouse infestation, multiple times my mother saw and heard mice running around the floor of her room. The facility placed sticky mouse traps on the floors to remedy the problem, twice my mothers clothing was ruined because they allowed her clothes to drop to the floor where it had contact to these sticky traps, her clothing needed to be thrown out. The furniture is very old and frayed, the laminate on both the mobile eating tray and bed foot/head boards were peeling making the surfaces unsanitary. There were some very caring people that worked there, but the facility is very understaffed, my mother often had to wait to get help to go to the restroom. Prior roommates toothbrushes left in the bathroom. The food was awful, mystery meat, canned vegetables nothing fresh. She repeatedly asked for crackers to help take her pills and they never gave them to her, one night she got her pills lodged in her throat and couldn't breath. When I heard this I asked if they had a vending machine so I could buy some crackers for my mom until my next visit. One of the care staff led me into a kitchen closet where there was a very large container of individually wrapped crackers. Why couldn't they have given this to my mom the prior evening? I'm so glad we were able to get my mom out of this place, no one should have to stay there. read more

I had to give one star because is not possible to write a review without. this place in Lexington is the worse place ever! the stuff is rude, yelling, disrespectful and I can go on and on about the way they treat elderly people there. I was there as a companionship with a 92 years old man and they was brutal on him. if you really care for your loved ones think again before make the decision to leave them on the hands of those unprofessional people. read more

Care None, where you go to die. Just ask any local VNA nurse or PT or a physician at a local hospital and they'll roll their eyes and say, " well I wouldn't send my mother there." Unfortunately, hospitals continue to discharge patients to here because there simply isn't room anywhere decent. Check their Medicare ratings, fined 75k just this past year alone for lack of care or substandard. A Whooping 17% of their staff is fully vaccinated according to Medicare's latest report. So assume your loved one will get covid and because they don't follow standard precautions, add a little Cdiff or Flu too. Hard to believe it's still running, shows how desperate health care has become. Mothers PCP, said "get her out, either to hospital or home." read more

AVOID AVOID!!!! The most inadequate staff ever... including doctors, nurses and nursing supervisors. My mom asked a nurse to help her overnight to walk to the bathroom... he told her no. When I got there the next day at 3pm, i noticed the left side of her face looked swollen or odd... she was lifting her left arm with her right. I asked her what was wrong... she said she just felt weak. I asked 3 different nurses how long they had been like that.. they said all day (so at least 6-7 hours). I asked how come they didn't call me or call the doctor or 911... he kept repeating "for what!?? Her vitals are fine". I pressed and pressed... he called the nurse supervisor who wasn't even in house.. who told him as long as her vitals were fine.. don't call anybody. I kept on pressing him... he came back 30 minutes later to tell me that the ambulance would be there in 2-3 hours. I asked why would 911 take so long, he said there was no reason to call 911 because what would he tell them. So I called myself. EMT's came in minutes and already assumed what it was. They were disgusted and confused at the lack of judgement of the staff at CareNONE. When we got to the ER, not only were the doctors confused by the lack of judgement by the staff... we also found a HUGE bruise on my mom's thigh becuase when the overnight nurse denied her help to the bathroom.. she fell really bad. Turned out I made the right call to 911... my mom had had 3-4 mini strokes. Not COOL. Don't send even a stuffed animal here. Also, from the moment we arrived, no staff offered us help. You have to go up a ramp or stairs to go to the main area. I asked for a wheelchair and it seemed to have bothered the nurse. She pushed it down the ramp and left me to fend for my mom alone (bags included). And don't try to call here... nobody answers. I had to call ANOTHER location to try to get the staff to answer. read more

I was a patient in there rehab unit and was very happy with my care. Everyone was friendly and empathetic. The therapists were great at getting me moving back to my baseline to go home. The nursing staff was helpful and happy to answer any questions. read more

I want to give this zero stars - it is an absolute hell-hole. Do not under any circumstances send a loved one to this place. It is dirty and disgusting. My mother had a broken shoulder and pelvis and was sent there for "rehabilitation" she was there for 48 hours and they did not get her out of bed once! Instead of helping her to the restroom they put a diaper on her because it was "easier". Staff is essentially non-existent and completely disinterested, they were giving my mother all these pills and no one could tell us what they were - finally someone said "probably vitamins"! Once we said we were moving her out a nurse showed up (first medical professional we had seen) to ask why and was extremely defensive. Overall, if we left my mother there she would have died - I am so upset that she was there at all and that the hospital keeps sending patients there. It is complete negligence. read more

I was admitted to Care One's rehabilitation section for a fractured pelvis in October 2017. I stayed four and a half days before being transferred. Several issues, some extremely serious. Three incidents stand out. 1. When I told the male nurse assigned to me that I'd like a female nurse because I was wearing a catheter, he lifted my gown and grabbed my catheter. 2. A few days later, the doctor affiliated with this facility came straight up to my bed and lifted up my gown. When I asked "What are you doing?" he responded that he was giving me an examination. No other doctor has ever examined me without telling me what he was doing. 3. I was in a lot of pain one night, after being told that my pain medications had run out. When I left for a better facility two days later, my nurse didn't want to give me my remaining medications. When I checked, I found that several pain medication pills were missing. Other issues: the nurse I was assigned hadn't heard of one of my medications, and told me they didn't have it. They did. But I had to argue for it, spell it for him, etc, in order to get a medication crucial for my medical condition. Then whenever I pressed the call button, I'd have to wait 15-20 mins for a response. I could hear other patients in the rooms around me giving up and calling for help instead. And apart from a shower that was part of an occupational therapy exercise, I wasn't washed. A nursing aide told me that patients (who, remember, are injured or sick) wash themselves. Avoid this place. read more

I'm not sure how this place has good reviews... I sent my grandfather here based on the reviews I saw here. My grandfather was in the hospital for one week due to aspiration pneumonia and diverticulitis of his large intestine. He is also a Parkinson patient which is how he got the aspiration pneumonia his food was going into his lungs. Anyways, we were given this location and another the other had one star and everyone says this place is so amazing and has the best occupational therapy for patients. Since he needed PT, OT and speech therapy we were like lets choose care one. - Day one at rehab no lie they sent him potato chips and a sandwich. This is an 83 year old man with a care plan stating he needed special food due to aspiration... they also told my mother that they didn't know if they had his Parkinson's medication in the facility. At night we were told by another patient my grandfather had been screaming for over an hour for someone to attend him not one person came till the patient across the hall rang her buzzer and even for her it took them 40 minutes to get to her. My grandfather had been soiled for hours on his bed and no one ever came to change his bed sheets. - day 2 they lost his dinner my grandmother spent the night with him and the nurse told her that "the kitchen was closed her hands were cleaned and there was nothing she could do to help her" oh and they also needed to be reminded to give him his medication again before every meal. My mom pulls up to the facility to look for the supervisor and her response was "its been taken care of" and literally walked away... - day 3 me and my boyfriend show up at 5 pm I ask my grandmother if he's received his medication because it's almost dinner time she said no. The nurse named Caroline was just sitting at her desk looking at memes on Facebook looked at me and said she had no idea who the patient was or what medication he was on. Once both the nurse Caroline and PT Vanessa were in the room I told them our frustration with the staff about my grandfather and they both looked lost in the woods. Caroline even said that she never looked at the plan of care and that its hard on weekends when new patients to come in to adjust to their plan of care. Not to mention the nurse Caroline left an open bottle of pills just open in the hall way at the nurses station. The PT Vanessa was there just to raise my grandfathers arms for like 15 minutes that was his physical therapy for the day. The entire day my grandfather was not removed from his bed not even to eat his meal which was also on his plan of care. The staff here is so unprofessional has 0 bed side manner. I'm not quite sure how any one in their right mind would send a family member here I wouldn't send my enemy to this location. In the end we discharged my grandfather early the last straw was the staff shaving off his mustache that he has had for over 60 years without consenting with anyone. I really hope that my review gets the attention of the owners of this establishment, because we spoke to several other patients in the facility that had a lot of similar complaints. This is not the correct way to run any type of healthcare facility. I would like to thank John who is the social worker at this place and is the only one who truly listens and cares about the people in it. I pray and hope that with this review you think twice before placing someone you love here because this is not a healthy place for someone to be in. read more
