My father went there and after a week he was rushed to the hospital after they forced a handful of pills down his throat that made him aspirate then it became pneumonia in the hospital where he passed away. They are highly understaffed. He had to wait hours for any kind of assistance. I do not recommend. read more

My spouse went there for physical therapy after a hospital stay. Due to Covid restrictions I could not visit. My spouse was physically incapacitated not mentally. It took the both of us several days of making waves to even get a therapist in there. They tripled one medication and eliminated two others. They left him totally unable to move in a wheelchair for 6-8,hours a day. a quick answer to a buzzer was an hour. They failed to document two falls. I talked with administrators, social workers, head nurses - basically everyone and their brother multiple times and told them we had no family nearby, no help and that I worked outside. They didn't care and threw him out anyway . It took 4 people to get him into the car and I had to call EMS to get him into the house. When I complained I was told -well you told us you had 24/7 nursing care. He came home ten times worse than when he went in. My grandfather went there for physical therapy due to a massive stroke, not only every time my grandmother and I would try to visit him we would always notice clothes missing but we would also notice things that weren't his appear in his room, mind you my grandfather can't walk cause of his stroke, but not only did some clothes go missing his dentures did as well, and when we asked the nurses to look for them cause without them he can only eat soft food, they didn't try. They let their patients roam around the building and into other patients rooms, but not only that, my grandfathers health in the facility was also decreasing as he started becoming very pale and very noticeably looking as if he was starving. another thing is that one of the nurses left my grandfather in a wheelchair after trying physical therapy, he tried to get out of the wheelchair but fell in the process, i don't know exactly how long they left him there but when they got him up, the nurse that did said he hurt his shoulder pretty bad, but that wasn't the only time he got hurt in this facility as we always saw bruises on him. I don't think anyone should go to this facility. They obviously don't care about their patients and are highly unprofessional. read more

The facility looks good and is fairly well laid out. From there on out, this place is a failure. My mother had to spend time there after contracting pneumonia. She wasn't fed properly nor taken care of properly. Inside of two weeks, Peak Resources of Alamance had "kicked" her out to the local hospital where she subsequently died. The staff there were as callous and uncaring as any I've ever seen. I met some of the patients staying there, and they were miserable. Please do not allow your family, friends or any loved ones anywhere near this place. They may not survive. It deserves to be called "The killing field". read more

Peak Resources in Graham, NC is a BAD place to go for rehab for broken hips. The treatment my dad got was awful..Sit for 8 hours in his diaper, no response when requesting help to feed him, did not keep his brace on his legs per Dr. Instructions. My sister took my Dad to post OP dr appointment and when Dr. raised his pant leg to check, there was no leg brace there at all. That brace was a must to have when Dad was out of bed...I do not reccomend this place to anyone... read more

This place is an absolute JOKE. My grandfather was in there after surgery and they treated him like a toddler. They don't take care of him. Would not give him his medication that he is prescribed from his doctor. This place needs to be shut down. I will find a news reporter to help get this place shut down! I don't even know what to say. Please consider other long-term healthcare facilities and take time to speak with patients about their experiences and observe how health care professionals interact with patients if possible when considering long-term facilities. As a professional in the health field who also works with healthcare professionals of all cadres (who I often consult with on health matters) and observing my grandmother's health condition, I can tell there is gross negligence happening at this facility. The bad reviews you read below are true. Some of the staff need specialized training to provide care to patients facing a variety of health conditions. There is no reason a patient should not receive their medications or vital substances such as water and food or never receive the results of their blood work allegedly taken a week ago. It is sad that a family member has to tell staff that their loved one is dehydrated. It is criminal what is happening in that facility and sadly many residents are paying with their lives. Not to say there are not good healthcare professionals at this facility but you should want to feel comfortable with everyone at a facility you pay thousands to keep running. read more

Facility is beautiful but care is not good especially 3rd shift care. They are not trained properly to assist Alzheimer's patient. My dad went in with a broken hip for rehab. Braces dr said was a must were not on the majority of the time and when they were on they were not on properly. I have heard no rehab is good but I would hope there is something better for care than this place. I prayed God would hold dads hip in joint every time I had to leave him there. Brandy E., you are not alone! The staff did not take care of my mother either and I filed many complaints. Unfortunately, I have found that this is the culture of skilled care nursing facilities; a huge systemic issue that legislators are not addressing. Peak Resources is a beautiful facility. But, the CNA to patio ratio is low (1 CNA to 10 or more patients), the nursing staff is overworked (1 LPN to 20+ patients) and are spread too thin. Even worse, the medical notes made by the staff are not honest or trustworthy, but well-written enough to pass a State Survey investigation. I was in the building when a State Survey was being conducted. The staff intentionally kept the investigators away from my Mother's room while I was there. And, unfortunately, the investigation procedures do not include talking with random family members visiting their loved ones; only the families identified by the nursing facility! Something to note [lessons learned] is that although the facility passes a State Survey investigation and invite you to review their results does not mean that deficiencies did not occur and are not present!!! It means that the nursing facility have found a loophole and learned how to fool the State, so that the nursing facility can continue to defraud the government and receive Medicare/Medicaid funding! My prayers are with you and your family. Hopefully, this review will also help others. And, in response to the business response below, I already did. I reached out to you on a number of occasions to discuss my mother's healthcare decline while in your care. However, you refused to take my calls after complaining about the negligent care your facility provided. You and I both know this process doesn't work!! Be honest with the YELP Community and STOP with the standard copy & paste PR responses. Use your energy to fix the issues we are complaining about to improve the patient care provided in your facility!! We love our family members and only want the best care for them. While in your care, the least you could do is be an advocate for quality patient care and ensure that residents get it!!! You know the secrets that your facility keeps and so do I. But, I bet if you focused on providing better patient-care solutions, complaints would decrease, you and other care providers would sleep better, and your conscience would be clear!!! I agree 100% with the previous reviews. My husband was taken there for rehab after a 7 week hospital stay. When I was looking into places near home, I read the website reviews that said it was 5 star with Medicare. He was taken to Peak on a Friday afternoon. No one came to the room for 3 to 4 hrs. He did not receive his night meds. Saturday was uneventful except that no one checked in on him, luckily I was there as much as possible. Sunday morning I came to his room and he was in bed with only briefs on. I forgot to mention he needed help walking because of being in bed 7 weeks. I put a handmade sign on his door to inform everyone he needed help getting to the bathroom. His dirty clothes were on the bathroom floor with fences on floor and commode. I pulled the sheets back and he had dried fences on him. I went ballistic with the nurse aid, though it was not her working. I gave her an apology but there was not any staff to file a complaint to. We had another problem Sunday night at 3AM. Monday at 8AM when they unlock the doors I packed his clothes and told them I wanted him to be discharged and I took him home. He has in home PT and is flourishing. This place needs to be checked out by Medicare. read more

On Monday, February 25, 2019, my mother began to cry out stating that she was in pain. I was so heart broken with all my mom has been through. She rarely cries or complains. I pulled her light switch, Joy came IMMEDIATELY, gave her Tylenol for the pain and advised me she would have her checked right away for any breakdowns in the area. I want to say that due to the fact that my mother is a diabetic, this is urgent and Joy had a sense of urgency! I appreciate her as I do the entire staff at Peak. She could have waited five minutes, but no! Ms. Joy was our own EMS on feet! I want to thank Joy. She was able to find a small breakdown early in order that it may be treated and my find relief before the problem grew out hand [sic]. I thank Joy and I thank Peak for bringing on a valuable contribution to [the] team and for making it a great stay for my mom during her road to recovery. Based on our recent experience during a nearly 4 week stay, would rate 4.5 stars! I am a healthcare worker and consider myself more critical of the care to begin with, and my Mother was the patient which sets the bar even higher. We also have previous experiences with rehab centers in Durham to compare this rehab stay. We found only one "weak link" (thus, not a 5 rating). Beginning with the Nurse Liaison who personally came to the hospital to meet with us before my Mother's discharge, we were greeted by smiling, compassionate, encouraging people throughout the stay. The nursing staff (RNs, LPNs, CNAs) is to be commended for their assistance, patience, and accommodations (and all the sincere hugs and kind words we received before leaving). Physical Therapy, Occupational Therapy, and Speech Therapy all did a phenomenal job moving my Mother from a painful, weak, and unsafe mobility state to being able to get around her home with minimal assistance-- much more safely and her pain is controlled (which has been rare since 10/03/2017). We had multiple interactions with Peak's ancillary staff (Admin Assistants at front desk, Nutrition Services, Environmental Services, Engineering/Maintenance, and contract-transporter) as well as Administrators (Facility Director, Admissions, Nutrition, Financial/Business)-- all of whom are readily accessible, attentive, kind, and helpful. The facility itself is kept clean and uncluttered. The accommodations are very nice (exceptions: the toilet seat should be higher-- especially for those with hip and back precautions; the individual room HVAC thermostat was difficult to regulate; most of the food was quite average of such facilities, and noise travels easily down the large and open hallways). These exceptions were only a small part of the stay, and conflict with Peak's employees and administrators obvious care and intentions. Another reviewer wrote of the 'culture of skilled care nursing facilities'. I do not believe that the real and inherent problems mentioned are confined to such facilities. We see similar weak-links in hospitals and clinics as well. Thin staffing (resulting in call-light response delays), shortage of some medications and supplies (often blamed on post-hurricane Puerto Rico not producing supplies at this time) and shorter stays (as dictated by insurance companies) compromise healthcare throughout the system. As consumers, our best defense is being bold with participation and communication. Every patient needs an on-site advocate anymore to successfully navigate the healthcare process (realizing this is not always possible). Bottom line: We are so grateful for Peak's staff with their congeniality and helpfulness. Have already, and will continue to strongly recommend this facility. Thanks for what you all do ! read more
