These patients sing praises about Forte, from the doctors to the nurses to the efficient people at the front desk. From a visitor's point of view, Forte is great. They are very straightforward with their instructions, which I appreciate. Just tell me what to do and what to expect, and I'll do it and expect it. Everything went pretty much according to what they said. They have a smoothly running system here. Visitors can help themselves to the unlimited coffee and water at the coffee and water station. This is where the Little Free Library is located! The waiting room also has multiple seating arrangements, from high-top desks to comfy chairs to love seats. Or, if you need something to eat or drink, there's a cafe on the first floor. (See my separate review for La Verna Cafe.) The waiting room is airy and bright. One thing that's kinda weird is that a "cruise director" comes and talks about what to expect with your surgery ... right in the waiting room, among all those random people. That seems unusual, right? However, there are private rooms that you're taken to when the time comes to talk about your particular case with the doctor. As a visitor who is not a patient, Forte is great. But I've been told that it's also great being a patient here, so there's that.

They also sent reminder texts and emails, and sent me a COVID screening the morning of the appointment (mini online quiz to say if you'd been feeling sick or not). It was all very seamless! When I visited the office, they checked me in right away and I was brought back right at my appointment time. A PA named Kyler came in to assess my knees and she was super awesome and helpful. I was then brought back for knee x-rays and the x-ray women were SO nice too. As soon as I was brought out of the x-ray room, an attendant brought me back to my room and Dr. Shook came in to assess. He delivered good news (yay!) and was so kind and helpful. He communicated very clearly, seemed very patient, and was open to any questions. He printed off exercises for me to do at home and the attendant brought me to check out. All seven or so people that I interacted with were so friendly and sharp. The whole appointment was very efficient and Dr. Shook seemed to know everyone by name. I would highly recommend this place to anyone!

The services I received from A Meyers for a surgical site infection in my hand caused by one of his…read morepartners was substandard care and very disappointing. 1. A MeyerS allowed the infection to progress to osteomyelitis over a 4 month period. During this time, I was in constant pain and my finger had to remain in a splint for 6 months instead of the 6 weeks. I was on various ineffective oral antibiotic regimens during this period. I broke out in a sepsis rash twice and thought I was going to die of sepsis because the infection was so mismanaged. When I asked for a second opinion from an Infectious Disease(ID) physician other than the one A Meyers was using he refused to provide a referral and, despite being in practice in the area for 16 years, said he did not know another physician. His stonewalling me, the patient, for 4 months allowed the infection to progress to osteomyelitis. I finally identified another ID physician myself but A Meyers continued to refuse to provide a referral until my insistence was so strong that he was forced to issue it. The new ID physician (ID2) said that A Meyers said that there was no infection in my hand but ID2 believed otherwise after examining me and ordered the correct diagnostic test(an MRI of my hand). The MRI was performed the next day diagnosing osteomyelitis, a central IV (a PICC line) was inserted into my arm, and I was started on a 42 daily infusion of IV antibiotics at a hospital center. The side effects of the antibiotic included extreme nausea and dizziness that prevented me from driving or engaging in normal activities for 42 days. 2. As a former director of surgery in a large municipal hospital, I believe the infection control practices of both A Meyers' office and of the surgery center (owned by A Meyers and his partners) did not meet basic good infection control practice standards. While other medical providers in the area and the nation were still wearing masks for covid, no one in the office or surgery center wore masks except during surgery. I repeatedly had to ask A Meyers to wipe the surface of the office xray machine when filming my hand. And disturbingly, he either could not or would not provide me with copies of all my x-rays. 3. This was the first time I have had surgery at a physician owned facility. It is the last time as I believe both the infection control practices and the quality of the staff were substandard. 4. I found A Meyers to be very condescending and disrespectful. He is obviously unable to diagnose an infection and did not know the correct diagnostic test, an MRI, that should have been ordered to diagnose the infection. The ID2 physician told me it was my right as a patient to go to any physician I wished and readily provided a referral to another surgeon. Without this referral, I would have been unable to schedule an appointment with another surgeon as no one wants to clean up another physician's mess. 5. The additional care I required generated an additional $7500(based on insurance claims) in payments to A Meyers practice. I am left with a deformed finger, constant pain whenever the tip of my finger touches a surface, and the need for continued monitoring for the remainder of my life. A surgery which should have cost $2670 has now generated payments of at least $31,500(based on insurance claims to various providers to rectify the medical issues caused by the original procedure and mismanagement of the infection. I am now under the care of the professors and clinicians of one of the top five university medical centers in the United States. They are closely monitoring the status of my finger in order to prevent a possible amputation--all because A Meyers did not listen to me, mismanaged antibiotic treatment, could not recognize an infection, and did not know the proper diagnostic test to order to determine the problem. My life as I knew it has been taken from me and my family.

I've known him since he started the practice. He has treated various back issues for me successfully for seven years. I have probably referred six patients to him, all receiving superior care and outcomes. Today I am feeling hurt, betrayed, and yet in severe pain. I called the Carmel office two weeks ago to request an injection. The scheduler checked to insure that I was not violating a "waiting period" between shots. I explained that the pain had been more severe with a short three month span than I usually experience after nine months to a year after Dr. Mac's expert therapy. I'm back in my office after I was turned away, an hour ago because the front office had no record of my appointment. Bad show. The scheduler was pleasant and seemed pleased to slot me into an appointment within a relatively short wait given that my pain levels were so high. The front office staff this morning were agreeable and I believed attempted to fit me into the schedule after telling me that I did not have an appointment. I'm writing this arguably because I am disappointed. I will continue to depend on Dr Macadaeg and his care. Perhaps he and his partners might examine best practices regarding their support staff processes. He splits time here, another office and surgery, so it may be difficult to get a hold of his staff at times. I have also had to wait a long time after my scheduled appointment time to be seen. My hand surgery with D Dellacqua resulted in a permanent deformity of my finger, chronic and…read morepersistent pain of this finger, and the need to be monitored for the rest of my life for infection. 1. The hand infection was due to D Dellacqua's violation of the sterile surgical field. When the alleged sterile surgical bandage was removed 7 days post surgery my finger was swollen, red, and painful. The occupational therapist was so concerned about the appearance of my finger that she questioned whether the stitches should be removed. Oral antibiotics were immediately prescribed and a second surgery to incise and drain the infection(pus) was performed within 3 days of removal of the surgical bandage. Due to infection mismanagement the screw used to fuse the joint of my finger had to be removed. I had 3 surgeries in a 3 month period of time. Instead of the usual 6 week splinting of a finger with this type of surgery, I had to wear a splint for over 6 months. Mismanagement of the infection initially by D Dellacqua and then by one of his partners resulted in the development of osteomyelitis. There was concern that it would progress to sepsis. This necessitated that a special IV line be established in my arm and IV antibiotic infused at an hospital facility for 42 consecutive days.. 2. I have been in charge of surgery in a large inner city hospital. I found the infection control practices at both the office and the ambulatory surgery center owned by D Dellacqua grossly lacking. While other medical providers in the area were requiring masks for covid infection, masks were not required either in D Dellacqua's office or at the surgery center. I always wore a mask. In fact, there was a bravado sign on the front door of the surgery center stating "FACE MASKS NOT REQUIRED." The surgery center asked me to arrive an hour early and then left me sitting in a crowded waiting room filled with adults and children not wearing masks for over an hour. I do not believe my surgery was performed in a normal surgery rooms as they wheeled me by these rooms to the back of the facility where a lot of equipment was stored. There was considerable tension in the operating room itself during my procedure--I do not know why. D Dellacqua engaged in lengthy conversation with another physician during surgery who walked into room(without a mask) while Dellacqua was performing the procedure. At the office, masks were not worn by any of the personnel and I had to ask that the machine surface be wiped before my infected hand was placed on the surface for xays. 3. Because of Dellacqua's violation of the sterile surgical field, the payments for care have been excessive totaling $31,500(based on insurance claims) for a procedure which should have cost only $2670. Dellacqua and his practice received at least $7500(based on insurance claims) additional revenue due to his mistake. 4. My initial surgery was done under a local nerve block administered by D Dellacqua into the palm of my hand. It was stunningly painful and I now have a lump in palm of right hand that is sensitive to the slightest touch. When I asked D Dellacqua about it, he shrugged his shoulders and said "things happen during surgery." 5. This was the first time I have had surgery at a physician owned center. It will be the last. I had tried to obtain the infection rate of the Surgery Center prior to my procedure but was unable to do so as this information is not available to patients. 6. I am now under the care of expert professors and clinicians at one of the top 5 university medical schools in the United States who continue to try to rectify the poor care I received and prevent amputation of my finger. I am trying to put this incident behind me but it is difficult since I have pain every time the tip of the affected finger touches a surface.
