She has everything scheduled for you before you know you need it and has transportation to and from appointments as well. All of the staff was very friendly and easy to communicate with. If a loved one needs rehab and Congregational Home is a choice you have, choose it.

Since my father's passing (after my mother was already gone), I have been struggling for eight months with the accounting department. For one thing, they didn't close my father's account (under Alexian's contract) with Time Warner Cable and I had to deal with TWC directly in person to do so. They also sent an unauthorized debit to a closed bank account, incurring overdraft fees to me. Meanwhile, Alexian owes me money at this point for overbilling by their own admission, but they still haven't repaid me. My emails and phone calls are not returned by the ever changing staff, promises are not kept, and the whole situation has been exasperating. The administration is very disorganized and completely unprofessional. I can't believe I'm still having to deal with this so long after my father's death.

During this last year with Covid 19 they have done so much to keep us all safe . The complex is located in such a perfect setting ! It is within walking distance of downtown Hartland ! Many diverse shopping and eating opportunities. Palmer's Steakhouse, Zesti , the Phoenix, and Creole Cafe are only a few of your options. Piggly Wiggly is less than a mile away . Also Nixon Park is a great place to go walking and enjoying many activities they have . One we enjoy is free concerts during the summer . They have underground parking so you don't have to worry about taking your car ! If you want quite surroundings you can have that , or if you still want to enjoy nite life you can do that too . I enjoyed the residents, the delicious luncheons given by Nancy and Byron, the two libraries, easy access to exercise room, plus computer room. Nancy and Byron made sure that all was well at the Meadows, and so many visitors commented on the lovely decorations in the lobby. There was coffee hour three times a week, Thursday night Happy Hour, and often times residents went to Friday Night Fish Fries. If you are looking for a place to take care of a loved one who needs assistant with their basic…read moreeveryday care..... THIS IS NOT THE PLACE. My mother has been at this location for 6 week and the lack of basic care has been so subpar. My mother's safety has been at risk since day one (See Pictures). The staff sees nothing wrong with how she is on the wheelchairs in the pictures. Staff that doesn't know how to use the equipment properly, she goes the whole day without being checked. When pushing her button for help it goes hours with out anyone coming to see what she needs. She was there for 5 weeks and was never showered once. Simple things like brushing her teeth everyday can not even be done regularly. The final straw was when my mom was having a bad day and was nonresponsive when trying to wake her up. The young lady giving her pills decided since she could not wake her up she would take the pills and shove them in her mouth. Then after shoving them in her mouth tried to give her water and because she could not get any water in her mouth decided to leave the water and walk out of them room. Leaving the pills in her mouth for the potential to choke. This facility only has one care and that is the money that is being paid by the families. Your loved ones best interest is not their priority. The worst part is the facility puts the blame on the family and does not take accountability for their staff and their lack of care and training. I had several meetings with the directors and nothing got better.

My Mom has been a resident for about a year…read more I had chosen NP for her because of the wonderful atmosphere and the comparable care services. BUT, after this year , I must say that your strengths are far beyond comparison. Here are several things that are an example of your stellar service. Mom enjoyed your pool for many months, telling visitors not to come at certain times as she could NOT miss her Aquatics class. (Not a single facility that I toured had a pool except you) Both PT, OT and the other support services have been so helpful in coming forward with suggestion of products and things that will help Mom, before I even knew she needed them. When Mom was beginning to show signs of decreased verbal skills, staff called, recommended and enrolled her in Speech Therapy Caregivers of all shifts are so willing to share things (both the good and the difficult) about Mom's habits, quirks and routines. It says a lot about their care and interaction with Mom. It is a comfort to myself and our family to know that they are aware of all this and share it as well. It speaks volumes of your caring staff. They know what movies she watches, what books she is reading, where and what she does on Wednesday nights (and ask about it), amazing! The Nurse has been spectacular. Prompt at addressing all of my many questions and concerns, she literally jumped out of her chair once and said " Let's check on that". She follows thru with so many little things and has many suggestions to help Mom and myself, as Mom continues to change and decline. The personal touch on a weekend I was out of town: She left me a phone message saying she was working the weekend, and was watching and assessing Mom. Just give a call if I needed anything or had questions. It is difficult for both the individual and the family to travel this path. All these examples of the personalized care that your staff provides Mom and in extension our family is priceless. Thank you to ALL of your staff. You should be proud of the service and care you provide. Our family couldn't have found a better home for Mom. Thank you

I am gonna have to hold off on the care part of my review because the jury is out on this one and…read moreonly been recent that we moved a loved one to the assisted living from the independent living arrangement. it really is a long involved convoluted story , way to many details to write about. the move process to assisted living lacks support and any process. the left hand does not know what the right hand is doing. the actual move in day was supposed to have a nurse meet with us,but this never happened. we were accused of not arriving on time, even though we did. We had no idea when dinner would be served, or when the meds would be administered. I asked multiple staff members for help and who I should be talking to since our arrival meet and greet never happened. I finally lost it, yes I did, and demanded to speak to "the nurse". I did not yell, but was real firm and stressed out in talking with one of the care coordinators and so was our loved one. Finally the person who was to meet with us earlier in the day comes barreling in the room like she was ready for war. We were accused of yelling, yes yelling at her staff. Not true, yes I was firm, but did not raise my voice. she starting spewing that she had 60 people to care for! Guess our loved one was not in that number. She lied about an email that was sent to us about dinner times, nope nothing like that existed. we asked where she was for the agreed upon range of time for our arrival, said she didn't see an email or have a call. we emailed her at 7:40 am and called when we arrived. only could leave a voice mail. We had to fight to have a TV installed, another story for another time. Lastly the most rude and appalling comment out of this persons mouth was that if we didn't like it we should go back to where we came from. my loved one is not only stressed out because of this encounter he witnessed in his room (she should have had the professional courtesy to suggest that we go to her office to talk) but he is afraid that he won't get the care he needs! BTW we are paying $6500 for this type of treatment. Pathetic to say the least.

Our search for an appropriate memory care for my dad ended when we met the team at Azura. We just…read moreknew this was the right place. There is a positive energy with the team members here and they seem to try as hard as possible. My dad is not always agreeable to getting help with his bathing, medications, changing of clothes, incontinence, etc. but the team tries different interventions and makes every re-attempt at things as comfortable as they can for him. The home is clean, it smells pleasant, and it feels like home away from home. He started in the early memory class but then needed to move in full time when he no longer could participate and it was too difficult for his wife to take care of him at home. We have excellent communication because there are registered nurses, an assistant director and a good medical team that provide the care. The outdoor courtyard is a bonus. I visit once a month for a span of 2 days (living in Chicago), so I eat the meals, watch him in activities, and see how he is cared for.
