    Review of The Gardens Memory Care…read more Parent with a diagnosis of Dementia of Alzheimer's type was there just under 6 months when they requested we find another facility. This was due to increased falls and cognitive decline resulting in mobility issues. We were informed the facility was not staffed to provide walking assistance or wheelchair transfers. We were advised to apply to skilled nursing facilities, but several rejected our request since there are no health issues other than dementia that require skilled care. Administration promised to help find a new facility. However, it became apparent that their "help" was only sending out more referrals to other skilled nursing facilities with no regard to ratings or location. During this process, parent was suddenly moved to a different room with no prior notice. We expressed concerns that this was not in the best interest since we would be relocating to another facility soon. Our concerns were dismissed and the move occurred in less than an hour! This was a direct violation of patient rights of having a written notice before changing rooms or roommates. On the positive side, the CMTs and aides were extremely kind and attentive. These providers were expected to care for residents' needs, dispense medicines, unlock door for visitors, take phone calls, plate food that was sent from the Assisted Living side, serve meals, and clean up. Often there were only 2 people on the floor to do all of these tasks. Needless to say, there were times meals were late due to lack of help! Bottom line: This is Level 1 Memory Care, so as disease advances, they are not equipped to handle the decline. Administration talks a good talk, but it pretty much ends there. While we recognize this is a business, we believe better communication and understanding toward patient families are necessary. The staff members who care for residents are compassionate and positive. They bond with residents, and it is apparent they truly care about them. Staff deserves a 5-star rating, but the overall facility rating is lower due to administrative decisions.

It was a…read morevery clean and well laid out apartment complex with a nice common dining area, a nice library for the residents, a cute little Coke decorated ice cream dining area, and a nice common area inside for activities. I was very impressed and will definitely put this independent living home complex at the top of our family's list to consider and learn more about. Thank you to the associate that was so kind and gracious to show me around. This is such a big decision for a family to make and you sincerely made me feel that each resident is important there.

    My father was a resident in the Maple Wood Alzheimer's Special Care facility for the last five…read moremonths of his life. This facility was absolutely wonderful for him! He had previously spent a few weeks in a different high quality nursing home, but he did terribly there due to his desire to constantly be on the move and becoming overstimulated by the noise and activity. He actually was expelled from that facility due to his restlessness and interference with other residents - such as going into their rooms and bothering them. Since all of the patients in Maple Wood suffer from similar cognitive limitations, his behavior was not as disturbing to the other residents and he enjoyed the quieter environment. He loved to walk the hallways and could stop and sit in readily available and comfortable seating. [By regulation, chairs are not allowed in the hallways of most nursing homes.] The staff were WONDERFUL. They obviously cared for dad and did their best to keep him nicely dressed and groomed - which was not easy due to his resistance to "help" and his penchant for "borrowing" other peoples belongings. The food was also wonderful, which resulted in his gaining weight (he had been anorexic) shortly after moving in. When dad began his terminal decline, the administration and staff gave us the opportunity of involving hospice and he passed away peacefully in his room with family members at his side. I never fully relaxed prior to dad's living at Maple Wood. I always had concern in the back of my mind that he would get himself into some sort of trouble and would either harm himself (accidently) or that I or my sister would have to drop everything and run to his rescue. Once dad had moved to Maple Wood, I was able to fully relax when I was away from him, as I knew he was safe and being cared for better than I could!

    Turners Rock is a truly outstanding senior living community! The building is modern--only about…read morefive years old--and offers independent living, assisted living, and memory care. Unlike many other senior communities, Turners Rock is bright, contemporary, and impeccably clean. The floor plans are especially impressive, with spacious layouts and large bedrooms--something you rarely find in other senior living options around Springfield. But what truly sets Turners Rock apart is the warm, welcoming community and the exceptional staff. Brad, the director, is friendly, responsive, and genuinely devoted to the residents. The activity director, Will, is absolutely PHENOMENAL--charming, personable, and hilarious! He constantly goes above and beyond to keep residents engaged and entertained. One creative example: he prints "funny money" with residents' photos that they earn at activities and spend at a monthly auction. The Turners Rock driver, Lezlie, is incredible--she takes my mom to doctor's appointments and has even taken her shopping at the mall. She always goes the extra mile (pun intended!). My mom loves the variety of activities , events, and outings that Will plans. There are about six activities each day--like exercise classes, trivia, book club, Bible study, bingo, Wheel of Fortune, bocce ball, movies, and more. Residents can also take the shuttle bus for picnics, plays, movies, and restaurant outings. The on-site restaurant is contemporary and open for dining between 7 a.m. and 7 p.m. (I believe). The servers know the residents by name and even remember many of their preferences. The menu offers plenty of options along with daily specials. Sometimes the chef even names dishes after residents, which is such a thoughtful touch. The desserts are always amazing, and family members often visit during meal times--just $7 for breakfast and $10 for lunch or dinner. My parents moved into the independent living section in October 2023. My dad later transitioned to the Memory Care Unit for a few months before he passed away. It was a huge relief for my mom to know he was safe and well cared for during that time. And he was just steps away, so my mom could easily visit him each day. I live in California and have stayed at Turners Rock seven times since my parents moved in. Knowing my mom is surrounded by friends and supported by such a compassionate, capable team brings me tremendous peace of mind. I'm so grateful to everyone at Turners Rock. 10/10 recommend!

    Classic nursing home with all the nightmares. Low staffing with comments from staff that are…read morecringeworthy. Care is substandard. Prn meds given when not necessary. MIL accepted even though she has dementia, is a fall risk and an escape artist. She is NOt in memory care. Had to ask for a bed bath after 5 days in and still was not given until next evening after more requests! Family hired sitters 24/7. Pretty much a mess but very typical of how administrators run their facilities. Speaking of administrators: do your job! Get out of your office and check on the staff and the residents. Make sure your nurses and med tech are not on their phones!! I have seen this problem all over that building. The building is a nightmare to navigate. No room number guides on hallways. You just stumble around until you find someone to ask for help. And that's a big if. Also, the main door is some distance from the parking area. Elderly who are disabled have to walk quite far. There should be handicapped parking much closer to the front door. Would not recommend this place.

    Clinton Lowe is the administrator. He is an arrogant, defensive man who did not provide adequate…read moreoversight of our loved one's care. We have photos and video proof of the unacceptable treatment and negligent care of our family member, which we provided to the Springfield Police Department for their report as well as to the State of Missouri Department of Health and Senior Services, and the Center for Medicare Services in our formal complaints against Birch Pointe. On day 4 after her admission we found our loved one had not been checked on for well over 7 hours, which is totally unacceptable. We showed up shortly after 1 pm to find dried, caked blood on her pillow case and blanket, her laying in urine soaked sheets and her Depends Undergarment was soaked. She also had stage 2 bedsores on her back, shoulders, tailbone and the heels of her feet. This was the direct result from not being turned every two hours, which was directed by her physician. We were shocked by this and tracked down the CNA who was assigned to her room. She explained to us (on video) that the shift change was at 6 am, and she had not had a chance to check in on her to change her bedding or her Depends Undergarment because they had other "emergencies" and were really busy. Oddly, there was both a breakfast and lunch tray sitting on her bedside table even though had become unable to eat or drink but apparently Food Service was not notified. Our loved one was on hospice and was not being adequately monitored for her pain or the administration of her pain medication. The staff said they were crushing pills and putting it in applesauce but that she was spitting it out. We looked in her mouth and found a large amount of her hair wadded up in it. Because she was not being turned every two hours or even receiving daily basic hygiene care, her hair had become matted and tangled and some got stuck in her mouth. We have video of us finding it, and we had to cut the hair around her face to remove the hair from her mouth. This is why she was not able to swallow the pain meds. It was appalling to say the least to think that she probably was like this for at least 3 days, and the staff did not know. She just had broken both of her hips and had hip replacement surgeries, and was admitted there on hospice. It is no doubt she suffered because of their negligence. When we informed the Charge Nurse, LPN, Hospice Nurse and CNA of the wad of hair in her mouth and her bed sores, they are on video admitting that this was unacceptable; and when asked if they would want this substandard care for their loved one they all said no. This was when we had our first encounter with Clinton Lowe who was very defensive, unkind nor did he show any empathy toward the situation. We showed him the unacceptable state of her care and the many untreated bed sores. We shared our deep concern that she was not being turned every two hours as directed by her doctor. We also asked why many of his staff did not wear proper IDs and asked how were identity who was an aide, CNA, housekeeping or Nurse. We noticed a handful of staff not in appropriate medical attire, but in street wear. It was a concern because we could not identify if they were a staff member, a visitor or someone else. We shared that it was baffling that for such a large facility there was only one Charge Nurse on duty and that the LPN to patient ratio was too large to provide adequate medical care. We were alarmed that there was not enough staff to handle both emergencies and provide quality care for all patients. Our loved one suffered because of the mismanagement of this facility. It dawned on Clinton Lowe that we recognized this was elder abuse and gross negligence by his staff and went into total defensive mode and stated that he would "do his own investigation" of the matter, which we thought was a load of crap. We found the security at the front door is alarmingly lacking after hours and on weekends. There was no one at the front entrance. There was only a sign in sheet. With some residents in wheelchairs sitting in the halls, someone with bad intentions could walk in and roll them out the front door. Two workers shared with us that they were extremely overworked and stretched too thin. We saw for ourselves that the facility was grossly understaffed each and every time we were there. When we called on the weekends and overnight, no one answered the phone until 8:00 am. Please investigate all aspects of any nursing facility, and look beyond the flashy, new entry way and common areas. That is just a rouse to make the facility look better than the actual care they provide. Look at the staff top to bottom as this is what matters the most when placing a loved one in facility.
