I left Meadowbrook Care Center on Oct 18th and for now I would like to thank the entire Kitchen Staff. There food was excellent. The other four facilities that i have been in the last four months were terrible and could not compare to the food I was served in Meadowbrook. Their soup was surpurb, and any time I wanted more all I did was call the kitchen and they brought it to me right away. They really went out of their way to make sure all my needs were met. Run! RUN! RUN! .my fil needed short term rehab after a rather sudden decline from complete independence. We were presented with little choice since he is on 5l oxygen. First impression was fine. Nice people. All introduced us. Reassured. Then the truth appeared. Next morning Speech therapist saw him. Then leaned in whispered Get out of here this place is horrible. We were understandably shocke, upset . The SW called us to explain this whole process. We informed them what was said. Apologies all around. She explained rehab is offered only 3 to 6 times a week. Very different from the 7 days offered on the website. We asked when therapy doesn't happen. Do u help him out of bed. Yes absolutely every day. Eat in dining room. My fil.is completely intake mentally. No neuro deficit. Just physical decline from long hospitalization. Next morning no therapy ordered. They keep him in bed despite begging for help all day. When questioned. Nurse told us nurses DO Not ever get pts out of bed. Not my department. We asked for supervisor. No one was available. Next day came up to discuss. My fil still in bed call bell rang for 30min. His insurance paid for rehabilitation. Not for him to be in bed for 2 days and staff overall not nice. NEVER EVER COME HERE IF U LOVE YOUR FAMILY!!!!!!!!! read more

I spent enough time visiting family in this facility to understand 110% why the elderly don't want to be put in nursing homes. Understaffed, uncaring, unattended, unresponsive, etc., etc, etc. I'm not going to list the countless grievances I have for this place because my family member passed some time ago and I want to distance myself emotionally from the experience of Meadowbrook Care Center. I just want to communicate to any reader out there that if there is no possibility to keep your family member at home, don't let one day pass without visiting them, attending to their unmet needs, and advocating for their well-being. Are all nursing homes/rehab facilities for the elderly like this one? God help us all. read more

Upon arriving at the faculty you're taken in by the beautiful front desk and lobby. This was a return for my husband and he was disappointed in the unknown section he was assumed to. Before long we learned there's more than beauty that matters. The head nurse in charge, Marie, was stern but knew her stuff. In the end I saw how compassionate she truly was. ALL the assistant nurse were unbelievable...thank you Marie, Ms Jarret, et,c. Finally let's not forget about the fantastic PT/OT that's provided. 28 days later.... My father was admitted to the MeadowBrook center for a total of 28 days. 93 years old, dementia,pelvic fracture. Good health otherwise, eating, and verbal. -In his 28 day stay he and our family experienced the following: -While awaiting a bed in another facility, my dad experienced a total and frightening decline in his overall health and being. To make 4 weeks a story short I offer you this info- -My father lost 15 lbs in 4 weeks at Meadowbrook- despite family members feeding him at least 2 meals daily. -My father was sent out of the facility upon the insistence of the Day Nurse Manager for an orthopedic follow up appt . He was transported unaccompanied by center staff, in pain, disheveled and without his doctor prescribed hearing aids and glasses. Disgraceful. -My father spiked a fever, developed a urinary track infection(first ever in 93 years)was put on antibiotics, and left in wet/dirty diapers(urine/feces) -My fathers IV, when finally started, was often "infiltrated". Meaning the IV needle was not placed in his vein but just under the skin. Meaning my dad was not getting much needed fluids and medicine and his arm would be swollen from the fluids filling it. Medical care at its finest -My father, only saw a nurse during med rounds- once per shift. Nurses seemingly ignored the family while visiting. Always having to ask if he received his Tylenol. -My father, frequently sat in wheelchair for 8 hrs and when family requested a more comfortable chair the nurse manger response was " he is in a chair" . -My fathers rooms: His room was changed. The bed linens in the new room were dirty, blood stained and obviously had not been changed from the previous patient. The pillows had dried feces on them. I immediately brought this to the attention of the Day Nurse Manager who stated that the bed sheets had in fact been changled as it's a health safety concern. Ha. Well she wouldn't know because she never cared to check if this basic health safety concern was attended to. Multiple requests later...the sheets and pillows were changed. The smelly fece stained pillows were immediately thrown in the trash by staff. If I'm able to post pictures with this review I will post pics of the pillows -My father became non-verbal -My father was self discharged, to the emergency room by family, against the centers medical advice on the 28th day due to lack of care, escalating pain and the frightening decline in his medical condition. Day Nurse manager argued " we can take care of him- I don't agree with this and neither does Dr Khan" - " He's not in tremendous pain". He was in fact in tremendous pain. Who is this Dr. Kahn anyway? 28 days, declining health, doesn't agree on medical discharge, yet no one in my family was able to meet or speak with him....ever. Voice mail messages unreturned? -My father was promptly admitted to Intensive Care Unit upon discharge. He was quickly diagnosed with SEVERE DEHYDRATION, and as a direct result of the severe dehydration, he was further diagnosed with hypernatremia ( sodium 179) and encephalopathy(brain swelling) For a 93 year old man this equaled a death sentence. -My father passed away three (3) days after the family self discharge from the Meadowbrook center. The finest hospitalized intensive care unit medicine and care could not save him from his hellish 28 days at this facility. The Meadowbrook Care Center provided abhorrent medical care and were directly responsible for his demise. They could not recognize dehydration?? ***Nurse aide "Heather" was excellent and was attentive and caring towards my fathers care. I would be remiss if I did not mention this kind soul. And yes there were a few other employees that deserve thanks as well.. read more

I wanted to say thank you to the Meadowbrook Care Center for giving my father 15-20 minutes of PT a day during his 20 day stay. Also to say thank you for not releasing him when we asked on his 16th day there (we had home care set up) after which his roommate got sick with pneumonia (you kept the roommate in the room) which he obviously passed onto my father. When you released my father on that Friday (strange coincidence that that was his 20th day max on insurance) he wasn't feeling well. He had a horrible night and Saturday morning we brought him back to the hospital and found he now had double pneumonia w/covid and died Wednesday morning. THANKS AGAIN Meadowbrook !!! read more

My finances grandmother was recently at meadowbrooke. She was a fall risk and when she was in bed she was supposed to have her bed lowered to the floor ,have the sides of her mattress raised, and have mats on the floor next to her . the day before she was coming home she was brought to her room because she was exhausted . She somehow managed to got out of bed , fall,, hit her head and broke her neck. No one at meadowbrooke will take any responsibility for this. My finance and her family are devastated as this has made it even harder for their grandmother in so many different ways . Would never recommend this place read more

Zero stars this place is awful. My grandma is incontinent and she sat in her Urine so much and no matter how many times we complained nothing would be done about it. I was told by a nurse supervisor they hadn't implemented a toileting schedule or taken any notes to track her incontinence. After one month of her being there, she said "oh yeah maybe we should take notes." Nothing changed. "Team work-makes the dream work" is there slogan on there website. This is the biggest false advertisement I can imagine. Their lack of care was degrading and inhumane. One staff member named Winnie was very patient and kind and incredibly helpful to my grandma. However, the head nurse Joanne was so rude and unprofessional. She couldn't care less when we complained. All she had to say was she had so many patients to care for and my grandma isn't her only patient. Administration should take notes and listen right now- if you're understaffed you should spend less money on making renovations and hire more Aid staff and nurses to help your patients. We chose this facility bc we were told they had excellent respiratory therapy. So to our surprise my grandmother who has COPD and was sent to Meadowbrook to recover from pneumonia was left WITHOUT OXYGEN on multiple occasions. I had to connect her once to the proper oxygen machine bc they connected her to the portable tank which wasn't even turned on. Another time I was not there and she thought she was going to die until Jean the PT found her connected to the same empty tank. She was sitting next to an oxygen machine too which was fully functioning. When we told Joanne she decided it was appropriate to blame the PT saying she knows how to change the tank and said oh "she's fine now." Did not take any accountability for her staff and their lack of awareness. Once me and my mother wrote negative reviews initiated from a feedback text that the facility sent us, Joanne called my mom to update her and was pretending to be concerned. They immediately decided to discharge my grandma after they received our reviews. Meanwhile we had been working on trying to discharge her for a week with no success. They were trying to not be liable for their careless mistakes! I won't even mention the HIPPA violations. Read the rest of reviews and you'll get a good idea about this facility. please anyone thinking of having a loved one, get care in this nightmare of a facility needs to read. my father left a south hospital and stressd to the point of a cardic event that took his life. he was already weak when he was discharged and taken care in a professional staging care to recovery. dont allow your loved one suffer the way my father was helpless in the hands of these so called professinonals. read more

After 20 days we were given about 36 hours notice that our request for traditional medicare was never received. Tammy K. in the Business office dropped the ball or just doesn't care enough to do her job. Caring and compassion is not only for the healthcare workers on the floor. Now we are trying to ensure my moms health coverage is back to where it was before Tammy suggested to change it. The place was clean and the food was better than expected. Wait time after using the call bell is way too long. Rehab is only 1 hour per day, other 23 hours is spent sitting or in bed. Taking care of or seniors is big business but should be a time for caring. read more

I worked at Meadowbrook Care Center, when it was Doctors Hospital, and Lydia E. Hall Hospital. I must say, I was pleasantly surprised, at the care and overall experience I received here. The physical therapy department, is above excellent. I am going to go home, on Thursday; and because of Candy and Rodger, I will be able to return to work, in the fall. I highly recommend MCC, for anyone who needs skilled rehab services. read more

Horrible, just horrible. Every step in the rehab process has been horrendous. No one knows what anyone else is doing. CNAs are running ragged. Every staff member tells you something different. My dog gets more attention when I board him. I would not recommend this place to anyone. read more

My uncle was there for two weeks. Yesterday he was unceremoniously kicked due to insurance issues. No aide, no follow-up medical assistance, no nothing. I seriously fear for his health and the health of the elderly relative to whose house he was sent. While Meadowbrook does not make decisions about insurance, there is something wrong when I receive a voicemail requesting a credit card to pay for his continued stay a couple hours before he is kicked out. Not humane. Not acceptable. read more

Very rarely do I write reviews ,however my grandmothers experience at Meadowbrook was so horrific I feel the need to convey our experience there . My grandmother was at Meadowbrook for about 5 to 6 weeks December to January . She had broken two hips and was considered a fall precaution . Since physical restraints are not allowed by law in nursing homes I.e bed rails , she was supposed to have her bed lowered to the floor at all times , have the sides of her mattress raised, and have mats on the floor. The idea being that this would keep fall precautions from getting out of bed when they are alone. The evening before she was supposed to be discharged, 1/ 11 /23 she was brought into her room early from dinner as it was reported she was exhausted . We were informed that at some point my grandmother got up from bed with all these precaution properly in place as well as being exhausted and fell, hit her head which caused a large hematoma and open cut that needed stitches. ,and broke a vertebrae in her neck. No one of course can tell us how this happened ,as they are all saying everyone was following protocol .My grandmother is now immobile, and in a neck brace for the next 6 weeks . She is now starting to suffering from bedsore . They do not manage fall precaution patients there appropriately . Never did I see her on a 1:1. Their idea of monitoring those patients when not in bed , is putting them in a wheelchair and having them sit in front of the nurse desk , which I have witnessed vacant when patients are sitting there . In one instance a women in a wheelchair attempted to stand and start walking . Someone in the hall noticed and grabbed her before she could really start moving . They have also left my grandmothers medication with her aid who would come to visit her. The idea being she would give it to her . Nurses should be the ones to administer meds , not my grandmothers visitors . They always seemed to be functioning on a Skelton crew and the nurse supervisors was only ever nice to us after the incident with my grandmother. I write this in hopes that it deters ppl from sending their love ones there ,but if you do and an incident occurs please ensure you file an incident report with the NYS department of health and with your loved ones insurance company . Also ensure meadowBrooke takes all required steps and fills out all required paperwork by law with both the department of health and your loved ones insurance . read more

They don't check on their patients and neglect their wounds and other medical issues. If you're thinking about putting your loved one in this facility I would definitely try to find another rehab center. Heard positive things about this rehab so we were hopeful for our 90+ year-old parent when our first two rehab choices had no availability. The PT staff is good and recreation director is engaging and helpful. I found the nurses helpful but severely understaffed. However, the nurses aides are disinterested at best and appear as though working there in that role -- helping people in need who are dependent on them -- is the last thing on earth they want to be doing. I understand it is a difficult job and requires a very tough mindset, but these people have zero compassion. I would not have them look after any family member or friend ever. They clearly don't want to be there despite their choice to work in that field. The "dining area," where residents congregate and eat, isn't clean, particularly around the sink area. Dirty lunch trays remain until dinner. And, try finding Purell dispensers; there aren't any. Go elsewhere. read more

I would give 5 star to this location because during my mom's treatment in this center, everybody was so nice, couteous, respectful and compassionate. Nurses like , Laurent LPN was always very cheerful, dedicated to her job ,compassionate and funny. Nurse Laurene RN was one of our favourite. She was always softspoken, easy to be with, efficient and darling. Gelli Ann was awesome and nice. St. Hubert Adeline was sweet, caring and gentle. Lorraine Brown CNA was very nice to talk to. Jackie Gale was chipper, cute and compassionate. MIchael Colorio always greeted us with a big smile and helped us, PIerre Renon was well informed and gentleman. Jennifer Smith was always cooperative and full of youthful energy, Dianna Diaz was awesome, quick to help and kind. Cadet and Kenia were very sweet and kind as well. Social worker Nathalie Lafrance was very sweet , well informed, efficient and helpful. Arfa from (Recreation dept.) was very nice to my mom and spoke my moms dialect and made a very good rapport with my mom. Cathy the Nutrionist was very compassionate and easy to be with. MY MOM DOESNT SPEAK ENGLISH BUT DESPITE THIS BARRIER EVERYBODY MADE MY MOM VERY COMFORTABLE. MY GRATITUDE TO EVERYBODY AND SALUTE TO ALL THESE HEROES. Jay and Kanesha at front desk were always greeted cheerfully and respectfully. All together it was a very nice heart touching experience. The rating on communication alone would be a 1 star. Administration never calls you back. The director seems to have a very standoffish attitude. As if she can't be bothered. The nurses however are very nice. And hospice care was good. I would keep looking though. read more

We had to send my dad to this location. I am very impressed with their service. Paul is one of the nurses and he really helped us out. He's so kind and knew the right things to say when my father passed. The room they had us in was very clean. They changed my father often so he was cleaned twice a day. When the skilled nurse Paul knew my dad had a few hours left he pulled me to the side and told me. Know that there was hours left helped me. My father went peacefully under the care of this location. I'm just in shock with the big difference of how this place was compared to the hospital. The Nassau hospital was the worst hospital in the world. They allow you to visit your loved ones at anytime and you can even sleep there. read more

Wherever you take a sick loved one when they have to be in a nursing/rehab there is always a problem. My brother has been in (2) nursing homes since 2021 and to start with the staff is always RUDE, if this is not the type of work for you then don't, the last thing a sick person or the family needs is more problems, having to put a love one in one of these facilities is not an easy choice. When my brother was first sent to Meadowbrook I felt good and I was able to access the staff and discuss his case with no issues, now I am being told that my name is not an authorized family member, and to make matters worst someone is calling my 20 yr old nephew about my brother's bill, he's under medicare and never said anything to my brother. A young boy is not capable of handling this information and my brother said he ask daily when will he get to go home, its a fact sick people get better in their own homes, Now until I'm added to the family authorized list again this is the only way to vent my frustration. My mom had to go to rehab after hip surgery (which she had in Southampton) if it wasn't for Joyce in admitting who went over and beyond to get my mom admitted- our family would have been trekking over an hour to see her each day. Although she only needed to stay for 10 days, mom was very impressed with her physical therapists (Elaine and Jean as well as others) and the aids were always pleasant and attentive. According to mom: the food was fine, the room was clean and although she absolutely hated to be dependent on people, she felt safe and respected. (The noise level - constant beeping of hallway alerts, and loud TVs were her only real complaint) I really liked the video doctor available at all times and the ease of discharge. read more

For the most part the care is good, but they need more help. I was there the other day and my mother begged to go to the bathroom several times and was ignored. She had no alterative but to soil herself. Again, the problem is they are understaffed. They keep the place spotless but they should pay more attention to the resident. read more

My father in law spent a few weeks there and we were somewhat prepared that he wasn't going to get better. Because of Covid we weren't able to see him. We cannot comment on his care because we were never able to see him. We were never made aware that he was near the end. Someone did call but they were unable to communicate clearly. The facility needs to be more compassionate and establish better communication with the families during this time. We were only able to speak with them once a week and FaceTime him only for 10-15 minutes on Fridays. When asked for more updates we were told they were unable to do that. Dr Jaglall was very nice and supportive. Joyce was very helpful when getting us information. read more

Really can not say enough about the care center and there staff, my father was in rehab for 100 days between two facilities, the first one was a true nightmare. Meadowbrook and Mike C (Nursing Supervisor) were absolutely my saving grace!! They cared for my father like one of there own!! Even when we were locked out due to Covid-19 they did weekly check ins, offered family face times and made sure my father called home once a day!! Excellent aides as well, Lauren on 2c made my father and I laugh. Answered all of my questions and cared for Dad with a loving heart!! Way to go Meadowvbrook Care Center. If Rehab becomes a necessary again you will be the top of our list!! read more

I was extremely concerned regarding the care for my brother at this facility, the care that it seems he did not receive and the outright lies we received from both the administrative and clinical staff. Getting a return call about his condition and care issues was almost impossible without multiple follow up calls. We we were continuously lied to about matters of care and condition. Based on very limited visits we were allowed with my brother is seems he was not getting the level of care that we had been advised. I would never allow anyone I love to get "service" from this facility. Initially, I had such high hopes based on the reviews I read. But that expectation was shattered quickly. Keep in mind, my family was extremely involved in understanding my brothers condition, expected outcome and the treatment plan for his care. Perhaps to someone who was not that involved, they may be happy about the care that their family member received based upon staff updates received. This facility was especially bad at returning calls to answer family questions. Whenever we were able to speak with someone at the facility, we were given glowing progress reports that did not match reality when we dug deeper to get all of the details. Unfortunately, during the early part of his stay, we made decisions that were not consistent to the actual outcome based the facility lies or incompetent expectations we received. This cost my brother a very large amount of money because we did things on his behalf that turned out not to be warranted. The money we spent could have been better used for his future care needs. I can not begin to explain the the lack of professionalism I experienced while my family member was in the care of MCC. Admitted as a vent patient and unable to communicate, I managed his care. From day 1, I was unable to get any consistent, comprehensive information. I based costly and difficult decisions on erroneous info. His doctor was dismissive and rude. The administrator was condescending and aggressive...implying that I was not clearly assessing the situation. The director of recreation was.unorganized and did not schedule zoom calls for Christmas and Thanksgiving ....i scheduled these weeks in advance. I was promised return calls dromnthe medical director and the director of nursing to explain his conditions...no calls came. A care plan meeting basically discussed how great he was doing. They sighted interactions with my family member that never occurred. The entire executive staff showed a flagrant disregard for the family. I am totally exhausted and frustrated with this situation...tired of the lies and the obstacles. I am grateful for the PT and floor nurses who gave me updates when I could contact them. I would never recommend that anyone place a loved one in this facility. I realize that covid has placed a new dimension to nursing care...but there is no reason or excuse for family to be treated so disrespectful. There is no reason to expose a loved one to the erratic care and medical neglected. The only dept that returned calls and expressed interest was the business office... Don't even consider MCC. read more

I am very disappointed in the care provided at this facility. I am in long term care myself for over 30 years as a health care provider and was appalled at how my mother -in-law was treated here. I took her concerns with a grain of salt, until I saw for myself how she was being treated. When I approached the RN staff with concerns in a professional manner as I am fully aware of the regulations and how this business runs, I was treated rudely by RN supervisors that should be the foremost customer care providers. The DNS and ADON did not return my calls. The RN unit manager attempted customer recovery, however the fact that my mother-in- laws condition was not treated appropriately and she developed an open wound from delayed incontinent care is unacceptable. Her son documented it took an hour and a half for incontinent care to be provided after she requested assistance. She was transferred to the hospital with an acute condition and will not be returning to this facility. Shame on you Meadowbrook as our first obligation is to uphold the stanard of care to our residents. read more

Our father was a resident at Meadowbrook Care Center for about a year and a half when he passed away peacefully in May. The staff was absolutely wonderful in every way. He was treated at all times with the utmost dignity and respect. The aides on Unit 1b and the nurses took care of our dad as if he was a beloved member of their family. Great facility! Amazing therapy department! I have visited several facilities and this is the best in LI. Very pleased with the nursing staff in 1b as well as rehab. Highly recommend this place! Danielle and Andrea are two of the therapists working with my family member and they are exceptional. My grandmother was sent here for hospice care and it was the best care she has received. The staff and nurses were incredibly compassionate, friendly, and accommodating. They truly did everything in their power to keep her comfortable and at ease towards the end of her life. The staff was also unbelievably sensitive to my family and made sure they provided us with everything we needed in order to properly grieve. The facility was extremely clean, neat, and the rooms were very much modernized. It was such a relief knowing that my grandmother spent her last days somewhere comfortable, elegant, and with a wonderful staff. read more

The place itself does not look institutional. It has a B &B look to it and is both well kept and clean. My 90 year old mom came in after a hospital stay in which she was in a critical condition from covid 19. She was then released to a covid unit at Meadowbrook care center. Their dedicated unit was set up with a great staff that had as much knowledge as was available in treating and helping a covid patient. Due to this care she was able to graduate to a non covid unit and receive excellent physical and occupational therapy that helped her recover to the point that she could breathe without an oxygen tank and walk with a walker at the same level she did before covid. She was able to return to her assisted living facility. Being an only child who is very hands on with my mom it was a extremely stressful to navigate from afar because I was not able to be with my mother. The Head of Social Work, Ayelet Goldberg, made it all work. I felt as though she was my partner; communicating with me multiple times a day filling me in on developments, listening to my input and of course speaking to my mom daily to make her feel better while she was very fearful and lonely. I highly recommend Meadowbrook because as the world was spinning with this health crisis they were able to meet this challenge and restore my mom to the condition she was prior to the covid. read more
