Unfortunately my sister has been at this facility for over 2 months and she is in the same place from day one. She has not gotten better nor made any progress since day one and i find this unacceptable! I've called for over a month to talk the administrative staff and THEY WILL NOT CALL ME BACK. Jasmine the social worker! Mark the Director, Elsie the head nurse! And many others and I'll call 2/3 times a day to leave messages and nobody will get back to me. I'm now trying to figure out who runs MainPlace Acute and report at the highest level because this is unacceptable!! She had needed a new PCP that for over a month we have asked for. The doctor has not seen her. We have asked for referrals for her eyes, a cpac machine for breathing due to COPD and her being in hospital so many dies from breathing in poisonous carbon dioxide. Also she needs a wheelchair that she has not had since day one. PT can't even practice getting in and out of wheelchair b/c she does not have one and the request are many!! This place is a poor excuse for taking care of my sister!! For a business to ignore calls and not go over a plan w family is beyond anything I've ever experienced. My sister has been in terrible places but NEVER EVER have we had a facility that completely ignores calls and WILL NOT CALL BACK!! I've left 50 or more messages and I'm in another state trying to help the best I can. For MARK the head of this facility not to call back a family member when messages have been left over and over. This man NEEDS TO BE REPLACED!! As far as the nurses that car for my sister, most are very sweet and do what they can. They too have left messages for Jasmine the Social worker to call me back. They leave notes on her desk but for some reason does not feel the need to help in any possible way. They collect money from insurance company but do absolutely nothing to help with bettering her health . I'm beyond puzzled by this place and I do not know who to go to?? I will report this place to the insurance company and find out who MARK reports to!! I pray that whomever he reports to cares that this man thinks it's ok to have everyone on his staff that does not care to help or return calls. Again the administration staff is beyond terrible!! The nurses and receptionist that have heard my complaints are puzzled as well. They are kind and give out the messages. They know I've been calling for over a month. These are people that are in the highest positions that don't think returning calls are important! They have their lead MARK backing them up and it's said to have a man in this position that feels it not necessary to call and or give you a number for help?? Something needs to be done about this!! HELP!! PRAY SOMEONE IN A HIGHER THEN MARK SEES THIS AND CALKS ME BACK!! You need to know what is going on within your facility at main place acute!! As a 57 yo woman, I never thought I'd be in a facility like this, but life is be strange that way. The doctors kept thinking I was having strokes, when in reality I was being severely overdosed on Tacrolimus. Unable to walk or use my left side, I was finally diagnosed correctly and was sent here to recover. It really is a great place, filled with people who care about their patients and enjoy their jobs. I was here for about a month. Omar is wonderful and works very hard to make patients comfortable. I would highly recommend Main Place for my family and friends. read more

Still in the process of evaluating as elderly mother in law is currently a patient in their car. What I can say, is that the facility appears to be clean and pretty well staffed. According to the patient, the food is lacking flavor. Then again, what hospital food is actually good? Rooms are good sized and seem to be limited to two patients. Galway's are wide. My mom stayed here for about 1 month post knee surgery. She was hesitant at 1st but was happy she chose this facility and so was I, her daughter. Her favorite CNA was Gina. The care was great. My mom enjoyed working with their physical therapists. Lehong, i belive she was the floor supervisor, was always happy to answer any questions we had. I was at MainPlace Post Acute for 3 months. When I first got there I couldn't move either of my legs. Thanks to my therapy team, especially Melany, Leslie, and Tiffany Chen, I am now able to stand up and walk. Their positive energy and encouragement gave me the motivation I needed! During those 3 months, I also received excellent speech therapy from Tracy, Janelly, and Christine, as well as compassionate care from many LVNs: Sydney, Carla, Selenna, Danh, Kat, and Kathy; and CNAs: Claudia, George, Marisol, Israel, and Sean (sorry if I left out anyone else ~ you know who you are!). I left there happy to call many of them friends. Dr. V was also an excellent doctor who took very good care of me and advocated for me. SNFs I think are almost always what you make of it and your expectations. I was very motivated to get better and move onto an acute rehabilitation facility, and the staff there, especially those mentioned above, kept me motivated and cheered me on. The reason I'm only giving 4 stars is the food, which is terrible! read more

During my stay at Main Place, I had surgery and I also had a wound. Dr. Vee was very caring and made sure I received the medical attention I needed. He is one of the nicest, caring, and professional doctors I have met. For wound care Dr. Sor, Jose and Maggie provided excellent wound care. Nurses Jocelyn and Sydney are definitely assets to this facility. As for CNAs, Larissa, Jenny, Jordan, Evelyn, and Melissa were the best! Unfortunately, a couple of really good ones left. There is a high turnover and I can see why. The food is terrible. I ordered food from UBER almost daily. There are thieves at this facility. I've had personal items stolen such as a dress, perfume, air freshener, body wash, shampoo, and conditioner. Fortunately, all of these items were inexpensive, but that's not the point. If you decide to put a loved one at Main Place, I hope they are alert. Make sure they don't have anything valuable. Overall I have to say there is more bad than good unfortunately. read more

My Dad was here for over a year. He contracted C-diff, sepsis, covid and pneumonia throughout his stay. He had a brain tumor, after his surgery the nurse never ordered his medication. I called the health dept. they were found to be in violation of a few things and fined. Please DO NOT bring your loved ones here. Office staff and management pretend to care. Try calling after 6pm! Nobody answers, ever. Sadly, my Dad passed away last month. I blame this place. read more

Avoid at all costs. Their negligence, and that of Dr. Sadr, resulted in my dad getting a UTI and sepsis, in addition to a respiratory infection. He passed away three weeks later at St. Joseph Hospital. I've reported them to Calif. Dept. of Public Health. We had to constantly chase after the nursing assistants for help, and the LVNs were sullen and mean. As another review said, there were bugs flying around. He even had to be moved to a different room when we spotted a trail of bugs in his room. Thanks to their negligence and disregard for the well-being of patients, I'm planning my dad's memorial instead of celebrating Christmas with him next week. read more

Complete failure in rehabilitating my mother and preparing her physically to do something as simple as getting out of bed. 3 months and she is weaker than the day she went in. Although the people smiled, were very nice, and treated her well, the staff kept her in bed and her physical therapy was practically non existent until her Medicare benefits were about to run out and her time there was now limited. read more

Every person who put a good review out here is LYING. My sister was put here for hospice and I wouldn't leave my animals here. It's dirty, smelly, and disgusting. Bugs and flies flying around. There is no security and gardeners walk through the halls with blowers, rakes, and trash cans. There are no nurses at the nursing stations or the entrance. One of the nurses or CNAs was having sex in the front seat of a truck in front of the facility. I had to demand a new room because there were paper towels on the floor with feces from the other patient in her room and I had to ask three times before it was finally cleaned. In addition to that the first room had no air conditioning. We have been here for 2hrs no assessment. The nursing staff is overwhelmed and I will be filing a complaint with the State of California and immediately moving my sister. Shame on the owners of this facility. read more

my dad was here for some time before his condition worsened and was transferred back to the hospital, on the first day when he arrived and we visited, he needed to be changed and was not fed at all, the cna assigned to him was too busy with other tasks that we had to change him and feed him ourselves, this continued during his time here, during all my visits not once did the cna or lvn come to check up on him or his roommate, this place needs to seriously higher more workers to provide the attention needed in a post acute care and need to be trained properly, I'm only sharing my story to help you make a better decision on where you want to send your loved one read more

This facility is unsafe and a danger as well as a liability to patients. The staff all around is incompetent on every level. Patient care in non-existent and the general environment is nothing more than chaos. Patients are neglected, and do not receive basic caregiving needs, and those administering medications are confused and distracted. there is no chain of command and the nursing staff isn't capable of treating and caring for anyone. The rooms are filthy, the hallways are lined with trash and discarded paper, the public restroom is disgusting and there is no A/C. Lastly, the food is not fit for human consumption. Sadly, this facility is absolutely the worst of the worst. By the way, the reviews in which others give praise to this center are not true, valid or accurate. Main Place Post Acute recently hosted a Valentine themed Prom for residents. It included tuxes and prom dresses, hair and make up and a giant heart filled photo spot. Residents followed a red carpet to a white tent dinner complete with beautiful decorations, music, and delicious food. Kudos to facility Director Mark, Activities Director Flo, Food Manager Johanna, and all the staff. They created a night that made each resident feel truly special and we appreciated all the time and effort it took to pull it off. read more

A big shout out to Baebie on unit care and chuck and Laura for PT and OT therapy care. Although it's an older facility the short term wing treats you like family. I was there January 2019 thru June 2019. The worst of covid. I was treated with dignity care and respect. Funny note One of the lvns was big into q anon so that was funny watching him get excited over nothing. The administrator mark ruggles was top notch and both a listener a fixer and a caring individual. If I had to go back to a nursing home it would be Main Place. read more

My 92 year old Mom had a stroke in November of 2022 and after an amazing recovery ( kudos to St Jude Stroke Center) was hit with shingles at the end of January 2023. The shingles and anti viral meds knocked her for a loop. After an ER visit and stay at a local rehab facility (not Mainplace) in February 2023 she came home. Unfortunately, she was not better. ENTER MAINPLACE POST ACUTE in March of 2023.... Mom has been at Mainplace for 3-4 days now. (I will update my review every few days.) Mom went into the ER incoherent, sleeping all day and barely able to move on 03/20/23. ER ran the tests and blood was normal, no stroke and no UTI's. Normal tests and healthy. St Joes ER and social workers (Lucy and Dasha) were awesome as always and said her tests are clean but let's send her to rehab. Apprehensive to send her to another rehab facility as she left her last rehab 2 weeks prior and she came out loaded from the meds they gave her to stimulate appetite. I was given a couple choices of facilities that had beds available this time around. Before I could even choose a facility Val from Mainplace admissions called me with questions and some comforting chat. And the chat was not a push to go to her facility. It was more of a reassuring phone call and to tell me if Mainplace is my choice they will do they utmost to help Mom. I was impressed with Val. I could tell she knew her stuff and really cared. Mom entered Mainplace incoherent, 95 pounds and barely able to walk. 2nd day that Mom was at Mainplace I found myself on the 22 freeway saying a prayer that Mom will have a breakthrough. Just give me something darn it. I walked in and there mom is wide eyed, clean, sitting by the window. I was making sure that my contacts were in right as I could not believe I was in the right room. Engages with her roommate. Weird cuz my mom was one to be anti social and somewhat cold. Regular bathroom visits and a meal plan that was blessed by the dietician at Mainplace I believe is helping her recover. Light physical therapy until we can figure out the appetite and sleeping thing. Mom is smiling, does not smell and is diaper changed or taken to the bathroom as needed. She is eating...( maybe it is my crap*y cooking that is making her appetite go south????hahaha) If I have a questions for management I am encouraged to visit the case manager Mina or to have a chat with the onsite social workers. I walk over to their office during office hours and they chat with with me about care. Im welcomed into their offices..No questions... CNA's are pretty awesome too. Yanes is extremely kind and helpful. He speaks to my mom in French and German to engage her....., there is a young lady named LLelian (something like that) who is just awesome...Martina, Lupe, Fred. Gina...All amazing.. So far so good. Im not saying things are going to be rosy every day for Mom but I will take what I can. As stated I will update my review. Backstory for me is that I have dealt with Mom's rehab decisions, some smart and some not so much, I have dealt with the rehab decisions for my late father and my handicapped brother. Im 51 now and the 20 years of education I have received from medical professionals has been overwhelming at times. (Funny as I never finished college) But take it in. Write it down. Bad and good info. Just for those of you who are new to this very delicate subject, everyone has a valid opinion. My opinion is honest. I have chosen 5 star facilities rated on Yelp, Google and even Medicare reviews and I have been disappointed. I have chosen 2 star reviewed facilities and been blown away. I stress when choosing a rehab facility for your loved one, talk to the admissions dept, case manager or social workers first. Ask stupid questions cuz with your loved ones there is never a stupid question. If you are rebuffed for stupid questions you have the choice to just get up and leave. Visit if you can. If during your chat or during your tour you feel like you are being treated like a business transaction, most likely that is how the facility is run. Your loved one needs compassion and nurturing. Dont be turned off by unruly patients that may be loud during your visits. They are at every facility and the staff does their best to calm the situation down. You are always going to have diapers that need changing and trash cans that smell. CNA's and nurses will get to your loved one. Look for organization..Are items stocked for patients? Are hallways clear? Mainplace has no weird smells, organized, clear hallways. I dont get the sense of things being out of control at Mainplace. Never patients going down the hallway lost and alone...I have seen that at other facilities. SO FAR SO GOOD! When I started looking for a rehab facility for my father I was concerned about the reviews I saw on yelp from all of the places available that took my Dad's insurance. Main Place Post Acute rehab looked the best. My Dad fell and has a partial hip replacement. He needed to have physical therapy. He is 89 years old and has dementia. The facility isn't fancy but it is very clean. The nurses are nice and when I asked for something for my Dad they were helpful. I was most impressed with the physical therapists and that is the main reason he was there. ALL of the physical therapist were very kind and did a great job with my Dad. My Dad is not able to use the call light to get assistance due to his level of dementia so I did need to hire an outside agency to be with him during the day to help him. I would recommend Main Place Post Acute rehab. read more

I do not recommend this place to anyone. Does not matter what kind of need someone needs here. They have this huge runaround. The ON staff are TERRIBLE! I have pictures and conversations with the staff about how incredibly rude they were to my mom and her roommate. The way they talk to the patients is terrible. I understand the patients are upset and need things but my goodness, 1-4 hour wait to change an individual. I do not recommend this place at all!! I came in as a Level I Occupational Therapy student with Sabrina as my clinical instructor, and I had the best experience. The entire rehab team was friendly and supportive. Even though I was only there for a week, I felt like I was part of the family. I also witnessed the rehab team in action; they were well-prepared and knowledgeable about their residents, providing proper treatment. When I graduate from school, I would love to apply here for a full-time job. Hopefully, by then, they'll have space for me :) read more

My husband was here for a little over a month and received very good care. The staff was kind and personalized each visit to his room using his name and relating something pertinent to his stay. He received appropriate services from different specialists and they each gave me updates as well as had myself or another family member watch sessions so we could continue the activities at home. They insured my husband's participation in the social activities room regularly. And epically: he liked the food. He had minimum of three staff assigned each day on the health services side of things insuring he was clean, fed, wound care monitored, and received his medication.They also communicated with me regularly and asked about my wishes or goals for my husband. Social services and case management checked in with us during his stay . I so appreciated this facility. It may look a bit humble and the cable reception wasn't top Notch but the warmth of the staff from all departments was amazing. At the end of the day, our family felt our guy was well cared for. My father 94 has been in MainPlace Post Acute since 2019. My family feels extremely blessed that he has improved in so many ways. Dr. Suresh Veerramachanei is the best doctor my father has ever had. He is very committed to the wellbeing of his patients. There are some amazing staff members that have made a difference in my dad's life. Seeing my father smile is priceless. When I first visited Mainplace, I felt a positive energy throughout the facility. Little did I know how instrumental all the staff would be when Covid hit. The activity director, Nancy along with her awesome team did an amazing job keeping up the spirits of patients. In the past my dad was withdrawn and kept to himself the majority of his life. As time went by I noticed how my dads spirit was changing. Staff mentioned that my dad loved singing out loud through out the halls. The physical therapy director also mentioned how my dad was a workhorse and committed to his physical therapy. My dad also became very engaged with the activities at Mainplace; he was the only resident that sang during a talent show and volunteered to participate when asked. Nancy also assisted in making my dad's birthdays extra special. Seeing the happiness on my father's face was such a blessing. Beautiful inspirational themes filled with vibrant colors fill the halls at MainPlace. I always looked forward to all the celebrations. I really appreciated receiving pictures of dad having fun. Seeing these fun Instagram videos also lifted my spirit. When one loves their job and their mission, it is demonstrated through their work. The halls of Mainplace come alive with music, vibrant décor, creativity, and laughter for residents. read more

Don't send anyone you care about here. The lack of communication or common sense is appalling. Most information was not communicated to us and we would only find out something when visiting. My mom was moved 4 times in 2 weeks and not once did I get a call about the room changes. The rooms were not maintained well including squeaky doors, broken electrical outlets, crappy beds, etc. Most of the staff had awful attitudes and wouldn't help if it wasn't their job or section, but then would also not get help from someone who could. They would get annoyed if my mom didn't eat fast enough. The food is terrible so we brought dinner most nights and brought snacks. The few people who were nice are too few and far between. I was unable to visit for a few days due to the amount of sick people at work and I was afraid of bringing a disease into a nursing home. In those 3 days I didn't see her her condition deterated rapidly and I never even got a call. I made the biggest mistake possible by trusting this place with my mom's care. I thought that as professionals they knew best. How multiple medical professionals and aids could see my mom in that condition and not take her to the hospital just a couple hundred yards away or at the very least call me is absolutely astoundingly atrocious. The 2 days I visited before she passed had at least 3 staff checking on her while I was there and acting like everything was fine when she wasn't eating, drinking, barely aware of her surroundings, hardly able to speak and getting weaker by the moment. They mentioned things like medications and physical therapy they were supposedly doing. To top things off when picking up her belongings the day after her death they apparently didn't tell the receptionist because he acted like normal and was asking us what room she was in after we already told him she was gone. He took that as she went home and only after repeating she was gone and we were there to get her things did he decide it was a good idea to run his fingers across his neck to confirm our meaning. That was infuriating. That is what I will remember about this place. They couldn't be bothered to communicate properly when my mom was alive and getting better and they couldn't do it even when she died. Edit: I received a compliment asking if things were so bad why didn't we move her to a different place. I'm not overly familiar with Yelp so I'll just answer that here. Basically boils down to time. She was at Mainplace for a hair over 2 weeks. The 1st week was actually good. My mom became fast friends with her roommate, liked the nurses in that section, was happy with the physical therapy, she was getting stronger bit by bit, and there wasn't much aside from the food that she was unhappy with. It was only after that week when she was moved for the 1st time did things started going downhill. Bad roommate in the 2nd room, the lack of communication became more and more apparent by the day, she didn't like the nurses in the section where the 2nd thru 4th rooms were, and it wasn't until after the 9th or 10th day did her health take a turn. After the 1st move we were looking into ways to get her in the original room to be with her friend as she was much happier. Trying to get ahold of someone who we could talk to about that was difficult. Most of the time when we called noone answered and when they did they didn't know when the person we needed would be in. We did start talking about moving her if things didn't improve. Her health rapidly declined over a few days, some of which we weren't aware of because of the reasons above. While we did not like the facility more and more we still trusted that the medical professionals would take care of her and at least let us know if something was happening. When visiting after she was getting bad we still believed they would tell us if it was as bad as it seemed or not. Since they acted as if things were normal we held onto that sliver of hope. By the time things had gotten truly bad it was too late for us to do anything aside from trusting the people there. Hindsight says that was a mistake. I left this review because I don't want anyone else to deal with what my family did. It is hard not to think that maybe I would still have my mom if she had been in a different facility. Things can happen quickly with our loved ones and this place won't have your back when it does. She went from being on the road to recovery to dead in a matter of days and the only call I ever got during those few days was to tell me she had passed.
