I appreciate their willingness to really work with the whole family as a unit and keeping me involved. My dad is a patient of one of the doctors in the practice and they have taken great care of him. They members of this practice have been there every step of the way and are great at making sure that my dad is healthy, has his prescriptions and has access to the services that he needs. I am grateful to the members of this practice for keeping my father healthy and making sure that I was aware of his condition. As a member of the healthcare field myself, I am impressed with their quality of patient care, and the time they put into their patients.

The…read moregroup is meant to be the primary care provider of home bound potentially very sick patients. The nurse practioners, the owners who are nurse practioners and the office staff all are on a different page as to what is suppose to go in the practice and what actually does go on in the practice. The doctor who is listed and apparently by law has to sign off on referrals to the VNA is a figure head. As one nurse practioners who visited a very complicated and sick elderly patient told me I have never met or spoken to the doctor and he knows nothing about any of the patients. This is a problem as medical professionals regularly seek out opinions from other medical providers or specialists on complicated patients. What good is a team if other members of the team are not utilized. As a medical care provider myself I have been shocked and appalled at what is going on in this practice for many other specific reasons. First of all, the patient I am caring for was sent home with a diagnosis of CHF, a recent history of aspiration pneumonia both necessitating a hospital bed. Despite being at home for greater than 2 months and several phone calls by the VNA nursing and physical therapy staff no orders have ever been completed, nor calls from the office staff returned. This patient had an issue with significant diarrhea. The staff only make regular home visits and I was told are not equipped to make sick visits if there is a concern. I suggested given the patients confusion that the daughter request lab tests since this patient did have a history of an elevated potassium and low sodium previously when she was dehydrated due to her kidney disease. The labs were ordered but although the results were posted on Saturday the patients daughter had to call the owner of the practice on Monday to ask for the results only to learn that the potassium was 6.0, a critically high value that could have been deadly. The patient's nurse practioner didn't get back to the daughter for another day! This elderly women has had a history of urosepsis and despite many calls by the patients daughter orders were delayed to get a urine culture. Once the urine culture revealed a significant infection it was the daughter who had to notify the group about the results. The daughter was concerned that her mothers symptoms and confusion were similar to what she usually observed in her Mom and notified the nurse practioner last Wednesday. She spoke with them again on Thursday and someone was to give orders to the VNA to get a urinary catch specimen. The VNA called the group and never received a call back. On Friday the owner was notified and supposedly called in a order to the VNA but apparently that was not done either. The daughter had to take it upon herself to get the urine specimen and take it to the lab herself. She notified the group of the results on Monday of this week. As of this AM when she called the groups office at my urging to get a prescription she was told they would return her call by the end of the day. Why a women who has had urinary sepsis, has kidney disease was not treated 2 days ago and had to wait is beyond explaination. Needless to say this women woke with flank pain, fever, and chills. She needed to be admitted for pyelonephritis and treated with IV antibiotics. I believe that had the group had common medical knowledge last week there would not have been a delayed diagnosis and another hospitalization.
