I will say this: Get your MRI before you go. Pain management places tend to charge pretty hefty fees for meeting, and there's practically not much they can assess without your MRIs done. Everything is pretty much hypothetical with no treatment plan until they go through MRIs, so honestly I wouldn't even go until you have it. Saves you the cost on an entire visit. Also, please ask for your MRI reports. The doctor is super friendly, but they will only discuss matters associated with your accident. There could be major things on that MRI that you would 100% want to know or follow up with your primary about. You definitely should prepare to be proactive in order for anything to be seamless.

I was already at the lowest point of my life emotionally, and fitness was all I had to process my pain. As the nerve damage worsened, I tried **three epidurals** -- only one helped. I lost my insurance and couldn't afford surgery. For six months, I woke up every day with intense nerve pain, cramping, and ankle lock-up. But even among this elite group, Dr. Ehsan Saadat stood out as the one doctor who I would entrust with my spine, and by extension my life and the future of my children, above everyone else. As a scientist myself, I know a remarkable and detail-oriented mind when I see one, and Dr. Saadat is as good at it gets. My case was not straightforward: I saw 6 different and distinguished spine surgeons and got almost as many different opinions on what to do. Dr. Saadat spent a lot of time thinking through my case, he referenced the latest scientific evidence, and he consulted his trusted colleagues on my behalf to double check his own judgement. He also spent a lot of time emailing back and forth with me to discuss the pros and cons of various approaches rigorously, patiently, and honestly. On a human level, It became clear pretty early that Dr Saadat is caring and generous with his time and attention. I can imagine that when you do something as demanding as spine surgery for a living, day in and day out, then no matter your level of empathy it becomes easy to forget that what is routine to you can be one of the most scary and nerve-wracking experiences for each of your patients. Not Dr. Saadat; he treated my case with the same level of thoughtfulness, attention to detail, and meticulous planning that one would usually reserve for their own family. I felt that my spine is in good hands all throughout the process. Dr. Saadat is also a man of great integrity. The surgery we ended up going with is far from the most expensive for my condition, but it was the best tradeoff between risk and reward. He talked me out of a more expensive surgery --- which is something he also specializes in ---- because he believed something simpler is a better risk/reward tradeoff for me, despite it being a close call. He based this on evidence, data, sound judgement, and consultation with trusted colleagues. That's the kind of honesty, care, and impartiality you want in your corner. Leading up to surgery, Dr. Saadat made the time to sit down with me and explain in detail what he was planning to do. He had a well-thought out answer to every question I asked, and I asked many as I had spent the past two months reading and watching everything I can find related to my spine condition. It became clear that he left no stone unturned in preparing his team for my surgery, and making sure I'm comfortable with everything he planned to do in there. He explained how he will use every modality of Neuro monitoring available, how he would decide which size and angle of implant he would use based on what he finds when he goes in there, and many other details. Best of all, he assembled a team in place that would be able to handle every contingency, and he told me about them. Notably, he explained how he will be joined by an exceptional neurosurgeon colleague in the operating room; this turned out to be a prescient judgement call, as they discovered dangerous adhesions to the covering of my spinal cord once they opened me up, a grave challenge that Dr. Saadat's all-star team overcame with precision. Without Dr. Saadat's meticulous preparation, his refusal to leave anything to chance, and his dedication to leaving no stone unturned for the benefit of his patients, I might have been left permanently disabled or worse. I am thankful I chose someone who was able to keep me safe, give me my life back, and safeguard for my children their father. On surgery day, I experienced no hardships. The last thing I remember before surgery was joking around with the anaesthesia team as they injected something in my IV, and the next moment I was talking to Dr. Saadat as he was checking my strength after surgery while I told him how I was feeling remarkably little pain. I was released 24 hours later after staying overnight for observation, and left the hospital virtually pain free feeling as if nothing happened, having to consciously remind myself to take it easy. Once all was said and done, my surgical outcome has been great, and I'm continuing to recover nicely with all my pain and symptoms seemingly a thing of the past. Dr. Saadat has been with me every step of the way, generous with his time and his attention as always. I couldn't be more satisfied. I was also leaking a huge amount of fluid from my back, to the point that I would wake up with my bed completely drenched. I called Dr. Attinello and told him exactly what was happening, and he had me come in. I was told that the fluid was simply pus draining from the wound and that I should put gauze over it. I followed those instructions for about a week while my pain became unbearable. I was screaming from the pain constantly, and my neighbors even told me they were going to call the police because they could hear me screaming. Eventually, I went to the emergency room. At the hospital, I was told that what had been leaking from my back was cerebrospinal fluid. In my understanding, this had not been identified during my earlier evaluation, even though I had undergone imaging including a CT and MRI. I then developed osteomyelitis in my spine. I had to undergo another major surgery, during which a significant amount of infected tissue was removed from my back, and I spent close to a year receiving IV treatment to eradicate the infection. When I woke up after that surgery, I was paralyzed and had to relearn how to walk. My recovery took approximately a year. After finally learning to walk again, I later required a third surgery to fuse my spine. I woke up from that surgery unable to walk as well and had to go through the process of learning to walk yet again. This entire experience took roughly two years of my life. What began as one spinal surgery turned into two additional surgeries, a serious spinal infection, long-term IV treatment, paralysis, and years of rehabilitation. I considered pursuing legal action, but ultimately I could not bring myself to relive everything I had gone through. I am still deeply angry about what happened. From my perspective, the way my symptoms were handled after the original surgery had life-changing consequences. I would strongly encourage anyone considering spinal surgery to advocate for themselves, seek another medical opinion if something feels seriously wrong, and take symptoms such as severe pain or unusual fluid drainage extremely seriously.

I tried everything I could think of--massage, physical therapy, acupuncture, radiofrequency treatment--but nothing gave me lasting relief. Eventually, I could barely walk for five minutes before the pain and numbness would travel down my leg. I felt like I couldn't live my normal life anymore. When Dr. Ajiboye reviewed my MRI, he explained that the degeneration in my spine was compressing the nerves. I already understood that my condition was unlikely to get better without surgery, but I was still very nervous about making that decision. After seeking advice, i decided to schedule my L-5 S-1 fusion with Dr. Melikian even though he was out of network and was going to be much more expensive than if I went in network. They were very flexible on the date of the surgery. They not only told me exactly what to do to be ready for the surgery, but also helped me schedule some of the appointments (GP visit for physical, imaging, etc). They were awesome at communicating with the other doctors to get everything they needed prior to surgery. Because of the type of surgery, I also had to have a vascular surgeon perform some of the surgery. They introduced me to the vascular surgeon and set up a consult. That surgeon was also awesome. Super thorough consult with a great description of the entire surgery. Surgery day was a breeze. The surgery center and all the staff there were amazing. I was REALLY scared going in, but after the surgery I was amazed at how I felt. I have heard horror stories of this particular surgery from family and I didn't experience of those things. I am 9 days post surgery and I feel great. Don't bargain shop for spine and vascular surgeons. That's stupid. Get the best. It's way important than a car, house, or private school. You get what you pay for, pay for the best. Dr. Melikian is hands down the best. I will update this review in a few months.
