Kern River Transitional Care was recommended to me because they have (and we needed for our family member) a strong physical therapy department. I now know this is a fact of the care offered. Nursing care also did not disappoint. Caring people doing their best. Skilled nursing with rehab can be a challenging experience for patients and families, confidence in the care makes it all easier. Exceptional and quality care was given to my father who was there to receive rehab for strengthening and endurance. Despite my father being a bit non-compliant with recommendations, the nursing and PT/OT staff were supportive and accommodating of his challenging behavior. I have had a loved one in this facility since 2019. Overall the staff is amazing, going above and beyond for their residents/patients. Of course there are a few bad apples, but they never last long. I had a fair to good experience at this facility. My LO, the actual patient, was very unhappy with the staff and their ability to respond to a call light. He also very much disliked the food. He was pretty much forgotten about in a private room. On my end, there was always having to follow up with Nursing with orders. It is a beautiful facility and was kept very clean. The staff at KRTC was so professional and compassionate. They treated my family member with utmost respect and took the time to explain any concerns I had. Even after discharge, I was able to contact a Nurse with medication questions. She calmed my fears and even invited me to come in person with future questions or concerns. I pray we never need their services again, but equally draw comfort from knowing they will always be there should we need them again. This review is incredibly personal, but I hope it can help at least one other person. My father had been battling Alzheimer's for the last few years. He started steeply declining in June and was in and out of the hospital. Kern River Transitional was fantastic in his final days of care and a much better alternative than a noisy hospital. The facility was superbly clean. It also always smelled clean, but not that chemically clean smell. It's hard to describe, but you'll know what I mean if you've been in similar care centers. The facility had tons of natural light and small courtyards everywhere. It had a great indoor/outdoor feel. There was even a tiny veggie garden. All the staff at Kern River were pleasant and helpful. They didn't even bat an eye when we stayed late after visiting hours. Without a doubt, I'd recommend Kern River Transitional Care for whatever care you or your loved one needs. read more

3 stars because I don't know the facility, so this is based off of the van driver who is an employee of this company. 1st off I'd like to say kudos to the guy who answered the phone for following through ASAP fully agreeing with my complaint and concern about the van parking in a handicap spot. I don't think it's right for the van lady to be parked on her phone while waiting for a patient if they do not have the proper placard. She is a very rude lady for arguing with the man on the phone, yes she was being very loud and incredibly inconsiderate. She wants to argue that she doesn't see why she has to move, maybe for the fact that elderly people should be able to park their cars in that spot. Funny as I'm writing this another van pulled up, and had to park farther away to unload their patient but guess what?!? They're moving the van they came in out of the way and being very courteous. So kudos to Galvez Medical Transportation for having a decent company driver unlike Kern River transport. Honestly this lady I wouldn't recommend my family member to ever be driven by her just for the sole fact of how she spoke on the phone and her ignorance of the fact that she feels entitled because she dropped someone off. No placard no parking I'm not sure how complicated that is. There are dozens of people who could have used that spot over that lady in the bus who is so concerned about staying in a shady spot over the common courtesy of doing the right thing. She shouldn't even be speaking to the person who called her the way she was. She shouldn't be a driver for any handicapped person due to the fact that she doesn't have any respect for someone who is handicapped walking by her struggling after having to walk across the parking lot when there is a perfectly good spot in the front that is being blocked by this ignorant driver. read more

Kern River is incredible -- definitely a home away from home! We are delighted we chose this facility after much research to find just the right place for our beloved brother Edward who was accustomed to only the best. Ed had a stroke & lost use of his limbs. KRTC's quality care has made a great difference for Ed as well as for our family. We are pleased that he receives compassionate care and attention daily. KRTC's physical, occupational, and speech therapy are exceptional. The staff is remarkable and make their patients feel important & valued. KRTC regularly gives thorough care conferences with the family and are willing to make any adjustments and respond to family concerns. We could not ask for better! The facility is absolutely beautiful and modern. Among the things I like best is their indoor therapy garden that allows patients to nurture and care for the plants... possibly something they may have done at their own homes. The indoor atrium is so beautiful that I have since emulated it at my own home. In conclusion, KRTC is the best care facility ever! You will not be disappointed. Beware ! Dont pay attention to these good reviews! Do not use this facility if you care about your family member! My brother-in-law was just released in the last 4 months after suffering from a stroke! Because of these high ratings my sister chose this facility! Major Mistake! Not only did he not receive proper care! No Onsite visiting physician to keep family informed and therapy was sporadic or nonexistent! They kept calling my sister to tell her that they were going to send him home before he had recoverd enough to do simple tasks! He wasn't capable of making decisions for himself at this time and the staff kept pressuring him to sign papers he didn't understand stating that if his insurance didn't pay he would be responsible for entire cost! They put my sister through Hell! This place is a nightmare! After a lot of bad communication and lack of empathy he finally came home and my sister is struggling to do the therapy at home that should have been done there! Now they are billing her for $$$$$$ that should be covered by Medicare and his Insurance! These bills itemize nothing and the insurance company claims they haven't even been billed! This facility should be investigated for insurance fraud and taking advantage of people who can't defend themselves or make decisions for themselves! Please don't put anyone you love in this facility ! Please pray for my brother in-law! As a important side note the neighbors down the street told my sister that they send people here to die! I have spent about 6 to 7 hours a day in the facility while they provide quality care to my mother for two weeks and the staff is always positive, kind, encouraging, compassionate and caring. My elderly dad fell and had to go into skilled nursing from his injury and subsequent bedsore. We chose KRTC because of its light, bright interiors and his home health PT, who we learned, also worked there. We couldn't have asked for better care throughout his recovery. They made every accommodation that was needed, including when he came down with C-diff due to his 3 rounds of antibiotics. Their rapid response and re-rooming to stop any spread was truly phenomenal. In my search for a skilled nursing facility,,I was admitted to Kern River Transitional Care from my hospital stay and boy was I lucky. What a nice place it is, (I made it my permanent home). I am so happy here! The facility is so clean and smells so good it's a blessing to be in. The staff here are so eager to help me in my needs. And always around close to help me if I need something with just the push of my call light! The food is very good and arranged nicely on my tray and delivered to me wherever I might be, the dining room or my room . The Physical Therapy Dept. I'd excellent with all the latest equipment! There's so much more I can speak to but perhaps another time as my tremors from Parkinons are acting up and it's difficult to type. My boyfriend and I went to this facility as a favor for a friend who's sister is there. We went there to make sure she was being treated well and taken care of appropriately. It was such a pleasure to walk into such a clean, well designed building with comfortable furniture and friendly staff. There was not one person there who didn't treat us with respect. And they lovingly cared for the patient even though she was constantly being negative and condescending towards them. We give this care facility and every single person who works there a AAA rating. And I hope when I'm in my last days I can go to a place as wonderful. read more

I can't say enough about the care here at Kern. Cynthia a CNA was very caring with my sister. She went above and beyond in her hospice care. She is very dedicated to her work and very compassionate. After my dad being at KRTC for 2 months, and me visiting him every single day, sometimes more than once a day, seeing how things work, seeing how all patients are treated, watching nursing and CNA staff do their "stuff", listening and talking to other residents/patients, I can confidently say KRTC would be the skilled nursing facility I would want to be placed in if I ever need that type of care, and I would have no hesitation in having a family member or friend being placed there. I have to mention a few names, and I wish I could remember everyone's name, but there were so many staff members that assisted with my father's care, it is impossible to remember all those names, but I will never forget every person's face or what they all did for my dad. Kelly (LVN) who greeted me on day one and gave me the lay of the land, is an exceptional LVN and one of the kindest individuals you'll ever meet. He took care of my dad without hesitation, without fail, and I am truly grateful. Orlin (LVN) took care of my dad as if he was her own dad - she is caring, thoughtful, great at her job, and puts patients at ease with her beautiful soul. Jayson (PT), the best! Cora (Admin), the best! Gavin, Maria (front desk), the best!!! Nora (Social Worker), the best! Every single CNA, who has the not-so-glamourous jobs (personal hygiene care, etc), the best! In the 2 months my dad was at KRTC, I never saw staff members just idly standing around, messing around, or NOT taking care of patients. The facility was full, I think there are over 100+ beds/patients there, and at all times the CNAs, LVNs, Admin staff, were moving and hustling and getting stuff done. There were times dinner was late - oh well. There were times my dad's Norco came 15 minutes late, and he, being 85 and somewhat confused, would call me in the middle of the night to tell me they weren't taking care of him. I would then call the 2A wing nurses' station, speak with the nurse that night, they would explain to me when he had his last meds, and that they were on their way to give him his next set - and in a few minutes he was calmed. He wasn't being neglected, at all, he is 85 and had been in a hospital setting for months, and was a little confused and in pain. I was very happy with the responses I got, whenever I needed to call the facility in the middle of the night, or talk to them in person. I heard patients yelling (not often, but sometimes you do hear it) - that's nothing out of the ordinary for a skilled nursing facility. There are patients in cognitive decline there, there are non-verbal patients, there are patients who are short-timers and recovering from strokes and surgeries, and there are patients who are long-term forever patients. Sometimes elderly people yell. I can say, having spent so much time there, I personally NEVER saw any neglect of any patient. I got to know the long-term residents and became friends with many of these beautiful older people. I watched how the staff treated them - these seniors had become part of the KRTC family. I watched how the staff spoke with all the residents, even the cognitively challenged ones, and while I am sure there are times staff can become frustrated - like any of us - they never displayed that with their patients. The only place I would give KRTC 4 out of 5 stars would be for the wound care - my dad had a couple of ulcers on his legs and it took quite a few times for me to push to get him on the wound care list, and have his wounds looked at and treated. There is a wound nurse who makes rounds and does this, but evidently he/she is not there every day? I'm not sure, but I do hope that going forward wounds can be looked at and treated daily by the LVN if the wound specialist isn't there. After a little reminding from me, they got on a regular schedule of looking at his wounds and cleaning them up. The facility is beautiful and the newest in town - and I am ending with this because something can look great, but not be great - well here? it looks great and IS great. The place is clean, the PT area (or as my dad called it, "The Gym") is amazing! There was a carnival in the parking lot during my dad's stay - my dad doesn't "do carnivals" but guess what? He actually went outside in his wheelchair and rolled through the carnival and listened to the music and said it was great! There are dining areas for residents/patients to sit at if they don't want to eat in their room - there are great patios, there's a salon, and the lobby is also a wonderful, welcoming place to sit and visit with your family. There are vending machines if your loved ones want "not so great for you" snacks....and, there is a piano - which is horribly out of tune, hahaha - that I heard residents regularly playing. If anything, they should tune that piano so when the music is floating through the the facility, it doesn't sound so painful . I love the care you get here.Dr. Klang is the best doctor because he listens to you and does everything possible to help you get better. If you need c-rays or ultra sound or just rest, he follows through to get you better. I love this doctor. The nurses and CNA's are the best. They go out of their way to help you. Jackie, Sylvia, Matt,JT are just a few who are so awesome. Did I mention Ana? She is such a wonderful person & CNA,. I trust the care here better than any place . Gavin and Maria are great receptionists. They can get anything you need done. The therapy department has the very best therapists. Eric ,and Vicki, the three Jason's Art,Jessica, Dee, and others helped me to walk again after a fall. Two falls together were too much for me to adjust to. The social workers are so helpful. Regina was so helpful and getting me ready to go home. The activities department is so busy. The parties are increable. They give you a manicure,actually activities in the morning and bingo in the afternoon.. They even had a carnival complete with a rock band. I highly recommend Kern River Transistional care to anyone coming out of the hospital and needing more care. I love this place! read more

This facility has the lowest sense of human kindness I have ever seen first hand. My family would go visit our grandmother but because of Covid the visits were through her room window. We used a cell phone on speaker to communicate with her on our visits. My Grandma was here for months recovering from a stroke and was treated like she didn't matter. The nurses would deny her water stating the kitchen was closed. They were rude, never had time to adjust her position in bed, took hours to respond to simple requests. No one should be treated this way. On one visit my grandmother was yelling on the top of her lungs "enfermera ayuda!" And they wouldn't come into her room, I had to call the front and ask them to transfer me to the nurse assigned to her wing. When the nurse answered I told her that my grandmother was yelling for one of them to go help her and the nurse simply hung up the phone. It took her 15 minutes from the time I called her to actually walk into the room. I'd list names but the nurses would flip their name tags so we couldn't see their names read more

My husband was sent to Kern River Transitional Care from the hospital. They took him at about 7:30pm so I couldn't go up till the next day. He was in there about 14 hours and he called me, after begging them for hours to call me. Our son was on his way up there with the only car I have to drive. When I answered the phone my husband said get me out of here now! He didn't want to say too much because they were standing right there. MY son got there and had a bottle of water for his dad. He said his dad looked at it like it was a billion dollar gold bar. He was so thirsty. He said they only gave him one drink of water since he had been there. No pitcher of water, no cup of water was in the room, no way for him to get a drink. There was no call button for him to press to get help. He was down a hallway so he couldn't get anyone to come when he called for help. My son said the place was nice looking but there were multiple people in rooms yelling for help. He said they didn't look well cared for. Patients were asking him to get them out of there. Overall a horrible experience for my husband and our son. Our son brought him home. His Dr. got him into Bakersfield Rehab center and they have been great. I would NOT recommend Kern River under any circumstances! I wish i could give no stars! read more

Not a good place if your loved one is there longer then a few days , our family experience has not been a good one. My mom ended up with an infection so bad that the surgeon had to remove all the hardware he placed in her ankle. She was to have a weekly Dr. Visit to watch her leg closely and the facility failed to take her to them. by the third week it was to late they caused her more suffering. On top of that you can never get someone to answer the phones, all dept. Voice mail is full? I'm in nursing and have never heard of such a messed up system in 26 years in healthcare. My Dad who couldn't reach anyone or my mom in two days had to call the local police department to do a well check for my mom. After several, attempts by hitting redial I was able to speak with nursing and check on my mom, I was told 4 days later they couldn't speck to me about my mom or tell me if she was okay said I wasn't in her chart, which prior to speaking to a Kathryn I no longer could follow up on my mom until my dad called again to give my name it was as if they where playing games with us . Awful place in one review they lost a person's body and it took days to find out where they where, how terrible is that. no confidence in this facility, lack of communication , please beware. read more
