Somewhere between 5-7 years ago, I began suffering from arthritis in the tarsal sinus of both ankles, as the result of severe flatfeet, years of basketball and related ankle injuries in my youth. As the pain increased, my level of activity/athletics decreased. I distinctly recall a day I went to magic mountain with one of my good friends, like we used to as kids. I could barely make it out of the park I was in so much pain at the end of it, and the recovery from such an outing could take WEEKS... it was terrible.
This prompted me to seek out medical treatment to first fix my flat feet -- for good. I stumbled upon the Hyprocure stent as a possible solution. Initially, I visited a podatrist in Sherman Oaks (not to be named), who said I was a complete FIT for the procedure, which was great news.
However, I am always one to seek multiple opinions, which led me to Dr. Roth, THE top physician performing the procedure. My HMO at the time (Kaiser) did not support the hyprocure. In fact, their only solution was a triple arthrodesis (fusing the 3 main joints of the hindfoot), which I refused to consider at age 38..
Dr. Roth evaluated me for the Hyprocure, and was completely honest and straightforward with me -- something we don't get enough of in this day and age of ULTRA-sensitivity and fragile egos. Unfortunately, I was too close to the edge of "a fit" - and he explained why with data, images, etc - for the Hyprocure in its current form, so as to either not see enough correction, or to possibly see complications later. Dr. Roth offered an alternative long-term solution Kaiser made no mention of - the calcaneal osteotomy - along with the NOW solution of a set of his WONDERFUL custom orthotics, ALONG with a stem-cell treatment not yet used by any podiatrists Dr. Roth knows of. I agreed to take the chance on the treatment, one in each ankle, stems cells taken from my upper tibia, in lieu of more cortisone shots from Kaiser (forgot to mention those .. had 2 and then refused more due to degenerative effect).
It took a while - perhaps a month - after each shot, but I truly began noticing improvement. Overall a bit less acute pain, but more importantly to me, less RECOVERY from overexertion (long walks, hikes, standing on feet for 45+ mins, etc), Whereas in the past, I would be on the shelf and shuffling around for a week or so with terrible chronic soreness and SHOOTING pain, since these procedures, i might only be sore (in the tarsal sinus and arches) for a day, maybe two with the second day much better than the first.
Am I still in pain? Yes of course, but that wont be "fixed" until I either undergo calcaneal osteotomies (with expected new pain and soreness that come with them) or somehow qualify for a later version of the Hyprocure. Or God forbid, am now stuck with triple arthrodeses...
For the NOW/short-term problem, the orthotics provide GREAT support / stability, and allow me to walk/hike for longer periods before the onset of pain.
Whereas, stem-cell therapy has truly revolutionized my quality of life in 3 ways:
1) NO more shooting pain while resting! This as something truly terrible... after a long day of walking and constant pain, the only thing I could hope for was to kick my feet up on the couch.
No reprieve, for the arthritic joints in my ankle would begin to feel as if they were separating, moving around and i'd be hit with daggers of shooting pain, without warning. Ice, heat, meds, nothing helped but time and grinning and bearing it. These daggers are GONE
2) reduced the acute soreness somewhat (onset of pain while being active .. i know the orthotics are helping here as well). Basically, I can walk, hike, stand longer before soreness/pain starts up in both my ankles and arches.
3) Greatly reducing recovery time from walking, hiking, standing, etc -- down from 1-2 weeks to 1-2 days.
NEXT, i will be bringing my 16-year old daughter to get fitted for orthotics, for she sustained a complete ACL tear in her right knee while playing club soccer. Dr. Roth actually advised me that athletes were more prone to suffering ACL tears BECAUSE they did not wear orthotics at the time of the injurym, and that he had supplied entire football teams with said orthotics, resulting in near -elimination of ACL tears. Sadly, i did not take the steps that could have perhaps prevented my daughter's injury, but I will NOW do it to reduce the chance of future ones.
