The facilities are great. The residents rooms and bathrooms are private and spacious with high quality buildouts. The food is good. I've noticed generally high quality, tasty food getting served. The staff means well. I don't doubt the sincerity of the staff wanting to help improve the quality of life of the residents whether that mean providing activities in the common areas, get them medical care when needed, speak to them encouragingly when they need to be re-guided. The issues I ran into require a little bit of context. My father is averse to getting help on showers. The PVN nurse suggested we look into getting some meds to help on shower days and get some external extra help to provide showers for him. While I wasn't happy to pay for extra help, given the high cost of PVN I thought the request was presented in a professional manner. The issues come in with a lack of professional approach to medical management and follow-up. I consider these issues significant enough to ding PVN down to 2 stars. My father's doctor agreed that some medicines could help make shower days a little easier and that it was important for him to stay clean on a regular basis. The problem is that now, despite this being the PVN staff's idea, they are unable to get the medicines to my dad on the morning's of the shower days. They have missed this 3 out of the last 4 shower days, which then makes the task of giving showers a monumental task. It would seem that this would be easy to address given that they claim to give him his other medicines, but the one medicine that was introduced - effectively at their request - is something they seem unable or unwilling to give him. I have called numerous times and can get an answer about 50% of the time. I have sent 4 emails on this topic to the Head Nurse & her assistant and have a 0% response rate thus far. In my mind this boils down to poor leadership. I have no way of communicating with the senior leaders of PVN to get these issues addressed. The nurses ignore my emails, and worse, deny my father the access to medicines that will improve his and his fellow residents quality of life. it is my belief that PVN needs to fundamentally transform the culture and leadership of their organization. They are currently falling far short of their mission.

I would give Holding Hands Hospice 10+ stars if that was possible. I found them through Yelp &…read moreGoogle reviews when I was looking to change from a hospice group that was part of a large hospice organization, to one that could accommodate some special requests that we had. Holding Hands Hospice (HHH) is a boutique hospice group owned by a lovely, dedicated gentleman named Aqua Umoren. During the 12+ years they have been in business, he has built a team of amazing, dedicated individuals, the majority of whom have been with him for 5+ years. To me that speaks volumes, considering this is such a difficult industry to work in, with a lot of turnover. Our mother lives at The Preston of the Park Cities, an upscale assisted living facility where she is well cared for, however she is at the point in her long life whereby we needed the extra support hospice offers, which is provided through Medicare, at no additional cost. Mary, an RN is mom's case manager from HHH. Both she and Aqua made it a point of coming over and meeting me when I was in town a couple of weeks back since I live out of state. Prior to that I only knew their voices over the phone along with contact via text messages. They both are such genuine, personable individuals who love what they do, and have loads of compassion for those they are caring for. They matched my mother with an amazing woman named Lawana who assist mom Monday through Friday mornings, showering her & getting her ready for the day. Lawana arrives during the timeframe my mother requested, which is not an easy thing to. Lawana goes above and beyond, keeping our mother company after she has tended to her morning needs. They talk, and even go down to have coffee together. Aqua understood right away how impactful this type of high touch care was to our mother's well being, and rearranged Lawana's schedule to make this level of care possible. Aqua and Mary consistently keep me up-to-date on how mom is doing and any changes that are taking place. According to the Director of Resident Care at The Preston, HHH has had the best communication with her and her staff of any hospice group they have worked with. I just don't see how any other hospice organization can measure up to what HHH provides. They are truly the best of the best.

Anything less than 9 is acceptable but that's difficult to find in short or long term nursing and rehabilitation facilities. As a retired Army Nurse, I understand this. I thought I did my due diligence by using Google reviews, Yelp reviews, a list given by Medicare and the accepted facilities to consider by Medicare/Medicaid and United Healthcare. CHECK HOW MANY REVIEWS WERE GIVEN AS WELL AS WHAT WAS SAID. 5 REVIEWS IS NOT ENOUGH TO GO BY! My mother at 97 years of age is strong willed yet not demanding, has a will to continue living, does her best to maintain as much self care as possible but here, her emotional hopeful attitude and dignity and will were highly degraded which I had never seen before. She was broken. The only time she ever stated she wished the "LORD would take her". She was left to sit in feces for up to three hours out of a few times and daily, in her urine saturated diapers for hours, yes, hours. Repeated call button requests ignored, "I'll return" was hours later then treated poorly when she did get the issues addressed. Served meager amounts of poor quality food, all but two CNAs disrespectful, negligent, demeaning, negative, etc.. only two days of good physical rehabilitation care in two weeks. The times I tried to and connected to facility coordinators the problems were promised to be address but went unchanged. I'm filing a report to APS, HHS and Medicare. After her release from the hospital from a fall occurring at Le Reve my mother was placed at my second choice AVANTE Rehabilitation she is now receiving good quality care. Three days with just a few minor incidents. Update: The care at AVANTE in Irving was superior. Night and day comparison. Good food, empathetic caring staff. Two very minor incidents in two weeks that were addressed and handle positively.. Should my Mother need long term care, this will be the place if they have a place for her.

My father entered this facility for 20 days of rehabilitation, costing approximately $4,200, and his experience was absolutely horrific. He left in significantly worse condition than when he arrived. My father has taken psychiatric medication since he was 12 years old because of severe childhood trauma. Despite receiving discharge instructions from the hospital, this facility failed to provide his psychiatric medication for more than three weeks. Staff then called my elderly mother and said they were discharging him because of his behavior. We were stunned. How can a facility stop a lifelong psychiatric medication, fail to address the consequences, and then blame and discharge the patient for his behavior? My father also has serious swallowing difficulties and is at risk of aspiration. His hospital instructions required a pureed diet and specific swallowing precautions. When the facility sent him back to the hospital for approximately a week, they apparently failed to communicate those critical instructions. The hospital began serving him regular meals, placing him at further risk because he could not safely swallow that food. The neglect did not stop there. My father was frequently left alone in his room, urine was left on the floor, and he had to change rooms after ants were found in his bed. His bed was positioned with his back toward the doorway and his face toward the wall, so he could not see staff passing by or entering. There was no phone in his room. Our family repeatedly called staff about his medications, his behavior, and our concerns, yet our calls were not returned. Staff may sound friendly and reassuring on the phone, but their actions did not reflect genuine care. My father declined physically and mentally while he was there. Now my elderly mother is being left to care for him because the facility is discharging him over behavior that occurred while they were failing to provide his prescribed psychiatric medication. I have filed a formal report with Texas Health and Human Services. This facility's failures involving medication management, swallowing precautions, sanitation, supervision, and family communication created serious and potentially life-threatening risks. Our family trusted them to rehabilitate and care for my father. Instead, he was neglected, isolated, and returned to us in worse condition. This is not the last they will hear from our family. Please think very carefully before placing someone you love in this facility.

They started out in IL (independent living) and 3 years ago we lost Dad and Mom moved into DL (dependent living). All buildings are connected so Mom gets to see old friends at Sunday Brunch. (She is 93 now.) She has made many new friends in the DL building who are charming and so nice to her. I'm writing this because over the last 5-6 months I've been joining Mom for Happy Hour and have gotten to know her friends. I can't tell you how many stories I hear them share about other facilities in Dallas (Austin and Houston too) about how bad the food is and how they themselves (or friends) are ignored and not cared for. They told be several times that when they tell people how awesome everything is at The Forum... their friends don't believe it!! LOL, LOL. Well... it's true! Yes, I confess I take Mom to Sunday Brunch so I can get one of the best omelets in Dallas (in the IL building) and the rest of the food is so nice. My sisters take guests and we always celebrate birthdays and holidays at Sunday Brunch. Enough about that. Let's get to the brass tacks. The staff is probably the best I've ever encountered. If I'm with Mon and she has an ache and needs some Tylenol, I call the front desk and the nurse brings it to her within 15 min or so. VERY RESPONSIVE!! (Abbey is so awesome!) Mom has a Help Watch on her arm and can press the button and help comes immediately. As well, the staff will call me with every incident (like sliding out of her chair, etc.) Major or minor - they update the family immediately. AWESOME! Ok. So you are looking for a place for your parents. Absolutely check out The Forum. Have frank and direct conversations with the staff who show you around (and see if you can pay to sample the Sunday Buffet!!) Our family has so much peace knowing even the cleaning people care about a quality job and if they observe anything that needs attention - they alert the right people. I personally feel indebted to The Forum. THANK YOU ALL FOR TAKING SUCH GOOD CARE OF MY MOM!!!!!!
