The level of care, compassion, and professionalism shown by everyone at Sun Mar is truly remarkable. No matter the situation, their staff consistently go above and beyond for their patients. Like any place, there were some issues along the way, but what stood out most was how promptly and effectively they handled everything. Every issue was addressed and resolved in a timely and caring manner, and my dad's needs were always their top priority. From the nurses and physical therapists to the staff who assisted with meals, bathing, and medication, every single person played an important role in my dad's recovery. Their teamwork and dedication made such a difference. I'd especially like to acknowledge Jun, Julie, Andy, Brandon, Jasper, Michael, Dennis, Christine, Mark, Stephanie, Lisa, Lucy, Celina, Cindy, Sarah, Angie, Jaime, Ben, Mark, Ester, Navi, Balgit, Shainah,( I am sure I am forgetting alot of names...) for their kindness, responsiveness, and personal attention throughout my dad's stay. I'll never forget how accommodating Jun was when I reached out on short notice. I think it was 24 hours before we needed to transfer my dad. He took care of everything quickly and efficiently, ensuring my dad could be admitted without delay. That kind of compassion and professionalism is rare, and I truly appreciate it so much!! The facility was consistently clean and had a calm, welcoming atmosphere. It stayed organized without ever feeling chaotic, and the staff were professional, approachable, and responsive. That consistency carried across the entire facility--from the nurses, CNAs, and medical providers to social services, activities, dietary, housekeeping, and front-office staff. We received timely reminders and updates, which helped me stay on top of his care and next steps. As his primary caregiver, knowing I could reach someone when I had a question made my life easier and gave me much-needed peace of mind. Most importantly, my dad formed a genuine connection and friendship with the people caring for him. That certainly made his four-month stay feel much more personal and comfortable. PT and OT were especially important to his recovery. They were patient and encouraging while helping him rebuild his strength, mobility, confidence, and everyday abilities. They challenged him appropriately while adjusting to how he was feeling each day. After four months, his transition home was handled smoothly. I'm grateful to the entire Gordon Lane team for supporting my dad and making a difficult period easier for our family.

I told them this was my goal. Once they got me there they put me in a diaper! After putting up with this for several days I threatened to require transfer to another care center. Then they promised to get a commode for me for my bedside. They kept promising and promising, and never did it for the 6+ weeks I was there! In addition, the first space I was assigned had a TV that would only turn off if you unplugged it. Also, their TV channels did not include KTLA, a major local channel. Hygiene was also an issue. The curtain between me and my neighbor had smear of snot or other bodily fluids on it. It took more than a week for them to change it. Later on I had a chest infection and I got some fluid on the new curtain. When I told the cleaning crew, all they did was rehang the same curtain with the side toward my neighbor (who was there for cancer treatment)! There was also a security issue. One male resident, who apparently had some form of dementia, kept wandering the halls and entering my space, claiming he was visiting a friend (no males were in my unit, and the other resident also did not want him in our unit). I told the staff and they cautioned him ("you don't want to go back to the hospital (mental?), do you?"), but he continued to poke his head into my unit. I observed them lying to residents. They took away my neighbor's wheelchair because they didn't want her wandering unaccompanied in the halls, but told her they didn't know what happened to it. They promised physical therapy four times a week but I never received more than 10 to 15 minutes two or three times a week, and when I asked for more they said staff were unavailable.

We are writing to express our deepest gratitude for the exceptional care she received at your facility in Fullerton. We would like to extend a huge thank you to Jay in Physical Therapy for his excellent attention to detail. His diligent tracking of the additional days required for her stay ensured she could safely transfer from her bed to a wheelchair with the assistance of only one CNA. When our mother arrived a month ago, she was unable to stand or walk, and her progress has been remarkable. We were also very impressed by the medical care meeting provided for our family. It was well-organized, with up-to-date lab work and clear documentation that allowed us to easily understand her medical progress. Additionally, social workers Elvie and Ana, along with Bianca, Alejandra, and Renee from administration, were outstanding resources. They kept our family consistently informed and made the discharge process seamless. Our special gratitude goes to the housekeeping team (Gloria and Julio), the CNAs (Mayra and Alma), and the nursing staff. They kept the facility immaculately clean and welcoming, while ensuring our mother was comfortable, cared for, and able to participate in daily activities. The rehab department itself is well organized, and put together. I would hands-down recommend this facility to anyone looking for skilled rehab. Elvie the case manager is professional and very communicative. Returning your calls immediately. She's very up-to-date on the patient's situation at all times. It's a small and everyone works as a team. From the CNA's to the nursing staff to the physical therapist, occupational therapist. They all work well together.

As a healthcare provider myself, im very keen to the healthcare facilities and how well they are maintained. Im sure, like many other inpatient facilities, one cant expect service right away once called upon. So i guess that would be my only real complain. Other than that, i truly believe my lola (means grandma in Filipino) is in good hands. This facility has a great schedule for activities to keep them active or something fun like getting her nails painted or coloring. I remember walking in on the patients in a circle and they were hitting an air ball around. It was just too cute! They even take out all the patients once a month or so to a restaurant just to change things up! They provide costumes for fun parties and Halloween. Its so nice! They also have board games and playing cards and i loved borrowing their cards to play Go Fish with my lola. I love my lola with all my heart and the last time i visited her, I was very pleased to learn how much the staff truly cares about her as well as the other patients. There are all those horror stories of other nursing homes that are just inhumane and cruel and breaks your heart to find out the mistreatment from a nurse.     Finding a place to care for your loved one is hard. And believe me, I felt sorry for whoever was…read moregoing to take on my mom because not only am I in deep in healthcare (previously in acute neuro trauma and now in outpatient/hospital admin), but she is heavily SURROUNDED by people in healthcare ranging from directors, doctors, surgeons, and nurses. On top of that she has family who loves and protects her very deeply. Somethings that stood out about ActivCare to me from the rest that made me choose them over the others: 1. There's only 2 levels instead of 5+ levels. You have to have memory issues if you want to live there so there will be no confusion or issues trying to determine which patients need what as the general needs will be similar. 2. You don't get nickel and dimed. Other places might charge for laundry service, food, incontinence, etc etc. not here. You just might have to pay for medications or the beauty salon or doctors. 3. There's a licensed nurse at all hours of the day and wake staff 24 hours. 4. Appropriate staff are constantly cleaning. 5. They usually have daily exercises. 6. There are outings. 7. There is a set daily schedule. 8. There's a tiny putting green if they choose to putt. 9. There's a tiny garden. 10. We could go take food and eat with her whenever we wanted. 11. All of the above was everything we loved, but the cherry on top was that it was THE BEST COST!! What I was scared of: 1. She wouldn't accept it and start declining. 2. She wouldn't adapt. 3. She wouldn't be nice to them. 4. She wouldn't eat the food. 5. She would be mad at us. 6. They wouldn't care for her like we did at home. What actually happened after she became a resident despite her craziness and Sour Patch Kid personality: 1. She relied on them and they comforted her. 2. She ate the food and almost too much that she got the nickname the Cookiemonster. 3. She adapted (we told her it was a private hospital to get stronger since they had exercises everyday and people to help her walk). 4. They accepted her sour moments and loved her anyway. 5. She felt comforted by them and especially by certain people and they would do her hair and paint her nails. 6. She actually got stronger for a bit and actually started socializing and participating. 7. Of course she fell because thats is inevitable but they cared for her the right way each time (and if you think because she lives in a facility that it shouldn't have happened then I'll tell you right now you're never allowed to hire anyone to care for your loved one or send them somewhere to live because that's impossible). 8. I got to see an Elvis impersonator, whole room karaoke, chair exercises, the biggest puzzles of my life, so many things that will now also be core memories for my son. 9. I became her daughter again instead of the stressed and burnt out default extra caregiver since she lived at my house. 10. They loved and cared for her more than I could have ever imagined. My mom's brain cancer eventually took over and even when it enslaved her to be loud and demanding they still loved and cared for her until she couldn't function. Imagine this mixed with memory and dementia issues. It takes a special soul and personality to work in an environment like this; I can save your life but I admittedly am not that person. All of these people are that person and if God gave me back my mom from heaven then I would choose ActivCare Yorba Linda again to help me care for my mom. The care she received makes it 10 times more worth what was paid for her care which was already a great price. If you're reading this sentence and contemplating, I think you know where your family will be going. Tell them the Cookiemonster sent you.

I love that they know Mom and can distract her when she wants to be grumpy. When I call everyone is more than happy to talk to me and tell me all about Mom. When she has had any health issues I am notified right away and kept informed about everything. if you are looking for a place for Mom or Dad, ActivCare is certainly worth looking into.
