My father was released from Osborn Hospital in Scottsdale AZ on 7/15/26 & admitted to Osborn health & rehabilitation in Scottsdale, AZ on 7/15/26 in the evening. He passed away 7/28/26. Upon release he was talking and walking with assist and stayed an extra 3 days in the hospital to ensure he was his normal self and was ready to start activity rehab. He had pneumonia and an infection that cleared prior to leaving the hospital when he returned to the hospital upon evaluation they noticed it returned. The first night he was in the rehab facility he fell out of his bed around 4:00AM and a call was placed to my stepmother from Nadeera around 5:39AM stating he fell and has a slight bump on his bed and he was normal. My stepmother went to the facility around 8:30AM after work and my father was incoherent, staring at the walls and had a temperature of 80 degrees.I spoke with Chanelle on 7/19/26 stating his vitals was normal and she released information to me and I wasn't on the contact list, she placed me on the contact list after she gave me his medical information in which my stepmother didn't authorize and Bodie stated my dad spit his food and turned his head away during an attempted 1:1 feeding session. My father was completely incoherent and not responding to touch or anything so that's not true. He was left in a hypothermic state for 4 days, no food or water and an IV was not administered until day 4 or 5 of his stay. My father was not in a position to do any physical, occupational, speech or activity therapy. He immediately declined in less than 12 hrs of being there and his ICU doctor kept asking us why they (Osborn rehab) sit on him for 4 days and not send him back to the hospital right away (right across the street from the rehab is the hospital less than 30 seconds). THe rehab doctor spoke to the ICU doctor and stated my father never fell and we have a voicemail stating he did. My father got transported back to the hospital on Monday 7/20/26 and went to trauma for MRI and Scans which came back clear, after the evaluation in trauma he got rushed to ICU due to septic shock in his heart/lungs, and severe coma, lack of oxygen and immediately had to be incubated on a ventilator. Once my father fell they never gave him an XRAY or CT scan to check on his health status which is also on record. We also have photos and videos of our fathers condition/ health which was utterly disheartening. I walked into his room and immediately knew something was wrong and he needed immediate medical attention. The aide Chanelle stated he had no xray done and his vitals were normal which is untrue. MY father body temp was 80 degrees and we found out from the hospital his actual temperature due to a catheter being inserted into his groin for an actual temp. SO how did they get a temp of it being normal? The nurse stated my dad was normal after the fall he was non-responsive, blood shot red eyes, not responding to touch or conversation and was naked no clothes and a made up diaper, a mat on the floor to brace his fall had a feeding table on it locked in place. My dad was in no position to move anyway but he is a fall risk and the table was completely unsafe. My father experienced a severe decline in health and experienced malpractice, pain and suffering, gross negligience, delayed calls to 911 un an emergency, sepsis (leading to septic shock), sudden decline, neglect, malnutrition, inadequate resources (no clothes on my dad, no warmer to provide warming blankets which was a delay in adequate care. The rehab kept insinuating to my stepmother she could take him to the hospital herself and my dad was completely not in the position to move he needed an ambulance. The rehab sttd he was there under doctors orders and nothing could be done until the doctor came in. The facility wanted my stepmother to take him herself so they wouldnt be liable for the lack of care they provided my father while in their care. I would give all the earthly things in the world to have my father back with us, he was aiming to come home nothing about his stay was leaning towards him not fully rehabbing and coming home. My family and I are truly upset that due to sudden decline and delay of calling 911 is what contributed to him passing away. He was up and talking and walking when he got there and less than 12 hrs my father was in a position of a person who was left at the top of a mountain and froze to death. The staff is only there for a check and every person there needs to be fired. My family and I are currently taking legal matters and are currently represented by a firm that truly believes justice will prevail in our favor. Honestly, one star is too much zero.

Her room often smelled of urine, and while the care staff was great, her living situation quickly deteriorated. One evening I received a call where she was "lost" but they later found her in a courtyard talking to a friend. The financial accounting aspect was very frustrating. I was originally told she could eat in her room or in the dining room but then they began to charge a delivery fee. She fell a second time right before her 88th birthday and sustained a TBI. We were still expected to pay the rent portion for her 30 day notice, which I understand but I am now arguing about a $47 charge that cannot be explained. This is the last thing we should be worried about when my grandmother is now dying as a result of that last fall.

It was the second day it opened...a little over three years ago. From my apartment on the 10th floor I look east across the beautiful campus and see the McDowell Mountains and the rising sun each morning, as well as students scurrying, riding skateboards, and hurrying to class. Residents are also able to take classes tuition free and participate in many campus activities. It's an easy walk to the football and basketball facilities, and my favorite, baseball, is just four miles away. The Gammage Performance Center is an easy walk, as well as the many shops downtown. For transportation we have light rail that goes all over the Valley, just a short ride to see the Arizona Diamondbacks, the Phoenix Suns, or the Phoenix Symphony! Similar facilities are remotely located...go out the gate and see desert or a residential neighborhood. We have shops, groceries, a drug store, entertainment, and a movie theater! My one-bedroom apartment is spacious and comfortable with maintenance and cleaning included. Our computer/TV guy Trevor is one of the top five reasons to live here...get a new printer...he will make it work in no time!! Our maintenance people are prompt and pleasant for issues with appliances or repair of anything that comes up or to assist in special projects. We also have three restaurants serving a variety of new items as well as old favorites. Many ASU students work in the restaurants and our underground parking garage. None of us get into a hot car because it's all underground! Valets provide airport service and also drive those without with a car for appointments within a reasonable distance! Residents are also kept busy going all over the Valley in 15-passenger buses to varied locations. Many events and speakers are offered; it's sometimes impossible to do it all. One floor is designated as Assisted Living and another for Memory Care. So if you are thinking about moving from the big house that's heavy maintenance, don't wait until you have a fall or accident. Come here, meet new people, and join in the activities. If you have grown children, it's a great gift for us to all downsize and know there are people nearby to help as we grow older and less able to handle the burden of home ownership or living alone.
